8. Career Paths and Organizational Development: Expanding Alliances.

Abstract

The Synergistic Model of Organizational Career Development is an attempt to combine best practice principles from two domains: organizational development and individual career planning. The model assumes three levels of intervention within an organization: philosophical, strategic, and practical. Interventions at any of the levels may be directed toward the employees, the organization, or the balancing and interactive process that bring the two systems together. At the philosophical level, employees are concerned with becoming or managing to stay meaningfully connected to the world of work, organizations are concerned with defining their central purpose as an organization, and balancing/interactive processes are designed to balance employees' and the organization's long-term needs and goals. At the strategic level, employees are concerned with enhancing their careers, organizations are concerned with best meeting their organizational outcomes, and balancing/interactive processes are designed to balance short-term employees and organization goals. At the practical level, employees are concerned with staying employable, organizations are concerned ensuring that employees perform tasks essential to the organization, and balancing/interactive processes are designed to balance organizational demands with employee performance. The ultimate goal of balancing/interactive interventions must be to bring individual career planning into alignment with effective organizational development strategies. 9. A Synergistic Model of Organizational Career Development.

Abstract

The Synergistic Model of Organizational Career Development is a new model of organizational career development that combines the best of career development practice and organizational development into a unified, coherent model. The model has three levels of organization: philosophical, strategic, and practical. Expanding circles are used to illustrate movement from the broad philosophical vision to strategic plans and then to the practical need for acquisition and demonstration of specific competencies. The model encourages employees and organizations to dream (philosophical level), plan (strategic level), and perform (practical level). The personal and organizational vision circles are represented by the center rings to denote their role in regulating the other subsystems. The focus on competence is represented by the outer rings to denote their role in providing feedback to the rest of the system regarding the requirements of the world of work: the competencies that employees require to remain employable and organizations require to remain competitive. This feedback helps employees and organizations adjust to changes in the world of work and monitor their plans and strategies to ensure optimum fulfillment of their respective visions. Abstract

Background In Region Västra Götaland (VGR), the development of remote patient monitoring is given high priority, aiming for improvements for patients and reduction of healthcare costs. In this report we defined remote patient monitoring as continuous follow-up of relevant health-related parameters of patients located outside healthcare facilities (e.g. at home). Measurements taken by analogue or digital devices, objective and/or subjective assessments, are delivered digitally to the patient and to a healthcare professional. The healthcare professional provides the patient with feedback on the reported data (feedback may be automatically generated if data are within a predefined range). The plan in VGR is to introduce remote monitoring in selected diagnosis groups – some of which already started using remote monitoring. Aim The aim of this report was to provide an overview of systematic reviews regarding remote monitoring(as add on or replacement of visits in current standard of care) compared to standard of care in 25 selected diagnosis groups. Method In order to clarify how remote monitoring is intended to be used in the 25 diagnosis groups, representatives from the respective clinical areas were interviewed. As the scope of this project covered many diagnosis groups, the search was limited to systematic reviews (SRs) of randomised (RCTs) or non-randomised clinical trials. The relevance of each identified SR for our PICO(Population, Intervention, Comparator and Outcomes) was assessed by at least two project members (one clinical representative and one from HTA-centrum). Relevant SRs were assessed by at least two project members using SNABBSTAR, a tool developed by The Swedish Agency for Health Technology Assessment and Assessment of Social Services (SBU) for assessment of risk of bias/systematic errors in SRs. Abstract

The potential impacts that video games might have on players’ well-being are under increased scrutiny but poorly understood empirically. Although extensively studied, a level of understanding required to address concerns and advise policy is lacking, at least partly because much of this science has relied on artificial settings and limited self-report data. We describe a large and detailed dataset that addresses these issues by pairing video game play behaviors and events with in-game well-being and motivation reports. 11,080 players (from 39 countries) of the first person PC game PowerWash Simulator volunteered for a research version of the game that logged their play across 10 in-game behaviors and events (e.g. task completion) and 21 variables (e.g. current position), and responses to 6 psychological survey instruments via in-game pop-ups. The data consists of 15,772,514 gameplay events, 726,316 survey item responses, and 21,202,667 additional gameplay status records, and spans 222 days. The data and codebook are publicly available with a permissive CC0 license.

16. Affective uplift during video game play: A naturalistic case study

Abstract

Do video games affect players’ well-being? In this case study, we examined 162,325 intensive longitudinal in-game mood reports from 67,328 play sessions of 8,695 players of the popular game PowerWash Simulator. We compared players’ moods at the beginning of play session with their moods during play, and found that the average player reported 0.034 [0.032, 0.036] visual analog scale (VAS; 0-1) units greater mood during than at the beginning of play sessions. Moreover, we predict that 72.1% [70.8%, 73.5%] of similar players experience this affective uplift during play, and that the bulk of it happens during the first 15 minutes of play. We do not know whether these results indicate causal effects or to what extent they generalize to other games or player populations. Yet, these results based on in-game subjective reports from players of a popular commercially available game suggest good external validity, and as such offer a promising glimpse of the scientific value of transparent industry-academia collaborations in understanding the psychological roles of popular digital entertainment.

17. Changes to consultations and diagnosis of osteoarthritis in primary care during the COVID-19 pandemic

Abstract

Objective: General practitioners (GP) are often the first medical professionals to treat musculoskeletal complaints. Yet the impact of COVID-19 on primary care utilisation for musculoskeletal complaints is largely unknown. This study quantifies the impact of the pandemic on primary care utilisation for musculoskeletal complaints and specifically osteoarthritis (OA) in the Netherlands. Design: We extracted data on GP consultations in 2015–2020 from 118,756 patients over 45 years of age and estimated reductions in consultations in 2020 as compared to 5-year average. Outcomes were GP consultations for: any musculoskeletal complaints, knee and hip OA, knee and hip complaints, and newly diagnosed knee and hip OA/complaints. Results: The relative reductions in consultations ranged from 46.7% (95% confidence intervals (CI): 43.9–49.3%) (all musculoskeletal consultations) to 61.6% (95% CI: 44.7–73.3%) (hip complaints) at the peak of the first wave, and from 9.3% (95% CI: 5.7–12.7%) (all musculoskeletal consultations) to 26.6% (95% CI: 11.5–39.1%) (knee OA) at the peak of the second wave. The reductions for new diagnoses were 87.0% (95% CI: 71.5–94.1%) for knee OA/complaints, and 70.5% (95% CI: 37.7–86.0%) for hip OA/complaints at the peak of the first wave, and not statistically significant at the peak of the second wave. Conclusion: We observed 47% reduction in GP consultations for musculoskeletal disorders during the first wave and 9% during the second wave. For hip and knee OA/complaints, the reductions were over 50% during the first, and 10% during the second wave. This disruption may lead to accumulation of patients with severe OA symptoms and more requests for arthroplasty surgery.

Abstract

We present a new concept for marine research, applied in the EU-funded project EMERGE, “Evaluation, control and Mitigation of the EnviRonmental impacts of shippinG Emissions” (2020–2024; https://emerge-h2020.eu/). For the first time, both the various marine and atmospheric impacts of the shipping sector have been and will be comprehensively analyzed, using a concerted modelling and measurements framework. The experimental part of the project focuses on five European geographical case studies in different ecologically vulnerable regions, and a mobile onboard case study. The EMERGE consortium has also developed a harmonised and integrated modelling framework to assess the combined impacts of shipping emissions, both (i) on the marine ecosystems and (ii) the atmospheric environment. The first results include substantial refinements of a range of models to be applied, especially those for the STEAM and OpenDrift models. In particular, the STEAM (Ship Traffic Emission Assessment Model) model has been extended to allow for the effects of atmospheric and oceanographic factors on the fuel consumption and emissions of the ships. The OpenDrift model has been improved to take into account the partitioning, degradation, and volatilization of pollutants in water. The predicted emission and discharge values have been used as input for both regional scale atmospheric dispersion models, such as WRF-CMAQ (Weather Research and Forecasting—Community Multiscale Air Quality Model) and SILAM (System for Integrated modeLling of Atmospheric composition), and water quality and circulation models, such as OpenDrift (Open source model for the drifting of substances in the ocean) and Delft3D (oceanographic model). The case study regions are Eastern Mediterranean, Northern Adriatic Sea, the Lagoon of Aveiro, the Solent Strait and the Öresund Strait. Abstract

The atmosphere and cryosphere have recently garnered considerable attention due to their role in transporting microplastics to and within the Arctic, and between freshwater, marine, and terrestrial environments. While investigating either in isolation provides valuable insight on the fate of microplastics in the Arctic, monitoring both provides a more holistic view. Nonetheless, despite the recent scientific interest, fundamental knowledge on microplastic abundance, and consistent monitoring efforts, are lacking for these compartments. Here, we build upon the work of the Arctic Monitoring and Assessment Programme’s Monitoring Guidelines for Litter and Microplastic to provide a roadmap for multi-compartment monitoring of the atmosphere and cryosphere to support our understanding of the sources, pathways, and sinks of plastic pollution across the Arctic. Overall, we recommend the use of existing standard techniques for ice and atmospheric sampling and to build upon existing monitoring efforts in the Arctic to obtain a more comprehensive pan-Arctic view of microplastic pollution in these two compartments.

Abstract

Litter and microplastic assessments are being carried out worldwide. Arctic ecosystems are no exception and plastic pollution is high on the Arctic Council’s agenda. Water and sediment have been identified as two of the priority compartments for monitoring plastics under the Arctic Monitoring and Assessment Programme (AMAP). Recommendations for monitoring both compartments are presented in this publication. Alone, such samples can provide information on presence, fate, and potential impacts to ecosystems. Together, the quantification of microplastics in sediment and water from the same region produce a three-dimensional picture of plastics, not only a snapshot of floating or buoyant plastics in the surface water or water column but also a picture of the plastics reaching the shoreline or benthic sediments, in lakes, rivers, and the ocean. Assessment methodologies must be adapted to the ecosystems of interest to generate reliable data. In its current form, published data on plastic pollution in the Arctic is sporadic and collected using a wide spectrum of methods which limits the extent to which data can be compared. A harmonised and coordinated effort is needed to gather data on plastic pollution for the Pan-Arctic. Such information will aid in identifying priority regions and focusing mitigation efforts.

28. Predicting total knee arthroplasty from ultrasonography using machine learning

Abstract

Objective: To investigate the value of ultrasonographic data in predicting total knee replacement (TKR). Design: Data from the Musculoskeletal Pain in Ullensaker study (MUST) was linked to the Norwegian Arthroplasty Register to form a 5–7 year prospective cohort study of 630 persons (69% women, mean (SD) age 64 (8.7) years). We examined the predictive power of ultrasound (US) features, i.e. osteophytes, meniscal extrusion, synovitis in the suprapatellar recess, femoral cartilage thickness, and quality for future knee osteoarthritis (OA) surgery. We investigated 4 main settings for multivariate predictive modeling: 1) clinical predictors (age, sex, body mass index, knee injury, familial OA and workload), 2) radiographic data (assessed by the Kellgren Lawrence grade, KL) with clinical predictors, 3) US features and clinical predictors. Finally, we also considered an ensemble of models 2) and 3) and used it as our fifth model. All models were compared using the Average Precision (AP) and the Area Under Receiver Operating Characteristic Curve (AUC) metrics. Results: Clinical predictors yielded AP of 0.11 (95% confidence interval [CI] 0.05–0.23) and AUC of 0.69 (0.58–0.79). Clinical predictors with KL grade yielded AP of 0.20 (0.12–0.33) and AUC of 0.81 (0.67–0.90). The clinical variables with ultrasound yielded AP of 0.17 (0.08–0.30) and AUC of 0.79 (0.69–0.86). Conclusions: Ultrasonographic examination of the knee may provide added value to basic clinical and demographic descriptors when predicting TKR. While it does not achieve the same predictive performance as radiography, it can provide additional value to the radiographic examination.

Abstract

The purpose of the guidelines is to review existing knowledge and provide guidance for designing an Arctic monitoring program that will track litter and MP. The topics of litter, plastic pollution, and MP are addressed in many fora, including several of the Arctic Council working groups: Arctic Monitoring and Assessment Programme (AMAP; https://www.amap.no/documents/doc/amap-assessment-2016-chemicals-of-emerging-arctic-concern/1624), Protection of the Marine Environment (PAME, 2019), and Conservation of the Arctic Flora and Fauna (CAFF). The development of an Arctic monitoring program and its technical approaches will be based on the work that already exists in other programs such as those of OSPAR, the Helsinki Commission (HELCOM), the International Council for the Exploration of the Sea (ICES), the Organisation for Economic Co-operation and Development (OECD), and the United Nations Environment Programme (UNEP). Plastic pollution is typically categorized into items and particles of macro-, micro-, and nano-sizes. These guidelines address macrosized litter as well as MP (< 5 mm), essentially including smaller size ranges (>1 µm). However, determination of nanoplastic (< 1 µm) particles is still hampered by technical challenges, as addressed in Section 4.3 Analytical methods, and thus not currently considered in the current recommendations. Although most studies have addressed marine litter and MP, these guidelines also comprise the Arctic’s terrestrial and freshwater environments. Thus, the objectives of the guidelines are to: 1) support litter and MP baseline mapping in the Arctic across a wide range of environmental compartments to allow spatial and temporal comparisons in the coming years; 2) initiate monitoring to generate data to assess temporal and spatial trends; 3) recommend that Arctic countries develop and implement monitoring nationally via community-based programs and other mechanisms, in the context of a pan-Arctic program; 4) provide data that can be
