Tumours with hybrid morphological features between two different histological types are seldom observed in salivary glands. We report a hybrid malignant tumour arising in the right parotid gland of a 53 year-old Caucasian woman. Microscopically, two different components were present in the same tumour: one was represented by a biphasic neoplasm consistent with epithelial-myoepithelial cell carcinoma, the other was a poorly differentiated tumour with cytological features of a lymphoepithelial carcinoma. In both component, a marked lymphoplasmacytic infiltrate, with germinal center formation, was evident throughout the tumour, in both components. Neoplastic cells of both tumoural components were diffusely positive for Epstein Barr virus (EBV) mRNA by in situ hybridisation. This case appears to be unique, as the association between EBV and myoepithelial tumours has not been described, and lymphoepithelial carcinoma with epi-myoepithelial carcinomas have to be added to the possible combinations in hybrid tumours.

I tumori delle ghiandole salivari con aspetti morfologici misti tra due istotipi differenti sono rari. Nel caso che descriviamo, la paziente, una donna di 53 anni si presenta con una massa di 4 cm nella regione parotidea destra. Viene eseguito a una parotidectomia totale e all’esame istologico la neoplasia appare costituita da due differenti componenti: una è rappresentata da una proliferazione bifasica in cui si riconoscono nidi e strutture ghiandolari bordate da epitelio secernente e circondate esternamente da uno strato mioepiteliale, l’altra da un tumore scarsamente differenziato con caratteristiche citologiche di un carcinoma linfoepiteliale. In entrambe le componenti, si osserva un marcato infiltrato linfoplasmacellulare con formazione di centri germinativi. Le cellule neoplastiche d’entrambe le componenti del tumore sono risultate diffusamente positive alla ricerca dell’ mRNA per il virus di Epstein Barr (EBV) mediante la metodica dell’ ibridazione in situ. Questo caso è unico in letteratura, dato che nè l’associazione tra questi due istotipi, nè una correlazione tra EBV e i tumori mioepiteliali è stata mai descritta prima.

Introduzione: Le alterazioni genetiche piu’ frequenti nel carcinoma polmonare non a piccole cellule (CNPC) sono rappresentate da perdite del 3p, sede del gene FHIT, e varie alterazioni numeriche a carico dei cromosomi 9, 17, 21 e 22. tuttavia la maggior parte delle casistiche presenti in letteratura sono state studiate mediante FISH o ricerca della perdita di eterozigosi. Pochi sono i dati attuali sulla determinazione del cariotipo completo, con citogenetica classica in metafase su ampie casistiche. Metodi: Il cariotipo di 38 CNPC del polmone è stato studiato attraverso la tecnica delle colture a breve termine e l’analisi su piastre metafasiche. Risultati: La casistica è rappresentata da 19 carcinomi squamocellulari (SC), 18 adenocarcinomi (AC) e 1 carcinoma a grandi cellule (CGC). Nei CS le alterazioni piu’ frequenti osservate erano alterazioni numeriche, in particolare, a carico del cromosoma 21 ( in 6 casi) e l’acquisizione di un cromosoma 3 (in 9 casi: clonale in 2 e come singola metafase in 7). L’acquisizione del cromosoma 3 (trisomia 3) è stata riscontrata, sia associata alla perdita del 21, sia come unica alterazione. Conclusioni: Mentre la monosomia 21 è un fenomeno ben conosciuto nelle cellule tumorali squamose, piu’ sorprendente appare nella nostra casistica il riscontro di trisomia 3. Infatti, nel braccio corto del cromosoma 3 risiede il gene FHIT la cui perdita è descritta come evento precoce nella carcinogenesi polmonare. Al fine di confermare i risultati ottenuti, i casi sono stati testati mediante immunoistochimica con anticorpo anti-FHIT e casi selezionati sono stati sottoposti anche a FISH con sonda centromerica per il cromosoma 3.

Introduction: Although several clinico-pathologic studies on extra-nodal oral cavity lymphomas have been reported, our understanding of these tumours is incomplete. This is mainly because oral lymphomas are quite rare and the opportunity to apply modern techniques to their classification is limited. Propose: The aim of this study was to determinate both the histotype and phenotype of lymphomas presenting in the oral cavity. Materials and methods: Cases were classified according to the World Health Organization Classification of Tumours of Haematopoietic and Lymphoid Tissues 2001. Immunohistochemical stains were performed on tissue microarrays (TMA). Results: Forty-four cases of non-Hodgkin’s lymphoma (NHL) and 2 cases of primitive Hodgkin’s lymphoma (HL) were observed, with a slight female predominance (21M; 25F). Forty cases of NHL were primitive of the oral cavity, while six cases were secondary lesions. NHL were divided into five groups: 1) diffuse large B-cell lymphoma (DLBCL: 33 cases); 2) follicular lymphoma (FL: 5 cases); 3) extranodal marginal zone B-cell lymphoma (MZL: 3 cases); 4) mantle cell lymphoma (MCL: 1 case); 5) peripheral T-cell lymphomas, not otherwise specified (PTCL, NOS: 2 cases). HL occurred in two males, aged 25 and 65 years respectively. Both cases were diagnosed as nodular sclerosing HL. The lesions were variably located in different sites of the oral cavity, commonly affected sites being the soft and hard palate and tongue. The most frequent clinical appearance was a submucosal mass (34 cases). Dental waving was present in 5 cases and actinomycotic infection in one. Six cases appeared as slightly raised whitish lesions. Two cases (1 NHL and 1 HL) appeared in HIV infected patients. Conclusion: The present series shows that malignant lymphomas of the oral cavity more often represent de novo primary growths. The most frequent event is DLBCL affecting elderly patients in the palate or tongue. Although rare, HL should be considered among primary oral cavity lymphomas.
