2. Akutna kardiovaskularna stanja u povezanosti s praćenjem nogometnih utakmica

Nogomet i Fédération Internationale de Football Association (FIFA) Svjetsko nogometno prvenstvo predstavljaju jedan od najraširenijih društvenih fenomena u svijetu. Utakmice nacionalnih vrsta na prvenstvu praćene su velikim emocionalnim uključivanjem navijača. Poznato je kako emocionalni stres može biti okidač neželjenih kardiovaskularnih zbivanja. Cilj ovog retrospektivnog istraživanja bio je utvrditi da li je na objedinjenom bolničkom hitnom prijemu i Klinici za bolesti srca i krvnih žila Kliničkog bolničkog centra Sestre milosrdnice (KBC SM) došlo do povećanog broja prijama osoba sa kardiovaskularnim bolestima (KVB) za vrijeme praćenja utakmica hrvatske reprezentacije na Svjetskom nogometnom prvenstvu 2018. godine. Pregledana je baza podataka KBCa SM za datume dana utakmice te dva dana nakon u 2018. godini. Kao neizložena skupina uzeti su isti datumi u 2017. i 2019. godini. Izdvojena su 1093 slučaja, od toga 398 prijama u izloženom periodu. Nađeno je statistički značajno povećanje broja slučajeva koje je 1,15 (95% interval pouzdanosti[IP]; 1,02-1,31) puta veće u odnosu na neizloženi period. Kada se pogledaju omjeri odvojeno za muški i za ženski spol, nije nađeno statistički značajno povećanje za muški spol, dok je omjer za ženski spol 1,30 (95% IP; 1,1 – 1,54) značajan. Aritmije i angina pektoris su KV bolesti koje su se značajno češće javljale u periodu prvenstva. Istraživanje je pokazalo kako je praćenje utakmica hrvatske nogometne reprezentacije predstavljalo dodatni rizik za KV incident, prvenstveno u osoba ženskoga spola., FIFA World Cup represents one of the world’s greatest phenomena. The spectators watch the matches of national teams with great emotional involvement. It is well documented fact that emotional stress can be a trigger of unwanted cardiovascular (CV) event. The aim of this retrospective study was to determine whether there had been an increase in the number of the emergency admissions for CVD in the Emergency Room and Clinic for Cardiovascular Diseases of the Sestre milosrdnice University Hospital Centre during and after the matches that the Croatian national team played in the FIFA World Cup 2018. The hospital’s database was examined for the dates when Croatia played its matches, plus two more days after each match. A non-exposed period that included the same dates in 2017 and 2019 was formed. 1093 cases were assessed. The incidence of CV admissions during the exposed period was 1.15 (95% confidence interval [CI]; 1.02 to 1.31) times higher than during the non-exposed period. There was a 1.30 (95% CI; 1.1 to 1.54) times higher incidence in women compared to the non-exposed period. Arrhythmias and angina pectoris were the CVDs that occurred more frequently in the exposed period. This study showed that watching Croatian national team’s matches and cheering represented an additional risk for a CV incident, especially in women.

Cilj ovog retrospektivnog istraživanja bio je utvrditi da li je na objedinjenom bolničkom hitnom prijemu (OBHP) i Klinici za bolesti srca i krvnih žila Kliničkog bolničkog centra Sestre milosrdnice (KBC SM) došlo do povećanog broja prijama osoba sa KV bolestima (KVB) za vrijeme praćenja utakmica hrvatske reprezentacije na Svjetskom nogometnom prvenstvu 2018. godine te utvrditi druge parametre povezane s istim. Pregledana je baza podataka KBCa SM za datume dana utakmice te dva dana nakon u 2018. godini. Kao kontrolna skupina uzeti su isti datumi u 2017. i 2019. godini. Izdvojena su 1093 slučaja, od toga 398 prijama u ciljanom periodu. Nađeno je statistički značajno povećanje broja slučajeva koje je 1,15 (95% interval pouzdanosti[IP]; 1,02-1,31) puta veće u odnosu na kontrolni period. Kada se pogledaju omjeri odvojeno za muški i za ženski spol, nije nađeno statistički značajno povećanje za muški spol, dok je omjer za ženski spol 1,30 (95% IP; 1,1 – 1,54). Aritmije i angina pektoris su KV bolesti koje su se značajno češće javljale u ciljanom periodu, a nije nađena razlika između ciljanog i kontrolnog perioda što se tiče ishoda bolesti ili KV rizičnog profila. Time je pokazano kako je praćenje utakmica hrvatske nogometne reprezentacije predstavljalo dodatni rizik za KV incident, prvenstveno u osoba ženskoga spola., Soccer and Fédération Internationale de Football Association (FIFA) World Cup represent one of the world's biggest phenomena. Matches between national selections are viewed with great emotional involvement by the spectators. It is well documented fact that emotional stress can be a trigger of unwanted cardiovascular (CV) event. The aim of this retrospective study was to determine whether there has been an increase in the number of the emergency admissions for CV disease (CVD) in Emergency Room and Clinic for Cardiovascular Diseases in Clinical Hospital Centre Sisters of Charity during and after the period when Croatian national team played its matches on FIFA World Cup 2018 and to determine other parameters linked with that. The hospital's data base has been researched for the dates when Croatia played its matches and for the two following days in 2018. A control period was formed that included the same dates in 2017 and 2019. 1093 cases were assessed, out of which 398 were during the period of the World Cup. The incidence of CV admissions during that period was 1.15 (95% confidence interval [CI]; 1.02 to 1.31) times than during the control period. In this investigation was not observed statistically relevant increase for men, while for women there was a 1.30 (95% CI; 1.1 to 1.54) times higher incidence compared to the control period. Arrhythmias and angina pectoris are CV diseases that occurred significantly more often in the target period, and no difference was found between the target and control period in terms of disease outcome or CV risk profile. This study has shown that following Croatian national team's matches represented an additional risk for CV incident, especially for women.

Gotovo da i ne postoji osoba koja nije čula za prestižnu Nobelovu nagradu. Ta vrijedna nagrada koja se dodjeljuje još od 1901. godine, možda je i najveće priznanje koje jedan znanstvenik može primiti za svoj rad i doprinos boljitku čovječanstva. No unazad nekoliko godina, na hladnom, ali naprednom te znanstveno osviještenom sjeveru Europe dodjeljuje se još jedna prestižna nagrada. Nagrada Kavli je relativno novo priznanje u svijetu znanosti. Ta se vrijedna nagrada od 2008. godine dodjeljuje svake dvije godine. Nagradu, koja je ime dobila po Fredu Kavliju, dodjeljuju Norveška akademija znanosti i književnosti, Zaklada Kavli te Ministarstvo obrazovanja i istraživanja Kraljevine Norveške. Cilj toga priznanja je prepoznavanje izvanrednih i revolucionarnih otkrića u tri zasebna područja znanosti: astrofizici, nanotehnologiji te neuroznanosti. Dobitnike biraju odbori sastavljeni od članova koje predlaže šest najprestižnijih svjetskih znanstvenih društava i akademija (Kineska akademija znanosti, Francuska akademija znanosti, Institut Max Planck, Nacionalna akademija znanosti SAD, Norveška akademija znanosti i književnosti te Kraljevsko društvo UK), a odbore potvrđuje Norveška akademija znanosti i književnosti. Uz zlatnu medalju laureatima se uručuje i nagrada u iznosu od milijun američkih dolara. Ceremonija dodjele nagrade održava se u norveškoj prijestolnici Oslu, a domaćin je norveški kralj Harald.

8. Out-of-Hospital Cardiac Arrest in the Eye of the Beholder and Emergency Medical Service.

- Abstract
Purpose: Out-of-hospital cardiac arrest (OHCA) remains a global healthcare problem, with low survival and bystander cardiopulmonary resuscitation (CPR) rates. This study aimed to identify event-related factors in OHCA and their impact on return of spontaneous circulation (ROSC) achievement and maintenance until hospital admission., Patients and Methods: All data were collected from Utstein Resuscitation Registry Template for OHCA from The Institute of Emergency Medicine of Zagreb from January 2012 to August 2022. This cross-sectional research analyzed 2839 Utstein reports, including 2001 male, 836 female, and 8 subjects of unknown gender. The average age was 65.4 ± 16.2 years., Results: The most frequent place of collapse was private residence, and 27% of collapses were unwitnessed. Dispatcher-provided CPR instructions were provided in 39.7% of cases until the arrival of the emergency service team, which showed a very strong effect on bystander-provided CPR, and were followed in 68.4% of cases, while non-instructed bystander CPR was provided in only 7.9% of cases. Bystander CPR is more likely to be provided in public places than in private residences, often with both compression and ventilation. Bystander CPR was also more likely to be provided to men. Cases with bystander CPR, and compressions with ventilation compared to compression only CPR, showed a significantly greater success in maintaining ROSC later in CPR, both with moderate effects., Conclusion: Bystander CPR has been shown to have a significant role in achieving and maintaining ROSC until hospital admission. However, our results showed a location-dependent nature of bystanders' willingness to perform CPR as well as sex disparities in patients receiving CPR.
