3. The Effect of Magma Poor and Magma Rich Rifted Margins on Continental Collision Dynamics.

Abstract

The transition between non‐rifted continental lithosphere and oceanic lithosphere in rifted margins can display a wide range of characteristics, depending on the regional tectonic evolution. The velocity and duration of the rifting process as well as the geodynamic setting influence the properties and geometry of the margins, which are often grouped into two main categories: magma‐poor and magma‐rich. We show how different types of rifted margins can influence the dynamics of continental collision, focusing on the time and depth of slab break‐off after collision and the fate of margin material. We find that rifted margins have a noticeable impact on subduction dynamics, as we observe large variability in slab break‐off times and depths. In particular, the presence of a rifted margin can delay slab break‐off to up to 60 Myr after the onset of collision. Our results show that a large portion of the weak crust of magma‐poor margins is likely to detach from the subducting plate and accrete to the upper plate, while the dense and strong mafic and ultramafic component of magma‐rich margins causes most of the margin to subduct and be lost into the mantle, leaving only a small fraction of transitional and oceanic crust at the surface. Therefore, the volume of accreted material is much larger when the margin is magma‐poor than magma‐rich, which is consistent with geological observations that fossil magma‐poor rifted margins are preserved in many mountain ranges, whereas remnants of magma‐rich rifted margins are scarce. Plain Language Summary: The transition between continental and oceanic lithosphere at rifted margins in nature is not a sharp boundary, but it is gradual and often characterized by the presence of altered crust. Such margins can be either magma‐poor or magma‐rich, depending on the volume and timing of magmatism during extension; in nature, we can find traces of rifted margins in collisional mountain belts, with magma‐poor margins being more easily preserved than magma‐rich ones. The available models for continental collision, however, do not often consider the presence of such margins. We model continental collision with the presence of a magma‐poor or magma‐rich rifted margin, aiming to provide an explanation for these observations and to see how the margins can affect the subduction process. We find that the presence of a rifted margin can have a significant impact on the evolution of the subduction system, with slab break‐off being significantly delayed in all cases. In the models, we find that part of the margin is always transferred to the overriding plate, and our results show that magma‐poor margins are more likely to be recovered, matching the observations that magma‐poor margins are more easily preserved than magma‐rich. Item does not contain fulltext, Axillary lymphadenopathy is a common side effect of COVID-19 vaccination, leading to increased imaging-detected asymptomatic and symptomatic unilateral axillary lymphadenopathy. This has threatened to negatively impact the workflow of breast imaging services, leading to the release of ten recommendations by the European Society of Breast Imaging (EUSOBI) in August 2021. Considering the rapidly changing scenario and data scarcity, these initial recommendations kept a highly conservative approach. As of 2023, according to newly acquired evidence, EUSOBI proposes the following updates, in order to reduce unnecessary examinations and avoid delaying necessary examinations. First, recommendation n. 3 has been revised to state that breast examinations should not be delayed or rescheduled because of COVID-19 vaccination, as evidence from the first pandemic waves highlights how delayed or missed screening tests have a negative effect on breast cancer morbidity and mortality, and that there is a near-zero risk of subsequent malignant findings in asymptomatic patients who have unilateral lymphadenopathy and no suspicious breast findings. Second, recommendation n. 7 has been revised to simplify follow-up strategies: in patients without breast cancer history and no imaging findings suspicious for cancer, symptomatic and asymptomatic imaging-detected unilateral lymphadenopathy on the same side of recent COVID-19 vaccination (within 12 weeks) should be classified as a benign finding (BI-RADS 2) and no further work-up should be pursued. All other recommendations issued by EUSOBI in 2021 remain valid.

10. The Formation of Continental Fragments in Subduction Settings: The Importance of Structural Inheritance and Subduction System Dynamics

Abstract

Microcontinents and continental fragments are pieces of continental lithosphere, formed by extension and breakup, followed by plate boundary relocations. Microcontinents or continental fragments affiliated with passive margins are well documented, but those close to active margins are less studied. We use dynamic two- and three-dimensional numerical experiments to investigate how preexisting weaknesses within a continental upper plate affect extension and the possible formation of continental fragments. Our parametric study of the configuration (width and viscosity) of this imposed weakness indicates that stress localization and breakup of the upper plate are most efficient for narrow weak zones and a viscosity contrast between the weak zone and the surrounding crust of at least 1 order of magnitude. Moreover, upper plate extension and breakup occurs only if extension has a rotational component, here caused by the presence of a continental indenter on the downgoing plate. The width of the indenter relative to oceanic part of the downgoing plate controls differential slab pull that triggers trench retreat and upper plate deformation. A downgoing plate with a relatively large continental indenter yields large enough slab rotation to detach a continental block from the overriding plate and form wide back-arc basins. Variations in the weak zone angle with respect to the trench result in different basin geometries. We successfully modeled the first step in breakup of active continental margins and determined the settings that may facilitate microcontinent formation in a subduction framework.

Abstract

A comprehensive numerical and experimental study of the mechanism of a novel passive mode-locking technique called cascaded second-order nonlinearity mode locking (CSM) is presented. The theoretical model allows for the calculation of the nonlinear loss modulation of resonators used for CSM as a function of the resonator parameters and of the phase mismatch. The model's predictions are in good agreement with experimental results achieved with a longitudinally pumped CW Nd:YAG laser.

Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

Abstract

Aims: Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. Methods and results: Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. Conclusions: This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

Common sequence variants have recently joined rare structural polymorphisms as genetic factors with strong evidence for association with schizophrenia. Here we extend our previous genome-wide association study and meta-analysis (totalling 7 946 cases and 19 036 controls) by examining an expanded set of variants using an enlarged follow-up sample (up to 10 260 cases and 23 500 controls). In addition to previously reported alleles in the major histocompatibility complex region, near neurogranin (NRGN) and in an intron of transcription factor 4 (TCF4), we find two novel variants showing genome-wide significant association: rs2312147[C], upstream of vaccinia-related kinase 2 (VRK2) [odds ratio (OR) = 1.09, P = 1.9 × 10(-9)] and rs4309482[A], between coiled-coiled domain containing 68 (CCDC68) and TCF4, about 400 kb from the previously described risk allele, but not accounted for by its association (OR = 1.09, P = 7.8 × 10(-9)). Abstract

Increased mobilization of circulating endothelial progenitor cells may represent a new biological hallmark of myeloproliferative neoplasms. We measured circulating endothelial colony forming cells (ECFCs) in 106 patients with primary myelofibrosis, fibrotic stage, 49 with prefibrotic myelofibrosis, 59 with essential thrombocythemia or polycythemia vera, and 43 normal controls. Levels of ECFC frequency for patient's characteristics were estimated by using logistic regression in univariate and multivariate setting. The sensitivity, specificity, likelihood ratios, and positive predictive value of increased ECFC frequency were calculated for the significantly associated characteristics. Increased frequency of ECFCs resulted independently associated with history of splanchnic vein thrombosis (adjusted odds ratio = 6.61, 95% CI = 2.54-17.16), and a summary measure of non-active disease, i.e. hemoglobin of 13.8 g/dL or lower, white blood cells count of 7.8×10(9)/L or lower, and platelet count of 400×10(9)/L or lower (adjusted odds ratio = 4.43, 95% CI = 1.45-13.49) Thirteen patients with splanchnic vein thrombosis non associated with myeloproliferative neoplasms were recruited as controls. We excluded a causal role of splanchnic vein thrombosis in ECFCs increase, since no control had elevated ECFCs. We concluded that increased frequency of ECFCs represents the biological hallmark of a non-active myeloproliferative neoplasm with high risk of splanchnic vein thrombosis. The recognition of this disease category copes with the phenotypic mimicry of myeloproliferative neoplasms. Due to inherent performance limitations of ECFCs assay, there is an urgent need to arrive to an acceptable standardization of ECFC assessment.

Abstract

Item does not contain fulltext, Epidemiological and genetic data support the notion that schizophrenia and bipolar disorder share genetic risk factors. In our previous genome-wide association study, meta-analysis and follow-up (totaling as many as 18 206 cases and 42 536 controls), we identified four loci showing genome-wide significant association with schizophrenia. Here we consider a mixed schizophrenia and bipolar disorder (psychosis) phenotype (addition of 7469 bipolar disorder cases, 1535 schizophrenia cases, 333 other psychosis cases, 808 unaffected family members and 46 160 controls). Combined analysis reveals a novel variant at 16p11.2 showing genome-wide significant association (rs4583255[T]; odds ratio=1.08; P=6.6 x 10(-11)). The new variant is located within a 593-kb region that substantially increases risk of psychosis when duplicated. In line with the association of the duplication with reduced body mass index (BMI), rs4583255[T] is also associated with lower BMI (P=0.0039 in the public GIANT consortium data set; P=0.00047 in 22 651 additional Icelanders).

Abstract

Aims: To evaluate the occurrence of late and very late stent thrombosis (ST) following elective drug-eluting stent (DES) implantation in unprotected left main coronary artery (LMCA) stenosis in a large multicentre registry. Methods and results: All 731 consecutive patients who had sirolimus- or paclitaxel-eluting stent electively implanted in de novo lesions on unprotected LMCA in five centres were included. ST was defined according to Academic Research Consortium definitions. Four (0.5%) patients had a definite ST: three early (two acute and one subacute) and one late ST, no cases of very late definite ST were recorded. All patients survived from the event.
