Abstract

BACKGROUND: The SAfety and FEasibility of standard EVAR outside the instruction for use (SAFE-EVAR) Study was designed to define the attitude of Italian vascular surgeons towards the use of standard endovascular repair (EVAR) for infrarenal abdominal aortic aneurysm (AAA) outside the instruction for use (IFU) through a Delphi consensus endorsed by the Italian Society of Vascular and Endovascular Surgery (Società Italiana di Chirurgia Vascolare ed Endovascolare - SI CVE). METHODS : A questionnaire consisting of 26 statements was developed, validated by an 18-member Advisory Board, and then sent to 600 Italian vascular surgeons. The Delphi process was structured in three subsequent rounds which took place between April and June 2023. In the first two rounds, respondents could indicate one of the following five degrees of agreement: 1) strongly agree; 2) partially agree; 3) neither agree nor disagree; 4) partially disagree; 5) strongly disagree; while in the third round only three different choices were proposed: 1) agree; 2) neither agree nor disagree; 3) disagree. We considered the consensus reached when ≥70% of respondents agreed on one of the options. After the conclusion of each round, a report describing the percentage distribution of the answers was sent to all the participants. RESULTS: Two-hundred-forty-four (40.6%) Italian Vascular Surgeons agreed to participate the first round of the Delphi Consensus; the second and the third rounds of the Delphi collected 230 responders (94.3% of the first-round responders). Four statements (15.4%) reached a consensus in the first rounds. Among the 22 remaining statements, one more consensus (3.8%) was achieved in the second round. Finally, seven more statements (26.9%) reached a consensus in the simplified last round. Globally, a consensus was reached for almost half of the proposed statements (46.1%). Data were collected between August 2011 and February 2013, from 398 evaluable patients receiving treatment at 33 sites in the USA. This manuscript presents data collected from patients with CABP who received care in an intensive care unit (ICU) or in general medical wards (35% and 64% of evaluable patients, respectively). The majority of ICU and general medical ward patients had underlying comorbidities (78% and 74%, respectively), with structural lung disease being the most common (42% in the ICU and 40% in general medical wards). Patients admitted to the ICU had a longer duration of stay, a longer duration of symptoms before treatment, and a longer duration of ceftaroline fosamil therapy than did general medical ward patients. Most patients treated in the ICU and in general medical wards were given ceftaroline fosamil as second-line therapy (87% and 80%, respectively). The overall rate of clinical success for patients treated with ceftaroline fosamil was 68% in the ICU and 85% in the general medical wards. Clinical success for patients receiving ceftaroline fosamil as a second-line agent was 84% in the ICU and 86% in general medical wards. These findings indicate that ceftaroline fosamil is a viable treatment option for CABP, both in the ICU and in general medical wards. Keywords: CAPTURE, registry

Abstract

Background: Triple-negative breast cancer (TNBC) is a very heterogeneous disease. Several gene expression and mutation profiling approaches were used to classify it, and all converged to the identification of distinct molecular subtypes, with some overlapping across different approaches. However, a standardised tool to routinely classify TNBC in the clinics and guide personalised treatment is lacking. We aimed at defining a specific gene signature for each of the six TNBC subtypes proposed by Lehman et al. in 2011 (basal-like 1 (BL1); basal-like 2 (BL2); mesenchymal (M); immunomodulatory (IM); mesenchymal stem-like (MSL); and luminal androgen receptor (LAR)), to be able to accurately predict them. Methods: Lehman’s TNBCtype subtyping tool was applied to RNA-sequencing data from 482 TNBC (GSE164458), and a minimal subtype-specific gene signature was defined by combining two class comparison techniques with seven attribute selection methods. Several machine learning algorithms for subtype prediction were used, and the best classifier was applied on microarray data from 72 Italian TNBC and on the TNBC subset of the BRCA-TCGA data set. Results: We identified two signatures with the 120 and 81 top up- and downregulated genes that define the six TNBC subtypes, with prediction accuracy ranging from 88.6 to 89.4%, and even improving after removal of the least important genes. Network analysis was used to identify highly interconnected genes within each subgroup. Two druggable matrix metalloproteinases were found in the BL1 and BL2 subsets, and several druggable targets were complementary to androgen receptor or aromatase in the LAR subset. Abstract

The results of a multidisciplinary survey on the use of wood in Sicily are presented here. This research, based on a thorough review of the information available from numerous sources, mostly related to Socio-economic Sciences and Humanities, sheds light on the lasting, widespread and diversified use of many tens of woody species - native and cultivated - growing on the island. This work represents an initial contribution to a topic largely overlooked the national and regional literature concerning forest sciences and wood technology. Contrariwise, the very precise information available on the specific uses of wood from certain Sicilian woody species clearly testifies to the profound knowledge and high level of skills acquired not only by local craftsmen’s associations (carpenters, cabinet-makers, shipwrights, etc.) but also by local farmers and other woodworking sectors. Moreover, the use of several woody plants points to the probable impact that past use of forest resources had on the distribution of a number of tree species, many of which now rarely occur on a regional level, namely large-leaved lime trees (Tilia platyphyllos), common ashes (Fraxinus excelsior), yews (Taxus baccata) and black alders (Alnus glutinosa).

Abstract

Neoadjuvant chemotherapy is increasingly being employed in the management of breast cancer patients. Efforts and resources have been devoted over the years to the search for an optimal strategy that can improve outcomes in the neoadjuvant setting. Today, a multidisciplinary approach with the application of evidence-based medicine is considered the gold standard for the improvement of oncological results and patient satisfaction. However, several clinical complications and psychological issues due to various factors can arise during neoadjuvant therapy and undermine outcomes. To ensure that health care needs are adequately addressed, clinicians must consider that women with breast cancer have a high risk of developing “unmet needs” during treatment, and often require a clinical intervention or additional care resources to limit possible complications and psychological issues that can occur during neoadjuvant treatment. This work describes a multidisciplinary model developed at “Fondazione Policlinico Universitario Agostino Gemelli” (FPG) in Rome in an effort to optimize treatment, ease the application of evidence-based medicine, and improve patient quality of life in the neoadjuvant setting. In developing our model, our main goal was to adequately meet patient needs while preventing high levels of distress.

Abstract

The oxygen content of diamond is determined by a microactive-technique. The reaction is generated by using a tandem accelerator to bombard the target with an energetic beam. Positron emission causes fluorine to decay into oxygen. Natural types I and IIa diamonds, a yellow synthetic diamond sample and samples from which eclogitic and peridotitic mineral inclusions had been extracted were used as samples to be analyzed. Bands, indicative of C-O bonds, are not contained in the spectra, as seen by FTIR spectroscopy. The kinetics of the growing surface control the oxygen content of diamond, while the equilibrium thermodynamic conditions and crystal defect structure do not control it.

Abstract Background Distant spread from breast cancer is commonly found in bones, lungs, liver and central nervous system. Metastatic involvement of peritoneum and retroperitoneum is unusual and unexpected. Case presentation We report the case of a 67 year-old-woman who presented with gastrointestinal symptoms which revealed to be the clinical manifestations of peritoneal and retroperitoneal metastatic spread of an invasive lobular breast cancer diagnosed 15 years before. Conclusion To the best of our knowledge, the case presented is the third one reported in literature showing a wide peritoneal and extraperitoneal diffusion of an invasive lobular breast cancer. The long and complex diagnostic work up which led us to the diagnosis is illustrated, with particular emphasis on the multidisciplinary approach, which is mandatory to obtain such a result in these cases. Awareness of such a condition by clinicians is mandatory in order to make an early diagnosis and start a prompt and correct therapeutic approach.

Abstract

The mechanical characterisation of the asphalt concrete in terms of both the fatigue resistance and the stiffness modulus is necessary to use any design method of the flexible road pavements. Different kinds of test are usually used in experimental work such as bending tests, uniaxial tests, etc., but sometimes they do not give the same answer. In this paper mechanical characterization was carried out by means of fatigue tests undertaken with two most used testing machines for asphalt material: two point bending (2PB) test at IFSTTAR in Nantes (France) and four point bending (4PB) test at University of Palermo, in Palermo (Italy). Different strain controlled tests were undertaken for the same material under the same loading conditions, frequency and temperature (15 Hz and 20˚C), according to the European standard 12697 part 24 and 26. The first results of this interlaboratory activity are showed in this paper.

Abstract

Flexural fatigue due to repeated traffic loading is a process of cumulative damage and one of the main failure modes of flexible pavement structures. Typically, micro-cracks originate at the bottom of an asphalt concrete layer due to horizontal tensile strains. Micro-cracking starts to propagate towards the upper layers under repeated loading which can lead to pavement failure. Different approaches are usually used to characterise fatigue resistance in asphalt mixtures including the phenomenological approach, the fracture mechanics approach and the dissipated energy approach. This paper presents a comparison of fatigue resistance calculated for different dissipated energy models using 2 Point Bending (2PB) at IFSTTAR in Nantes. 2PB tests have been undertaken under different loading and environmental conditions in order to evaluate the properties of the mixtures (stiffness, dissipated energy, fatigue life and healing effect).

Different strain controlled tests were been undertaken for the same material under the same loading conditions, frequency and temperature (15 Hz and 20˚C), according to the European standard 12697 part 24 and 26. The first results of this interlaboratory activity are showed in this paper.
