Autoantibody assays are reported in a variety of formats. Results only slightly above established cut-offs provide lower likelihood ratios; therefore, their clinical significance may be more uncertain, which is not readily communicated with dichotomous qualitative reporting. Line immunoassays (LIA) are a common method for detecting antibodies to extractable nuclear antigens (ENA) and myositis-associated antibodies. However, recommended positive cut-offs are contentious. We distributed a survey via e-mail to participants in the Royal College of Pathologists of Australasia Quality Assurance Program (RCPAQAP) Immunology modules and to a dedicated immunology mailing list in Australasia. Questions explored general viewpoints surrounding autoantibody reporting, as well as current laboratory practices, with particular focus on interpretation and reporting of the most commonly used ENA LIA manufactured by Euroimmun. There were 31 responders, representative of at least 17 unique laboratories across Australia (8 public, 5 private) and New Zealand (4 laboratories). Responses suggest that autoantibody reporting is not standardised; there was variation in general viewpoints and reporting practices, particularly regarding the interpretation of and positive cut-offs used for the Euroimmun ENA LIA, which were contrary to the manufacturer's guidelines in a majority of the responses. Interpretative qualitative reporting based on results from other investigations and the clinical history was a common theme. There is large variation in the reporting of autoantibody assays within Australasia, especially by LIA. A majority of respondents report the most commonly used ENA LIA contrary to manufacturer's guidelines; alternative positive cut-offs are commonly utilised. LIA reports should indicate the level of positivity to enhance their relevance in the clinical decision-making process.

OBJECTIVE Patients with epilepsy who do not respond to two trials of appropriate antiepileptic drugs are considered to have drug-resistant epilepsy (DRE). The International League Against Epilepsy recommends patients with DRE be referred for surgical evaluation; however, prior literature suggests this is an underutilized intervention, especially in the pediatric setting. This study captures practices of North American pediatric neurologists regarding the management of DRE and factors that may promote or limit referrals for epilepsy surgical evaluation. METHODS A REDCap survey was distributed via the Child Neurology Society mailing list to pediatric neurologists practicing in North America. Ethics approval from the Children's Hospital of Eastern Ontario Research Ethics Board was granted prior to the start of data collection. RESULTS Ninety-eight responses were included in the analysis; 77% of participants currently practice in the United States; 73.5% of respondents reported they would refer a patient for surgical consultation after two failed medications. Of all potential predictors tested in a binary logistic regression model, only annual referral volume predicted whether participants refer patients after three or more failed medications. Centers with high referral volume were 37% more likely to adhere to the guideline of referral after two failed medications. SIGNIFICANCE Pediatric neurologists demonstrate fair knowledge of formal recommendations to refer patients for surgical evaluation after two failed medication trials, although referral rates remain unacceptably low. Participants continue to report that they would not refer patients with generalized electroencephalographic findings for surgical evaluation; this should continue to be addressed. Other modifiable factors reported, especially family perceptions of epilepsy surgery, should be prioritized when developing tools to enhance effective referrals and increase utilization of epilepsy surgery in the management of pediatric DRE.

Code review is an important quality assurance activity for software development. Code review discussions among developers and maintainers can be heated and sometimes involve personal attacks and unnecessary disrespectful comments, demonstrating, therefore, incivility. Although incivility in public discussions has received increasing attention from researchers in different domains, the knowledge about the characteristics, causes, and consequences of uncivil communication is still very limited in the context of software development, and more specifically, code review. To address this gap in the literature, we leverage the mature social construct of incivility as a lens to understand confrontational conflicts in open source code review discussions. For that, we conducted a qualitative analysis on 1,545 emails from the Linux Kernel Mailing List (LKML) that were associated with rejected changes. We found that more than half (66.66%) of the non-technical emails included uncivil features. Particularly, frustration, name calling, and impatience are the most frequent features in uncivil emails. We also found that there are civil alternatives to address arguments, while uncivil comments can potentially be made by any people when discussing any topic. Finally, we identified various causes and consequences of such uncivil communication. Our work serves as the first study about the phenomenon of in(civility) in open source software development, paving the road for a new field of research about collaboration and communication in the context of software engineering activities.

This paper aims at exploring socio-cultural stance and perspective in writing practices for condolence emails. The social purpose of condolence emails is to express deep sadness on the passing souls. Therefore, such texts note life stories and commemorate inspirations of the deceased both famous and infamous so account for the genre analysis (Christie & Martin, 1997). Since personal emails are written by the individuals concerned so necessarily outline significant cultural elements. The study builds on the topological genre analysis (James R Martin & Rose, 2008) of the condolence emails mainly looking into staging (sequential and ascriptional) and describing linguistic features (Halliday & Matthiessen, 2014). The analysis shows significant socio-cultural variations in writing condolence emails. Therefore, genre features of the selected texts reveal that differences in perspectives and stance in constructing such texts are mainly attributes of the socio-cultural distinction’s peculiar to different dominant cultures. For example, texts from the European cultures highlight the deceased’s professional achievements then services to the wider community; while emails from the Asian cultures construe interpersonal relationships in the orientation stage then append personal attributes of the deceased followed by the professional services rendered to a wider SFL community. The classified data is obtained from the sys-func and sysfling mailing list archives and has been anonymized to secrete identity.

Objectives Pediatric cervical spine injuries are rare but potentially life threatening. Although published guidelines for assessment of such injuries exist, there is less uniformity in its implementation in out-of-hospital settings. Our purpose was to assess the knowledge and practice patterns for pediatric cervical spine immobilization among prehospital emergency medical services (EMS) providers in Arizona. Methods A cross-sectional web-based survey was conducted (October-December 2018), using an electronic mailing list of certified EMS providers (ground and air) in Arizona. A 20-question structured web-based survey was developed and deployed. Results One hundred eight EMS stations were contacted with the survey. Sixty-eight providers responded; majority were emergency medical paramedics (73.1%). Most of the stations surveyed did not have a pediatric trauma center (66.2%). When treating children younger than 3 years, half of the respondents stated they did not know of a specific cervical spine clearance criterion; 59.3% felt that cervical spine immobilization was required "sometimes," and 40.0% were unaware of the state's EMS pediatric cervical spine clearance algorithm; 93.9% of EMS providers felt that an age-based algorithm for cervical spine clearance in children would be useful. Conclusions In this statewide survey involving prehospital EMS providers, we found that pediatric cervical spine clearance and immobilization practices, even within a specific geographic location, remain inconsistent.
