Abstract

Osteomyelitis, a local infection of the bone or bone marrow, is often caused by infection with more than one organism, and usually occurs after a traumatic injury. The usual treatment is surgical removal of dead tissue and intravenous antibiotics, given for a duration of four to six weeks. In most cases these patients must be hospitalized for this period of time in order to receive the intravenous antibiotics. A medication that can be given orally and is effective against most organisms is ciprofloxacin. However, resistance to this drug develops quickly and therefore it cannot be used indiscriminately. The authors carried out a randomized comparison of oral ciprofloxacin and parenteral (intravenous) antibiotic therapy for treatment of bacterial osteomyelitis in adults. There were 14 patients treated orally with ciprofloxacin and 12 treated with parenterally administered antibiotics, including one or a combination of the following drugs: nafcillin, clindamycin, and gentamicin. Treatment was successful in 10 of the 12 patients given parenteral antibiotics and ll of the 14 treated with ciprofloxacin. In the ciprofloxacin group, minor side effects of the drug occurred in three cases, but therapy could be continued. It was concluded that administration of oral ciprofloxacin was as effective and safe as parenteral antibiotics in treating adult patients with osteomyelitis. The use of oral medication has major advantages because it allows for a shorter, less expensive hospital stay. The authors note that their study was limited in its scope by the small number of patients and the relatively short period of follow-up.
