A rise in fungal infections has been observed worldwide among patients with extended hospital stays because of the severe infection caused by the new coronavirus pandemic. A 62-year-old female patient was admitted with a severe form of Coronavirus disease 2019 (COVID-19) and spent four weeks in the intensive care unit (ICU) requiring mechanical ventilation support before being moved to a tertiary hospital for further testing. Aspergillus fumigatus filamentous fungus, Candida spp., and positive bacteriology for multidrug-resistant Klebsiella pneumoniae and Proteus mirabilis were identified by bronchial aspirate cultures. The patient's progress was gradually encouraging while receiving oral antifungal and broad-spectrum antibiotic therapy along with respiratory physical therapy; but ultimately, thoracic surgery was necessary. Long-lasting tissue damage and severe, persistent inflammatory syndrome were the two main pathophysiological mechanisms that led to significant outcomes regarding lung lesions that were rapidly colonized by fungi and resistant flora, cardiac damage with sinus tachycardia at the slightest effort, and chronic inflammatory syndrome, which was characterized by marked asthenia, myalgias, and exercise intolerance. This study assesses the effectiveness of such an approach in locating TB cases among vulnerable groups, such as homeless persons, injecting drug users, those detained in prison, and people living in rural areas. Methods: The study focuses on socio-economic characteristics and TB detection rates across Romanian counties using modern techniques including computer-aided detection of lesions on chest X-ray and GeneXpert tests. Results: The results highlight the disproportionate burden of TB in vulnerable groups, by revealing significant differences in TB detection rates between regions. Notably, the TB detection rates among these vulnerable groups (250.85 per 100,000 population) are five times higher than the national incidence rate (46.1). Discussion: These findings underscore the imperative integration of ACF into National TB Program to provide customized and efficient solutions for diverse vulnerable groups, thereby informing crucial public health initiatives and interventions. Pulmonary tuberculosis (TB) still represents a chalenge for the healthcare systems, in the active but also in the chronic phase, when TB sequelae may appear. Anatomical changes and destruction of lung parenchyma lead to loss of pulmonary volumes. The patients with post TB sequelae are expresing dyspnea, chronic cough, muscle weakness and reduced daily activity. We present the case of a 58 years old patient, with post TB sequelae (2 episodes of relapses), presenting dyspnea, chronic fatigue and bronchiectatic syndrome. He was included in a 8 weeks multidiciplinary pulmonary rehabilitation program, which comprised exercise trainng, airway clearance techniques, education. The results of PR program were positives: decrease of dyspnea (one point in mMRC scale), improvement of exercise tolerance (58 m gain at 6MWT), improvement in life quality (SGRQ decrease with 5.2 points). In conclusion, pulmonary rehabilitation may be a useful therapy in the management of post tuberculosis sequelae.

Background and objectives: Intuitive nutrition represents a disputed topic when subjects about weight loss and diets are approached worldwide. The current study aims to emphasize the causal relationship between emotional status and intuitive eating, starting from the hypothesis that a nutritional imbalance can be determined by an emotional imbalance. Materials and Methods: This prospective study included 73 subjects, aged between 18-70, with a sex ratio of 21:52 male to female and it was designed to use digital methods to collect data from the sample of subjects through online questionnaires, collection software and automatic statistical analysis of data. The collected data was analysed in parallel using the Minitab software and Office Excel. Results: Data processing confirmed the working hypothesis, namely that emotional status influences the way of eating and intuitive eating can contribute to maintaining emotional balance. Also, the statistical analysis show Mean=24.33, SD= 14.46, a Median of 17 and a level of Confidence of 35.94%. Conclusions: Intuitive nutrition can be a helpful factor in maintaining emotional balance directly related to nutrition. Disturbance of emotional balance facilitates the adoption of a way of nutrition that can be classified under the category of eating disorders, attracting a procession of somatic and psychological consequences. Background: Tuberculosis (TB) is the leading infectious cause of mortality worldwide. In the last years, resistant strains of the etiological agent, Mycobacterium tuberculosis, have emerged, thus demanding more triage tests to identify active pulmonary TB (PTB) patients and to evaluate their disease severity. Therefore, acute-phase reaction serum tests are required for monitoring TB patients, among WHO symptom screening recommendations. C-reactive protein (CRP) is a non-specific inflammatory biomarker that has been recently proposed for TB screening and can be quantitatively analyzed through cost-effective point-of-care assays. A previous meta-analysis found CRP to be highly sensitive and moderately specific for active PTB with confirmed HIV infection. Methods: We performed a meta-analysis update of diagnostic tests, pooling sensitivities, and specificities in order to assess the accuracy of CRP as a potential test for the screening of HIV-associated PTB in outpatients. We searched MEDLINE, Web of Science, and SCOPUS for eligible articles before 19 October 2021. Results: We identified 13 eligible studies with HIV-positive patients with PTB. At a CRP threshold of 10 mg/L, CRP pooled sensitivity was 87% (76%-93%) and pooled specificity was 67% (49%-81%), with an area under the curve (AUC) of 0.858. Using a CRP threshold of 8 mg/L, pooled sensitivity was 82% (72%-89%) and pooled specificity was 82% (67%-92%), with an AUC of 0.879. We found that CRP has a high sensitivity in the screening of PTB in HIV-positive outpatients, consistent with findings reported previously. Conclusions: Regardless of pooled specificity, better results were found using the CRP threshold of 8 mg/L as a test screening of PTB, meeting the need of further approaching specific TB diagnostic methods and reducing resource consumption.
