IntroductionThere are different types of COVID-19 vaccines approved worldwide. Since no national studies focus on vaccine-related adverse reactions and breakthrough cases, this study aimed to investigate the rate of adverse events and COVID-19 infection in medical students in Iran.MethodsThis retrospective cohort study included Iranian medical students who received two doses of COVID-19 vaccines. The medical team gathered the demographic characteristics, comorbidities, type of vaccine, adverse events following vaccination, and history of COVID-19 infection data through a phone interview. The frequency of adverse events and breakthrough infection was stratified by vaccine type (ChAdOx1-S, Gam-COVID-Vac, and BIBP-CorV).ResultsA total of 3,591 medical students enrolled in this study, of which 57.02% were females, with a mean age of 23.31 + 4.87. A PCR-confirmed and suspicious-for-COVID-19 breakthrough infection rate of 4.51 and 7.02% was detected, respectively. There was no significant relation between breakthrough infection and gender, BMI, blood groups, and comorbidities. However, there was a significant difference in breakthrough infection rate among different types of vaccines (p = 0.001) and history of COVID-19 infection (p = 0.001). A total of 16 participants were hospitalized due to COVID-19 infection after vaccination for reasons such as dyspnea, abnormal imaging, or decreased oxygen saturation. No severe infection or death was observed in the studied population.ConclusionVaccination prevented severe COVID-19 infection, although a high breakthrough infection rate was evident among Iranian medical students during the Delta variant’s peak. Vaccine effectiveness may be fragile during emerging new variants and in high-exposure settings. Moreover, adverse events are rare, and the benefits of vaccination outweigh the side effects. However, many limitations challenged this study, and the results should be cautious.

Immobilized cell reactor (ICR) was developed as a novel bioreactor to convert hydrolyzed sugars to organic acids. Microbial cells were immobilised by cross-linkage of the cell wall in a tubular bioreactor. Kinetic model was obtained using Propionibacterium acid-propionici on solid support. ICR efficiency was measured by consumption of glucose and xylose for production of propionic and acetic acids at several hydraulic retention time (5, 8, 12, 20 and 28 h). First order kinetic model was applied for the present research. The obtained rate constants were 0.79, 0.69, 0.68, 0.68 and 0.51 h-1 at 214, 135, 90, 55 and 38 ml.h-1 flow rates, respectively.

Key Clinical Message Giant pedunculated hepatic hemangiomas, mostly seen in women, are considered a rare type of giant hepatic hemangioma, with challenging diagnosis. Unlike other types of liver hemangiomas, they can manifest different kinds of symptoms, and are prone to life‐threatening manifestations like rupture or torsion. Abstract Hemangioma is the most common benign liver primary tumor. Hepatic hemangioma >4 cm (some studies suggest >10 cm) is referred to as a giant hemangioma. Although hepatic hemangioma does not manifest symptoms in most cases, a giant hepatic hemangioma can manifest different kinds of symptoms. Giant pedunculated hepatic hemangiomas are considered a rare type of giant hepatic hemangioma, with challenging diagnosis, as the thin pedicle could be hard to be detected on imaging. A 41‐year‐old woman was admitted to our hospital, with dull discomfort of the right upper quadrant and epigastric region and early satiety for the past 7 months, with the history of taking oral contraceptive (OCP) for 10 years. Ultrasound and computed tomography revealed a 130 × 124 × 76 mm solid mass, with central cystic lesion, located in the midline of the epigastric region, attaching to the inferior surface of the third segment of the left lobe of the liver. Due to the potential risk for torsion, and rupture of the hemangioma, the management of the patient proceeded to surgical excision. Pathological examination of the specimen confirmed the diagnosis of hepatic hemangioma. Giant pedunculated hepatic hemangioma is a rare benign tumor. It demonstrates higher incidence rate in women, as some hemangiomas have estrogen receptors, and estrogen can lead to endothelial cell proliferation and organization in vascular structure. Most hemangiomas do not express any symptoms; therefore, no treatment is needed except for the patients who manifest symptoms, or in giant pedunculated hemangiomas, as they are prone to rupture or torsion. In this review most cases were female, and most of them presented with abdominal pain, in most cases the tumor located in the left lobe of the liver. Almost all the reviewed cases underwent surgery. Giant hepatic hemangioma is a differential diagnosis of palpable mass, or other symptoms of the right upper quadrant, and epigastric region specially in women taking OCP. Imaging is needed to rule out these tumors, and most often, pedunculated hemangioma is harder to be defined on imaging. It requires surgery because of the risk of acute problems, such as torsion and rupture.

BACKGROUND: Bad news may be defined as “any information which adversely and seriously affects an individual’s view of his or her future.” It seems necessary for physicians to use a specific method to break the bad news to patients properly. Due to the importance of this skill and its effects on patient’s hope and motivation to continue his treatment process, in this study, we evaluate the interns of Guilan University of Medical Sciences’ attitude to breaking bad news (BBN) to the patients based on strategy for BBN, perception of condition or seriousness, invitation from the patient to give information, knowledge: giving medical facts, explore emotions, and sympathize (SPIKES) model in 2020–2022. MATERIALS AND METHODS: In this cross-sectional study in Iran, 153 Guilan University of Medical Sciences interns were selected as a census sample in 2020–2022. A self-administered questionnaire collected the information with standard tests confirming its reliability and validity. The collected data were described and analyzed using Statistical Package for the Social Sciences (SPSS) 16. The Chi-square test was used to measure the statistical relationship between the demographic variables and the entire questionnaire. Also, a one-way analysis of variance (ANOVA) test was used to measure the relationship between the average age and the scores obtained from the four main areas and the entire questionnaire. A statistical level of less than 0.05 was considered significant. RESULTS: 43.1% of the interns were men, and 56.9% were women. The mean attendance age was 26.12 ± 1.32, the minimum age was 23, and the maximum was 33. Only 8.5% of the interns in this study had been taught about BBN, and most participants announced that they feel pressure and anxiety when BBN to patients. The attitude of interns in this study was not satisfying in all four parts of the study: individual preference (54.2% of participants showed poor attitude), preparing environmental conditions for BBN (60.8% of participants showed poor attitude), how to break bad news (52.3% of participants showed poor attitude), and the things that are done after BBN (52.9% of participants showed poor attitude). CONCLUSION: Based on the results, the attitude of the interns who had participated in this study was not satisfactory. Due to the importance of this communication skill to reduce physician anxiety and best control patients’ reactions, managing courses in the undergraduate curriculum seems necessary.
