Balibrea Abstract: Background: We sought to assess the relationship between tissue concentration of erb -b-2 or neu oncogene-encoded protein (p185.sup.neu) with overall survival in patients with non-small cell lung cancer. Methods: Levels of protein p185.sup.neu were determined in 102 patients with the diagnosis of non-small cell lung cancer. Concentration of p185.sup.neu protein was determined by using enzyme immunoassay and evaluated by using several variables. The relative prognostic importance of this marker and its influence on other prognostic factors was evaluated by using the Cox regression model. Results: The mean p185.sup.neu value in these samples was 250 [+ or -] 200 U/mg (95% confidence interval, 210-290). This distinguished two groups within the tumoral population: those with less than 350 U/mg and those with 350 U/mg or greater (80th percentile). Multivariable analysis established an independent prognostic value for protein p185.sup.neu. Patients with p185.sup.neu values of the 80th percentile or greater had a risk of death that was 2.11-fold (95% confidence interval, 1.10-4.05) that of patients with values of less than 350 U/mg (P = .03), and increases in the neu oncogene of 100 U/mg increased the probability of death by 17% (P = .02; 95% confidence interval, 1.04-1.31). Conclusion: This study shows that the p185.sup.neu expression is an objective and comparable variable for the assessment of phenotypic aggressivity in non-small cell lung cancer, and in the future, it could be included in daily clinical practice. 11. Caracterización biomolecular del cáncer de laringe: relación entre ploidia, actividad proliferativa y LOH en la región 9p21

Abstract

El carcinoma escamoso de laringe o LSCC (Laryngeal Squamous Cell Carcinoma) representa más del 95% de las neoplasias de laringe. Representa cerca del 2% de todos los cánceres humanos en el mundo Occidental y es una importante causa de muerte entre los tumores de cabeza y cuello. A pesar de los avances en el estudio de LSCC los indicadores pronóstico "clasicos" no permiten identificar los pacientes con elevado riesgo de recaída o muerte. En los últimos años se intentan identificar nuevos factores biológicos para predecir la evolución del tumor. El objetivo de este trabajo es el estudio de los factores biológicos implicados en el carcinoma escamoso de laringe: actividad proliferativa (SPF), contenido celular de DNA y la determinación de posibles alteraciones genéticas en el gen supresor p16, pérdida de heterocigosidad (LOH) y mutaciones puntuales. Este estudio se realizó en 56 pacientes intervenidos quirúrgicamente de cáncer de laringe. Se realizó la evaluación de la ploidía y de la SPF mediante citometría de flujo. Para la valoración de la LOH en el gen p16 se realizó extracción de DNA, electroferesis en un gel de poliacrilamida al 8% y coloración con nitrato de plata. Las mutaciones en el gen p16 fueron detectadas mediante análisis SSCP después de la amplificación mediante PCR del exón 1. Se detectó aneuploidía en el 63% de los pacientes y de ellos el 9% presentaban multiclonalidad. En nuestra serie de pacientes, el 21% presentaba LOH en la región 9p21 (locus D9S1747). Abstract

The p16INK4a gene, localized within chromosome 9p21, has been identified as a cyclin-dependent kinase inhibitor and may negatively regulate the cell cycle acting as a tumor suppressor. Genetic alterations involving the 9p21 region are common in human cancers. A consecutive series of 64 untreated patients (median of follow up 53 months) undergoing surgical resection for locally advanced laryngeal squamous-cell carcinomas (LSCCs) has been studied prospectively. Our purpose was to investigate p16 alterations (9p21 allelic loss, hypermethylation and point mutations) and their possible association with clinico-pathological data and flow cytometric variables (DNA-ploidy and S-phase fraction (SPF)), and to determine the possible prognostic role of this gene in these tumors. PCR-based techniques were used for investigating 9p21 loss of heterozygosity (LOH) and methylation promoter status of the p16 gene. p16 mutations were detected by PCR-SSCP (single strand conformation polymorphism) and sequencing. 9p21 LOH was detected in 16/62 (26%) informative tumors, point mutations in 5% (3/64) and hypermethylation in 9% (6/64) of the cases. p16 alterations were significantly associated with high SPF and DNA-aneuploidy. By univariate analysis, poor histologic differentiation, stage IV, DNA-aneuploidy and p16 point mutations proved to be significantly related to quicker relapse, whereas these same factors, and in addition high SPF, 9p21 LOH and any p16 alterations were significantly related to shorter overall survival. By Cox proportional hazards analysis only histologic grade (G3) and p16 point mutations were independently related to both disease relapse and death. Our study has identified p16 point mutations as important biomolecular indicators in LSCCs. © 2002 Wiley-Liss, Inc.
