2. A visual quality control scale for clinical arterial spin labeling images

Abstract

Abstract Background Image-quality assessment is a fundamental step before clinical evaluation of magnetic resonance images. The aim of this study was to introduce a visual scoring system that provides a quality control standard for arterial spin labeling (ASL) and that can be applied to cerebral blood flow (CBF) maps, as well as to ancillary ASL images. Methods The proposed image quality control (QC) system had two components: (1) contrast-based QC (cQC), describing the visual contrast between anatomical structures; and (2) artifact-based QC (aQC), evaluating image quality of the CBF map for the presence of common types of artifacts. Three raters evaluated cQC and aQC for 158 quantitative signal targeting with alternating radiofrequency labelling of arterial regions (QUASAR) ASL scans (CBF, T1 relaxation rate, arterial blood volume, and arterial transient time). Spearman correlation coefficient (r), intraclass correlation coefficients (ICC), and receiver operating characteristic analysis were used. Results Intra/inter-rater agreement ranged from moderate to excellent; inter-rater ICC was 0.72 for cQC, 0.60 for aQC, and 0.74 for the combined QC (cQC + aQC). Intra-rater ICC was 0.90 for cQC; 0.80 for aQC, and 0.90 for the combined QC. Strong correlations were found between aQC and CBF maps quality (r = 0.75), and between aQC and cQC (r = 0.70). A QC score of 18 was optimal to discriminate between high and low quality clinical scans. Conclusions The proposed QC system provided high reproducibility and a reliable threshold for discarding low quality scans. Future research should compare this visual QC system with an automatic QC system.

Abstract

Zusammenfassung: Zielsetzung: Vergleich von Clomethiazol mit Oxazepam im Alkoholentzug im Hinblick auf die Inzidenz von Delirien und Krampfanfällen, die Reduktion der Entzugssymptome und die Nebenwirkungen. Methodik: Multizentrische, offene Studie. Zunächst wurden alle Patienten scoregesteuert gemäß der Hamburger Alkoholentzugs-Skala (HAES) mit Oxazepam (n= 202) entzogen, danach mit Clomethiazol (n= 218). Es wurden Patienten mit aktuell schweren Entzügen (mind. 50 mg Oxazepam / 10 Kps. Clomethiazol / 24h) und / oder Komplikationen eingeschlossen. Ergebnisse: In der Oxazepam-Gruppe traten mit 3 gegenüber 1 Delirien und 3 gegenüber 2 Krampfanfällen mehr Komplikationen auf als in den Clomethiazol-gestützten Entzügen (n. s.). Die vegetativen Scorepunkte sanken unter beiden Medikamenten etwa gleich stark. Unter Clomethiazol traten signifikant mehr Nebenwirkungen auf als unter Oxazepam. Schlussfolgerungen: Weder für die Prävention von Delirien und Krampfanfällen noch für die Kupierung der vegetativen Entzugssymptome ließ sich eine eindeutige Überlegenheit eines der beiden Medikamente nachweisen. Allerdings ergaben sich starke Hinweise für ein besseres Nebenwirkungsprofil von Oxazepam. Obwohl nur schwere Entzüge in die Studie eingeschlossen wurden, traten Delire nur bei 1,0 % der Patienten auf, Krampfanfälle bei 1,2 %. Das in der Studie verwendete Entzugskonzept mit der Hamburger Alkoholentzugs-Skala kann daher als eine sehr sichere Methode angesehen werden.

4. Plate Fixation for Irreducible Proximal Humeral Fractures in Children and Adolescents—A Single-Center Case Series of Six Patients

Abstract

Background: Recommended treatment for severely displaced proximal humeral fractures in children is the closed reduction and percutaneous fixation by K-wires or intramedullary nailing. Methods: From January 2016 to January 2017 6, 21 children/adolescents (range 8 to 16 years) with proximal humeral fractures were treated surgically for severe displacement. In these six patients, several attempts of closed reduction were unsuccessful, and an open reduction was performed. The humeral head was fixed with a 3.5 mm T-plate without affecting the growth plate. Plate removal was performed at a mean interval of 132 days after initial surgery. Two years after initial surgery, the clinical outcome was assessed by the Constant–Murley score and QuickDASH score (including sport/music and work) and the shoulder joint was evaluated with a standardized sonographic examination for the rotator cuff and the conjoint tendon. Results: In all six patients, dorsal displacement of the fracture was irreducible due to the interposition of tendinous or osseous structures. Intraoperatively, the interposed structures were the long biceps tendon in two, periosteal tissue in two, a bony fragment in one, and the long biceps tendon together with the conjoint tendon in one case. At mean follow-up of 26 months (range 22 months to 29 months), patients showed very good clinical results with an excellent mean Constant–Murley score of 97.5 (range 91 to 100) and mean QuickDASH score (including sport/music and work) of 5.5 (range 0–20.8). An X-ray follow-up 6 weeks after surgery demonstrated early consolidation and correct alignment in all patients. A sonographic evaluation at 2 years post injury showed that the biceps and the conjoined tendon were intact in all patients. Conclusions: If a proximal humeral fracture is not reducible by closed means, a tissue entrapment (most likely biceps tendon) should be considered. Treatment with an open reduction and plate fixation yields very good clinical and radiological results and preserves interposed structures as the biceps and conjoint tendon.

Abstract

We evaluated whether task-related fMRI (functional magnetic resonance imaging) BOLD (blood oxygenation level dependent) activation could be acquired under conventional anaesthesia at a depth enabling neurosurgery in five patients with supratentorial gliomas. Within a 1.5 T MRI operating room immediately prior to neurosurgery, a passive finger flexion sensorimotor paradigm was performed on each hand with the patients awake, and then immediately after the induction and maintenance of combined sevoflurane and propofol general anaesthesia. The depth of surgical anaesthesia was measured and confirmed with an EEG-derived technique, the Bispectral Index (BIS). The magnitude of the task-related BOLD response and BOLD sensitivity under anaesthesia were determined. The fMRI data were assessed by three fMRI expert observers who rated each activation map for somatotopy and usefulness for radiological neurosurgical guidance.The mean magnitudes of the task-related BOLD response under a BIS measured depth of surgical general anaesthesia were 25% (tumour affected hemisphere) and 22% (tumour free hemisphere) of the respective awake values. BOLD sensitivity under anaesthesia ranged from 7% to 83% compared to the awake state.
