Abstract

We used survival analysis to quantify the impact of postdischarge evaluation and management (E/M) services in preventing hospital readmission or death. Our approach avoids a specific pitfall of applying machine learning to this problem, which is an inflated estimate of the effect of interventions, due to survivors bias -- where the magnitude of inflation may be conditional on heterogeneous confounders in the population. This bias arises simply because in order to receive an intervention after discharge, a person must not have been readmitted in the intervening period. After deriving an expression for this phantom effect, we controlled for this and other biases within an inherently interpretable Bayesian survival framework. Abstract

We developed an inherently interpretable multilevel Bayesian framework for representing variation in regression coefficients that mimics the piecewise linearity of ReLU-activated deep neural networks. We used the framework to formulate a survival model for using medical claims to predict hospital readmission and death that focuses on discharge placement, adjusting for confounding in estimating causal local average treatment effects. We trained the model on a 5% sample of Medicare beneficiaries from 2008 and 2011, based on their 2009--2011 inpatient episodes, and then tested the model on 2012 episodes. The model scored an AUROC of approximately 0.76 on predicting all-cause readmissions -- defined using official Centers for Medicare and Medicaid Services (CMS) methodology -- or death within 30-days of discharge, being competitive against XGBoost and a Bayesian deep neural network, demonstrating that one need-not sacrifice interpretability for accuracy. Crucially, as a regression model, we provide what blackboxes cannot -- the exact gold-standard global interpretation of the model, identifying relative risk factors and quantifying the effect of discharge placement. Abstract

SUMO-1, a novel peptide of 101 amino acids, has been found to be covalently linked to the mammalian Ran GTPase-activating protein RanGAP1 in an adenosine triphosphate-dependent reaction. SUMO-1 exhibits a homology to ubiquitin, a 76-residue protein found in eukaryotes. Data indicate that the polypeptide is involved in protein targeting and not degradation. At least three additional intranuclear proteins are modified with SUMO-1 or with a protein closely resembling it.

Abstract

Regulated assembly of myosin II in Dictyostelium discoideum amoebae partially regulates the orderly formation of contractile structures during cytokinesis and cell migration. Kinetic and structural studies indicate that Dictyostelium myosin II assembles by a sequential process of slow nucleation and controlled growth that differs in rate and mechanism from other conventional myosins. The unique assembly mechanism and regulation used by Dictyostelium myosin II illustrates that myosins from smooth muscle, striated muscle and two types of amoebae form novel thick filaments through various pathways.

Abstract

The new coronavirus COVID-19 disease was declared a global public health emergency by the World Health Organization on January 2020. In the current dismal situation of the COVID-19 pandemic, effective management of patients with pneumonia and acute respiratory distress syndrome is of utmost importance. Due to the current lack of effective pharmacological concepts, this situation has caused interest in re-considering historical reports on the treatment of patients with low-dose radiation therapy for pneumonia. Although these historical reports are of low-level evidence per se, hampering recommendations for decision-making in the clinical setting, they indicate effectiveness in the dose range between 0.3 and 1 Gy, similar to more recent dose concepts in the treatment of acute and chronic inflammatory/degenerative benign diseases with, for example, a single dose per fraction of 0.5 Gy. Thus, we review the effects and mechanism and highlight the evidence for low-dose radiation that may be viable and useful in counteracting the acute inflammatory state induced by critical stage COVID-19 in the treatment of COVID-19 pneumopathy. Abstract

Incorporating evaluation metrics with GUI development tools will help designers create consistent interfaces in the future. Complexity in design of interfaces makes efficient evaluation impossible by a single consistency checking evaluation tool. Our f ocus is on developing a family of evaluation tools in order to make the evaluation process less cumbersome. We have developed a dialog box typeface and color table to facilitate detection of anomalies in color, font, font size, and font style. Concordance tools have been developed to spot variant capitalization and abbreviations globally in the interface and specifically in the button widgets. As buttons are frequently used widgets, a button layout table has been created to spot any inconsistencies in height, width and relative position between a given group of buttons if present. Finally, a terminology basket tool has been created to identify unwanted synonyms of computer related terms used in the interface which may be misleading to the end user. SHERLOCK, a family of consistency checking tools, has been designed to evaluate visual design and textual pro perties of interface, make the GUI evaluation process less arduous, and aid usability testing. SHERLOCK includes a dialog box summary table to pro vide a compact overview of visual properties of hundreds of dialog boxes of the interface. Terminology specific tools, like Interface Concordance, Terminology Baskets and Interface Speller have been developed. Button specific tools including Button Conco rdance and Button Layout Table have been created to detect variant capitalization, distinct typefaces, distinct colors, variant button sizes and inconsistent button placements. This paper describes the design, software architecture, and the use of SHERLOC K. An experiment with 60 subjects to study the effects of inconsistent interface terminology on user's performance showed 10-25% speedup for consistent interfaces.
