2. Genetic diversities and drug resistance in Mycobacterium bovis isolates from zoonotic tuberculosis using whole genome sequencing

Abstract Background Zoonotic human tuberculosis (TB) caused by Mycobacterium bovis (M. bovis) is as vital as Mycobacterium tuberculosis, however with scarce available information. We aimed to use whole-genome sequencing (WGS) technology to take a deep insight into the circulating genotypes of human M. bovis and the genomic characteristics underlying virulence and drug resistance. Methods The study included smear positive Ziehl-Neelsen samples from patients with suspected tuberculosis. Samples were cultured on Lowenstein-Jensen media and suspected colonies of M. bovis were selected to undergo DNA extraction and WGS. Data was analysed using the Bacterial and Viral Bioinformatics Resource Center (BV-BRC), and online bioinformatics tools. A phylogenetic tree was constructed for our sequenced strains, in addition to a set of 59 previously sequenced M. bovis genomes from different hosts and countries. Results Out of total 112 mycobacterial positive cultures, five M. bovis were isolated and underwent WGS. All sequenced strains belonged to Mycobacterium tuberculosis var bovis, spoligotype BOV_1; BOV_11. Resistance gene mutations were determined in 100% of strains to pyrazinamide (pncA and rpsA), isoniazid (KatG and ahpC), ethambutol (embB, embC, embR and ubiA), streptomycin (rpsl) and fluoroquinolones (gyrA and gyrB). Rifampin (rpoB and rpoC) and delamanid (fbiC) resistance genes were found in 80% of strains. The major represented virulence classes were the secretion system, cell surface components and regulation system. The phylogenetic analysis revealed close genetic relatedness of three sequenced M. bovis strains to previous reported cow strains from Egypt and human strains from France, as well as relatedness of one M. bovis strain to four human Algerian strains. One sequenced strain was related to one cow strain from Egypt and a human strain from South Africa. Conclusions All sequenced M. bovis isolates showed the same spoligotype, but diverse phylogeny. Resistance gene mutations were detected for anti-TB drugs including pyrazinamide, isoniazid, streptomycin, ethambutol, fluoroquinolones, cycloserine, rifampin and delamanid. The virulence profile comprised genes assigned mainly to secretion system, cell surface components and regulation system. Phylogenetic analysis revealed genetic relatedness between our isolates and previously sequenced bovine strains from Egypt as well as human strains from other nearby countries in the region.

Abstract Colorectal cancer (CRC) is the third most common cancer affecting people. The discovery of new, non-invasive, specific, and sensitive molecular biomarkers for CRC may assist in the diagnosis and support therapeutic decision making. Exosomal miRNAs have been demonstrated in carcinogenesis and CRC development, which makes these miRNAs strong biomarkers for CRC. Deep sequencing allows a robust high-throughput informatics investigation of the types and abundance of exosomal miRNAs. Thus, exosomal miRNAs can be efficiently examined as diagnostic biomarkers for disease screening. In the present study, a number of 660 mature miRNAs were detected in patients diagnosed with CRC at different stages. Of which, 29 miRNAs were differentially expressed in CRC patients compared with healthy controls. Twenty-nine miRNAs with high abundance levels were further selected for subsequent analysis. These miRNAs were either highly up-regulated (e.g., let-7a-5p, let-7c-5p, let-7f-5p, let-7d-3p, miR-423-5p, miR-3184-5p, and miR-584) or down-regulated (e.g., miR-30a-5p, miR-99-5p, miR-150-5p, miR-26-5p and miR-204-5p). These miRNAs influence critical genes in CRC, leading to either tumor growth or suppression. Most of the reported diagnostic exosomal miRNAs were shown to be circulating in blood serum. The latter is a novel miRNA that was found in exosomal profile of blood serum. Some of the predicted target genes of highly expressed miRNAs participate in several cancer pathways, including CRC pathway. These target genes include tumor suppressor genes, oncogenes and DNA repair genes. Main focus was given to multiple critical signaling cross-talking pathways including transforming growth factor β (TGFβ) signaling pathways that are directly linked to CRC. In conclusion, we recommend further analysis in order to experimentally confirm exact relationships between selected differentially expressed miRNAs and their predicted target genes and downstream functional consequences.

Background: Stroke presents as the second most prominent factor contributing to global mortality. Immediate brain imaging can be valuable for assessing functional recovery potential. This study investigated the association between Alberta Stroke Program Early CT Score (ASPECTS) and functional outcomes in patients with left and right-hemisphere strokes. Methods: A prospective cohort study was conducted in July-Dec2 022, at a tertiary care hospital in Karachi including patients of either gender presenting within 2 days of stroke while excluding posterior circulation strokes, TIA & unwilling patients using a non-probability consecutive sampling technique. A total of 152 patients with acute ischemic stroke involving anterior circulation were analyzed and patients were categorized into two groups: the left hemisphere group (n=76) and the right hemisphere group (n=76) accordingly. ASPECTS scores were calculated from brain CT scans, while functional outcomes were measured using the modified Rankin Scale (mRS) at the three-month mark. Descriptive analysis and chi-square test were applied using SPSS vr25. Results: Patients (n=152) had a mean age of 61.75 ± 13 years, with males comprising 67% of the cohort. ASPECTS scores were notably higher in left hemisphere strokes (median 9, IQR 2) than right hemisphere strokes (median 8, IQR 3) (p=0.036). Higher ASPECTS scores (≥7) correlated with improved outcomes (mRS ≤2) in both hemispheres. There was no statistically significant difference in both groups’ functional outcomes (p=0.182). Conclusion: ASPECTS predicts functional outcomes in acute ischemic strokes equally well regardless of the affected hemisphere.

Dempster–Shafer Theory (DST) relies significantly on belief and plausibility measures to handle ambiguity and uncertainty; however, DST has been extended to fuzzy sets (FSs) and intuitionistic fuzzy sets (IFSs) with only a few extensions focusing on belief and plausibility intuitionistic fuzzy distance (BP-distance) and similarity (BP-similarity) until now. In this work, we propose a novel framework for the belief and plausibility of intuitionistic fuzzy sets (BP-IFSs) and their BP-distance and BP-similarity measures. We modified steps 4 and 5 of the classical TOPSIS method, utilizing both distance and similarity measures to rank the alternatives that satisfy all necessary axioms of distance and similarity. We present numerical examples involving pattern recognition, linguistic variables, and clustering to illustrate the efficiency of these measures, and we develop belief and plausibility TOPSIS (BP-TOPSIS) using the proposed criteria and apply it to complex multicriteria decision-making (MCDM) challenges. The results demonstrate the practicality and effectiveness of our approach.

BackgroundCOVID-19 has probably contributed to sleep disturbance among nurses, especially those working at emergency departments (EDs). Sleep disorders in nurse managers can negatively impact their health and impair work performance and decision-making. This study aimed to explore the quality of sleep among nurses working in EDs and its influencing factors during the COVID-19 pandemic.MethodIn this study, a cross-sectional design was employed to assess the sleep quality of nurses working in EDs during the COVID-19 pandemic. The research recruited a convenience sample of emergency nurses, who were selectively sourced from four hospitals in Hail City. This recruitment occurred over the period from April to July 2022. Descriptive data analysis was conducted using SPSS, with the significance level set at 0.05.ResultsAmong the 216 participants in the study, the majority (55.6%) were aged between 30 and 39 years, and 73.6% were female. Additionally, 64.4% were married, while 69.4% had a bachelor’s degree, 20% held a diploma, and the remaining had a master’s degree. Notably, a significant 81.5% of the nurses reported poor sleep quality, as assessed by the Pittsburgh Sleep Quality Index (PSQI), with an overall mean score of 10.55 indicating poor sleep. The study highlighted that poor sleep quality among nurses was linked to being female, being married, and not exercising regularly. Better sleep was associated with nurses who manage fewer patients per shift and have adequate monthly income.ConclusionThe prevalence of poor sleep quality among Emergency nurses during the COVID-19 pandemic is high. There is a pressing need for targeted interventions to enhance sleep quality among ED nurses. Improving sleep quality is not only essential for the wellbeing of these nurses but is also likely to contribute to better patient care.

Introduction: Increased Actin-like 6A (ACTL6A) expression is associated with various cancers, but its comprehensive investigation across different malignancies is lacking. We aimed to analyze ACTL6A as a potential oncogene and therapeutic target using bioinformatics tools.Methods: We comprehensively analyzed ACTL6A expression profiles across human malignancies, focusing on correlations with tumor grade, stage, metastasis, and patient survival. Genetic alterations were examined, and the epigenetic landscape of ACTL6A was assessed using rigorous methods. The impact of ACTL6A on immune cell infiltration in the tumor microenvironment was evaluated, along with molecular docking studies and machine learning models.Results: Our analysis revealed elevated ACTL6A expression in various tumors, correlating with poor prognostic indicators such as tumor grade, stage, metastasis, and patient survival. Genetic mutations and epigenetic modifications were identified, along with associations with immune cell infiltration and key cellular pathways. Machine learning models demonstrated ACTL6A's potential for cancer detection.Discussion: ACTL6A emerges as a promising diagnostic and therapeutic target in cancer, with implications for prognosis and therapy. Our study provides comprehensive insights into its carcinogenic actions, highlighting its potential as both a prognostic indicator and a target for anti-cancer therapy. This integrative approach enhances our understanding of ACTL6A's role in cancer pathogenesis and treatment.

9. The Association Between Periodontal Disease and Chronic Migraine: A Systematic Review

Migraine is a neurologic illness that produces intense throbbing pain on one side of the head and affects roughly 1 billion people worldwide. Recent research indicates a relationship between periodontitis and chronic migraines. This study aimed to review the association between chronic migraines and periodontitis through a systematic literature review.Four research databases (Google Scholar, PubMed, ProQuest, and SpringerLink) were searched according to PRISMA guidelines to retrieve the studies included in this review. A search strategy was developed to answer the study question with appropriate inclusion and exclusion criteria. Out of 34 published studies, 8 studies were included in this review. Three of the studies were cross-sectional, 3 were case-control, and 2 were clinical report and medical hypothesis papers. Seven of the 8 included studies showed that there is an association between periodontal disease and chronic migraine. The elevated blood levels of some biomarkers such as leptins, ProCalcitonin (proCT), calcitonin gene-related peptides (CGRPs), Pentraxin 3 (PTX3), and Soluble Tumor Necrosis Factor-like Weak Inducer Of Apoptosis (sTWEAK) play a significant role in this association. The limitations include a small sample size, the influence of anti-inflammatory drugs, and a self-reported headache measure that is subject to misclassification bias.This systematic review reveals a supposed correlation between periodontal disease and chronic migraine, as evidenced by various biomarkers and inflammatory mediators. This suggests that periodontal disease could potentially contribute to the development of chronic migraine. However, to further assess the potential benefits of periodontal treatment in patients with chronic migraine, additional longitudinal studies with larger sample sizes and interventional studies are needed.

Abstract Background Ward rounds are a cornerstone in the educational experience of junior doctors and an essential part of teaching patient care. Here, we aimed to assess the doctors’ perception of ward rounds as an educational opportunity and to identify the obstacles faced in conducting a proper ward round in Sudanese hospitals. Method A cross-sectional study was conducted from the 15th to the 30th of January 2022 among house officers, medical officers, and registrars in about 50 teaching and referral hospitals in Sudan. House officers and medical officers were considered the learners, while specialist registrars were considered the teachers. Doctors’ perceptions were assessed using an online questionnaire, with a 5-level Likert scale to answer questions. Results A total of 2,011 doctors participated in this study (882 house officers, 697 medical officers, and 432 registrars). The participants were aged 26.9 ± 3.2 years, and females constituted about 60% of the sample. An average of 3.1 ± 6.8 ward rounds were conducted per week in our hospitals, with 11.1 ± 20.3 h spent on ward rounds per week. Most doctors agreed that ward rounds are suitable for teaching patient management (91.3%) and diagnostic investigations (89.1%). Almost all the doctors agreed that being interested in teaching (95.1%) and communicating appropriately with the patients (94.7%) make a good teacher in ward rounds. Furthermore, nearly all the doctors agreed that being interested in learning (94.3%) and communicating appropriately with the teacher (94.5%) make a good student on ward rounds. About 92.8% of the doctors stated that the quality of ward rounds could be improved. The most frequently reported obstacles faced during ward rounds were the noise (70%) and lack of privacy (77%) in the ward environment. Conclusion Ward rounds have a special value in teaching patient diagnosis and management. Being interested in teaching/learning and having good communication skills were the two major criteria that make a good teacher/learner. Unfortunately, ward rounds are faced with obstacles related to the ward environment. It is mandatory to ensure the quality of both ward rounds' teaching and environment to optimize the educational value and subsequently improve patient care practice.

In this article, we present some novel dynamic Hilbert-type inequalities within the framework of time scales T. We achieve this by utilizing Hölder’s inequality, the chain rule, and the mean inequality. As specific instances of our findings (when T=N and T=R), we obtain the discrete and continuous analogues of previously established inequalities. Additionally, we derive other inequalities for different time scales, such as T=qN0 for q>1, which, to the best of the authors’ knowledge, is a largely novel conclusion.

Abstract Background Elevated levels of S-100B in serum are increasingly considered a potential biochemical marker of nervous system damage. To our knowledge, limited number of research studies have tested the serum S-100B protein levels in children with epilepsy. This paper introduces novel generalizations of dynamic inequalities of Copson type within the framework of time scales delta calculus. The proposed generalizations leverage mathematical tools such as Hölder’s inequality, Minkowski’s inequality, the chain rule on time scales, and the properties of power rules on time scales. As special cases of our results, particularly when the time scale T equals the real line (T=R), we derive some classical continuous analogs of previous inequalities. Furthermore, when T corresponds to the set of natural numbers including zero (T=N0), the obtained results, to the best of the authors’ knowledge, represent innovative contributions to the field.

We explored the self-reported antibiotic stewardship (AS), and infection prevention and control (IPC) activities in intensive care units (ICUs) of different income settings. A cross-sectional study was conducted using an online questionnaire to collect data about IPC and AS measures in participating ICUs. The study participants were Infectious Diseases–International Research Initiative (IDI-IR) members, committed as per their institutional agreement form. We analyzed responses from 57 ICUs in 24 countries (Lower-middle income (LMI), n = 13; Upper-middle income (UMI), n = 33; High-income (HI), n = 11). This represented (~5%) of centers represented in the ID-IRI. Surveillance programs were implemented in (76.9%−90.9%) of ICUs with fewer contact precaution measures in LMI ones (p = 0.02); (LMI:69.2%, UMI:97%, HI:100%). Participation in regional antimicrobial resistance programs was more significantly applied in HI (p = 0.02) (LMI:38.4%,UMI:81.8%,HI:72.2%). AS programs are implemented in 77.2% of institutions with AS champions in 66.7%. Infectious diseases physicians and microbiologists are members of many AS teams (59%&50%) respectively. Unqualified healthcare professionals(42.1%), and deficient incentives(28.1%) are the main barriers to implementing AS. We underscore the existing differences in IPC and AS programs’ implementation, team composition, and faced barriers. Continuous collaboration and sharing best practices on APM is needed. The role of regional and international organizations should be encouraged. Global support for capacity building of healthcare practitioners is warranted.

Introduction: Angiosarcoma is a rare and highly aggressive soft tissue malignancy originating from vascular and lymphatic endothelial cells. Epithelioid angiosarcoma is the rarest subtype of angiosarcoma, characterized by the proliferation of large polygonal cells with an epithelioid feature. The occurrence of these tumors in the oral cavity is highly uncommon, and immunohistochemistry staining is essential to differentiate epithelioid angiosarcoma from mimicking lesions. Aim: To present a case of intraoral angiosarcoma with an unusual clinical presentation and behavior and to report, to the best of our knowledge, a first primary appendix epithelioid angiosarcoma with metastasis foci in the oral cavity. Objectives: To discuss the clinical, histological, and immunochemical features of an unusual case of intraoral angiosarcoma. Case report: A 53-year-old Saudi female with an uncommon clinical presentation of intraoral angiosarcoma. The patient reported the lesion being painless, slowly growing, and of a six-month duration. The microscopic examination and immunohistochemical evaluation showed epithelioid angiosarcoma. The tumor cells were positive to ERG, FLI 1, and CD31 (focal) and negative to CK HMW, CD45, S100, HMB 45, D2-4, and CD 34. Discussion: Due to the extremely rare occurrence and non-characteristic presentation of angiosarcoma in the oral cavity, many lesions maybe included in the differential diagnosis. Thus, making the diagnosis of intraoral angiosarcoma difficult.

In this article, we establish some new generalized inequalities of the Hilbert-type on time scales’ delta calculus, which can be considered similar to formulas for inequalities of Hilbert type. The major innovation point is to establish some dynamic inequalities of the Hilbert-type on time scales’ delta calculus for delta differentiable functions of one variable and two variables. In this paper, we use the condition aj(sj)=0 and aj(sj,zj)=aj(wj,nj)=0, ∀j=1,2,…,n. These inequalities will be proved by applying Hölder’s inequality, the chain rule on time scales, and the mean inequality. As special cases of our results (when T=N and T=R), we obtain the discrete and continuous inequalities. Also, we can obtain other inequalities in different time scales, like T=qZ−, q>1.

HEV-Ag ELISA assay is a reliable diagnostic test in resource-limited areas. HEV genotype 1 (HEV-1) infections are either self-limited or progress to fulminant hepatic failure (FHF) and death if anti-HEV therapy is delayed. Limited data is available about the diagnostic utility of HEV Ag on HEV-1 infections. Herein we aimed to study the kinetics of HEV Ag during HEV-1 infections at different stages, i.e., acute HEV infection, recovery, and progression to FHF. Also, we evaluated the diagnostic utility of this marker to predict the outcomes of HEV-1 infections. Plasma of acute hepatitis E (AHE) patients were assessed for HEV RNA by RT-qPCR, HEV Ag, and anti-HEV IgM by ELISA. The kinetics of HEV Ag was monitored at different time points; acute phase of infection, recovery, FHF stage, and post-recovery. Our results showed that the level of HEV Ag was elevated in AHE patients with a significantly higher level in FHF patients than recovered patients. We identified a plasma HEV Ag threshold that can differentiate between self-limiting infection and FHF progression with 100% sensitivity and 88.89% specificity. HEV Ag and HEV RNA have similar kinetics during the acute phase and self-limiting infection. In the FHF stage, HEV Ag and anti-HEV IgM have similar patterns of kinetics which could be the cause of liver damage. In conclusion, the HEV Ag assay can be used as a biomarker for predicting the consequences of HEV-1 infections which could be diagnostically useful for taking the appropriate measures to reduce the complications, especially for high-risk groups.

In this study, we apply Hölder’s inequality, Jensen’s inequality, chain rule and the properties of convex functions and submultiplicative functions to develop an innovative category of dynamic Hardy-type inequalities on time scales delta calculus. A time scale, denoted by T, is any closed nonempty subset of R. In time scale calculus, results are unified and extended. As particular cases of our findings (when T=R), we have the continuous analogues of inequalities established in some the literature. Furthermore, we can find other inequalities in different time scales, such as T=N, which, to the best of the authors’ knowledge, is a largely novel conclusion.

Abstract Tramadol is an opioid extensively used to treat moderate to severe pain; however, prolonged therapy is associated with several tissues damage. Chronic use of tramadol was linked to increased hospitalizations due to cardiovascular complications. Limited literature has described the effects of tramadol on the cardiovascular system, so we sought to investigate these actions and elucidate the underlying mechanisms. Mice received tramadol hydrochloride (40 mg/kg body weight) orally for 4 successive weeks. Oxidative stress, inflammation, and cardiac toxicity were assessed. In addition, eNOS expression was evaluated. Our results demonstrated marked histopathological alteration in heart and aortic tissues after exposure to tramadol. Tramadol upregulated the expression of oxidative stress and inflammatory markers in mice heart and aorta, whereas downregulated eNOS expression. Tramadol caused cardiac damage shown by the increase in LDH, Troponin I, and CK-MB activities in serum samples. Overall, these results highlight the risks of tramadol on the cardiovascular system.

Abstract Background Substance use disorders (SUD) are considered as serious mental illnesses, with variability of age of onset, duration of illness, and type of used substance among patients. The effect of type and number of substances used on the severity of such an illness are worth scientific investigation. In this study, we tried to investigate the probable relationship between type and number of used substances with the degree of severity of substance used disorders in a sample of upper Egyptian patients. To predict risk factors associated with the severity of SUD. Results Opiates (including tramadol) were the most frequently used substances followed by cannabis and then heroin. Patients using poly substances scored significantly higher than the ones using a single substance on AIS (p = 0.001), as well as on the legal, drug, and family and social subscales of ASI (0.012 0.006, 0.002 respectively). Multiple stepwise linear regression analysis demonstrated that the number of used substances was the most prominent factor in determining severity of substance-related disorder. Conclusions Using more than one substance is common in patients with substance use disorders. This is usually associated in more severe form and consequences of illness. So, some substances and combinations are associated with more morbidity than others.

Albumen, silver gelatin and chromogenic prints are found abundantly among photographic collections in Egypt. Due to the uncontrolled environment in archives and libraries, this precious visual heritage with its high protein and cellulose content provides the right culture medium required for fungal growth. Many essential oils have been proven to have antibacterial and antifungal properties. Essential oils offer a safe alternative to other common disinfection methods; however, their effect on the properties of photographs have not received much study. This paper studies the effect of vapors of anise, cinnamon, clove, lavender and thyme oils on albumen, silver gelatin and chromogenic prints, to find a proper disinfection method that is user-friendly and environmentally safe and respects the nature of photographic materials. Essential oils were provided by the National Research Center (NRC) in Cairo, Egypt. Artificially aged albumen prints, and naturally aged silver gelatin and chromogenic prints were exposed in desiccators to the selected essential oils in the vapor phase for a period of 5 days. All samples were artificially aged at a temperature of 80°C and 65% RH for a period of 5 days to study the long-term effects of the tested treatments. Treatments were evaluated using several techniques including visual inspection, microscopic inspection, colorimetric measurements, and attenuated total reflectance Fourier transform infrared spectroscopy. Results showed that all tested essential oils had a very slight effect on the tested photographic samples; however lavender oil was found to be the best option, specifically in terms of preserving the chemical properties of the photographic surfaces.

Through the paper, we present several inequalities involving C-monotonic functions with C≥1, on nabla calculus via time scales. It is known that dynamic inequalities generate many different inequalities in different calculus. The main results will be proved by applying the chain rule formula on nabla calculus. As a special case for our results, when T=R, we obtain the continuous analouges of inequalities that had previously been proved in the literature. When T=N, the results, to the best of the authors’ knowledge, are essentially new. Symmetrical properties of C-monotonic functions are critical in determining the best way to solve inequalities.

Background: Warfarin is the most widely used oral anticoagulant for the prevention and treatment of thromboembolic disorders. Because of narrow therapeutic index and various genetic and non-genetic factors that influence the disposition of the drug, its dose undergoes a great variability. The aim of this study was to determine the allelic variants of CYP2C9 and VKORC1 genes in Iraqi patients, and to investigate the contribution of genetic on warfarin dose requirements.Methods: A cross sectional study was carried out on a sample of Iraqi patients from Baghdad city who were admitted to Ibn AL-Bitar Specialized Center for cardiac surgery. Blood samples of all patients were collected for both hematological and genetic analysis utilizing standard techniques.Results: The frequency of CYP2C9*3 allele was 9.4% whereas that of CYP2C9*2 allele was 13.7%. The frequency of (VKORC1-1639G) allele was 58.75% and the frequency of (VKORC1-1639A) allele was 41.25%. Patients’ daily warfarin doses were administered according to their genotype.Conclusion:It can be concluded that CYP2C9*3 and VKORC1 had significant effect on warfarin dose. New warfarin-dosing algorithm was developed based on CYP2C9*3 and VKORC1genotypes for predicting the required dose of warfarin.

Abstract Background Chronic Hypersensitivity Pneumonitis (CHP) is caused by an immune mediated response in the lung tissue after exposure to an inhaled environmental antigenic stimulant. We previously documented the ways in which CHP impacts patients’ lives and have now developed a disease-specific instrument, the CHP-HRQOL instrument, to measure health-related quality of life (HRQOL). The objective of this study was to assess content validity for the CHP-HRQOL. Methods Cognitive interviews were conducted among adults with CHP. The instrument was revised and refined between each round of interviews. Feedback was obtained on the instructions, items, response options, and recall period. Items where participants had difficulty with comprehension, wording, or misinterpretation were marked by the interviewer and participant feedback was reviewed to make revisions, add or delete items when appropriate. Readability statistics were calculated using Flesch-Kincaid grade level and reading ease scores. Results Ten participants were interviewed over three rounds, with revisions made to the questionnaire in an iterative process. In the initial 39 item instrument, we identified 7 items where two or more participants reported difficulty. Participants preferred a four-week recall period (compared to a two-week recall period) and response options with a 5-point response scale. The final version of the CHP-HRQOL includes 40 items with a median reading level between 6th and 7th grade. Conclusion The CHP-HRQOL instrument demonstrated high content validity and is ready for psychometric testing in further validation studies.

Objective. Our study aimed to determine the prevalence, location, and morphological differences of the septa using CBCT images. Methods. This retrospective study was conducted on CBCT examinations. The sample included both mixed and permanent dentition and edentulous patients. The images were viewed in 3 planes (sagittal, coronal, and axial) and the left and right maxillary sinuses were evaluated for the septa’s prevalence, location, and morphological differences. Results. The measurements were statistically analyzed using SPSS software. Out of 200, 122 patients showed one or more bony septa in the maxillary sinus. The septal location and angulation were not limited to a specific area or a specific range. Significant differences between genders were found. Conclusions. The increased surface area of the septa using plane 2-dimensional radiographs is impossible. CBCT has improved the planning of any sinus procedure and offers adequate information compared to conventional radiographs.

Background. Breastfeeding provides unsurpassed natural nutrition to the newborn and infant. It has a nearly perfect mix of food elements and vitamins that infants need to grow up. Nonetheless, the tendency for breastfeeding remains below the expected levels. Objectives. To explore the attitudes and barriers to breastfeeding among mothers in Princess Nourah Abdulrahman University (PNU), Riyadh, Saudi Arabia. Methods. A cross-sectional study was conducted, from January to April 2019; 399 PNU students, employees, and faculty mothers aged 18 years and above with experience of childbirth and breastfeeding were included in the study using a predesigned validated questionnaire. The questionnaire consisted of four scales: sociodemographic, attitude toward breastfeeding, barriers to breastfeeding, and induced lactation knowledge. Results. The participants’ mean age was 34.1 ± 10.4 years; most (87.8%) were Saudi; 92.8% were married; 62% had a bachelor’s degree; and 43% had “enough income.” While 40% of the mothers reported >6 months “exclusive breastfeeding” for the first baby, only 34.8% did so for the last baby, and 54.5% did so for most of all babies altogether. The mothers’ parity ranged between 1 birth and 4 births in 23.5% and 17.5% of the participants, respectively. An overall score of breastfeeding attitude averaged 59.6 ± 7.3. Internet of Medical Things (IoMT) deals with a patient-data-rich segment, which makes security and privacy a severe concern for patients. Therefore, access control is a significant aspect of ensuring trust in the IoMT. However, deploying existing authentication and authorization solutions to the Internet of Medical Things (IoMT) is not straightforward because of highly dynamic and possibly unprotected environments and untrusted supply chain for the IoT devices. In this article, we propose Soter, a Zero-Trust based authentication system for the IoMT. Soter Incorporates trust negotiation mechanisms within the Zero Trust framework to enable dynamic trust establishment. When a user or device seeks access to a resource, initiate a trust negotiation process. During this process, credentials, attributes, and contextual information are exchanged between the requester and the resource owner. Soter defines access rules based on various factors, including user identity, device health, and location. Access is granted or denied based on these conditions.

Brassica oleracea var. italica ,phosphorus sources ,total phenols ,chemical composition ,total carbohydrates ,Agriculture - Abstract
Broccoli is a popular vegetable throughout the world and contains important nutritional features. This study was conducted to evaluate the effect the fertilization with different phosphorus sources (i.e., soil application of rock phosphate (RP) at 428 kg ha−1, calcium superphosphate (CSP) at 670 kg ha−1, phosphoric acid (PA) at 126 L ha−1 and monoammonium phosphate (MAP) at 334 kg ha−1) combined with biofertilizers (i.e., inoculation with phosphorin or mycorrhiza) on plant growth aspects, antioxidants enzyme activity, chemical constituents, yield and quality of broccoli leaves and heads (Brassica oleracea var. italica Plenck). The experiment was performed according to the randomized complete block design with three replications (n = 3), for two growing seasons (2018–2019) at the experimental farm of Benha University, Egypt. The obtained results showed that the highest values of plant height, leaf area, fresh and dry weight of leaves/plant, head weight and diameter, and the highest yield of heads ha−1 were recorded in plants that received MAP fertilizer and were inoculated with mycorrhiza. On the other hand, the highest value of head length, total carbohydrate and total soluble solids (TSS) content were observed in plants fertilized with PA and inoculated with mycorrhiza. The highest leaf nitrogen % and head ascorbic acid content, as well as the lowest head total phenol content were recorded in plants supplemented with MAP fertilizer and inoculated with mycorrhiza. Moreover, broccoli plants fertilized with PA and inoculated with mycorrhiza showed promising results in terms of phosphorus, potassium and total carbohydrate content, antioxidant activity and dehydrogenase activity. In conclusion, phosphorus may affect plant growth, yield and chemical parameters in broccoli plants depending on the fertilizer source, while inoculation with mycorrhiza may also have a beneficial impact on these parameters.

Stevia (Stevia rebaudiana, L.) is receiving increasing global interest as a diabetes-focused herb associated with zero-calorie stevioside sweetener glycoside production. This study was conducted to determine whether the arbuscular mycorrhiza (AM), as a biofertilizer integrated with nano boron (B), zinc (Zn), and molybdenum (Mo), would improve stevia growth and stevioside content. A factorial experiment with four replicates was conducted to evaluate the effect of AM at 0, 150, and 300 spore/g soil and three nano microelements B at 100 mg/L, Zn at 100 mg/L, and Mo at 40 mg/L on growth performance, stevioside, mineral contents, and biochemical contents of stevia. Results indicated that the combination of AM at 150 and B at 100 mg/L significantly increased plant height, number of leaves, fresh and dry-stem, and herbal g/plant during the 2019 and 2020 growing seasons. Chlorophyll content was increased by the combination between AM at 150 spore/g soil and B at 100 mg/L during both seasons. Stevioside content in leaves was increased by AM at 150 spore/g soil and B at 100 mg/L during the second season. In addition, N, P, K, Zn, and B in the leaf were increased by applying the combination of AM and nano microelements. Leaf bio constituent contents were increased with AM at 150 spore/g soil and B at 100 mg/L during both seasons. The application of AM and nano B can be exploited for high growth, mineral, and stevioside contents as a low-calorie sweetener product in stevia.

Abstract Introduction Media interventions can potentially play a major role in influencing health policies. This integrative systematic review aimed to assess the effects of planned media interventions—including social media—on the health policy-making process. Methods Eligible study designs included randomized and non-randomized designs, economic studies, process evaluation studies, stakeholder analyses, qualitative methods, and case studies. We electronically searched Medline, EMBASE, Communication and Mass Media Complete, Cochrane Central Register of Controlled Trials, and the WHO Global Health Library. We followed standard systematic review methodology for study selection, data abstraction, and risk of bias assessment. Results Twenty-one studies met our eligibility criteria: 10 evaluation studies using either quantitative (n = 7) or qualitative (n = 3) designs and 11 case studies. None of the evaluation studies were on social media. The findings of the evaluation studies suggest that media interventions may have a positive impact when used as accountability tools leading to prioritizing and initiating policy discussions, as tools to increase policymakers’ awareness, as tools to influence policy formulation, as awareness tools leading to policy adoption, and as awareness tools to improve compliance with laws and regulations. In one study, media-generated attention had a negative effect on policy advocacy as it mobilized opponents who defeated the passage of the bills that the media intervention advocated for. We judged the confidence in the available evidence as limited due to the risk of bias in the included studies and the indirectness of the evidence. Conclusion There is currently a lack of reliable evidence to guide decisions on the use of media interventions to influence health policy-making. Additional and better-designed, conducted, and reported primary research is needed to better understand the effects of media interventions, particularly social media, on health policy-making processes, and the circumstances under which media interventions are successful. This study aims at first identifying the kinds of tannins compounds in tea extract, second specifying the effect of temperature used in extracting tea on the amount of protein precipitant tannin. The amount of tannin has been estimated after being extracted from five pure types of cylanain , Indian, Indonesian tea and other than these ones. Then, it has been revealed that the higher the temperature the more amount of tannin is extracted in most of the extracts of tea. Tannins compounds have been identified in the five extracts ,by using the technique of "thin layer chromatography" (TLC) and the result was that the five extracts ; involve ;Tannic acid, Gallic acid and Ellagic acid; in addition to more four compounds with equal values of relative flow ,these compounds might be Epicatchin (EC), Epigallocatchin(EGC),Epicatchin gallate(ECG) and Epigallocatchin gallate(EGCG).

37. The Effect of Circular Perforation on a V-Corrugated Fin Performance under Natural Convection

An experimental investigation has been made to study the influence of using v-corrugated aluminum fin on heat transfer coefficient and heat dissipation in a heat sink. The geometry of fin is changed to investigate their performance. 27 circular perforations with 1 cm diameter were made. The holes designed into two ways, inline arrangement and staggered in the corrugated edges arrangement. The experiments were done in enclosure space under natural convection. Three different voltages supplied to the heat sink to study their effects on the fins performance. All the studied cases are compared with v-corrugated smooth solid fin. Each experiment was repeated two times to reduce the error and the data recorded after reaching the steady state conditions. The results showed that the v-corrugated fin dissipate heat twice and triple times than flat plate mentioned in past research with the same dimension. Also, the inline perforated fin gave higher enhancement percentage than solid one by 15, 32 and 36% for 110, 150 and 200 V voltages supplied. Finally, the staggered perforation arrangement gave the higher enhancement percentage with 22, 42 and 45% for the same voltages supply.

Objectives Oral lichen planus (OLP) is a chronic inflammatory oral mucosal disease. Cytokines play an important role in the pathogenesis and disease progression of OLP. Various reports have implicated cytokine gene polymorphisms in susceptibility to develop some immune mediated conditions including OLP. The purpose of this study was to investigate the association of tumor necrosis factor (TNF)-α, TNF-β and interleukin (IL)-10 gene polymorphisms with the OLP risk. Material and Methods Forty two unrelated patients with OLP and 211 healthy volunteers were genotyped for TNF-α (-308 G/A), TNF-β (+252A/G), IL-10 (-1082G/A), IL-10 (-819C/T), and IL-10 (-592C/A) polymorphisms. Results The frequencies of allele A and genotype GA of TNF-α (-308G/A) were significantly higher while allele G and GG genotypes were lower in OLP patients as compared to the controls (P0.05). However, haplotype ATA extracted from 1082G/A, -819C/T, -592C/A polymorphisms of IL-10 were more prevalent in OLP patients when compared to controls indicating its possible association with OLP susceptibility. Conclusion It is concluded that TNF-α (-308G/A), TNF-β (+252A/G) and IL-10 (-1082G/A, -819C/T and -592C/A) polymorphisms are associated with the susceptibility of OLP, thus giving additional support for the genetic basis of this disease.

39. Assessment of trace and heavy metal distribution by four sequential extraction procedures in a contaminated soil

Four sequential extraction procedures (Sposito, Tessier, Silveira and Bureau Communautaire de Reference (BCR)) were used to evaluate the distribution of some metals (Fe, Cu, Cd and Zn) in a contaminated soil around a mining area. The results showed that Fe and Zn were mainly recovered in the recalcitrant soil fractions, while Cd was primarily localized in the exchangeable fraction. Soil Cu was highly associated with organic matter fraction. The amorphous Fe fraction in Silveira could be recognized as part of the Fe-Mn oxide fraction in Tessier and BCR procedures, while the crystalline Fe oxide fraction was classified into the residual fraction in Sposito, BCR and Tessier schemes. Although the same reagent was used to extract target fraction, less carbonate-bound Cu, Cu and Zn were extracted in Tessier procedure as compared to Silveira method, while Tessier scheme yielded a higher proportion of Fe, Cu and Zn in the Fe-Mn oxide fraction than BCR method. Due to the lack of uniformity of experimental conditions and the differences in extraction reagents, the extraction efficiency of metal species varied with the sequential extraction schemes. Therefore, care should be taken when comparing the results obtained by different sequential extraction procedures.

This experiment has been carried out in Aqrai, Atroosh and Zawita zone on stand of Pinus brutia Ten. In Northern of Iraqi to determinate on accumulation the effect of density between trees for the growth of basal area for the length of rotation of trees. Different types of measurements have been taken for (30) random samples with an area of (0.1) ha. Measurement cover basal area , mean height and number of tree per hectare , by using different regression in statgraf system to derive mathematical equations. In order to select on equation that give the best fit , the following criteria were employed in the evolution process coefficient of determination R2 , standard error (S.E) , a mathematical relation between basal areas in the hectare as , dependent variable , on the other hand , age ,number of the tree in hectare , mean height as independent variable , we obtain the following equation: G=-2.6559+2.08333 A (1-e (-0.001020N)) Culmination of basal area length cutting cycle of Pinus brutia Ten. in Iraqi was estimated by using the equation above,we found increasing for growth basal area, when the number of tree increase to (750) tree| ha and age year. While the annual basal area growth was deceased, when the number of tree increased. From that the manager can select the best number and age which, he wanted in the produced of the stand Pinus brutia Ten.

An experimental study was conducted to determine the performance of a solar electric refrigeration system. The system contained flat photovoltaic solar panel which absorbs the solar energy and convert it to electrical energy, used to run the refrigeration cycle. Two refrigeration cycles with electrical solar panel were used over a period of 12 months, the first one with classical parts known in refrigeration cycle, while the second one introduced heat exchanger which improves the coefficient of performance by saving the consumed energy. The coefficient of performance of these refrigeration cycles with compressor efficiency 85% are 2.102 and 2.57 respectively. The overall efficiency of the two systems are 18.9% and 23.13%.

In this work, we defined an attack vector for networks utilizing the Internet of Medical Things (IoMT) devices and compute the probability distribution of IoMT security threats based on Markov chain and Common Vulnerability Scoring System (CVSS). IoMT is an emerging technology that improves patients' quality of life by permitting personalized e-health services without restrictions on time and site. The IoMT consists of embedded objects, sensors, and actuators that transmit and receive medical data. These Medical devices are vulnerable to different types of security threats, and thus, they pose a significant risk to patient's privacy and safety. Because security is a critical factor for successfully merging IoMT into pervasive healthcare systems, there is an urgent need for new security mechanisms to prevent threats on the IoMT edge network. Toward this direction, the first step is defining an attack vector that an attacker or unauthorized user can take advantage of to penetrate and tamper with medical data. In this article, we specify a threat model for the IoMT edge network. We identify any vulnerabilities or weaknesses within the IoMT network that allow unauthorized privileges and threats that can utilize these weaknesses to compromise the IoMT edge network. Finally, we compute the probability distribution of IoMT threats based on the Markov transition probability matrix.
