[Departement_IRSTEA]Eaux [TR1_IRSTEA]RIVAGE; National audience; The aim of Recommendations theme of ERINOH’s French project was to provide three guidelines which gather and capitalize the knowledge gained during these studies. This guide suggests recommendations on reconnaissance and surveillance methods used on dikes and proposes a diagnosis methodology to detect internal erosion on long linear structures. The guide is composed of three parts. The first part is the main text and contains five chapters. Chapter 1 defines the context and stakes of maintenance of earthen dikes, reminds the French 2007 safety regulation demand, clarifies the perimeter and the objectives of the guide and indicates the public preferentially concerned by the guide. Chapter 2 suggests a short description of hydraulic structures (earthen dikes and embankments) preferentially concerned by this guide, e.g. levees and dikes. Internal erosion laws are also reminded in this chapter. Chapter 3 gives a description of material’s properties of dikes and their foundation and quickly reminds soil’s classifications methods. Relationships between physical properties of materials and geophysical measurements are also given in this chapter. Chapter 4 provides an inventory of surveillance and reconnaissance methods able to detect anomalies related to leakage or seepage, gives theirs advantages, disadvantages and limits. In this charter, we also present some new developments which will permit to improve robustness of methods or obtain some quantitative hydraulic data. Chapter 5 defines and develops an efficient diagnosis methodology for assessment of the internal erosion risk on dikes. Some erosion criteria are used to lead to the diagnosis. The second part of the guide gathers technical sheets on geophysical and geotechnical methods commonly used on dikes. The third part gathers technical sheets which synthesize onsite investigations realized during ERINOH’s program.; Le thème Recommandations du Projet National ERINOH (ERosion INterne dans les Ouvrages Hydrauliques) avait pour objectif de produire trois guides permettant de capitaliser les connaissances acquises pendant ces recherches. Le présent guide propose des recommandations sur l’emploi des méthodes de reconnaissance et de surveillance des digues et suggère une méthodologie de diagnostic de l’érosion interne pour ces ouvrages à long linéaire. Le guide est composé de trois parties. La première partie, corps de texte principal de ce guide, est structurée en cinq chapitres. Le chapitre 1 introduit le guide. Il cerne le contexte et les enjeux de la maintenance des ouvrages hydrauliques en remblai, notamment des digues, rappelle les exigences de la réglementation de 2007 et précise le contour et les objectifs du guide ainsi que le public visé. Le chapitre 2 propose une description succincte des ouvrages hydrauliques en remblai préférentiellement visés par le guide. Les lois d’érosion interne sont aussi rappelées dans ce chapitre. Le chapitre 3 aborde une description des propriétés des matériaux constituant les digues et leur fondation et rappelle succinctement les procédés de classification des sols. Les relations entre les propriétés physiques des sols et les grandeurs géophysiques mesurées sont aussi données dans ce chapitre. Le chapitre 4 propose un inventaire des méthodes de reconnaissance et de surveillance capables de détecter les anomalies dues aux fuites et précise les avantages, inconvénients et les limites d’emploi. Dans ce chapitre, nous illustrons quelques nouveaux développements qui vont permettre d’améliorer la robustesse des méthodes de détection. Le chapitre 5 développe une méthodologie de reconnaissance et de diagnostic des digues vis-à-vis du risque d’érosion interne. Des critères d’érosion sont exploités pour mener au diagnostic. La seconde partie du guide rassemble des fiches techniques descriptives des méthodes de reconnaissance géophysique et géotechnique. La troisième partie résume, sous la forme de fiches synthétiques, les essais sur sites réalisés pendant le programme ANR ERINOH.

AIMS To compare the efficacy and safety of lanoteplase, a single-bolus thrombolytic drug derived from alteplase tissue plasminogen activator, with the established accelerated alteplase regimen in patients presenting within 6 h of onset of ST elevation acute myocardial infarction. METHODS AND RESULTS 15,078 patients were recruited from 855 hospitals worldwide and randomized in a 2:1 ratio to receive either lanoteplase 120 KU. kg(-1)as a single intravenous bolus, or up to 100 mg accelerated alteplase given over 90 min. The primary end-point was all-cause mortality at 30 days and the hypothesis was that the two treatments would be equivalent. By 30 days, 6.61% of alteplase-treated patients and 6.75% lanoteplase-treated patients had died (relative risk 1.02). Total stroke occurred in 1.53% alteplase- and 1.87% lanoteplase-treated patients (ns); haemorrhagic stroke rates were 0.64% alteplase and 1.12% lanoteplase (P=0.004). The net clinical deficit of 30-day death or non-fatal disabling stroke was 7.0% and 7.2%, respectively. By 6 months, 8.8% of alteplase-treated patients and 8.7% of lanoteplase-treated patients had died. CONCLUSION Single-bolus weight-adjusted lanoteplase is an effective thrombolytic agent, equivalent to alteplase in terms of its impact on survival and with a comparable risk-benefit profile. The single-bolus regimen should shorten symptoms to treatment times and be especially convenient for emergency department or out-of-hospital administration.

- Abstract
The inherent surface charges and small diameters that confer colloidal stability to gold particle conjugates (immunogold) are detrimental to rapid cell surface labeling and distinct cluster definition in flow cytometric light scatter assays. Although the inherent immunogold surface charge prevents self aggregation when stored in liquid suspension, it also slows binding to cells to timeframes of hours and inhibits cell surface coverage. Although the small diameter of immunogold particles prevents settling when in liquid suspension, small particles have small light scattering cross sections and weak light scatter signals. We report a new, small particle lyophilized immunogold reagent that maintains activity after 42°C storage for a year and can be rapidly dissolved into stable liquid suspension for use in labelling cells with larger particle aggregates that have enhanced scattering cross section. Labeling requires less than 1 min at 20°C, which is ∼30 times faster than customary fluorescent antibody labeling. The labeling step involves neutralizing the surface charge of immunogold and creating specifically bound aggregates of gold on the cell surface. This process provides distinct side-scatter cluster separation with blue laser light at 488 nm, which is further improved by using red laser light at 640 nm. Similar comparisons using LED light sources showed less improvement with red light, thereby indicating that coherent light scatter is of significance in enhancing side-scatter cluster separation. The physical principles elucidated here for this technique are compatible with most flow cytometers; however, future studies of its clinical efficacy should be of primary interest in point-of-care applications where robust reagents and rapid results are important. © 2011 International Society for Advancement of Cytometry [ABSTRACT FROM AUTHOR]

We previously confirmed the existence of two ground-state isomers of crystal violet (CV) in alcohol (Maruyama, Y.; Ishikawa, M.; Satozono, H. J. Am. Chem. Soc.
1996 , 118, 6257). These isomers are called solvation isomers because they are differentiated by the solvation of alcohol. Here, to investigate if solvation isomers are possible in triphenylmethane (TPM) dyes other than CV, we measured transient differential absorption (ΔOD) spectra of two classes of TPM dyes: symmetric (ethyl violet and parafuchsin) and asymmetric (malachite green and brilliant green) using a femtosecond spectral hole-burning technique. The symmetric TPM dyes that we studied (including CV) showed two spectral holes in the early part of photobleaching, whereas the asymmetric dyes showed three spectral holes. The ΔOD spectra of all of the TPM dyes depended on the pump wavelength. This dependence is the key experimental observation that confirms the existence of ground-state isomers, and thus the asymmetric TPM dyes have three ground-state isomers. Solvation isomers are possible in TPM dyes in alcohol. To obtain insight into the deactivation mechanism of TPM dyes, we examined a plateau region that appears in the early part of photobleaching recovery. The plateau is evidence for the existence of an intermediate state between the lowest singlet excited state (S1 ) and the ground state. The absence of the plateau only in parafuchsin, which is due to the difference in a positive charge distribution on a nitrogen atom, suggests that the intermediate state is a distorted excited state in which a positive charge is partially shifted to a nitrogen atom in an amino group.- Published

- Abstract
Purpose: To evaluate the efficiency and safety of intravitreal implant of 0.7mg dexamathasone in visual impairment due to diabetic macular edema (DME)., Materials and Methods: This was a retrospective, multicenter, study. Seventy-four patients, with a mean age of 65 years, followed for at least 6 months (mean follow-up: 9.8 months) were included in 5 French eye clinics (P 1.5 collective). The mean systolic blood pressure was 138mmHg and the mean HbA1c was 7.2%. We monitored 2 systemic parameters: blood pressure and glycemic balance. Best-corrected visual acuity (BCVA), central retinal thickness (CRT, Spectralis OCT), intraocular pressure (IOP) and cataract progression are studied at baseline and then at 1, 2, 4 and 6 months., Results: The average CRT decrease was: 239μm at month 2 (M2) and 135μm at month 6 (M6). The mean improvement from baseline of BCVA is 8.5 letters at M2 and 7.6 letters at M6. A gain greater than 15 letters is found in 27% of patients at M6. For naive patients the BCVA is 71 letters versus 60 letters (P<0.05). Patients with a baseline CRT <500mmHg have a BCVA of 66 letters versus 57 letters (P<0.05). The mean rate injections was 1.2 at 6 months with an average of 5.4 months for reinjection. Ocular hypertension greater than 25mmHg, managed by topical treatment, is observed in 13.4% of patients. No glaucoma surgery was necessary., Conclusion: Dexamethasone has an anatomical and functional effectiveness in the treatment of DME. Outcomes for naive patients and lower CRT suggest that the duration of diabetes mellitus and previous treatments are negative factors of recovery. Side effects are rare and manageable. Ozurdex(®) seems to be a treatment for visual impairment due to DME with a favorable safety profile. - Abstract
Purpose: Optical coherence tomography has been used for many years, in particular in the field of the retinal imaging, and is a promising technique in the analysis of the anterior segment, providing a similar resolution to histology for ocular tissue., Material and Methods: We present a collection of images obtained with the OCT3 (Carl Zeiss Meditec, Dublin, CA), with parameters set to obtain good-quality images of the iris and corneal structures., Results: The OCT provides detailed analysis of the cornea, precise assessment of the dimensions and locations of corneal lesions - in the anterior-posterior plane as well as transversally or obliquely - with 10-microm precision, and the study of the iridocorneal angle and its dynamics., Discussion: Using the highly precise OCT3 for retinal imaging, it is possible to obtain good-quality anterior segment imaging, which makes the device highly versatile and thus increases reasons for acquiring this type of apparatus., Conclusions: OCT3 users should not neglect this potential of the device, already in wide use for examining the retina.

Introduction: We report a case of a 66-year-old woman presenting a central retinal artery occlusion with no cardiovascular risk factor, with assessment using supra-aortic artery ultrasonography showing total internal carotid artery thrombosis., Observation: When vascular thrombosis risk factors are absent, more in-depth assessment such as a supra-aortic artery angioscan can provide the diagnosis of arterial dissection on arterial fibromuscular dysplasia., Conclusion: Central retinal artery occlusion is a rare but severe pathology. Therefore it is very important not to neglect the etiological assessment, because it can be the revealing element of a severe pathology.
