- Abstract
The dimension of care and the educational relationship in the pre-school context has always played a very important role in the tradition of studies and pedagogical literature. In recent years, in particular, interest in this specific age group, especially in the early years, has been renewed and strengthened. This is due to the evidence offered by neuroscientific studies, but also because of the new and complex demands arising from ongoing social, cultural and legislative transformations. Indeed, numerous studies have highlighted and continue to highlight the enormous potential contained, in particular, in the first years of life of every human being and, consequently, the inestimable value of educational and care actions.

- Abstract
Chronic Obstructive Pulmonary Disease (COPD) is a globally widespread pathology that causes an irreversible obstruction of airflow and it is associated with low levels of emotional and psychological well-being. In this context, Virtual Reality (VR) enhanced with savoring, that is the ability to generate and intensify positive emotions, can be employed to promote well-being and relaxation in patients with COPD. This ongoing study aims to investigate the feasibility of a two-week VR-based relaxation intervention enhanced with savoring in COPD patients in increasing emotional and psychological well-being, positive emotions, relaxation, oxygen saturation levels (SpO2) and decreasing negative emotions. Seventeen hospitalized patients of the Pulmonary Rehabilitation Unit of IRCCS INRCA Casatenovo have been enrolled so far in this randomized controlled trial study. Together with the traditional pulmonary rehabilitation, the experimental group took part in the four-session VR-based intervention, while the active control group listened to relaxing music. In each session, the experimental group experienced a relaxing virtual scenario, and then participants completed a savoring exercise. Both groups complete the questionnaires before and after the intervention, and before and after each session. Independent samples t-tests based on delta scores revealed significant differences between the two groups in positive emotions and psychological well-being, suggesting that VR enhanced with savoring can be a promising path for promoting well-being in COPD patients. Indeed, savoring can amplify the positive effects of VR and create a connection with the positive experiences in patients' lives. Consequently, participants can build positive resources to counteract the harmful psychological effects caused by COPD.

- Abstract
Background and aims: The potential impact of coronary atherosclerosis, as detected by coronary artery calcium, on clinical outcomes in COVID-19 patients remains unsettled. We aimed to evaluate the prognostic impact of clinical and subclinical coronary artery disease (CAD), as assessed by coronary artery calcium score (CAC), in a large, unselected population of hospitalized COVID-19 patients undergoing non-gated chest computed tomography (CT) for clinical practice. Methods: SARS-CoV 2 positive patients from the multicenter (16 Italian hospitals), retrospective observational SCORE COVID-19 (calcium score for COVID-19 Risk Evaluation) registry were stratified in three groups: (a) “clinical CAD” (prior revascularization history), (b) “subclinical CAD” (CAC >0), (c) “No CAD” (CAC = 0). Primary endpoint was in-hospital mortality and the secondary endpoint was a composite of myocardial infarction and cerebrovascular accident (MI/CVA). Results: Amongst 1625 patients (male 67.2%, median age 69 [interquartile range 58–77] years), 31%, 57.8% and 11.1% had no, subclinical and clinical CAD, respectively. Increasing rates of in-hospital mortality (11.3% vs. 27.3% vs. 39.8%, p < 0.001) and MI/CVA events (2.3% vs. 3.8% vs. 11.9%, p < 0.001) were observed for patients with no CAD vs. subclinical CAD vs clinical CAD, respectively. The association with in-hospital mortality was independent of in-study outcome predictors (age, peripheral artery disease, active cancer, hemoglobin, C-reactive protein, LDH, aerated lung volume): subclinical CAD vs. No CAD: adjusted hazard ratio (adj-HR) 2.86 (95% confidence interval [CI] 1.14–7.17, p=0.025); clinical CAD vs. No CAD: adj-HR 3.74 (95% CI 1.21–11.60, p=0.022). Among patients with subclinical CAD, increasing CAC burden was associated with higher rates of in-hospital mortality (20.5% vs. 27.9% vs. 38.7% for patients with CAC score thresholds≤100, 101–400 and > 400, respectively, p < 0.001). Tra le molteplici definizioni di “benessere”, pare utile, ai fini del nostro discorso, considerare quella che lo ritiene come “uno stato in cui l’individuo realizza le proprie capacità, può far fronte alle normali tensioni della vita, può lavorare in modo produttivo e fruttuoso, ed è in grado di dare un contributo alla sua comunità” , indipendentemente dalla presenza o assenza di una condizione di fragilità. A partire da tale prospettiva, si comprende pertanto, come esso debba essere inteso sempre di più come fine, ma anche come mezzo sia per la promozione di processi di apprendimento adeguati, sia per il raggiungimento di obiettivi esistenziali, creando un circolo virtuoso in cui le varie dinamiche relazionali possano influenzarsi reciprocamente in vista di un obiettivo comune: lo star bene di tutti., Among many definitions of "well-being," it seems useful, for the purposes of our discussion, to consider it as "a state in which the individual realizes his or her abilities, can cope with the normal stresses of life, can work productively and fruitfully, and is able to make a contribution to his or her community" , regardless of the presence or absence of a frailty condition. From this perspective, it is therefore understood, how it should be understood more and more as an end, but also as a means both for the promotion of adequate learning processes and for the achievement of existential goals, creating a virtuous circle in which the various relational dynamics can influence each other in view of a common goal: the well-being of all.

- Abstract
Rationale: In the exploratory Phase II STEM-AMI (Stem Cells Mobilization in Acute Myocardial Infarction) trial, we reported that early administration of G-CSF (granulocyte colony-stimulating factor), in patients with anterior ST-segment-elevation myocardial infarction and left ventricular (LV) dysfunction after successful percutaneous coronary intervention, had the potential to significantly attenuate LV adverse remodeling in the long-Term. Objective: The STEM-AMI OUTCOME CMR (Stem Cells Mobilization in Acute Myocardial Infarction Outcome Cardiac Magnetic Resonance) Substudy was adequately powered to evaluate, in a population showing LV ejection fraction ≤45% after percutaneous coronary intervention for extensive ST-segment-elevation myocardial infarction, the effects of early administration of G-CSF in terms of LV remodeling and function, infarct size assessed by late gadolinium enhancement, and myocardial strain. Methods and Results: Within the Italian, multicenter, prospective, randomized, Phase III STEM-AMI OUTCOME trial, 161 ST-segment-elevation myocardial infarction patients were enrolled in the CMR Substudy and assigned to standard of care (SOC) plus G-CSF or SOC alone. In 119 patients (61 G-CSF and 58 SOC, respectively), CMR was available at baseline and 6-month follow-up. Paired imaging data were independently analyzed by 2 blinded experts in a core CMR lab. The 2 groups were similar for clinical characteristics, cardiovascular risk factors, and pharmacological treatment, except for a trend towards a larger infarct size and longer symptom-To-balloon time in G-CSF patients. ANCOVA showed that the improvement of LV ejection fraction from baseline to 6 months was 5.1% higher in G-CSF patients versus SOC (P=0.01); concurrently, there was a significant between-group difference of 6.7 mL/m2 in the change of indexed LV end-systolic volume in favor of G-CSF group (P=0.02). Uno scenario, già di per sé non privo di nodi critici e di sfide quello con cui la scuola italiana si è presentata alle porte del mese di marzo 2020, quando si è trovata a dover fare i conti con la necessità di ripensare, in modo improvviso, ed anche improvvisato, la struttura alla base di quella che, tutto sommato, poteva essere percepita come una rassicurante quotidianità, e di accelerare il ricorso ad alcuni strumenti, strategie, approcci didattici ed organizzativi appartenenti, fino a pochi giorni prima, solo ad immaginari ed orizzonti avveniristici. Tutto ciò che ne è seguito, e che si è inevitabilmente tradotto in un nuovo, significativo, capitolo della storia dell’insegnamento, tuttora ancora aperto, pare essere perlopiù noto. O forse solo in parte, se è vero, come alcuni ritengono , che ad oggi è possibile tracciare un bilancio parziale di quello che tutto ciò ha comportato, soprattutto per alcuni bambini e ragazzi, sul piano delle conoscenze e degli obiettivi didattici e, forse ancor più, delle competenze emotive, comunicative, relazionali e delle opportunità di crescita all’interno di un contesto sociale adeguato ed intenzionalmente predisposto

- Abstract
Background: Coronavirus disease 2019 (COVID-19) has spread worldwide determining dramatic impacts on healthcare systems. Early identification of high-risk parameters is required in order to provide the best therapeutic approach. Coronary, thoracic aorta and aortic valve calcium can be measured from a non-gated chest computer tomography (CT) and are validated predictors of cardiovascular events and all-cause mortality. However, their prognostic role in acute systemic inflammatory diseases, such as COVID-19, has not been investigated. Objectives: The aim was to evaluate the association of coronary artery calcium and total thoracic calcium on in-hospital mortality in COVID-19 patients. Methods: 1093 consecutive patients from 16 Italian hospitals with a positive swab for COVID-19 and an admission chest CT for pneumonia severity assessment were included. At CT, coronary, aortic valve and thoracic aorta calcium were qualitatively and quantitatively evaluated separately and combined together (total thoracic calcium) by a central Core-lab blinded to patients’ outcomes. Results: Non-survivors compared to survivors had higher coronary artery [Agatston (467.76 ± 570.92 vs 206.80 ± 424.13 mm2, p < 0.001); Volume (487.79 ± 565.34 vs 207.77 ± 406.81, p < 0.001)], aortic valve [Volume (322.45 ± 390.90 vs 98.27 ± 250.74 mm2, p < 0.001; Agatston 337.38 ± 414.97 vs 111.70 ± 282.15, p < 0.001)] and thoracic aorta [Volume (3786.71 ± 4225.57 vs 1487.63 ± 2973.19 mm2, p < 0.001); Agatston (4688.82 ± 5363.72 vs 1834.90 ± 3761.25, p < 0.001)] calcium values. Coronary artery calcium (HR 1.308; 95% CI, 1.046–1.637, p = 0.019) and total thoracic calcium (HR 1.975; 95% CI, 1.200–3.251, p = 0.007) resulted to be independent predictors of in-hospital mortality. Conclusion: Coronary, aortic valve and thoracic aortic calcium assessment on admission non-gated CT permits to stratify the COVID-19 patients in-hospital mortality risk.

Emerge, in misura crescente negli ultimi anni, la necessità di indagare modalità attraverso le quali favorire l’attivazione di forme di generatività sociale in grado di promuovere e valorizzare quel bagaglio di competenze, risorse e potenzialità insite in molte famiglie con figli con disabilità. E’ evidente come tale affermazione non debba tradursi in un atteggiamento di ingenuo ottimismo o di irri-spettoso misconoscimento delle tante ed inevitabili fatiche che l'arrivo di un bambino con difficoltà spesso di per sé comporta, ma sia piuttosto da intendere come lente di in-grandimento mediante la quale osservare quei piccoli, ma in realtà grandi, tesori che si nascondono nei gesti di cura quotidianamente agiti da molti genitori

Il contributo, partendo da un affondo sull'istituzione familiare quale luogo per la crescita armonica di ciascun bambino e diritto imprescindibile, approfondisce inizialmente i concetti di ciclo di vita personale e familiare che si intrecciano nella definizione della storia di vita di ogni individuo e i caratteri delle crisi di sviluppo, necessarie per la formazione della persona e delle crisi accidentali, impreviste, che possono creare arresti e ripartenze nei percorsi di vita di ciascuno. Successivamente focalizza l'attenzione sulla disabilità in quanto evento critico, che può sconvolgere equilibri, scardinare routine e ritmi di vita, vivere fasi di adattamento, disvelare potenzialità impreviste, aprire a nuovi orizzonti e percorsi di vita. La costruzione di un autentico progetto di vita, frutto del lavoro congiunto di tutti, in cui la persona con disabilità e la sua famiglia siano poste al centro di una rete sociale dinamica, aperta al confronto e foriera di opportunità per tutti diventa pertanto l'obiettivo prioritario di tutti coloro che si occupano della promozione di processi educativi, di accompagnamento, supporto e sostegno.

In the exploratory Phase II STEM-AMI (Stem Cells Mobilization in Acute Myocardial Infarction) trial, we reported that early administration of G-CSF (granulocyte colony-stimulating factor), in patients with anterior ST-segment-elevation myocardial infarction and left ventricular (LV) dysfunction after successful percutaneous coronary intervention, had the potential to significantly attenuate LV adverse remodeling in the long-term.The STEM-AMI OUTCOME CMR (Stem Cells Mobilization in Acute Myocardial Infarction Outcome Cardiac Magnetic Resonance) Substudy was adequately powered to evaluate, in a population showing LV ejection fraction ≤45% after percutaneous coronary intervention for extensive ST-segment-elevation myocardial infarction, the effects of early administration of G-CSF in terms of LV remodeling and function, infarct size assessed by late gadolinium enhancement, and myocardial strain.Within the Italian, multicenter, prospective, randomized, Phase III STEM-AMI OUTCOME trial, 161 ST-segment-elevation myocardial infarction patients were enrolled in the CMR Substudy and assigned to standard of care (SOC) plus G-CSF or SOC alone. In 119 patients (61 G-CSF and 58 SOC, respectively), CMR was available at baseline and 6-month follow-up. Paired imaging data were independently analyzed by 2 blinded experts in a core CMR lab. The 2 groups were similar for clinical characteristics, cardiovascular risk factors, and pharmacological treatment, except for a trend towards a larger infarct size and longer symptom-to-balloon time in G-CSF patients. ANCOVA showed that the improvement of LV ejection fraction from baseline to 6 months was 5.1% higher in G-CSF patients versus SOC (P=0.01); concurrently, there was a significant between-group difference of 6.7 mL/mEarly administration of G-CSF exerted a beneficial effect on top of SOC in patients with LV dysfunction after extensive ST-segment-elevation myocardial infarction in terms of global systolic function, adverse remodeling, scar size, and myocardial strain.URL: https://www.clinicaltrials.gov. - Abstract
Il paradigma della complessità permea in tutti i contesti di vita della persona e pertanto anche in quelli che si occupano di progettare percorsi educativi e formativi lungo tutto l’arco della vita. La formazione si riferisce all’agire concreto della persona, non esclusivamente finalizzato solo all'attività produttiva; esso deve essere considerato come risorsa permanente per la crescita e il benessere di ogni persona. Formare l’adulto non vuol dire semplicemente istruirlo, dargli la possibilità di avere gli strumenti professionali per essere in grado di svolgere la propria attività lavorativa, ma offrirgli l’opportunità, l’occasione di ripercorrere la personale esperienza, valorizzandola ed interpretandola per poter generare cambiamento. Essere in grado di rileggere i propri vissuti sia personali sia lavorativi, saper riconoscere le proprie competenze ed abilità e saper identificare le proprie criticità diventano quindi gli obiettivi principali che qualsiasi processo formativo dovrebbe consentire di raggiungere. Innovare da un punto di vista educativo e didattico significa, pertanto, proprio questo: promuovere un processo riflessivo individuale, supportato dal gruppo/équipe di appartenenza, che sappia accompagnare ciascun educatore verso una maggiore comprensione sia dei propri bisogni, sia di quelli dei soggetti, compresi quelli più fragili e vulnerabili, a cui si rivolge il proprio intervento educativo.

Il benessere professionale è ritenuto, in misura crescente, un elemento fondamentale sul quale ogni realtà, pubblica o privata, è chiamata a far leva, sia per implementare la produttività in termini di efficacia e di efficienza, sia per consentire la realizzazione di ogni operatore coinvolto, come riconosciuto anche dalla specifica normativa sulla valutazione del rischio stress lavoro-correlato (D. Lgls 81/08). È evidente come tutto ciò assuma significati e valori ulteriori se applicato a quelle realtà lavorative, tra cui strutture educative quali gli asili nido, che potrebbero essere definite generatori di output complessi (Borzaga, Musella, 2003), ossia produttori di beni e servizi relazionali e per i quali la qualità delle risorse umane costituisce l’effettivo fattore critico di successo.

Aims Patients with acute coronary syndromes (ACSs) who are managed without coronary revascularization represent a mixed and understudied population that seems to receive suboptimal pharmacological treatment. Methods and results We assessed patterns of antithrombotic therapies employed during the hospitalization and in-hospital clinical events of medically managed patients with ACS enrolled in the prospective, multicentre, nationwide EYESHOT (EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units) registry. Among the 2585 consecutive ACS patients enrolled in EYESHOT, 783 (30.3%) did not receive any revascularization during hospital admission. Of these, 478 (61.0%) underwent coronary angiography (CA), whereas 305 (39.0%) did not. The median GRACE and CRUSADE risk scores were significantly higher among patients who did not undergo CA compared with those who did (180 vs. 145, P < 0.0001 and 50 vs. 33, P < 0.0001, respectively). Antithrombotic therapies employed during hospitalization significantly differ between patients who received CA and those who did not with unfractioned heparin and novel P2Y12 inhibitors more frequently used in the first group, and low-molecular-weight heparins and clopidogrel in the latter group. During the index hospitalization, patients who did not receive CA presented a higher incidence of ischaemic cerebrovascular events and of mortality compared with those who underwent CA (1.6 vs. 0.2%, P = 0.04 and 7.9 vs. 2.7%, P = 0.0009, respectively). Conclusion Almost one-third of ACS patients are managed without revascularization during the index hospitalization. In this population, a lower use of recommended antiplatelet therapy and worse clinical outcome were observed in those who did not undergo CA when compared with those who did. - Abstract
The family plays a very important role in the education process of a child with disability. In recent years, studies have increasingly highlighted its significant role not only in terms of care, but also as a protagonist of a network of support services. The whole community is, therefore, called upon to take responsibility in front of the awareness that family is central and needs support: only through becoming capable of listening and understanding the parents’ needs, the community will offer support to rebuild the parental identity and to accompany the child with disability along the life path. One of the main resources is certainly the family association reality, a form of solidarity and support, which in recent decades is acquiring a growing relevance and social visibility. The group dimension, which is essential for every human being, thus acquires an higher value when the link between its members is represented by experiences of struggle and crisis: the group becomes the place where the family is listened and respected, in cooperation and solidarity with the community providing support and solutions. Based on these aspects, the article aims to analyze some emerging dimensions in recent pedagogical studies on family education in the presence of disability or frailty condition

- Abstract
Il benessere lavorativo ha acquisito in tempi recenti una rilevanza sempre maggiore in ambito scientifico e normativo. Il D.Lgs 81/08, richiamando l'accordo europeo dell'8 ottobre 2004, ha posto l'attenzione su un tema così complesso, rendendo obbligatoria in ogni contesto lavorativo l'analisi del livello di benessere dei professionisti e le possibili azioni di miglioramento. Differenti sono i lavori di ricerca che hanno indagato, a livello nazionale ed internazionale, il costrutto della demotivazione al lavoro e delle sue possibili implicazioni. Tuttavia, nel contesto italiano risulta essere ridotto, il numero di studi dedicati alla comprensione dello stato di benessere di educatori e docenti che operano in ambito prescolare. In tale ottica si inserisce il presente progetto di ricerca-azione, realizzato dal Centro Studi e Ricerche sulla Disabilità e la Marginalità dell'Università Cattolica del Sacro Cuore, in collaborazione con Gemeaz Elior, nella sua divisione di servizi educativi per la prima infanzia. L'indagine ha avuto lo scopo di rilevare il livello di benessere individuale di 98 professionisti che prestano servizio all'interno del network di nidi e di identificare possibili strategie di miglioramento organizzativo, specifiche per ciascuna struttura. Lo studio, anche sulla base della rilevazione dei bisogni formativi, ha favorito la riflessione in merito allo sviluppo di interventi pedagogici ad hoc, a supporto del lavoro di cura nella fascia 0-3 anni.

Aims In the last decades, several new therapies have emerged for the treatment of acute coronary syndromes (ACS). We sought to describe real-world patterns of use of antithrombotic treatments in the catheterization laboratory for ACS patients undergoing percutaneous coronary interventions (PCI). Methods EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units was a nationwide, prospective registry aimed to evaluate antithrombotic strategies employed in ACS patients in Italy. Results Over a 3-week period, a total of 2585 consecutive ACS patients have been enrolled in 203 cardiac care units across Italy. Among these patients, 1755 underwent PCI (923 with ST-elevation myocardial infarction and 832 with non-ST-elevation ACS). In the catheterization laboratory, unfractioned heparin was the most used antithrombotic drug in both ST-elevation myocardial infarction (64.7%) and non-ST-elevation ACS (77.5%) undergoing PCI and, as aspirin, bivalirudin and glycoprotein IIb/IIIa inhibitors (GPIs) more frequently employed before or during PCI compared with the postprocedural period. Any crossover of heparin therapy occurred in 36.0% of cases, whereas switching from one P2Y12 inhibitor to another occurred in 3.7% of patients. Multivariable analysis yielded several independent predictors of GPIs and of bivalirudin use in the catheterization laboratory, mainly related to clinical presentation, PCI complexity and presence of complications during the procedure. Conclusion In our contemporary, nationwide, all-comers cohort of ACS patients undergoing PCI, antithrombotic therapies were commonly initiated before the catheterization laboratory. In the periprocedural period, the most frequently employed drugs were unfractioned heparin, leading to a high rate of crossover, followed by GPIs and bivalirudin, mainly used during complex PCI. Item does not contain fulltext BACKGROUND: Bococizumab is a humanized monoclonal antibody that inhibits proprotein convertase subtilisin-kexin type 9 (PCSK9) and reduces levels of low-density lipoprotein (LDL) cholesterol. We sought to evaluate the efficacy of bococizumab in patients at high cardiovascular risk. METHODS: In two parallel, multinational trials with different entry criteria for LDL cholesterol levels, we randomly assigned the 27,438 patients in the combined trials to receive bococizumab (at a dose of 150 mg) subcutaneously every 2 weeks or placebo. The primary end point was nonfatal myocardial infarction, nonfatal stroke, hospitalization for unstable angina requiring urgent revascularization, or cardiovascular death; 93% of the patients were receiving statin therapy at baseline. The trials were stopped early after the sponsor elected to discontinue the development of bococizumab owing in part to the development of high rates of antidrug antibodies, as seen in data from other studies in the program. The median follow-up was 10 months. RESULTS: At 14 weeks, patients in the combined trials had a mean change from baseline in LDL cholesterol levels of -56.0% in the bococizumab group and +2.9% in the placebo group, for a between-group difference of -59.0 percentage points (P/=70 mg per deciliter [1.8 mmol per liter] and the median follow-up was 7 months), major cardiovascular events occurred in 173 patients each in the bococizumab group and the placebo group (hazard ratio, 0.99; 95% confidence interval [CI], 0.80 to 1.22; P=0.94). In the higher-risk, longer-duration trial (in which the patients had a baseline LDL cholesterol level of >/=100 mg per deciliter [2.6 mmol per liter] and the median follow-up was 12 months), major cardiovascular events occurred in 179 and 224 patients, respectively (hazard ratio, 0.79; 95% CI, 0.65 to 0.97; P=0.02). The hazard ratio for the primary end point in the combined trials was 0.88 (95% CI, 0.76 to 1.02; P=0.08). Aims: The objective of the study is to determine the demographics and the in-hospital outcome of diabetic and non-diabetic patients treated with percutaneous coronary interventions (PCI) in Europe, to report the type of equipment and technology used for PCI procedures in diabetics and to clarify whether the treatment of diabetic patients complies with current European Society of Cardiology (ESC) guidelines. Methods and results: A total of 14,458 patients treated with PCI were enrolled from 29 member countries of the ESC between June 2005 and January 2006. Data were collected on patient characteristics and treatment, using new Cardiology Audit and Registration Data standards. In total, 3,603 patients (24.9%) were diabetic. Diabetics were older, more often female and had a higher body mass index than non-diabetics. Diabetics had higher rates of hypercholesterolaemia and hypertension, while current smokers were more frequent in the non-diabetics. Diabetics also had significantly higher rates of previous cardiovascular events. Clopidogrel was administered only in 48.1% of diabetic patients before PCI, while IIb/IIIa inhibitors were 22.9% during PCI. At discharge, there was a major adjustment of treatment with increases in the use of Beta-blocker (80.4%), angiotensin converting enzyme inhibitor (ACEI, 71.3%) and statins (89.8%) compared with on admission (Beta-blocker 60.9%, ACEI 55.0%, statin 63.1%). Inhospital mortality was higher in diabetics (1.8% vs 1.2%) although the in-hospital MACCE rate was not significantly different (3.6% vs 3.0%, p=0.09). Conclusions: Diabetic patients treated with PCI were older with more comorbidity. According to ESC guideline, the under-usage of clopidogrel, GP IIb/IIIa inhibitors should be improved. PCI is now taken as a good opportunity to adjust the use of appropriate medication. © Europa Edition. All rights reserved.

29. ROLE OF VERY LOW DOSE STATIN THERAPY IN PCSK9 INIB. TREATED PATIENTS

Female patient 62 years old Cardiovascular risk factors: Arterial hypertension, smoking, obesity, dyslipidemia, hypercholesterolemia, family history of cardiovascular diseases (father who died from miocardial infarction (MI) and brother with 3 MI). Intolerance to various statins (myalgia) Non–critical atherosclerosis of the supra–aortic trunks. Therapy with PCSK9 inib. started. (evolocumab 140 mg every 2 weeks). Subsequently the patient suffers from acute coronary syndrome (NSTEMI). CACG: fusiform aneurysm of proximal left anterior descending artery (LAD) The new LDL target to be reached < 55 mg/dl is far from the value achieved with the co–administration of PCSK9 inib. and ezetimibe (minimum LDL value reached with this therapy 123 mg/dl) Even with a certain time delay it was decided to introduce a statin at an extremely low dose (rosuvastatin 5 mg 3 times a week). The effect of this strategy is very effective, allowing a reduction in LDL by over 50% and up to a value of 60 (only slightly higher than the target value). However, the patient again suffers from myalgia (without an increase in CPK) and decides to suspend treatment with a new increase in LDL values to 112 mg/dl. The case emphasizes: 1) the synergistic effect of statin with PCSK9 inhibitor therapy. This synergy is already evident with extremely low doses of statin and should always be sought 2) the usefulness of an aggressive drug therapy that tends to bring LDL values within the therapeutic targets in the shortest possible time. Moreover this could highlight intolerances that make it impossible to continue with a specific therapeutic scheme so as to allow the study of new options.

- Abstract
The growing attention, on a scientific, cultural and social level, that the world of early childhood services has acquired in recent decades has contributed to encouraging reflection and incentivising research regarding the potential factors capable of significantly influencing the promotion of levels of quality which, broadly speaking, characterise such structures. It is thus essential to understand which aspects are particularly important in relation to the distinctive features of the crèche environment and to the professional identity of the educators offering their services. CeDisMa (The Research and Study Centre on Disability and Marginality of the Catholic University of the Sacred Heart) and the association Gemeaz Elior devised and created the research-action project «Benessere lavorativo nella realtà Pulcini & Co.® e supporto psicopedagogico per la prevenzione del burnout» (Well-being at work at Pulcini & Co.® and psychopedagogic support for preventing burnout). Interpreting it from a pedagogical viewpoint this article therefore aims to examine potential strategies and actions for improvement in order to facilitate the creation of a suitable relational and organisation environment within crèches.

BACKGROUND: Current fibrinolytic therapies fail to achieve optimum reperfusion in many patients. Low-molecular-weight heparins and platelet glycoprotein IIb/IIIa inhibitors have shown the potential to improve pharmacological reperfusion therapy. We did a randomised, open-label trial to compare the efficacy and safety of tenecteplase plus enoxaparin or abciximab, with that of tenecteplase plus weight-adjusted unfractionated heparin in patients with acute myocardial infarction. METHODS: 6095 patients with acute myocardial infarction of less than 6 h were randomly assigned one of three regimens: full-dose tenecteplase and enoxaparin for a maximum of 7 days (enoxaparin group; n=2040), half-dose tenecteplase with weight-adjusted low-dose unfractionated heparin and a 12-h infusion of abciximab (abciximab group; n=2017), or full-dose tenecteplase with weight-adjusted unfractionated heparin for 48 h (unfractionated heparin group; n=2038). The primary endpoints were the composites of 30-day mortality, in-hospital reinfarction, or in-hospital refractory ischaemia (efficacy endpoint), and the above endpoint plus in-hospital intracranial haemorrhage or in-hospital major bleeding complications (efficacy plus safety endpoint). Analysis was by intention to treat. FINDINGS: There were significantly fewer efficacy endpoints in the enoxaparin and abciximab groups than in the unfractionated heparin group: 233/2037 (11.4%) versus 315/2038 (15.4%; relative risk 0.74 [95% CI 0.63-0.87], p=0.0002) for enoxaparin, and 223/2017 (11.1%) versus 315/2038 (15.4%; 0.72 [0.61-0.84], p

Intubation is necessary in 7 to 20% of patients with severe acute cardiogenic pulmonary oedema despite optimal treatment. This study evaluated the usefulness of parameters largely available in clinical practice to predict the need for intubation in a population of acute cardiogenic pulmonary oedema patients treated with medical therapy and continuous positive airway pressure. The present retrospective cohort study involved 142 patients with severe acute cardiogenic pulmonary oedema who were admitted to coronary care or the intensive care unit of a university hospital and were treated by an in-hospital protocol. Physiological measurements and blood gas samples were evaluated at 'baseline' (just after admission), 'early' (one to three hours after beginning treatment) and 'late' (eight to 10 hours after beginning treatment). Twenty-two patients (15.5%) required intubation. A systolic blood pressure at admission lower than 140 mmHg was significantly associated with a higher risk for intubation, while hypercapnic patients or those with a reduced left ventricular ejection fraction at admission did not show a worse prognosis. A simple score based on largely available parameters (1 point for each: age >78 years, systolic blood pressure <140 mmHg at admission, arterial blood gas acidosis and heart rate >95 bpm at early time) is proposed. The rate of intubation according to this score ranged from 0% (score of 0) to 90% (score of 3). Our study found that simple parameters available in clinical practice are significantly associated with the need for intubation in acute cardiogenic pulmonary oedema patients treated with continuous positive airway pressure and medical therapy. A simple score to evaluate the need for endotracheal intubation is proposed.

A multicentre, randomised, open trial with a 2 x 2 factorial design was conducted to compare the benefits and risks of two thrombolytic agents, streptokinase (SK, 1·5 MU infused intravenously over 30-60 min) and alteplase (tPA, 100 mg infused intravenously over 3 h) in patients with acute myocardial infarction admitted to coronary care units within 6 h from onset of symptoms. The patients were also randomised to receive heparin (12 500 U subcutaneously twice daily until discharge from hospital, starting 12 h after beginning the tPA or SK infusion) or usual therapy. All patients without specific contraindications were given atenolol (5-10 mg iv) and aspirin (300-325 mg a day). The end-point of the study was the combined estimate of death plus severe left ventricular damage. 12 490 patients were randomised to four treatment groups (SK alone, SK plus heparin, tPA alone, tPA plus heparin). No specific differences between the two thrombolytic agents were detected as regards the combined end-point (tPA 23·1%; SK 22·5%; relative risk 1·04, 95% Cl 0·95-1·13), nor after the addition of heparin to the aspirin treatment (hep 22·7%, no hep 22·9%; RR 0·99, 95% Cl 0·91-1·08). The outcome of patients allocated to the four treatment groups was similar with respect to baseline risk factors such as age, Killip class, hours from onset of symptoms, and site and type of infarct. The rates of major in-hospital cardiac complications (reinfarction, post-infarction angina) were also similar. The incidence of major bleeds was significantly higher in SK and heparin treated patients (respectively, tPA 0·5%, SK 1·0%, RR 0·57, 95% Cl 0·38-0·85; hep 1·0%, no hep 0·6%, RR 1·64, 95% Cl 1·09-2·45), whereas the overall incidence of stroke was similar in all groups. SK and tPA appear equally effective and safe for use in routine conditions of care, in all infarct patients who have no contraindications, with or without post-thrombolytic heparin treatment. The 8·8% hospital mortality of the study population (compared with approximately 13% in the control cohort of the GISSI-1 trial) indicates the beneficial impact of the proven acute treatments for AMI.

- Abstract
Cirrhosis is associated with cardiovascular abnormalities. Scanty information is available as to whether these include left ventricle diastolic dysfunction and wall thickness increase. To this aim in 27 cirrhotic patients with tense ascites, 17 cirrhotic patients with previous episodes of ascites (not actual), and 11 controls we investigated by echocardiography and echocolor Doppler left ventricle diastolic function (E wave, A wave, E/A ratio, deceleration time of E wave), systolic function (ejection fraction), and wall thickness (left ventricle posterior wall thickness + interventricular septum thickness) along with neurohumoral variables. All measurements (supine position) were repeated after total paracentesis (10.7 +/- 0.6 L of ascites) in ascitic patients. Both in patients with and without ascites E/A ratio was reduced as compared with controls (0.93 +/- 0.07 and 0.97 +/- 0.06 vs. 1.18 +/- 0.08, P < .05) while left ventricle wall thickness was increased (18.6 +/- 0.6 and 20.1 +/- 0.8 vs. 17.2 +/- 0.7, P < .05 and P < .01, respectively), irrespective of the postviral or alcoholic cause of liver disease. In all cirrhotics both right and left atrial and right ventricle diameters were significantly greater. Ejection fraction was slightly but significantly (P < .01) reduced in ascitic patients. Paracentesis induced a reduction of the highly increased basal plasma renin activity, aldosterone, norepinephrine (P < .01), and epinephrine (P < .05) and improved diastolic function (E/A, P < .05). Systolic function was unaffected. Thus, irrespective of ascites and cause, advanced cirrhosis is associated with left ventricle diastolic dysfunction and wall thickness increase. We can speculate that neurohumoral overactivity, known to stimulate cardiac tissue growth, may challenge the heart, promoting fibrosis and exerting a further hindrance to ventricular relaxation in patients with cirrhosis experiencing episodes of ascites.

Aims: The aim of this study was to assess the effect of granulocyte colony-stimulating factor (G-CSF) on left ventricular (LV) function and volumes in patients with anterior ST-elevation myocardial infarction (STEMI) and depressed LV ejection fraction (EF).Methods and Results: Sixty consecutive patients with anterior STEMI, undergoing primary angioplasty percutaneous coronary intervention (PCI), with symptom-to-reperfusion time of 2-12 h and EF ≤45% after PCI, were randomized to G-CSF 5 μg/kg b.i.d. subcutaneously (n = 24) or placebo (n = 25) for 5 days, starting <12 h after PCI. The primary endpoint was an increase from baseline to 6 months of 5% in left ventricular ejection fraction (LVEF), as measured by magnetic resonance imaging (MRI). Co-primary endpoint was a ≥20 mL difference in end-diastolic volume (EDV). Infarct size and perfusion were evaluated with late gadolinium enhancement (LGE) and gated (99m)Technetium Sestamibi single-photon emission computed tomography (SPECT). Left ventricular EDV and end-systolic volume (ESV) increased from baseline to 6 months in the placebo group (81.7 ± 24.4 to 94.4 ± 26.0 mL/m(2), P < 0.00005 and 45.2 ± 20.0 to 53.2 ± 23.8 mL/m(2), P = 0.016) but were unchanged in the G-CSF group (82.2 ± 20.3 to 85.7 ± 23.7 mL/m(2), P = 0.40 and 46.0 ± 18.2 to 48.4 ± 20.8 mL/m(2), P = 0.338). There were no significant differences in EF or perfusion between groups. A significant reduction in transmural LGE segments was seen at 6 months in the G-CSF vs. placebo groups (4.38 ± 2.9 to 3.3 ± 2.6, P = 0.04 and 4.2 ± 2.6 to 3.6 ± 2.7, P = 0.301, respectively). Significantly more placebo patients had a change in left ventricular end-diastolic volume abovethe median (9.3 mL/m(2)) when reperfusion time exceeded 180 min (median time-to-reperfusion) (P = 0.0123). Severe adverse events were similar between groups.Conclusion: Early G-CSF administration attenuates ventricular remodelling in patients with anterior STEMI and EF ≤45% after successful PCI. Background. Current knowledge of risk assessment in survivors of myocardial infarction is largely based on data gathered before the advent of thrombolysis. It must be determined whether and to what extent available information and proposed criteria of prognostication are applicable in the thrombolytic era. Methods and Results. We reassessed risk prediction in the 10 219 survivors of myocardial infarction with follow-up data available (ie, 98% of the total) who had been enrolled in the GISSI-2 trial, relying on a set of prespecified variables. The 3.5% 6-month all-cause mortality rate of these patients compared with the higher value of 4.6% found in the corresponding GISSI-1 cohort, originally allocated to streptokinase therapy, indicates a 24% reduction in postdischarge 6-month mortality. On multivariate analysis (Cox model), the following variables were predictors of 6-month all-cause mortality: ineligibility for exercise test for both cardiac (relative risk [RR], 3.30; 95% confidence interval [CI], 2.36-4.62) and noncardiac reasons (RR, 3.28; 95% CI, 2.23-4.72), early left ventricular failure (RR, 2.41; 95% Cl, 1.87-3.09), echocardiographic evidence of recovery phase left ventricular dysfunction (RR, 2.30; 95% CI, 1.78-2.98), advanced (more than 70 years) age (RR, 1.81; 95% Cl, 1.43 -2.30), electrical instability (ie, frequent and/or complex ventricular arrhythmias) (RR, 1.70; 95% Cl, 1.32-2.19), late left ventricular failure (RR, 1.54; 95% Cl, 1.17-2.03), previous myocardial infarction (RR, 1.47; 95% CI, 1.14-1.89), and a history of treated hypertension (RR, 1.32; 95% Cl, 1.05-1.65). Early post-myocardial infarction angina, a positive exercise test, female sex, history of angina, history of insulin-dependent diabetes, and anterior site of myocardial infarction were not risk predictors. On further multivariate analysis, performed on 8315 patients with the echocardiographic indicator of left ventricular dysfunction available, only previous myocardial infarction was not retained as an independent risk predictor. Conclusions. A decline in 6-month mortality of myocardial infarction survivors, seen within 6 hours of symptom onset, has been observed in recent years. Ineligibility for exercise test, early left ventricular failure, and recovery-phase left ventricular dysfunction are the most powerful (RR, >2) predictors of 6-month mortality among patients recovering from myocardial infarction after thrombolysis. Qualitative variables reflecting residual myocardial ischemia do not appear to be risk predictors. The lack of an independent adverse influence of early post-myocardial infarction angina on 6-month survival represents a major difference between this study and those of the prethrombolytic era.
