Questo articolo costituisce la seconda parte del Vademecum per il farmacista che, facendo seguito alla precedente, dove venivano trattati i Dispositivi di Protezione Collettiva (DPC), affronterà il tema dei Dispositivi di Protezione Individuale (DPI).

Questo articolo costituisce la terza ed ultima parte del Vademecum per il farmacista ed affronterà il Regolamento Classification, Labelling and Packaging (CLP) con particolare riferimento alla metodologia per la classificazione delle sostanze chimiche (o loro miscele) e la loro etichettatura. Tale argomento, insieme ai due precedenti (Dispositivi di Protezione Collettiva e Dispositivi di Protezione Individuale), vuole rappresentare un valido e rapido strumento di consultazione per permettere al farmacista di lavorare in sicurezza secondo le disposizioni della normativa vigente.

Nel titolo di questo libro è presente la parola vademecum, questo perché, secondo la nostra visione del lavoro, il termine stesso, nelle sue varie declinazioni, racchiude in se lo spirito che ha animato gli autori di quest’opera (dal latino “vade mecum” che significa “vieni con me - ti posso aiutare”). Pertanto la finalità di questo documento è quello di offrire una guida pratica di agevole consultazione nella quotidiana attività del farmacista e per un’applicazione, uniforme in tutto il territorio nazionale, delle principali norme interessanti il nostro servizio. Attraverso una collaborazione attiva tra le due aree scientifico culturali, Galenica e Nutrizione clinica, è stato avviato un percorso di revisione che ha permesso di individuare alcuni settori, del nostro lavoro, che necessitavano di essere approfonditi. La realizzazione di questo documento ha richiesto lavoro metodologico ed un rigoroso impegno da parte di tutti gli autori. Particolare attenzione è stata data nel condurre i lavori nel modo più “aperto“ possibile, attraverso una condivisione trasversale che ha caratterizzato il nostro modus operandi. Due sono gli obiettivi che hanno guidato la stesura di questo libro: Tutelare il lavoratore; Tutelare il prodotto. Proposito di questo documento è quello di definire le specifiche tecniche ed i requisiti strutturali, organizzativi e tecnologici per l’esecuzione delle attività e la gestione delle stesse in sicurezza per operatore e prodotto. Siamo riconoscenti a coloro che hanno prestato la loro collaborazione a quest’opera, lavorando con dedizione e passione. Riteniamo che i risultati ottenuti con questo lavoro non debbano considerarsi un punto di arrivo, bensì un nuovo punto di partenza. Un lavoro come questo potrà mantenere il suo valore se i suoi eredi terranno fede ai precetti fondamentali perseguiti da tutti coloro che hanno partecipato alla realizzazione.

Con questo breve articolo e con i prossimi due, si vuole dare al lettore un documento che sia utile ad informare l’operatore che lavora in un laboratorio galenico sui dispositivi di protezione collettiva, sui dispositivi di protezione individuale e sulla normativa che regolamenta l’uso delle sostanze che vengono utilizzate presso il laboratorio.

The use of antiemetics for vomiting in acute gastroenteritis in children is still a matter of debate. We conducted a double-blind randomized trial to evaluate whether a single oral dose of ondansetron vs domperidone or placebo improves outcomes in children with gastroenteritis. After failure of initial oral rehydration administration, children aged 1-6 years admitted for gastroenteritis to the pediatric emergency departments of 15 hospitals in Italy were randomized to receive one oral dose of ondansetron (0.15 mg/kg) or domperidone (0.5 mg/kg) or placebo. The primary outcome was the percentage of children receiving nasogastric or intravenous rehydration. A p value of 0.014 was used to indicate statistical significance (and 98.6% CI were calculated) as a result of having carried out two interim analyses. 1,313 children were eligible for the first attempt with oral rehydration solution, which was successful for 832 (63.4%); 356 underwent randomization (the parents of 125 children did not give consent): 118 to placebo, 119 to domperidone, and 119 to ondansetron. Fourteen (11.8%) needed intravenous rehydration in the ondansetron group vs 30 (25.2%) and 34 (28.8%) in the domperidone and placebo groups, respectively. Ondansetron reduced the risk of intravenous rehydration by over 50%, both vs placebo (RR 0.41, 98.6% CI 0.20-0.83) and domperidone (RR 0.47, 98.6% CI 0.23-0.97). No differences for adverse events were seen among groups. In a context of emergency care, 6 out of 10 children aged 1-6 years with vomiting due to gastroenteritis and without severe dehydration can be managed effectively with administration of oral rehydration solution alone. In children who fail oral rehydration, a single oral dose of ondansetron reduces the need for intravenous rehydration and the percentage of children who continue to vomit, thereby facilitating the success of oral rehydration. Domperidone was not effective for the symptomatic treatment of vomiting during acute gastroenteritis.

La sedazione procedurale al di fuori della sala operatoria è sempre più frequentemente gestita da pediatri non anestesisti, adeguatamente addestrati nella selezione dei pazienti, nell’utilizzo dei farmaci e nella gestione delle eventuali complicanze legate alla somministrazione degli stessi. La dexmedetomidina ha proprietà farmacologiche uniche, in primis quella di indurre il sonno, senza deprimere l’attività respiratoria o influire sulla pervietà delle prime vie aeree. Diversi studi pediatrici ne dimostrano l’efficacia e la sicurezza in corso di sedazione procedurale, anche utilizzata da medici non anestesisti. Tra i numerosi strumenti a disposizione del medico in questo ambito riteniamo che la dexmedetomidina somministrata per via intranasale avrà un ruolo sempre più di primo piano.

Summary. A method to obtain and guarantee a multidose sterile oxybutynin solution useful in treating pediatric neurogenic bladder. Objective. Oxybutynin hydrochloride represents one of the anticholinergic drugs used in pediatric neurogenic bladder, and it is available either orally in generic formulation as tablets and syrup, or as a transdermal patch and as a topical gel. Despite the efficacy of oxybutynin, these systemic delivery systems present significant side effects, and therefore a number of studies have explored an intravesical oxybutynin solution as an alternative. Since a commercial intravesical oxybutynin solution for use in pediatric neurogenic bladder is currently not available, it could be useful to find a method to guarantee the sterility of prepared oxybutynin solution in order to prevent bacterial growth in the urinary tract after catheterization. Methods. A simple method to prepare a stable and sterile multidose solution of oxybutynin for pediatric neurogenic bladder was developed. The stability of the solution was evaluated using two techniques: ultraviolet (UV) spectrophotometry and spectrometric Nuclear Magnetic Resonance (NMR)”. In order to maximize the sensitivity of both the proposed methods solvent system, wavelength of detection, and mass spectrometry conditions were optimized. Results. No spectrum changes have been observed with UV/Vis method and no quantitative modification has been noticed in the NMR method. The solution after six months conforms perfectly with that calculated on the same solution when initially prepared. Conclusions. It was concluded that both methods that were developed are equally accurate, sensitive, precise, reproducible, and efficient, and both could be applied directly and easily to evaluate the pharmaceutical preparation of oxybutynin. The stability and sterility of the oxybutynin solution were determined to last six months.

2006/2007 I flavonoidi sono metaboliti secondari presenti in numerose piante commestibili che rappresentano una parte integrante dell’alimentazione umana. Numerosi studi epidemiologici hanno dimostrato che una dieta ricca di flavonoidi può avere un ruolo rilevante nella prevenzione di diverse patologie, come alcune neoplasie e le malattie cardiovascolari. L’endotelio vascolare svolge un ruolo fondamentale nel mantenimento dell’omeostasi vascolare e alterazioni a questo livello, in particolare se croniche, possono causare l’avvio di lesioni arteriosclerotiche. I flavonoidi esplicano le loro azioni a livello dell’endotelio vascolare attraverso meccanismi diversi, che vanno dalle loro attività anti-infiammatorie, alla loro capacità di modulare la sintesi e l’espressione di proteine e di secondi messaggeri coinvolti in numerose reazioni cellulari. Le proprietà biologiche di questi composti naturali sono state oggetto di numerosi studi, ma ancora oggi il meccanismo che spiega l’entrata di queste sostanze all’interno delle cellule, ed in particolare, nelle cellule endoteliali, rimane poco conosciuto. La maggior parte dei flavonoidi è presente negli alimenti in forma glicosilata, altamente idrofilica, pertanto non in grado di attraversare le membrane biologiche se non attraverso specifici trasportatori di membrana. In particolare, tra le proteine di membrana coinvolte nel trasporto di questi composti, sembra ricoprire un ruolo importante la bilitranslocasi (TC 2.A.65.1.1), un trasportatore di anioni organici situato al polo sinusoidale della cellula epatica e presente anche nella mucosa gastrica, che ha dimostrato di mediare il trasporto della quercetina, un flavonolo, e di alcuni antociani, una classe di flavonoidi contenuta soprattutto nel vino e nella frutta rossa. Anche alcuni componenti della superfamiglia delle proteine ABC (ATP-Binding Cassette), come le proteine P-glicoproteina (ABCB1), MRP1 (Multi-Drug Resistance related Protein 1) (ABCC1), MRP2 (Multi-Drug Resistance related Protein 2) (ABCC2) e BCRP (Breast Cancer Resistance related Protein) (ABCG2), hanno dimostrato di interagire con numerosi flavonoidi. Queste proteine agiscono da pompe di membrana e, attraverso un meccanismo energia –dipendente, legato all’idrolisi dell’ATP, permettono l’estrusione dei loro substrati dall’interno verso l’esterno delle cellule. I trasportatori in entrata ed in uscita possono, quindi, avere un ruolo importante nel regolare le concentrazioni intracellulari dei flavonoidi, necessarie per le loro azioni biologiche. Lo scopo di questa ricerca è stato analizzare tre modelli di cellule endoteliali nei quali sono stati studiati l’espressione della bilitranslocasi e delle proteine ABC ed il loro possibile ruolo nel trasporto di flavonoidi. Sono state utilizzate cellule endoteliali primarie di aorta umana e la linea cellulare endoteliale umana Ea.hy 926, che deriva dalla fusione di cellule endoteliali HUVEC (Human Umbilical Vein Endothelial Cells) con la linea cellulare di carcinoma polmonare A549. I dati di queste cellule sono stati confrontati con quelli ottenuti in cellule endoteliali primarie di aorta di ratto, espiantate mediante un metodo messo a punto nei nostri laboratori. Sono state condotte analisi di immunocitochimica, di Western blot e di valutazione dell’espressione dell’RNA messaggero mediante tecnica RT-PCR, per studiare l’espressione delle proteine di membrana nei tre diversi modelli cellulari. Infine, è stato messo a punto un saggio di uptake della quercetina per studiare il trasporto dei flavonoidi nei queste cellule. I risultati ottenuti mediante le tecniche immunochimiche hanno evidenziato l’espressione della bilitranslocasi in tutti e tre i tipi di cellule endoteliali. Tra i trasportatori ABC, le proteine MRP1, MRP2 e BCRP sono state tutte identificate in queste cellule, mentre la P-glicoproteina è risultata meno espressa in tutti e tre i modelli cellulari. Il saggio di trasporto messo a punto in queste cellule ha permesso di dimostrare che la quercetina entra nell’endotelio vascolare mediante la bilitranslocasi, come trasportatore in entrata, e viene estrusa dalle proteine ABC nelle cellule endoteliali. Il saggio, inoltre, ha permesso di studiare il trasporto anche di altri polifenoli, contenuti nella dieta, come la malvidina 3-glucoside, un antociano presente soprattutto nel vino rosso, e dell’acido caftarico, che si trova principalmente nel vino bianco. I risultati ottenuti hanno messo in luce un ruolo della bilitransloacasi nel trasporto anche di questi composti. 1979

Purpose: Compare ibuprofen and ketorolac for children with trauma-related acute pain. Methods: We conducted a multicentre randomized, double-blind, controlled trial in the Paediatric Emergency Department setting. We enrolled patients aged 8 to 17 who accessed the emergency department for pain related to a limb trauma that occurred in the previous 48 hours. At the admission, patients were classified based on numeric rating scale-11 (NRS-11) in moderate (NRS 4-6) and severe (NRS 7-10) pain groups. Each patient was randomized to receive either ibuprofen (10 mg/kg) or ketorolac (0.5 mg/kg) and the placebo of the not given drug in a double dummies design. NRS-11 was asked every 30 minutes until 2 hours after drug and placebo administration. The primary outcome was NRS-11 reduction at 60 minutes. Results: Among 125 patients with severe pain, NRS-11 reduction after 60 minutes from drug administration was 2.0 (IQR 1.0-4.0) for ibuprofen and 1.0 (IQR 1.0-3.0) for ketorolac (p=0.36). Ibuprofen was significantly better, considering secondary outcomes, at 90 minutes with a lower median of NRS-11 (p 0.008), more patients with NRS-11 less than 4 (p 0.01) and a reduction of pain score of more than 3 NRS-11 points (p 0.01). Among 87 patients with moderate pain, the NRS-11 reduction after 60 minutes from drug administration was 1.63 (±1.8) for ibuprofen and 1.8 (±1.6) for ketorolac, with no statistically significant difference. Conclusions: Oral ibuprofen and ketorolac are similarly effective in children and adolescents with acute traumatic musculoskeletal pain. ClinicalTrial.gov registration number: NCT04133623

Sex differences play a relevant role in cancer susceptibility, incidence and survival. Exploring such differences is difficult because of the close interplay of genetic, epigenetic and hormonal factors. However, a better understanding of the role of such disparities in cancer mechanisms could improve its prevention and therapy. Our study explores how sex differences in pediatric outcomes vary after undergoing first and advanced-line therapy for hematological malignancies. The primary goal was to evaluate if sex differences in pediatric outcomes after first-line therapy persist after allogeneic hematopoietic stem cell transplantation (HSCT). The secondary goal was to analyze sex differences in disease risk at onset and pediatric outcomes after first-line therapy to compare our results with the literature's reported results. Among a total of 485 patients (280 males, 205 females) admitted for hematological malignancies, disease risk at the onset was significantly higher in males (P < .05). One hundred and seventy-four patients (111 males and 63 females) had a high-risk disease requiring HSCT. Before HSCT, all patients underwent myeloablative conditioning, which substantially impaired gonadal function. Although the number of boys undergoing HSCT was almost double that of girls, there were no sex-related differences in overall survival, cancer relapse and complications after HSCT exposure (P > .05). These findings suggest that the existing sex differences in cancer risk ab initio can be somehow flattened by a conditioning regimen, shedding new light on the role of hormonal factors in cancer mechanism and management.

Acute graft-versus-host disease (GvHD) remains the second leading cause of death, after disease relapse, in patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). The medical records of 112 pediatric patients who underwent allo-HSCT from matched unrelated and haploidentical donors were analyzed. Patients were divided into two groups, according to the GvHD prophylactic regimen used. In the control group, GvHD prophylaxis consisted of cyclosporine A (CsA) and methotrexate (MTX) or CsA and mycophenolate mofetil (MMF) at a standard daily dose of 30 mg/kg. All subjects in the study group received tacrolimus (FK506) and MMF. In this group, MMF was subjected to therapeutic drug monitoring (TDM) through mycophenolic acid (MPA) area under the curve AUC0&ndash, 12. We found a statistically significant difference in both overall acute GvHD (p &lt, 0.0001) and overall chronic GvHD (p &lt, 0.05) incidence between the study and the control group. The initial daily MMF dose and the age at transplant in the study group proved to be inversely correlated (r = &minus, 0.523, p &lt, 0.0001). The children under six years of age required a significantly higher daily MMF dose (p &lt, 0.008). This study showed that pharmacological monitoring of MPA AUC0&ndash, 12 concentration allowed a reduction in the incidence of acute and chronic GvHD. MMF showed age-dependent pharmacokinetics due to greater drug clearance in younger children.

Background: Total body irradiation (TBI) is a mandatory step for patients with acute lymphoblastic leukemia (ALL), undergoing allogeneic hematopoietic stem cell transplantation (HSCT). In the past, amylases have been reported to be a possible sign of TBI toxicity. We investigated the relationship between total amylases (TA) and transplant-related outcomes in pediatric recipients. Methods: We retrospectively analyzed the medical records of all the patients who underwent allogeneic HSCT between January 2000 and November 2019. The inclusion criteria were the following: recipient’s age between 2 and 18, diagnosis of ALL, no previous transplantation, and use of TBI-based conditioning. The serum total amylase and pancreatic amylase were evaluated before, during, and after transplantation. Cytokines and chemokines assays were retrospectively performed. Results: 78 patients fulfilled the inclusion criteria. Fifty-seven patients were treated with fractionated TBI, and 21 with a single-dose regimen. The overall survival (OS) was 62.8%. Elevated values of TA were detected in 71 patients (91%). The TA were excellent in predicting the OS (AUC = 0.773, 95% CI = 0.66–0.86, p &lt, 0.001). TA values below 374 U/L were correlated with a higher OS. The highest mean TA values (673 U/L) were associated with a high disease-progression mortality rate. The TA showed a high predictive performance for disease progression-related death (AUC = 0.865, 95% CI = 0.77–0.93, 0.0001). Elevated TA values were also connected with significantly higher levels of proinflammatory cytokines, such as TNF-α, IL-6, and RANTES (p &lt, 0.001). Conclusions: this study shows that TA is a valuable predictor of post-transplant OS and increased risk of leukemia relapse.

Background Describe the efficacy of a galenic glycopyrrolate formulation and its impact on patients with sialorrhea Quality of Life (QoL), including costs analysis. Methods We performed a retrospective observational study on 21 patients who received a custom-formulated galenic glycopyrrolate syrup for sialorrhea for an average period of 14.3 months. We analyzed the telephone interviews with elaborated and validated questionnaires and the therapy costs comparing the brand marketed drug with the galenic formulation. Results Overall, 16 out of 21 patients (76.2%) reported a significant improvement in sialorrhea and QoL. In 14 subjects (66.7%), there was a remarkable decrease in the drooling severity; 10 individuals (47.6%) reported a reduction in drooling frequency. Nine patients experienced at least one adverse effect of glycopyrrolate therapy, and three of them stopped the treatment. No severe side effects were observed. The galenic drug significantly reduced costs for patients. Conclusions An oral glycopyrrolate solution easily administered to children with brain injuries is not commercially available in many European countries. This study demonstrates the efficacy of a compounded glycopyrrolate syrup on drooling severity, frequency and ensures a better QoL in patients and their caregivers.
