Abstract

IntroductionWe evaluated the impact of a 'Team-Based Goals and Incentives' (TBGI) intervention in Bihar, India, designed to improve front-line (community health) worker (FLW) performance and health-promoting behaviours related to reproductive, maternal, newborn and child health and nutrition.MethodsThis study used a cluster randomised controlled trial design and difference-in-difference analyses of improvements in maternal health-related behaviours related to the intervention's team-based goals (primary), and interactions of FLWs with each other and with maternal beneficiaries (secondary). Evaluation participants included approximately 1300 FLWs and 3600 mothers at baseline (May to June 2012) and after 2.5 years of implementation (November to December 2014) who had delivered an infant in the previous year.ResultsThe TBGI intervention resulted in significant increases in the frequency of antenatal home visits (15 absolute percentage points (PP), p=0.03) and receipt of iron-folic acid (IFA) tablets (7 PP, p=0.02), but non-significant changes in other health behaviours related to the trial's goals. Improvements were seen in selected attitudes related to coordination and teamwork among FLWs, and in the provision of advice to beneficiaries (ranging from 8 to 14 PP) related to IFA, cord care, breast feeding, complementary feeding and family planning.ConclusionResults suggest that combining an integrated set of team-based coverage goals and targets, small non-cash incentives for teams who meet targets and team building to motivate FLWs resulted in improvements in FLW coordination and teamwork, and in the quality and quantity of FLW-beneficiary interactions. These improvements represent programmatically meaningful steps towards improving health behaviours and outcomes.Trial registration numberNCT03406221.

Abstract

NiTi Shape Memory Alloy (SMA) finds wide applications in high-performance engineering industries such as medical devices, micro-electrical and electronic components, aerospace, aircraft structure, and so on, to name a few. This is owing to its superior properties like shape memory effect (SME), superelasticity, and biocompatibility. However, getting a proper joining of NiTi alloys is hard considering its unique characteristics. Therefore, joining this alloy has been a domain of great attention for scientists. The current research work investigates the weldability of NiTi wires using Laser spot welding. The microhardness and the tensile strength of the weld joint are acquired according to Taguchi's design of experiment. The significance of the control factors (laser power, scan speed, and focal length) is identified through ANOVA, and an ideal factor setting for optimum individual responses is established. Moreover, simultaneous optimisation of multi-performance characteristics is also attempted through Grey Relational Analysis (GRA) method. From the GRA, 300 W laser power, 1 mm/sec scan speed, and 0.5 mm focal position are found to be giving optimum results for both the responses. The result from the confirmation runs showed that the predicted optimal welding parameters improve the individual as well as the multiple performance characteristics. [ABSTRACT FROM AUTHOR]

Abstract

Background Visceral leishmaniasis (VL) has been targeted by the WHO for elimination as a public health problem (< 1 case/10,000 people/year) in the Indian sub-continent (ISC) by 2020. Bihar State in India, which accounts for the majority of cases in the ISC, remains a major target for this elimination effort. However, there is considerable spatial, temporal and sub-population variation in occurrence of the disease and the pathway to care, which is largely unexplored and a threat to achieving the target. Methods Data from 6081 suspected VL patients who reported being clinically diagnosed during 2012–2013 across eight districts in Bihar were analysed. Graphical comparisons and Chi-square tests were used to determine differences in the burden of identified cases by season, district, age and sex. Log-linear regression models were fitted to onset (of symptoms)-to-diagnosis and onset-to-treatment waiting times to estimate their associations with age, sex, district and various socio-economic factors (SEFs). Logistic regression models were used to identify factors associated with mortality. Results Comparisons of VL caseloads suggested an annual cycle peaking in January-March. A 17-fold variation in the burden of identified cases across districts and under-representation of young children (0–5 years) relative to age-specific populations in Bihar were observed. 35. Magnetohydrodynamic viscous flow separation in a channel with constrictions

Abstract

An analysis is made of the flow of an electrically conducting fluid in a channel with constrictions in the presence of a uniform transverse magnetic field. A solution technique for governing magnetohydrodynamic (MHD) equations in primitive variable formulation is developed. A coordinate stretching is used to map the long irregular geometry into a finite computational domain. The governing equations are discretized using finite difference approximations and the well-known staggered grid of Harlow and Welch is used. Pressure Poisson equation and pressure-velocity correction formulas are derived and solved numerically. It is found that the flow separates downstream of the constriction. With increase in the magnetic field, the flow separation zone diminishes in size and for large magnetic field, the separation zone disappears completely. Wall shear stress increases with increase in the magnetic field strength. It is also found that for symmetrically situated constrictions on the channel walls, the critical Reynolds number for the flow bifurcation (i.e., flow asymmetry) increases with increase in the magnetic field. Abstract

Inborn errors of metabolism belong to heterogeneous group of disorders which cause a number of morbidities and mortality in pediatric population and come under the class of genetic rare diseases. With the advent of newer molecular tools and techniques, so for several hundreds of disorders have been defined after the first description by Garrod in the 20th century. Early and timely diagnosis of the disease may prevent the life of a patient, but there are many reasons persist, restricting the timely diagnosis of the disease.
