Resumo O objetivo deste artigo foi analisar a prevalência e os fatores associados a quedas em idosos adscritos a uma Unidade Básica de Saúde (UBS), no município de Natal/RN. Estudo transversal realizado com 280 idosos cujos dados foram coletados nas dependências da UBS. A associação entre os desfechos “queda” e “queda recorrente” e as variáveis independentes foi verificada mediante análise bivariada e regressão de Poisson, com cálculo das respectivas razões de prevalências. Prevaleceram os idosos do gênero feminino (68,2%), com média de idade de 71,6 anos (± 6,7), alfabetizados (54,6%), não aposentados (73,5%) e sedentários (87,1%). Registraram queda 53,6% dos idosos, porém 27,8% caíram duas ou mais vezes. O modelo preditivo de queda incluiu o gênero feminino (RP = 1,81), presença de doenças osteoarticulares (RP = 1,71) e comprometimento do equilíbrio (RP = 0,88), enquanto a mobilidade funcional (RP = 0,94), medo de cair (RP = 1,21) e déficit de equilíbrio (RP = 0,80) compuseram o modelo final de quedas recorrentes. Encontrou-se maior prevalência de episódio único de queda e os fatores associados incluíram variáveis sociodemográficas, de saúde e performance física. Por outro lado, associaram-se a ocorrência de duas ou mais quedas apenas as variáveis de performance física. Abstract The scope of the study was to analyze the prevalence and factors associated with falls among the elderly registered in a Primary Healthcare Unit (PHU) in Natal, State of Rio Grande do Norte. It involved a cross-sectional study conducted with 280 elderly individuals with data collected at the primary healthcare unit. The association between falls, recurrent falls and independent variables was assessed using bivariate analysis and Poisson regression with calculation of their respective prevalence ratios. It involved predominantly elderly females (68.2%) with mean age of 71.6 years (± 6.7), literate (54.6%), non-retired (73.5%) and sedentary (87.1%). 53.6% of the elderly recorded falls and 27.8% fell twice or more. The predictive model of falls included the female gender (PR= 1.81), osteoarticular diseases (PR = 1.71) and balance impairment (PR = 0.88), while functional mobility (PR = 0.94), fear of falling (PR = 1.21) and balance deficit disorders (PR = 0.80) constituted the definitive model of recurrent falls. A higher prevalence of single fall episodes was found and the associated factors included sociodemographic, health and physical performance variables. On the other hand, the occurrence of two or more falls were only associated with physical performance variables.

OBJECTIVE: The presence of peripheral arterial disease (PAD) increases the risk and vulnerability to adverse clinical outcomes in the elderly. Therefore, we investigated the relationship between functional performance and indicators of PAD in elderly women. METHODS: Cross-sectional study in which 54 elderly were assessed by means of the Mini Mental State Examination (MMSE), Short Physical Performance Battery (SPPB), Ankle-Brachial Index (ABI), Human Activity Profile (HAP) and Edinburgh questionnaire. Statistical analysis was performed using ANOVA, Student's t test and Pearson correlation. We considered p < 0.05 as the significance level. RESULTS: The mean age, SPPB and ABI were 69.2 (± 6.9) years, 9.42 (± 2.55) and 1.04 (± 0.14), respectively. The prevalence of PAD was of 16.3%. There was a significant correlation between ABI and gait speed (r = 0.75, p = 0.001) and between PAH and SPPB (p = 0.001). CONCLUSIONS: It is suggested that the decline in functional performance in the elderly, expressed in the gait velocity component of SPPB, is related to the presence of PAD.

A fragilidade em idosos resulta da complexa interação entre fatores clínicos e sociais envolvidos na sua gênese. Porém, ainda não se sabe, de forma clara, se esta síndrome pode ser agravada devido à ausência do apoio social. Assim, o objetivo do presente estudo foi analisar a associação entre este e a síndrome da fragilidade em idosos residentes na comunidade. Trata-se de estudo observacional analítico de caráter transversal, com uma amostra de 300 idosos. Foram coletadas informações sobre os dados sociodemográficos e econômicos e de saúde física, fragilidade e apoio social. Para análise estatística, foram realizados o teste qui-quadrado de Pearson, o teste t de student e a regressão logística binária, adotando-se um nível de significância de 5%. Não foram observadas associações da fragilidade com as variáveis do apoio social, com exceção da modalidade tarefas domésticas (p = 0,04). Na regressão logística, permaneceram associadas a fragilidade à idade (OR = 1,19), à atividade física (OR = 2,56) e à saúde percebida (OR = 3,33). Acredita-se que mais estudos precisam ser desenvolvidos para uma caracterização da vulnerabilidade social, como também os serviços de saúde necessitam reconhecer a importância do apoio social como parte integrante da prestação de cuidados aos idosos. Frailty among the elderly is a result of the complex interplay between social and clinical factors involved in its genesis. However, it is still unclear whether the fraailty syndrome can be aggravated due to lack of social support. The scope of this study was to analyze the association between social support and the frailty syndrome among elderly residents in the community. It is an analytical observational cross-sectional study, with a sample of 300 elderly people. Socio-demographic, economic and physical health, frailty and social support data were collected. For statistical analysis, the Pearson chi-square, the Student t and the binary logistic regression were performed, adopting a significance level of 5%. No associations were observed between frailty and the social support variables, except for the housekeeping aspect (p = 0.04). In logistic regression frailty with age (OR = 1.19), physical activity (OR = 2.56) and self-perceived health (OR = 3.33) remained associated. It is considered that more studies need to be conducted to characterize social vulnerability and health services also need to recognize the importance of social support as an integral part of care for the elderly.

OBJETIVO: Este estudo teve como propósito analisar a capacidade da velocidade da marcha em identificar idosos com medo de cair. METODOLOGIA: Trata-se de estudo transversal analítico, realizado com amostra de 60 idosos da comunidade. Os dados foram obtidos por meio de questionário estruturado, análise da velocidade da marcha para percorrer a distância de 4,6 m e do medo de cair, avaliado pela escala de Autoeficácia de Quedas. The aging process if characterizes for a complex events network, from multidimensional nature, that encloses biological, social, psychic and functional aspects. The alteration of one or more aspects can speed up the aging process, anticipating limitations and until the death in the aged. For an adjusted confrontation of this question is necessary an interdisciplinary vision, in which the some areas of the knowledge can interact and with this to intervenes of the best possible form. Then, information derived from studies of aspects related to incidence, morbidity-mortality and transition patterns, involved in the health-illness process can more accurately identify risk groups thereby establishing links between social factors, illness, incapacity and death. Thus, this study aimed to identify, by a multidimensional vision, the risk factors of mortality in a coorth of elderly in a city in the interior of the state of Rio Grande do Norte (RN), Brazil. A prospective study carried out in Santa Cruz RN, where 310 elderly were randomly selected to form a baseline. The follow-up was 53 months. The predictive variables were divided into sociodemographic, physical health, neuropsychiatric and functional capacity. The statistical analysis carried out by bivariate analysis, survival analysis, followed by binary logistic regression and Cox regression, in the multivariate analysis, considering significant levels p < 0.05 and confidence interval (CI) of 95%. A total of 60 (19.3%) elderly died during the follow-up, where cardiovascular disease was the main cause. The survival was approximately 24.8 months. The study of general survival showed, at 12, 24, 36, and 48 months of observation, a survival rate of 97%, 54%, 31%, and 5% respectively, with a statistical difference in survival only observed for the variables of cognitive function and Basic Activities of Daily Living. In the logistic regression analysis, the risk factors identified were cognitive deficits (OR = 8.74), poor perception of health (OR = 3.89) and dependence for Basic Activities of Daily Living (OR = 3.96). In the Cox analysis, as well as dependence for Basic Activities of Daily Living (HR = 3.17), cognitive deficit (HR = 4.30) and stroke (CVA) (HR = 3.49) continued as independent risk factors for death. The risk factors found in the study can be interpreted as the primary predictors for death among elderly members of the community. Therefore, improvements in health conditions, with actions towards sustaining an autonomous life with special attention for elderly with cognitive impairment, could mean additional healthy quality of life, resulting in the reduction of premature mortality in this population O processo de envelhecimento se caracteriza por uma complexa rede de eventos, de natureza multidimensional, que abrange aspectos biológicos, sociais, psíquicos e funcionais. A alteração de um ou mais aspectos pode acelerar este processo, antecipando limitações e até o óbito no idoso. Para um adequado enfrentamento desta questão é necessária uma visão interdisciplinar, nas quais as várias áreas do conhecimento possam interagir e com isso intervir da melhor forma possível. Neste sentido, informações oriundas de estudos dos aspectos relacionados à incidência, morbi-mortalidade e padrões de transição, envolvidos no processo saúde-doença podem identificar com mais precisão grupos de risco, estabelecendo vínculos entre os fatores sociais, enfermidades, incapacidade e morte. Desta forma, este estudo objetivou identificar, dentro de uma visão multidimensional, os fatores de risco às condições de saúde e mortalidade em uma coorte de idosos numa cidade do interior do Estado do Rio Grande do Norte (RN), Brasil. Foi realizado um estudo prospectivo na cidade de Santa Cruz-RN, onde 310 idosos aleatoriamente selecionados formaram a linha de base. O tempo de acompanhamento foi de 53 meses. As variáveis preditoras foram divididas em sociodemográficas, saúde física, neuropsiquiátricas e capacidade funcional. A análise estatística foi feita mediante análise bivariada, análise de sobrevida, seguida de regressão logística binária e regressão de Cox, na análise multivariada, considerando nível de significância p< 0,05 e intervalo de confiança (IC) de 95%. Um total de 60 (19,3%) idosos morreu durante o seguimento, no qual a doença cardiovascular foi a principal causa de óbito. A sobrevida foi de aproximadamente 24,8 meses. O estudo da sobrevida geral demonstrou, aos 12, 24, 36 e 48 meses de acompanhamento, sobrevidas de 97%, 54%, 31% e 5%, respectivamente, sendo apenas, nas variáveis função cognitiva e atividades básicas da vida diária observadas diferenças estatísticas nas curvas de sobrevida. Na análise de regressão logística, os fatores de risco identificados foram déficit cognitivo (OR= 8,74), má percepção de saúde (OR= 3,89) e dependência para ABVD´s (OR= 3,96). Na análise de Cox permaneceram como fatores de risco independentes para o óbito, além da dependência para atividades básicas da vida diária (HR= 3,17), o AVC (HR= 3,49) e o déficit cognitivo (HR= 4,30). Os fatores de risco encontrados no estudo podem ser interpretados como os principais preditores para o óbito, em idosos residentes na comunidade. Desta forma, melhorias nas condições de saúde, com ações voltadas a manutenção de uma vida com autonomia e atenção especial aos idosos com déficit cognitivo pode significar qualidade de vida adicional saudável, resultando na diminuição da mortalidade prematura desta população

OBJETIVO: Analisar a influência dos fatores sociodemográficos, de saúde física e neuropsiquiátricos sobre a capacidade funcional de idosos residentes no município de Santa Cruz, Rio Grande do Norte, Brasil. MÉTODOS: Desenvolveu-se um estudo transversal, no qual uma amostra representativa de 310 idosos residentes na zona urbana da cidade de Santa Cruz-RN foi entrevistada, a partir da utilização de um questionário que continha informações sobre perfil sociodemográfico, condições de saúde física, função cognitiva e sintomatologia depressiva, além dos Índices de Katz e Lawton, que mediram as atividades básicas (ABVD's) e instrumentais (AIVD's) da vida diária. A análise estatística foi feita mediante análise bivariada (Qui-quadrado de Pearson), seguida de regressão logística binária na análise multivariada, com a respectiva odds ratio (OR), considerando nível de significância p< 0,05 e intervalo de confiança (IC) de 95%. RESULTADOS: Para o Índice de Katz teve-se que 86,9% eram independentes e no Índice de Lawton apenas 47,4%. Na análise multivariada houve associação com as ABVDs a idade (OR= 1,06) e má percepção de saúde (OR= 3,64). Nas AIVD's houve associação com a idade (OR= 1,18), sexo feminino (OR= 4,30), analfabetismo (OR= 3,78), estado civil (OR= 2,14), sintomatologia depressiva (OR= 4,54) e a má percepção de saúde (OR= 4,47). CONCLUSÃO: As variáveis associadas à incapacidade funcional revelam uma complexa relação entre o estado de saúde dos idosos e possíveis fatores de risco de declínio físico. OBJECTIVE: This study aims to assess the influence of sociodemographic factors, physical health, and neuropsychiatric factors on the functional capacity of the elderly who live in the city of Santa Cruz, Rio Grande do Norte, Brazil. METHODS: It was a cross-sectional study, where a representative sample of 310 elderly who live in the urban area of the city of Santa Cruz-RN was interviewed, with a questionnaire, which contained information on sociodemographic profile, physical health conditions, cognitive functioning and depressive symptoms, in addition to Katz's and Lawton's indices to measure basic activities of daily living (BADL's) and instrumental activities of daily living (IADL's). The statistical analysis was made through bivariate analysis (Pearson chi-square tests), followed by binary logistic regression in multivariate analysis, with the respective odds ratio (OR), considering a p< 0.05 significance level and 95% confidence intervals (CI 95%). RESULTS: In the Katz's Index, 86.9% were independent and in Lawton's Index, only 47.4% were so. In the multivariate analysis, age (OR= 1.06) and poor health perception (OR= 3.64) were associated with BADL's. For IADL's, there was association with age (OR= 1.18), female gender (OR= 4.3), illiteracy (OR= 3.78), marital status (OR= 2.14), depressive symptoms (OR= 4.54) and poor health perception (OR= 4.47). CONCLUSION: Variables associated with functional disability showed a complex relationship between the health status of the elderly and possible risk factors for physical decline.

OBJETIVO: Analisar a influência de fatores sociodemográficos, de saúde física, capacidade funcional e função cognitiva sobre a sintomatologia depressiva de idosos do município de Santa Cruz, no Rio Grande do Norte. MÉTODOS: Estudo com delineamento transversal de base populacional, incluindo 310 idosos, acima de 60 anos, residentes na zona urbana da cidade, nos quais se aplicou a Escala de Depressão Geriátrica (GDS-15).Aanálise estatística foi realizada com nível de significância p = 0,05, com cálculo da respectiva odds ratio (OR) na regressão logística binária. RESULTADOS: Encontrou-se uma prevalência de 25,5% de sujeitos considerados casos de depressão, nos quais, a partir de análise multivariada, verificou-se associação significativa com idade acima de 75 anos (p = 0,046), analfabetismo (p = 0,037), má percepção de saúde (p < 0,001) e dependência para atividades instrumentais da vida diária (AIVD) (p = 0,001). CONCLUSÕES: As variáveis idade acima de 75 anos, analfabetismo, má percepção de saúde e dependência para AIVD estiveram associadas de forma independente à presença de sintomatologia depressiva nos idosos da nossa população. Os autores discutem que a identificação de fatores que influenciam o surgimento de sintomas depressivos em idosos constitui passo fundamental para o planejamento das ações que visem reduzir os efeitos dessa enfermidade na qualidade de vida dessas pessoas. OBJECTIVE: To analyze the influence of socio-demographic factors, physical health, functional status and cognitive function on depressive symptomatology of the elderly from Santa Cruz, Rio Grande do Norte. METHODS: A cross-sectional study was conducted in a sample constituted by 310 elderly above 60 old years who live in an urban zone of the town in which the Geriatric Depression Scale (GDS-15) was applied. The statistical analysis was carried out with significance level p = 0.005, and the respective odds ratio (OR) was calculated in the binary logistic regression. RESULTS: A prevalence of 25.5% of subjects considered cases of depression was found, and after a multivariate analysis significant association with age over 75 years (p = 0.046), illiteracy (p = 0.037), poor health perception (p < 0.001) and dependency for instrumental activities of daily living (p = 0.001) was verified. CONCLUSIONS: The variables age above 75 years, illiteracy, bad health perception and being dependent in basic activities of daily living were independently associated with the presence of depressive symptoms in this population. The authors suggest that the identification of factors that influence the onset of depressive general situations in the elderly is a fundamental step to plan actions to reduce the effects of this condition on these people's living quality.
