We examined cause-specific death of DM patients with and without CAD.Methods: We conducted a cohort study of all patients who underwent CAG in Western Denmark between 2003 and 2016. Using Danish health registries, patients were followed for a maximum of 10 years and stratified according to their DM and CAD status. Outcomes included all-cause-, cancer-, circulatory-, and endocrinologic death. Ten-year cumulative risks were computed as well as adjusted and unadjusted hazard ratios (aHR and HR).Results: A total of 132,432 patients (28,524 deaths, median follow-up of 6.2 years) were included. Compared to patients with neither DM nor CAD, DM patients without CAD had a higher 10-year risk of all-cause death (27.9% versus 19.7%, aHR 1.43 [95% CI 1.35– 1.52]), cancer death (7.2% versus 5.4%, aHR 1.29 [95% CI 1.15– 1.46]), circulatory death (9.1% versus 6.9%, aHR 1.35 [95% CI 1.22– 1.49]), and endocrinologic death (3.9% versus 0.3%, aHR 14.02 [95% CI 10.95– 17.95]). Among endocrinologic deaths, 87% were due to classical complications of DM, such as diabetic nephropathy and ketoacidosis, in DM patients without CAD.Conclusion: Diabetes patients without CAD exhibit a higher risk of all-cause mortality, driven primarily by elevated rates of cancer, circulatory, and endocrinologic deaths, particularly related to diabetic microvascular complications.Keywords: diabetes, coronary artery disease, death

Moreover, an age- and sex-matched general population cohort was established. Patients were stratified according to estimated glomerular filtration rate (eGFR). Presence of CAD was defined as ≥ 1 obstructive stenosis or non-obstructive diffuse disease. Major adverse cardiovascular events (MACE) were defined as a composite of myocardial infarction, ischemic stroke, and cardiac death.Results: A total of 42,611 patients were included with a median follow-up of 7.3 years. Patients without and with CAD had MACE rates per 100 person-years that were 0.52 and 1.67 for eGFR ≥ 90 mL/min/1.73 m2, 0.68 and 2.09 for eGFR 60– 89 mL/min/1.73 m2, 1.27 and 3.85 for eGFR 30– 59 mL/min/1.73 m2, and 2.27 and 6.92 for eGFR < 30 mL/min/1.73 m2. Comparing to eGFR ≥ 90 mL/min/1.73 m2, the adjusted incidence rate ratios for MACE were 1.29 (1.10– 1.51) for eGFR 60– 89 mL/min/1.73 m2, 1.86 (1.49– 2.33) for eGFR 30– 59 mL/min/1.73 m2, and 3.57 (1.92– 6.67) for eGFR < 30 mL/min/1.73 m2 in patients without CAD, and 1.11 (1.03– 1.20), 1.71 (1.55– 1.90), and 2.46 (1.96– 3.09) in patients with CAD. We hypothesized that risks of myocardial infarction and death were associated with extent of coronary artery disease in diabetes patients.Patients and methods: We conducted a cohort study of patients with type 1 and type 2 diabetes, who underwent coronary angiography from 2004 to 2012. Patients were stratified according to extent of coronary artery disease: 0-, 1-, 2- or 3-vessel disease or diffuse vessel disease. Endpoints were myocardial infarction, all-cause death, and major adverse cardiovascular events (MACE), defined as the composite of myocardial infarction, cardiac death, or ischemic stroke. Adjusted incidence and mortality rate ratios (IRRsadj) were calculated using patients with 0-vessel disease as the reference group. Median follow-up was 3 years for a total of 45,164 person-years.Results: The study included 12,594 diabetes patients. Of these, 3,147 (25.0%) had 0-vessel disease, 1,195 (9.5%) had diffuse vessel disease, 3,001 (23.8%) had 1-vessel disease, 2,220 (17.6%) had 2-vessel disease, and 3,031 (24.1%) had 3-vessel disease. The myocardial infarction rate was 0.4 per 100 person-years (95% CI: 0.3–0.5) in patients with 0-vessel disease. These patients were evaluated for ischemic events, including AMI, during follow-up using an end point committee adjudication of AMI as reference standard.Results: Of 5,719 included patients, 285 patients suffered AMI within a mean follow-up time of 3 years after stent implantation. An AMI discharge diagnosis (primary or secondary) from any acute or elective admission had a sensitivity of 95%, a specificity of 93%, and a positive predictive value of 42%. Restriction to acute admissions decreased the sensitivity to 94% but increased the specificity to 98% and the positive predictive value to 73%. Further restriction to include only AMI as primary diagnosis from acute admissions decreased the sensitivity further to 82%, but increased the specificity to 99% and the positive predictive value to 81%. Restriction to patients admitted to hospitals with a coronary angiography catheterization laboratory increased the positive predictive value to 87%.Conclusion: Algorithms utilizing additional information from the Danish National Patient Registry yield different sensitivities, specificities, and predictive values in registry-based detection of AMI following PCI. We were able to identify AMI following PCI with moderate-to-high validity. However, the choice of algorithm will depend on the specific study purpose. Background: Randomized clinical trials often focus on short-term efficacy and safety in a controlled environment. Trial follow-up may be linked with study-related investigations and differ from routine clinical practice. Because treatment and control in randomized trials differ from daily practice, trial results may have reduced general applicability and may be of limited value in clinical decision-making. Further, it is economically very costly to conduct randomized clinical trials. Methods and results: Population-based health care databases collect data continuously and prospectively, and make it possible to monitor lifelong outcomes of cardiac interventions in large numbers of patients. This strengthens external validity by eliminating the effects of study-related monitoring or diagnostic tests. Further, follow-up data can be obtained at low expense. Importantly, data sources encompassing a complete population are likely to reflect clinical practice. Because population-based health care databases collect data for quality-control and administrative purposes unrelated to scientific investigations, certain biases, such as nonresponse bias, recall bias, and bias from losses to follow-up, can be avoided. Conclusion: Event detection using population-based health care databases is a new research tool in interventional cardiology that may allow large, low-cost, randomized clinical trials to reflect daily clinical practice, covering a broad range of patients and end points with complete lifelong follow-up. Abstract

Background: The multicentre European Bifurcation Club Trial (EBC TWO) showed no significant differences in 12-month clinical outcomes between patients randomised to a provisional stenting strategy or systematic culotte stenting in non-left main true bifurcations. Aims: This study aimed to investigate the 5-year clinical results of the EBC TWO Trial. Methods: A total of 200 patients undergoing stent implantation for non-left main bifurcation lesions were recruited into EBC TWO. Inclusion criteria required a side branch diameter ≥2.5 mm and side branch lesion length >5 mm. Five-year follow-up was completed for 197 patients. The primary endpoint was the composite of all-cause mortality, myocardial infarction, or target vessel revascularisation. Results: The mean side branch stent diameter was 2.7±0.3 mm and mean side branch lesion length was 10.3±7.2 mm. At 5-year follow-up, the primary endpoint occurred in 18.4% of provisional and 23.7% of systematic culotte patients (hazard ratio [HR] 0.75, 95% confidence interval [CI]: 0.41-1.38). No significant differences were identified individually for all-cause mortality (7.8% vs 7.2%, HR 1.11, 95% CI: 0.40-3.05), myocardial infarction (8.7% vs 13.4%, HR 0.64, 95% CI: 0.27-1.50) or target vessel revascularisation (6.8% vs 9.3%, HR 1.12, 95% CI: 0.37-3.34). Stent thrombosis rates were also similar (1.9% vs 3.1%, HR 0.63, 95% CI: 0.11-3.75). There was no significant interaction between the extent of side branch disease and the primary outcome (p=0.34). Conclusions: In large non-left main true bifurcation lesions, the use of a systematic culotte strategy showed no benefit over provisional stenting for the composite outcome of all-cause mortality, myocardial infarction, or target vessel revascularisation at 5 years. The stepwise provisional approach may be considered preferable for the majority of true coronary bifurcation lesions.

Abstract

Background: Cardiovascular heart disease is the leading cause of mortality worldwide, with the rupture or thrombosis of an atherosclerotic plaque being the main reason behind an acute coronary syndrome. It has already been established that the morphology of atherosclerotic plaques determine their stability. A lipid rich lesion with a thin fibrous cap is more prone to rupture compared to solid fibrous lesions. In the PROSPECTII study we used Near infrared spectroscopy (NIRS) and Intravascular ultrasound (IVUS) to identify atherosclerotic plaques in the coronary arteries; NIRS-derived lipid core burden index (LCBI) and IVUS-derived plaque burden (PB) identified plaques that caused adverse cardiovascular events. Purpose: Our aim is to find biomarkers associated with LCBI or PB, to understand the development of vulnerable plaques. Methods: 902 patients were enrolled in this study after successful percutaneous coronary intervention (PCI). A combined NIRS-IVUS catheter was then used to analyze approximately 200m of coronary arteries. Blood samples for biomarker analysis were taken before the PCI procedure and plasma levels of 182 proteins associated with cardiovascular disease were assessed using a novel method for measuring proximity extension assay. Adjusted linear regression models were calculated between the biomarkers and the outcomes of interest, followed by a false discovery rate (FDR) correction. Results: We found 24 proteins associated with plaque burden and 28 proteins associated with LCBI after using a cut off of two tailed P value <0.05. An overlap of 8 biomarkers could be seen between the two groups. After adjusting the P values with FDR, Angiopoeitin like 3 (ANGPTL3) retain edits association to LCBI, and Interleukin 18 receptor 1 (IL18R1) and colony stimulating factor 1 (CSF-1) to plaque burden. Conclusion: We were able to identify different biomarker patterns associated with plaque burden compared to lipid rich vulnerable plaques. Abstract

Aims The aim of this study was to determine the contemporary use of reperfusion therapy in the European Society of Cardiology (ESC) member and affiliated countries and adherence to ESC clinical practice guidelines in patients with ST-elevation myocardial infarction (STEMI). Methods and results Prospective cohort (EURObservational Research Programme STEMI Registry) of hospitalized STEMI patients with symptom onset Conclusions The use of reperfusion therapy for STEMI in the ESC member and affiliated countries was high. Primary PCI was the most frequently used treatment and associated total in-hospital mortality was below 5%. However, there was geographic variation in the use of primary PCI, which was associated with differences in in-hospital mortality.

However, it remains unclear whether revascularisation should be performed at the index procedure or at a later stage. METHODS AND ANALYSIS: The instantaneous wave-free ratio (iFR) Guided Multivessel Revascularisation During Percutaneous Coronary Intervention for Acute Myocardial Infarction trial is a multicentre, randomised controlled prospective open-label trial with blinded evaluation of endpoints. After successful primary percutaneous coronary intervention (PCI), eligible STEMI patients with residual non-culprit lesions are randomised, to instantaneous wave-free ratio guided treatment of non-culprit lesions during the index procedure versus deferred cardiac MR-guided management within 4 days to 6 weeks. The primary endpoint of the study is the combined occurrence of all-cause death, recurrent myocardial infarction and hospitalisation for heart failure at 12 months follow-up. Clinical follow-up includes questionnaires at 3 months and outpatient visits at 6 months and 12 months after primary PCI. Furthermore, a cost-effectiveness analysis will be performed. ETHICS AND DISSEMINATION: Permission to conduct this trial has been granted by the Medical Ethical Committee of the Amsterdam University Medical Centres (loc. VUmc, ID NL60107.029.16). Abstract

Background: The FFRCT Planner has been shown to be accurate and precise to predict post-PCI fractional flow reserve (FFR). The impact of coronary CT angiography (CCTA) image quality on the accuracy of this tool remains to be studied. Method(s): In this multicenter prospective study, patients with chronic coronary syndromes and lesions with an invasively measured FFR <=0.80 were included. All patients had CCTA performed with the latest-generation scanners. CCTA image quality was adjudicated as excellent, good, or sufficient with the use of a 4-point Likert-type scale at the per-vessel level by an independent committee. Images were processed for FFRCT, and the FFRCT Planner was applied, mirroring the PCI blinded to the invasively acquired data. The objective was to assess the accuracy of the FFRCT Planner across different levels of CCTA image quality. Result(s): Overall, 120 patients (123 vessels) were included. Mean age was 64.2 +/- 8.9 years and 24% were diabetic. FFR before PCI was 0.66 +/- 0.13 and mean percentage diameter stenosis was 50.1 +/- 16. Invasively-determined FFR after PCI was 0.88 +/- 0.06 and FFRCT Planner was 0.86 +/- 0.06 (mean difference 0.02 FFR units; autoregulation lower limit: -0.12; upper limit: 0.15). CCTA image quality was assessed as excellent (Likert score 4) in 47%, good (Likert score 3) in 46%, and sufficient (Likert score 2) in 3.6% of patients. The FFRCT Planner remained accurate across the different levels of image quality (mean difference between FFRCT Planner and invasively assessed post-PCI FFR in Likert score 4, 0.02 +/- 0.07; Likert score 3, 0.02 +/- 0.07; and Likert score 2, 0.03 +/- 0.08; ANOVA P = 0.898). Abstract

Introduction: Fractional flow reserve (FFR) measured after percutaneous coronary intervention (PCI) has been identified as a surrogate marker for vessel related adverse events. FFR can be derived from standard coronary computed tomography angiography (CTA). Moreover, the FFR derived from coronary CTA (FFRCT) Planner is a tool that simulates PCI providing modeled FFRCT values after stenosis opening. Aim(s): To validate the accuracy of the FFRCT Planner in predicting FFR after PCI with invasive FFR as a reference standard. Method(s): Prospective, international and multicenter study of patients with chronic coronary syndromes undergoing PCI. Patients will undergo coronary CTA with FFRCT prior to PCI. Combined morphological and functional evaluations with motorized FFR hyperemic pullbacks, and optical coherence tomography (OCT) will be performed before and after PCI. The FFRCT Planner will be applied by an independent core laboratory blinded to invasive data, replicating the invasive procedure. The primary objective is to assess the agreement between the predicted FFRCT post-PCI derived from the Planner and invasive FFR. A total of 127 patients will be included in the study. Result(s): Patient enrollment started in February 2019. Until December 2020, 100 patients have been included. Mean age was 64.1 +/- 9.03, 76% were males and 24% diabetics. The target vessels for PCI were LAD 83%, LCX 6%, and RCA 11%. The final results are expected in 2021. Abstract

Background: The amount of myocardium subtended by a stenotic vessel influences fractional flow reserve (FFR) measurements. FFR measured after percutaneous coronary intervention (PCI) has been shown to carry prognostic information in patients with stable coronary artery disease. We hypothesize that higher myocardial mass would be associated with lower post-PCI FFR. Method(s): This was an international prospective study of patients with significant lesions based on an invasive FFR <=0.80. All patients underwent optical coherence tomography-guided PCI followed by post-PCI FFR measurement. Per vessel myocardial mass was calculated from cardiac computed tomography angiography using the Voronoi algorithm. Fractional myocardial mass was defined as myocardial mass in grams divided by left ventricular mass in grams and multiplied by 100. The median myocardial mass value was used to define equal groups of low and high mass. Result(s): Overall, 120 patients (123 vessels) were evaluated (94 left anterior descending coronary arteries, 13 left circumflex arteries, and 16 right coronary arteries). The mean myocardial mass subtended by the target vessels was 61 +/- 23.1 g, and the mean left ventricular fractional myocardial mass was 39.6% +/- 11.7%. The mean post-PCI FFR was 0.88 +/- 0.06 FFR units. Vessels with higher subtended mass had lower post-PCI FFR compared with vessels with low mass (post-PCI FFR: 0.86 +/-0.05 high mass vs 0.89 +/- 0.06 low mass, P = 0.041). There was a significant negative correlation between the fractional myocardial mass and the post-PCI FFR (r = -0.35; 95% confidence interval: -0.5 to -0.19; P < 0.001; Figure 1). In multivariate analysis, fractional myocardial mass was an independent predictor of post-PCI FFR. [Formula presented] Conclusion(s): FFR after PCI is influenced by the subtended myocardial mass. Abstract

Background: The FFRCT Planner has been shown to be accurate and precise to predict post-percutaneous coronary intervention (PCI) fractional flow reserve (FFR). Calcific plaques hamper the coronary computed tomography evaluation due to blooming artifacts. In addition, severe calcifications are associated with lower stent expansion, potentially affecting post-PCI FFR. The accuracy of the FFRCT Planner in calcified vessels remains to be determined. Method(s): In this investigator-initiated, multicenter, prospective study, we recruited patients with chronic coronary syndromes and significant lesions based on an invasive FFR <= 0.80. All patients had computed tomography angiography (CTA) performed in the context of standard of care. CTA images were processed for FFRCT. The FFRCT Planner was applied blinded to the invasive data mirroring the PCI. Calcium was quantified using 2 metrics: 1) calcium burden (calcium volume/plaque volume x 100) and 2) the per-vessel Agatston calcium score. The objective was to assess the accuracy of the FFRCT Planner stratified by calcium burden and score. Result(s): Overall, 120 patients (123 vessels) were included. The mean age was 64.2 +/- 8.9 years, and 24% were diabetics. Invasive FFR post-PCI was 0.88 +/- 0.06, and Planner FFRCT was 0.86 +/- 0.06 (mean difference = 0.02 FFR units, lower limits of agreement = -0.12, and upper limits of agreement = 0.15). The mean calcium burden was 19% +/- 17%, and the per-vessel calcium score was 228.2 +/- 340.7. The mean differences between invasive post-PCI FFR and Planner FFRCT in vessels with low and high calcium burden were 0.03 +/- 0.06 versus 0.01 +/- 0.07 (P = 0.192). Likewise, there was no statistically significant difference in the accuracy of the FFRCT Planner stratified by low or high calcium score (0.02 +/- 0.06 vs 0.01 +/- 0.07, P = 0.526). Conclusion(s): The FFRCT Planner demonstrated high accuracy and precision to predict post-PCI FFR independent of the amount and burden of calcification.

Item does not contain fulltext, OBJECTIVES: The instantaneous wave-free ratio cutoff value of <0.90 for hemodynamic significance of coronary stenoses has been validated in stable patients. We examined different cutoff values in the evaluation of nonculprit stenoses in patients with ST-segment elevation myocardial infarction. METHODS: We measured instantaneous wave-free ratio across nonculprit stenoses in the acute setting and at follow-up in 120 patients with ST-segment elevation myocardial infarction and 157 nonculprit stenoses, of which, 113 patients with 147 nonculprit stenoses completed follow-up. METHODS: The prevalence of nonculprit stenosis hemodynamic significance was 52% in the acute setting and 41% at follow-up. With follow-up, instantaneous wave-free ratio as reference, acute instantaneous wave-free ratio >0.90 had a negative predictive value of 89%. Acute instantaneous wave-free ratio <0.90 had a positive predictive value of 68%. Acute instantaneous wave-free ratio >0.93 had a negative predictive value of 100%. Acute instantaneous wave-free ratio <0.86 and <0.83 had positive predictive values of 71 and 77%. Using acute instantaneous wave-free ratio <0.90 as cutoff for hemodynamic significance yielded the highest degree of classification agreement between acute and follow-up instantaneous wave-free ratio. CONCLUSIONS: In patients with ST-segment elevation myocardial infarction, acute instantaneous wave-free ratio with the cutoff values <0.90 for hemodynamic significance appears optimal in the evaluation of nonculprit stenoses and has a high negative predictive value and a moderate positive predictive value.

Contains fulltext : 225311.pdf (publisher's version ) (Open Access), Objective: To evaluate agreement between instantaneous wave free ratio (iFR) and fractional flow reserve (FFR) for the functional assessment of nonculprit coronary stenoses at staged follow-up after ST-segment elevation myocardial infarction (STEMI). Results: We measured iFR and FFR at staged follow-up in 112 STEMI patients with 146 nonculprit stenoses. Median interval between STEMI and follow-up was 16 (interquartile range 5–32) days. Agreement between iFR and FFR was 77% < 5 days after STEMI and 86% after ≥ 5 days (p = 0.19). Among cases with disagreement, the proportion of cases with hemodynamically significant iFR and non-significant FFR were different when assessed < 5 days (5 in 8, 63%) versus ≥ 5 days (3 in 15, 20%) after STEMI (p = 0.04). Overall classification agreement between iFR and FFR was comparable to that observed in stable patients. Time interval between STEMI and follow-up evaluation may impact agreement between iFR and FFR.

Abstract

Aims The Acute Cardiac Care Association (ACCA)–European Association of Percutaneous Coronary Intervention (EAPCI) Registry on ST-elevation myocardial infarction (STEMI) of the EurObservational programme (EORP) of the European Society of Cardiology (ESC) registry aimed to determine the current state of the use of reperfusion therapy in ESC member and ESC affiliated countries and the adherence to ESC STEMI guidelines in patients with STEMI. Methods and results Between 1 January 2015 and 31 March 2018, a total of 11 462 patients admitted with an initial diagnosis of STEMI according to the 2012 ESC STEMI guidelines were enrolled. Individual patient data were collected across 196 centres and 29 countries. Among the centres, there were 136 percutaneous coronary intervention centres and 91 with cardiac surgery on-site. The majority of centres (129/196) were part of a STEMI network. The main objective of this study was to describe the demographic, clinical, and angiographic characteristics of patients with STEMI. Other objectives include to assess management patterns and in particular the current use of reperfusion therapies and to evaluate how recommendations of most recent STEMI European guidelines regarding reperfusion therapies and adjunctive pharmacological and non-pharmacological treatments are adopted in clinical practice and how their application can impact on patients’ outcomes. Patients will be followed for 1 year after admission. Conclusion The ESC ACCA-EAPCI EORP ACS STEMI registry is an international registry of care and outcomes of patients hospitalized with STEMI. It will provide insights into the contemporary patient profile, management patterns, and 1-year outcome of patients with STEMI.

Abstract

Objectives: The aim of this study was to compare the thin-strut biodegradable-polymer everolimus-eluting platinum-chromium stent (EES) with the biodegradable-polymer biolimus-eluting stainless-steel stent (BES). Background: Currently available drug-eluting coronary stents have been refined to reduce the risk for coronary events following implantation. Methods: This randomized, multicenter, all-comers, noninferiority trial was undertaken at 3 sites in western Denmark. Patients with clinical indications for percutaneous coronary intervention were eligible for inclusion. Patients were randomly assigned (1:1) to either EES or BES. The primary endpoint, target lesion failure, was a composite of safety (cardiac death and myocardial infarction not clearly attributable to a nontarget lesion) and efficacy (target lesion revascularization) at 12 months, analyzed using intention-to-treat principles. The trial was powered to assess target lesion failure noninferiority of the EES compared with the BES with a predetermined noninferiority margin of 3%. Results: A total of 1,385 patients were assigned to treatment with EES and 1,369 patients to treatment with BES. The analysis showed that 55 patients (4.0%) assigned to the EES and 60 (4.4%) assigned to the BES met the primary endpoint (absolute risk difference 0.4%; upper limit of 1-sided 95% confidence interval: 1.7%; p < 0.001). Conclusions: At 1-year follow-up, the EES was found to be noninferior to the BES with respect to target lesion failure.
