Abstract

Internet of Things (IoT) devices are proliferating in the agricultural sector as more and more farmers depend on these tools to manage their operations. From completely autonomous milking parlors to crop management and greenhouse automation technology is being used in an unprecedented variety of ways to improve productivity. The advantages are enormous, allowing farms to grow to accommodate more resources (such as water) and less need for human labor; in fact, in some areas, it is now economically impossible for farmers to return to more conventional methods of production. Regrettably, the risk of cyber assaults has also increased alongside technological advancements. The safety of the food supply as a whole and specific farm may be jeopardized if any one of them were to be successful. Collars used to track cow health in an intelligent dairy farm are thoroughly examined in this study. To our knowledge, this is the first study of its kind to focus on the cyber defences of actual farm equipment. We were able to effectively decipher the wireless protocol and show that we could trick the system into thinking we were one of the sensors by injecting bogus data. According to the results of the tests, the data endpoint software and the system that receives signals from the sensors are both susceptible to data injection. In this study, we focus on the vulnerabilities that pose the greatest risk and propose some solutions that could be built into sensors down the road. [ABSTRACT FROM AUTHOR]

Abstract

An emerging paradigm known as "fog computing" improves real-time data processing and analysis by bringing cloud capabilities to the edge of the network. An adaptive scheduling technique is the centre of this study's optimization efforts for fog computing systems. Depending on the level of protection needed, it sorts received tasks into three categories: public, semi-private, and private. Then, it assigns each category to a certain Cloud Data Center (CDC) or Mobile Data Center (MDCs). To guarantee the best possible allocation of tasks, the algorithm considers a number of criteria, such as the quantity of jobs, the capacities of MDCs and CDCs, and communication delays. Examining the efficacy of three scheduling algorithms in homogeneous as well as hetero-geneous fog conditions sheds light on distribution of work, system efficacy and resource management. In order to optimise resource consumption and meet the varied needs of Internet of Things (IoT) applications, this study offers helpful recommendations for designing and operating fog computing infrastructures. 15. Drug Utilization Pattern of Proton Pump Inhibitors in the Department of General Medicine

Abstract

Objectives: In current clinical practice, utilization of Proton Pump Inhibitors (PPIs) has increased enormously as OTC drug as well as the commonly prescribed drug in majority of the prescriptions. PPIs are predominantly indicated in prescriptions for the purpose of prophylaxis of antibiotics and analgesics. The study aimed to assess the prescribing pattern of PPIs in the Department of General Medicine of a tertiary care hospital. Methods: This is a prospective observational study conducted in the Department of General Medicine for a period of 1 year from August 2017 to July 2018 to analyse the prescribing pattern of PPIs. The medication charts of both inpatients and outpatients of General Medicine department was reviewed. Core information such as demographic characteristics, diagnosis, and current medications were recorded. The patients were regularly monitored for drug – drug interactions. Results: Of 1019 prescriptions, 908 (89%) prescriptions were found to have PPIs. Pantoprazole 749 (82.49%) was the most commonly prescribed PPI followed by esomeprazole 124 (13.66%). The indications for PPI therapy were acute gastroenteritis 103 (11.34%), ulcers 7 (0.77%), gastro esophageal reflux disease 5 (0.55%), gastrointestinal bleed 3 (0.33%), ulcerative colitis 2 (0.22%), inflammatory bowel disease 1(0.11%), esophageal and gastric injury 1(0.11%) and gastric erosion 1 (0.11%). PPIs were found to be prescribed with antibiotics 681 (75%) followed by non-steroidal anti-inflammatory drugs 179 (19.7%). Conclusions: The study showed inappropriateness in prescribing patterns of PPIs leading to increased therapeutic burden and treatment cost. This necessitates formulation of standard therapeutic guidelines for rationalizing the utilization of PPIs.

19. Prescribing Pattern of Antimicrobials in the Department of General Medicine

Abstract

Background: Antibiotics are extensively used for the treatment of minor, moderate both life threatening and minor infections. Irrational use of antibiotics increases the risk of bacterial resistance. Hence, the present study was aimed to assess the most commonly prescribed antibiotics in the Department of General Medicine in a tertiary care hospital. Methodology: A prospective observational study was carried out at the Department of General Medicine, M.S. Ramaiah Medical Teaching hospital, Bangalore, India over a period of five months from January 2015 to May 2015. Patient’s case sheets, treatment charts and laboratory investigation were used as the source of data. Results: The mean age of the study population and duration of hospitalization was 38.54±12.27 and 6.15±2.56 days respectively and the mean number of antimicrobials per prescription was 1.44±0.74. The total number of antibiotics prescribed was 197. The most commonly prescribed antibiotic classes were cephalosporin (62.43%) among which ceftriaxone was the most commonly prescribed drug. Conclusions: As antibiotics are the most commonly prescribed drugs therefore development of antibiotic use policy is very essential not only to prevent antibiotic resistance but also to reduce the treatment expenditure.

Abstract

Background: Long term administration of antiepileptic drug phenytoin is reported to cause behavioral abnormalities mediated via oxidative stress. The effect of an antioxidant alpha lipoic acid (ALA) against phenytoin induced behavioral abnormalities was investigated. Methods: The study was carried out in albino wistar rats. The rats were divided into five groups of six animals each. Group 1 received 0.2% carboxy methyl cellulose (CMC, p.o), group 2 received 20 mg/kg phenytoin (p.o), groups 3,4 and 5 received 50, 100 and 200 mg/kg (p.o) of ALA in 0.2% CMC, respectively 1 h prior to phenytoin for 45 days. Motor coordination, exploratory behavior, memory and spontaneous motor activity were evaluated by Rota rod, Hole board, Elevated plus maze and Actophotometer respectively. On day 45, regional brain lipid peroxidation and acetylcholinesterase (ACh E) activity along with brain histopathological investigation were performed after euthanasia. In addition, pharmacokinetic and pharmacodynamic drug interactions between phenytoin and ALA were also studied. Results: Long term administration of phenytoin showed behavioral abnormalities, increased regional brain malondialdehyde (MDA) and ACh E activity. The histopathological investigation showed congested and damaged cells in brain regions. ALA substantially reversed phenytoin induced behavioral abnormalities, oxidative stress and alleviated the histopathological abnormalities. There were no significant differences in the serum levels of phenytoin and the degree of protection offered by phenytoin in ALA supplemented groups revealing that there were no pharmacokinetic and pharmacodynamic interactions between phenytoin and ALA. Conclusion: This study reports the effectiveness of ALA against phenytoin induced behavioral abnormalities and oxidative stress in rats without altering the bioavailability of phenytoin and its therapeutic effect.
