Abstract

Background: Contemporary management of patients with synchronous colorectal cancer and liver metastases is complex. The aim of this project was to provide a practical framework for care of patients with synchronous colorectal cancer and liver metastases, with a focus on terminology, diagnosis, and management. Methods: This project was a multiorganizational, multidisciplinary consensus. The consensus group produced statements which focused on terminology, diagnosis, and management. Statements were refined during an online Delphi process, and those with 70 per cent agreement or above were reviewed at a final meeting. Iterations of the report were shared by electronic mail to arrive at a final agreed document comprising 12 key statements. Results: Synchronous liver metastases are those detected at the time of presentation of the primary tumour. The term 'early metachronous metastases' applies to those absent at presentation but detected within 12 months of diagnosis of the primary tumour, the term 'late metachronous metastases' applies to those detected after 12 months. 'Disappearing metastases' applies to lesions that are no longer detectable on MRI after systemic chemotherapy. Guidance was provided on the recommended composition of tumour boards, and clinical assessment in emergency and elective settings. The consensus focused on treatment pathways, including systemic chemotherapy, synchronous surgery, and the staged approach with either colorectal or liver-directed surgery as first step. Management of pulmonary metastases and the role of minimally invasive surgery was discussed. Conclusion: The recommendations of this contemporary consensus provide information of practical value to clinicians managing patients with synchronous colorectal cancer and liver metastases.

13. Polaron relaxation in ferroelectric thin films

Abstract

We report a dielectric relaxation in ferroelectric thin films of the ABO3 family. We have compared films of different compositions with several growth modes: sputtering (with and without magnetron) and sol-gel. The relaxation was observed at cryogenic temperature (T<100K) for frequencies from 100Hz up to 10MHz. This relaxation activation energy is always lower than 200meV. It is very similar to the polaron relaxation that we reported in the parent bulk perovskites. Being independent of the materials size, morphology and texture, this relaxation can be a useful probe of defects in actual integrated capacitors with no need for specific shaping

Abstract

Three types of BaTiO3 core - amorphous nano-shell composite ceramics were processed from the same core-shell powder by standard sintering, spark-plasma sintering and two-step sintering techniques and characterized by XRD, HRSEM and broad-band dielectric spectroscopy in the frequency range 10^3 - 10^13 Hz including the THz and IR range. The samples differed by porosity and by the amount of interdiffusion from the cores to shells, in correlation with their increasing porosity. The dielectric spectra were also calculated using suitable models based on effective medium approximation. The measurements revealed a strong dielectric dispersion below the THz range, which cannot be explained by the modeling, and whose strength was in correlation with the degree of interdiffusion. We assigned it to an effect of the interdiffusion layers, giving rise to a strong interfacial polarization. It appears that the high-frequency dielectric spectroscopy is an extremely sensitive tool for detection of any gradient layers and sample inhomogeneities even in dielectric materials with negligible conductivity.

Several Niobium oxides of formula Ba2LnFeNb4O15 (Ln = La, Pr, Nd, Sm, Eu, Gd) with the Tetragonal Tungsten Bronze (TTB) structure have been synthesised by conventional solid-state methods. The Neodymium, Samarium and Europium compounds are ferroelectric with Curie temperature ranging from 320 to 440K. The Praseodymium and Gadolinium compounds behave as relaxors below 170 and 300 K respectively. The Praseodymium, Neodymium, Samarium, Europium and Gadolinium compounds exhibit magnetic hysteresis loops at room temperature originating from traces of a barium ferrite secondary phase. The presence of both ferroelectric and magnetic hysteresis loops at room temperature allows considering these materials as composites multiferroic. Based on crystal-chemical analysis we propose some relationships between the introduction of Ln3+ ions in the TTB framework and the chemical, structural and physical properties of these materials., Comment: Revised version after identification of a spurious barium ferrite phase responsible for composite multiferroic properties at RT. Abstract

In this paper, we report the electrical and structural properties of the oxide-based metal/ferroelectric/metal (MFM) junctions. The heterostructures are composed of ultrathin layers of La0.7Ca0.3MnO3 (LCMO) as a metallic layer and, BaTiO3 (BTO) as a ferroelectric layer. Junction based devices, having the dimensions of 400 x 200 micom2, have been fabricated upon LCMO/BTO/LCMO heterostructures by photolithography and Ar-ion milling technique. The DC current-voltage (I-V) characteristics of the MFM junctions were carried out. At 300 K, the devices showed the linear (I-V) characteristics, whereas at 77 K, (I-V) curves exhibited some reproducible switching behaviours with well-defined remnant currents. The resulting resistance modulation is very different from what was already reported in ultrathin ferroelectric layers displaying resistive switching. Abstract

Extended X-ray absorption fine structure (EXAFS) experiments at the Zr K-edge were carried out on perovskite relaxor ferroelectrics BaTi(1-x)Zr(x)O3 (BTZ) (x = 0.25, 0.30, 0.35), and on BaZrO3 for comparison. Structural information up to 4.5 A around the Zr atoms is obtained, revealing that the local structure differs notably from the average Pm-3m cubic structure deduced from X-ray diffraction. In particular, our results show that the distance between Zr atoms and their first oxygen neighbors is independent of the Zr substitution rate x and equal to that measured in BaZrO3, while the X-ray cubic cell parameter increases linearly with x. Furthermore, we show that the Zr atoms tend to segregate in Zr-rich regions. We propose that the relaxor behavior in BTZ is linked to random elastic fields generated by this particular chemical arrangement, rather than to random electric fields as is the case in most relaxors., Comment: 13 pages, 12 figures, 4 tables. Abstract

In the vicinity of their resonance frequency, piezoelectric resonators are highly sensitive to small perturbations. The present report is focussed on the magnetic field as a perturbation source. First, magneto-dielectric modulation of more than 10% is achieved at room temperature on both ferroelectric single crystals and quartz discs. Since such piezoelectric resonators are now available as membranes directly integrated on Silicon wafer, we have checked the magneto-dielectric modulation in such resonators. We show here for the first time that a moderate magnetic field of 2.104 Oersteds is able to efficiently tune the impedance of these resonators in their resonance window., Comment: 11 Pages, 4 figures

Abstract

Thin films of the recently discovered magnesium diboride (MgB2) intermetalic superconducting compound have been grown using a magnetron sputtering deposition technique followed by in-situ annealing at 830 C. High quality films were obtained on both sapphire and MgO substrates. The best films showed maximum Tc = 35 K (onset), a transition width of 0.5 K, a residual resistivity ratio up to 1.6, a low temperature critical current density Jc > 1 MA/cm2 and anisotropic critical field with gamma = 2.5 close to the values obtained for single crystals. Abstract

We present a study on the power dependence of the microwave surface impedance in thin films of the novel superconductor MgB2. 500 nm thick samples exhibiting critical temperatures ranging between 26 and 38 K are synthesized by an ex-situ post-anneal of e-beam evaporated boron in the presence of an Mg vapor at 900 C. Preliminary results on films grown in situ by a high rate magnetron sputtering technique from stoichiometric MgB2 and Mg targets are also reported. Microwave measurements have been carried out employing a dielectrically loaded niobium superconducting cavity operating at 19.8 GHz and 4 K. The study shows that the electrodynamic response of MgB2 films is presently dominated by extrinsic sources of dissipation, appearing already at low microwave power, likely to be ascribed to the presence of grain boundaries and normal inclusions in the samples., Comment: 10 pages, 3 figures, proceedings of the conference EUCAS 2001, submitted to Physica C

Abstract

Aim: to assess the influence of the crown height (CH), root length (RL) and crown-to-root ratio (CRR) on the survival of te- eth subjected to surgical endodontic retreatment and classi- fied as periapically healed. Methods: a single operator performed all the endodontic mi- crosurgery interventions. The present analysis selected the te- eth classified as ‘‘complete periapical healing’’ according to the Molven-Halse-Grung scale. The periapical radiographs were analyzed by two independent calibrated examiners, who measured CH and RL in a blind manner. The CRR was calcula- ted as the ratio of the two variables CH and RL. The measure- ments were performed by comparing the post-operative radio- graphs (t0) with those taken for a previous retrospective analysis (t1) and the most recent available (t2). An independent statistician conducted a survival analysis using Kaplan-Meier plots and a log-rank test (a = 0.05). Results: thirty-eight patients were evaluated, each one contri- buting to the study with a single tooth. The mean follow-up pe- riod was 5,96 ± 3,36 years. Comparing the CRR and RL values between t1 and t2, the difference was found statistically signifi- cant (p = 0.03). Survival was improved for the teeth with roots longer than 7 mm. There were no statistically significant diffe- rences among the remaining comparisons. Conclusions: root length ≥ 7mm exhibited better chances of long-term survival. Over time, a risk of further decrease of cli- nical RL due to periodontal disease and consequent increase of CRR could be critical by a mechanical point of view. Other studies are needed.

Abstract

This study aims to describe pathologic fractures of the jaws due to cystic bone lesions. Their daily clinical practice findings are not very common, so their treatment can be challenging. In addition, endosseous benign lesions like a radicular, residual, solitary, aneurysmal cyst, and odontogenic keratocyst, might weaken the bone so that trauma, or usual chewing, could break it. We follow from the first clinical examination to the X-ray or CBCT examination and surgical management of our patients, reporting all procedures and results to explain our approach to these cases. We also reviewed the literature briefly to determine if our operative management is in line with the scientific community. Mandibular angle and body are common locations, while symphysis and condyle are less so. Young men are the most affected, and trauma is the most triggering event. Cyst enucleation, followed by fracture reduction and fixation, is the treatment of choice for our clinical team and the scientific community. We observe how impactful it is to approach the cavity of the cystic lesion from the fracture line as the surgical gold standard

Abstract

The aim of this study was to identify features mainly involved in determining the partial response (PR) to the Electrochemotherapy (ECT) in patients with recurrent and/or metastatic head and neck (HN) tumor; the identified features were also used in a decision chart in order to provide the clinician with a support tool in deciding further therapies.131 patients (186 treatment sessions) with recurrent and/or metastatic HN neoplasm were subjected to ECT. Treatment response was evaluated based on Response Evaluation Criteria in Solid Tumors (RECIST) v. 1.1 two months after the ECT. The grade of bleeding and pain before, at the end and one week after ECT treatment were evaluated. Univariate and multivariate analysis were performed to identify features involved in determining the patient PR.In the context of the univariate analysis, tumor size significantly influenced the response to ECT, with higher PR rate of 58.3%: 28 among 48 patients with lesion size ≤ 3 centimeters (p-value0.001 at Chi-square test). Pain and bleeding pre-treatment were positively correlated to PR (p-value0.001 at Chi-square test). A difference in the current flowing in the tissue during treatment was also observed in partially responsive patients, where the median current value (6.6 A) was higher than that achieved in patients that did not show PR (3.3 A). In the context of the multivariate analysis, the best performances are achieved with the BART method (accuracy of 84%). The main clinical factors to predict the partial response, among investigated features, that have shown to be considered were the pain value felt before performing the treatment and the median current delivered during the ECT treatment. A decision-making support tool to predict the patient prognosis in terms of response rate could be represented by the decision tree obtained with CART algorithm, where a pain pre-treatment more than 5 and a median delivered current not less than 2.8 A led to the prediction a partial responsive patient with an accuracy of 75%.The study confirmed that ECT is an interesting antitumoral therapy in advanced chemo- and radio-refractory HN neoplasms, able to reduce frequent symptoms and to improve the quality of life. Pain pre-treatment and delivered current are the most important variables when predicting the partial response of patients.

37. Barrier height at (Ba,Sr)TiO₃/Pt interfaces studied by photoemission

Abstract

The interface formation of Nb-doped SrTiO₃ single crystals and (Ba,Sr)TiO₃ thin films with Pt has been studied by using photoelectron spectroscopy with in situ sample preparation. For the single crystal sample, a Schottky barrier height for electrons of 0.5–0.6 eV is determined after deposition of Pt in vacuum environment. After annealing in 0.05 Pa oxygen pressure, a strong increase in the barrier height to ≥1.2 eV is observed. X-ray induced photovoltages of up to 0.7 eV are observed in this case and have to be taken into account for a proper determination of the barrier height. A subsequent annealing in vacuum reduces the barrier again. Hence, the barrier height can be reversibly switched between an oxidized state with a large barrier height and a reduced state with a low barrier height. Quantitative analysis of the barrier heights indicates that the changes are related to the changes of interfacial defect concentration. Due to the occurrence of a Ti³⁺ related signal, the defects are identified as oxygen vacancies. The same effects are observed at interfaces between Pt and (Ba,Sr)TiO₃ thin films with a smaller absolute value of the barrier height in the oxidized state of ∼1 eV. Deposition of (Ba,Sr)TiO₃ onto a metallic Pt substrate also results in a barrier height of 1.0 eV.

Abstract

Aim: the following work assessed the behavior of human Dental Pulp Stem Cells (hDPSC) on PEEK and Titanium samples, in particular their ability to adhere, proliferate and differentiate into osteoblasts. Methods: surface modifications were made to the samples by pre-established protocols of sandblasting and laser micropatterning, to modulate surface roughness. After roughening treatments, samples were divided into two groups: nontreated and treated with air-plasma cleaning process to modulate surface wettability. The hDPSCs were harvested from dental elements and, after expansion in the proper medium, were seeded on the samples to study their behavior. The analyses for the adhesion and the proliferation were performed by Scanning Electron Microscopy and Alamar Blue test, while the differentiation in osteoblasts was evaluated by measuring AlkalinePhosphatase activity. Results: all the samples tested are able to support hDPSCs adhesion, proliferation and differentiation in osteoblasts with differences depending on the material nature and surface. Laser micropatterned titanium samples, in particular, were the best performing in terms of cell differentiation. Air-plasma treatment does not affect cell adhesion and proliferation but improves cell differentiation, in particular on Titanium samples. Conclusion: PEEK and Titanium are confirmed as excellent materials to support the adhesion, the proliferation and the differentiation of the hDPSCs. The biological behavior is strong

Abstract

Aim: the aim of this study is to evaluate the efficacy of two different dosages of dexamethasone, i.e. 2 and 4 mg injected submucosally to reduce postoperative discomfort after mandibular third molar surgery. Methods: the study was conducted on 150 patients subjected to surgical removal of an impacted third molar. Patients were randomly divided into three groups: G1 (n = 50) received 4 mg/1 ml dexamethasone submucosally; G2 (n = 50) received 2 mg/0,5 ml dexamethasone submucosally; G3 (n = 50) received saline submucosally. The drug was administered after wound closure through silk sutures. Pain, swelling, trismus, analgesic consumption and operation time were comparatively evaluated at three different time point: T0 before surgery, T1 on 3rd and T2 on 7th day after surgery. The sutures were removed on the 7th day. Results: G2 showed statistically significant reduction in swelling considering male gender. The differences between the time points where statistically significant in G2 under 25 years old when trismus was analyzed. Difference between G2 and G3 was statistically significant at T1 when compared in painkillers assumption. Conclusion: it can be concluded that better post-operative values were obtained with 2 mg dexamethasone infiltration in order to reduce trismus in subjects under 25 years old. The statistical analysis showed positive effects in female patients with 4 mg dose of dexamethasone in the aperture evaluation and oedema.

Abstract

Owing to its ease of application, noninvasive nature, and safety, echocardiography is an essential imaging modality to assess cardiac function in patients affected by ischemic heart disease (IHD). Over the past few decades, we have witnessed a continuous series of evolutions in the ultrasound field that have led to the introduction of innovative echocardiographic modalities which allowed to better understand the morphofunctional abnormalities occurring in cardiovascular diseases. This article offers an overview of some of the newest echocardiographic modalities and their promising application in IHD diagnosis, risk stratification, management, and monitoring after cardiac rehabilitation.

Abstract

Aim: Pathological fractures of the maxillary bones are quite rare; they occur when a weakening of the bone is generated by unrecognized bone diseases or surgical practices and so the fracture margin results in close proximity to the lesion. In this paper we describe the different surgical management of two maxillary bone’s fractures related to cysts, in order to promote a new surgical approach. Methods: Examining the CT scan of our rst patient, a 54-year-old woman with a history of accidental trauma, not only a zygomatic-maxillary complex (ZMC) fracture was found, but also an intrasinusal cystic lesion (22.5 x 28.8 mm) that had been expanded massively in the anterior and lateral walls. At rst, alterations in cutaneous sensitivity or damage to eyesight were excluded; subsequently oral antibiotic therapy was prescribed and after 4 days a surgical session in GA was planned. During surgery we recornized that the right buccal cortex of the maxilla was deformed by the lesion. After isolation of the right infraorbital nerve, the maxillary fracture was reduced, xing it with a microplate and titanium screws. Necrotic teeth 15 and 16 were extracted (the apexes were included in the lesion) and the cyst was easily enucleated using the bone gap created by the fracture’s margin. Then the anterior maxillary wall was replaced and, in order to overcome bone fragility, a large titanium plate was xed. The procedure ended with reduction and xation of other fractures and intraoral and cutaneous sutures. Further on, a 21-year old patient came to our observation with a traumatic mandibular fracture resulted from a scuf e. The physical examination excluded paresthesia, but showed altered occlusion and mandibular excursions. OPT and CT showed a left paramedian fracture of the mandible and another one near tooth 38; moreover, as occasional nding, a cyst was detected. After 6 days during surgicery under GA, a full thickness ap, from tooth 33 to 43, was raised and a traditional approach was chosen by opening a vestibular bone trap helped by piezoelectric instrument. Within the endosseous cavity only blood and minimal residuals of soft tissue were found. Finally the bone trap was repositioned, the fractures were reduced using miniplates and xing screws and the ap was repositioned and sutured. At the end of the surgery, an intermaxillary xation was applied and maintained for 15 days. Results: Histological examination of the rst case gave a diagnosis of odontogenic cyst. At 13 days after surgical session the facial symmetry and the physiological eye mobility were restored; the surgical wound was on way of consolidation. The histological examination of the fragments found within the bone de ciency that had caused the mandibular fracture con rmed a diagnosis of SBC. The last check (within 21 days) showed: good stability of the applied titanium plates, optimal healing of hard and soft tissues and physiological mandibular movements. Conclusions: Using the same surgical time to enucleate cyst and reduce a pathological fracture has the rationale to improve postoperative morbidity and prognosis. Therefore, if the more traditional approach has its undoubted value, the opportunity to access the cystic lesions by using the gap caused by the fracture line, the way we decided to approach our rst case, should be, in our opinion, taken into serious consideration in similar clinical and surgical situations.
