Vertical loading rate (VLR) and pelvic floor muscle activity (PFA) increase with running velocity, which may indicate a relationship between VLR and PFA. Foot strike pattern has been shown to influence VLR while running, but little is known about its influence on PFA. Twenty healthy women ran on a treadmill for 2 conditions: with a rearfoot strike and with a forefoot strike. PFA was measured with electromyography. Running kinematics associated with VLR were collected using inertial measurement units and tibial accelerometers. Change scores between conditions were calculated for average PFA and running kinematics: peak vertical tibial acceleration, vertical excursion of the center of mass (VO), and cadence. Paired t tests assessed differences between running conditions for all variables. Pearson correlations assessed the relationships between changes in PFA and running kinematics. PFA was significantly higher during the forefoot compared with the rearfoot strike condition. Change in vertical tibial acceleration was positively correlated with change in PFA during the right stance. Change in cadence was negatively correlated, and change in vertical excursion of the center of mass was positively correlated with change in PFA during left stance. To identify pre-operative patient characteristics associated with early recovery of knee range of motion and ambulation without an assistive device after TKA. 95 subjects completed testing 2-4 weeks prior to surgery and 1 month after surgery. Binary logistic regression models were performed using patient's pre-operative measures to predict knee functional knee range of motion (ROM) and early independent mobility 1 month after TKA. Functional ROM was defined as active knee flexion ≥100° and Ext ≤5°. Independent mobility was defined as no need for an assistive device during the Six Minute Walk (6 MW) test. Nagelkerke's R2, and model and step significance were assessed. One month after surgery, 56 (58.9%) subjects did not have functional ROM. 75 (78.9%) subjects did not use an AD during the 6 MW. There were no significant predictors of early ROM recovery (R2=0.142, p-value = 0.06) or independent mobility (R2N=0.138, p-value = 0.116). Pre-operative measures were not predictive of early recovery rates one month after TKA, making pre-operative ROM and mobility risk stratification difficult. Future research should evaluate other factors, such as surgical pain and swelling, which may be better predictors of early outcomes after TKA. [ABSTRACT FROM AUTHOR]

Background: Few studies have compared the biomechanical outcomes of different surgical approaches for hip arthroplasty. The purpose of this study was to compare hip, pelvic, and trunk kinematics and kinetics between individuals who underwent a posterior or anterolateral approach.Methods: Forty-five individuals between 40 and 80 years old underwent motion analysis during overground gait prior to hip arthroplasty and 3 months after surgery. Walking speed, hip flexion angle, hip extension angle, adduction angle and moment, trunk angle, trunk lean, and pelvis drop were compared between approaches.Findings: There were 30 subjects in the posterior group and 15 subjects in the anterolateral group. The groups did not change differently over time as there were no significant interaction effects. However, there were main effects for time; walking speed increased 19.9% (p < .001), hip flexion angle increased 3.3 degrees (p = 0.014) and peak hip extension increased 4.5 degrees (p = .001), and peak hip adduction significantly increased 1.9 degrees (p = .004) for the sample as a whole. Trunk angle (p = .283) and trunk lean (p = .401) did not significantly change between time points, but there was a significant increase in pelvic drop (p = .003).Interpretation: Surgical approach did not affect biomechanical outcomes 3 months after arthroplasty. Both groups showed improvement in sagittal plane hip kinetics and kinematics. However, increased pelvic drop may be indicative of residual hip weakness in both groups. [ABSTRACT FROM AUTHOR]

Background and Purpose: Most rehabilitation interventions after total hip arthroplasty (THA) are not designed to return patients to high-levels of physical activity and, thus, low levels of physical activity and residual weakness are common. The purpose of this case series was to describe the feasibility and preliminary efficacy of an exercise and education intervention for patients after THA who have already completed formal outpatient physical therapy. Study Design: Case series Case Description: Two participants underwent unilateral THA seven (case A) or eight (case B) months prior to the intervention. Individuals participated in 18 treatment sessions that included progressive aerobic and strengthening exercises and meetings with a health coach. Change in function, strength, and self-reported physical activity were measured. Outcomes 12 months after surgery were compared to a historical cohort of patients after THA. Outcome: There were no adverse events during the intervention. At the end of the intervention, hip and knee strength on the surgical side increased approximately 30% compared to baseline in both cases. Activity level, and recreational performance, including walking up stairs and hiking uphill (case A), and running and golfing (case B), improved by the end of the intervention. Most changes were maintained at follow-up, although hip strength for case B decreased 27% after discharge from the intervention. Outcomes for both cases exceeded historical averages for patients 12 months after THA, with the exception of strength in case B. Discussion: The exercise intervention was well tolerated and without negative effects in two participants. Both participants increased their ability to complete demanding recreational and sports-related activities, physical activity, and demonstrated improved hip abductor and knee extensor strength.
