Nawrotowe zapalenie przedniego odcinka błony naczyniowej oka(ZPOBN) to choroba autoimmunologiczna, która u ok. 20% chorychdoprowadza do trwałego ciężkiego upośledzenia widzenia lub ślepotyi, co z tego wynika, ciężkiego kalectwa i istotnego pogorszeniajakości życia. Dlatego oprócz intensywnego leczenia fazy ostrejzapalenia i jej powikłań bardzo ważne jest leczenie przewlekłe wceluutrzymania remisji w możliwie jak najdłuższym czasie. Aż u 50%chorych na nawrotowe ZPOBN nie udaje się potwierdzić współistnieniażadnej choroby układowej. Sytuacja ta, określana mianemidiopatycznego ZPOBN (IZPOBN), stwarza duże problemy w doborzestrategii terapeutycznej. Tym niemniej uważa się, że IZPOBN możebyć pierwszym, zwiastunowym objawem spondyloartropatii, którynawet o kilkadziesiąt lat może wyprzedzać pojawienie się objawówtypowych dla tego zespołu chorobowego. Dotychczas na tematskuteczności terapii IZPOBN lekami z grupy leków przeciwreuma -tycz nych, które modyfikują przebieg tej choroby, opublikowanojedynie pojedyncze doniesienia uwzględniające obserwacje małychgrup pacjentów.Przedstawione w niniejszym artykule zalecenia do leczenia systemowegoIZPOBN zostały opracowane na podstawie doświadczeńośrodków klinicznych, w których praktykują autorzy artykułu, orazanalizy dostępnych opracowań naukowych.

3. Does trastuzumab-related cardiotoxicity influence long-term outcome in patients with HER-2 positive breast cancer? A prospective study

IntroductionTrastuzumab is a monoclonal antibody directed against the HER-2 receptor that has led, in an adjuvant setting, to higher disease-free (DFS) and overall survival (OS) in HER-2 positive breast cancer (BC) compared with chemotherapy alone. Cardiotoxicity often results in early discontinuation of trastuzumab, which may elevate the risk of cancer recurrence or mortality. Our study aimed to assess how early interruption or early permanent termination of adjuvant trastuzumab treatment influences DFS and OS of patients with HER-2 positive BC.Material and methodsThis is a prospective observation of 253 women (55 ±10 years of age) with HER-2 positive unilateral, non-metastatic BC treated with trastuzumab in an adjuvant setting. To monitor the safety of the treatment echocardiography was performed at baseline and every 3 months up to 12 months after the end of therapy. If cardiotoxicity developed, trastuzumab was stopped early. Overall survival and DFS were assessed.ResultsTrastuzumab-associated cardiac complications resulting in treatment discontinuation developed in 52 (20.55%) patients. Median DFS time was 21.1 months in the group with interruption compared with 25.7 months in the group with full trastuzumab treatment, being significantly shorter (HR = 2.32, 95% CI: 1.15–4.71, p = 0.0106). Two year OS in the interruption and no-interruption groups were 80.8% and 88.5%, respectively, which were not statistically significantly different (p = 0.268). In a multivariate regression analysis the cumulative dose of anthracycline (OR = 1.01, 95% CI: 1.00–1.01, p = 0.002) and LVEF at baseline (OR = 0.83, 95% CI: 0.70–0.99, p = 0.0344) were independent predictors of a cardiotoxic effect.ConclusionsTrastuzumab-related cardiotoxicity resulting in early treatment discontinuation negatively influences DFS, but does not seem to influence OS.

Vitamin D deficiency is common in rheumatoid arthritis (RA) and may be related to disease activity. Population-based studies have shown the influence of vitamin D deficiency on quality of life (QoL), but it was not investigated in RA patients.The aim of the study was to determine possible relationship between vitamin D deficiency, QoL, physical activity (PA), and disease activity in RA.In 97 consecutive RA patients without vitamin D supplementation (86 women and 11 men, aged 59.4 ± 12 years), serum 25-hydroxycholecalciferol (25(OH)D), calcium, phosphorus, and parathyroid hormone were measured. The patients completed Short Form 36 (SF-36), Beck Depression Inventory, and Health Assessment Questionnaire, assessed the intensity of pain, fatigue, and PA. Disease Activity Score in 28 Joints was used to assess disease activity. A comparison control group consisted of 28 osteoarthritis patients (25 women and 3 men aged 56.2 ± 15 years).Vitamin D deficiency was detected in 76.3% of RA and in 78.6% of osteoarthritis patients (P = 0.75). There was a negative correlation between 25(OH)D serum concentration and Disease Activity Score in 28 Joints in patients with active arthritis. There was a positive correlation between serum 25(OH)D and the level of PA and most aspects of SF-36, and negative correlation between serum 25(OH)D and Health Assessment Questionnaire and Beck Depression Inventory in patients with disease duration of 1 year or longer. After inclusion of PA into multivariable analysis, only the correlations between 25(OH)D and SF-36 mental subscale (MCS) and pain remained significant.Vitamin D deficiency is highly prevalent in RA patients and is associated with higher disease activity and worse QoL indices. Regular PA correlates with higher vitamin D titers and better QoL in RA. Further studies are needed to explain possible influence of vitamin D on RA activity.

Probiotics and isoflavones have a promising therapeutic effect on calcium status and bone health, but there are only limited studies focusing on these products. The present study aimed to 1) determine the potential bioaccessibility of calcium from soybean products (raw soybean and tempeh) and a probiotic strain (Lactobacillus acidophilus) and to 2) evaluate whether the studied products and strain have an impact on the potential bioaccessibility of calcium from inorganic and organic calcium salts. The bioaccessibility of calcium was determined based on enzymatic in vitro digestion, and calcium content by flame atomic absorption spectrometry. The average potential calcium bioaccessibility from tempeh was 10% and from raw soybean was 2%, while probiotics could release over 85% of potential bioaccessible calcium during enzymatic digestion. In conclusion, the potential bioaccessibility of calcium from tempeh and soybeans is low. Soybean products decrease the bioaccessibility of calcium from organic and inorganic calcium salts, while probiotics could improve the bioavailability of calcium from these salts.

Cel pracy: Zbadanie jakości życia chorych na reumatoidalne zapalenie stawów (RZS) oraz określenie jej związku z wiekiem, czasem trwania, aktywnością i stopniem zaawansowania choroby oraz stężeniem witaminy D3 w surowicy. Materiał i metody: Do badania zakwalifikowano kolejnych 119 chorych na RZS (107 kobiet i 12 mężczyzn w wieku 59,7 ±11,7 roku) leczonych w Klinice Chorób Wewnętrznych i Reumatologii WIM CSK MON, u których oznaczono stężenie witaminy D3 w surowicy. Chorzy wypełnili kwestionariusze: SF-36, skalę depresji Becka, HAQ, oraz określili w skali VAS stopień nasilenia bólu i zmęczenia. W badaniach laboratoryjnych oznaczono: morfologię krwi obwodowej, odczyn Biernackiego, stężenie kreatyniny, wapnia, fosforanów i 25-hydroksycholekalcyferolu w surowicy. Aktywność choroby określono według wskaźnika DAS28. W analizie statystycznej zastosowano metodę Statistica wersja 10. Wyniki: Wskaźnik DAS28 korelował ujemnie z wynikami skali SF-36 i skali Becka. Chorzy z niedawno rozpoznanym RZS wykazywali gorsze wyniki w skali depresji i w podskalach SF-36 dotyczących wpływu emocji i bólu. Wiek chorych, czas trwania choroby, stopień uszkodzenia stawów oraz stężenie witaminy D3 w surowicy nie korelowały istotnie ze wskaźnikami jakości życia. Wnioski: Reumatoidalne zapalenie stawów wywiera znaczący wpływ na jakość życia poprzez ból i upośledzenie fizyczne. Chorzy z niedawno rozpoznanym RZS częściej cierpią z powodu bólu i obniżonego nastroju niż chorzy przewlekle leczeni. Aktywność choroby wykazuje niewielki negatywny wpływ na jakość życia chorych na RZS. Konieczne są dalsze badania, aby określić znaczenie niedoboru witaminy D3 dla jakości życia tych chorych.

9. Isoflavones and probiotics effect on bone calcium and bone cells in rats

Isoflavones and probiotics have shown the therapeutic potential to alter calcium absorption and bone cell metabolism. This study sought to ascertain the effect of isoflavones and probiotics on calcium status and bone health in healthy female rats. Forty-eight adult female Wistar rats were grouped and fed: a standard diet (control); and standard diets with tempeh; soy; daidzein and genistein; Lactobacillus acidophilus; and a combination of daidzein, genistein, and L. acidophilus. The biochemical serum parameters, such as alanine transaminase, aspartate transaminase, glucose, and triacylglycerol concentrations, were measured, and calcium contents in tissues were determined. After staining the bone with hematoxylin and eosin, the number of osteoblasts, osteocytes, and the percentage of bone marrow adipocytes were counted. Compared with the control group, the soy group showed a significantly lower triacylglycerol concentration. The L. acidophilus group considerably increased the calcium content in the femoral bone. The daidzein and genistein, L. acidophilus, and a combination of daidzein, genistein, and L. acidophilus groups showed significantly lower calcium contents in the heart and kidneys. The daidzein and genistein group significantly enhanced the number of osteoblasts and osteocytes. A substantial inverse correlation was observed between calcium contents in kidneys and osteoblasts. In conclusion, the combination of daidzein, genistein, and L. acidophilus may improve bone calcium concentrations and bone cells. However, no synergistic effect between isoflavones and probiotics was detected in this study.

10. Influence of Isoflavones and Probiotics on Magnesium Status in Healthy Female Rats

Isoflavones and probiotics are promising nutrients for bone health, and magnesium (Mg) is essential for bone metabolism. This study aimed to determine the effects of daidzein, genistein and Lactobacillus acidophilus on the Mg status of healthy female rats. Forty-eight rats were randomly assigned to six groups, with the control group receiving a standard diet (AIN 93M). The remaining groups were fed the same diet with added ingredients such as tempeh flour; soy flour; pure daidzein and genistein; L. acidophilus or a combination of daidzein, genistein, and L. acidophilus. Tissue samples were collected after the eight-week intervention, and Mg concentrations were analysed using flame atomic absorption spectrometry. Myeloid and erythroid cells were determined using the haematoxylin and eosin bone staining method. Statistical analyses were conducted using one-way ANOVA with Tukey’s post hoc test and Pearson’s correlation coefficient. The threshold for significance was p < 0.05. Compared with the control group, adding tempeh to the diet of rats resulted in significant changes in Mg concentrations in various tissues, with a decrease in the kidneys and an increase in the fur. Although not statistically significant compared to the control group, the tempeh group showed increased Mg concentrations in the femur and spleen. The myeloid-to-erythroid cell ratio did not differ significantly among groups, but all intervention groups showed higher ratios than the control group. A strong negative correlation was observed between Mg concentrations in the kidneys and fur. Conversely, a positive correlation was identified between Mg concentrations in the pancreas and fur. Daily consumption of tempeh may improve Mg status in the organism. Intake of pure daidzein, genistein, or probiotic seems to have no effect on Mg concentrations in healthy rats.

The utilization of food industrial by-products, especially pomace after juice processing, could be a big challenge. Chokeberry pomace is a valuable source of phenolic compounds, including procyanidins and anthocyanins. These compounds have, e.g., antioxidant, anticancer, hypoglycemic, hypotensive, hepatoprotective and cardioprotective effects. In this work, dried chokeberry pomace (2.0 g, 2.5 g and 3.0 g) was added to green tea leaves or white or red hibiscus flowers (0.5 g of each) to obtain mixtures for preparing infusions. Such products could be a valuable source of bioactive compounds. After brewing and cooling, selected quality parameters of the obtained infusions were tested: pH, total phenolic content (TPC), anthocyanin content and the instrumentally and sensorily determined color. The composition of the infusion had an impact on the color. Those from a mixture of chokeberry by-products and hibiscus possessed lower L* and higher a* and b* values than those composed of chokeberry and green tea leaves. Infusions prepared from the mixture containing dried chokeberry pomace (3.0 g) and green tea leaves (0.5 g) had a higher content of anthocyanins (17%) and phenols (48%), respectively, than other samples with the same proportions of ingredients. According to PCA analysis, the highest content of the tested bioactive compounds was obtained in infusions of green tea leaves with the addition of chokeberry pomace.

Recurrent anterior uveitis (AU) is an autoimmune disease that in 20% of patients leads to severe visual loss or blindness, thereby a severe disability and significant quality of life impairment. That is why, it is important to conduct a long term treatment after an acute phase of the disease, to keep the remission as long as possible. Up to 50% of patients with recurrent AU have no additional symptoms of systemic disease. This situation, classified as idiopathic AU (IAU), causes a lot of problems when a therapeutic strategy has to be chosen. Nevertheless, IAU is considered to be a first symptom of spondylarthropathy (SpA), even several dozen years prior to other typical manifestations of SpA. There have been published few papers concerning only small groups of patients with IAU, treated with disease modifying anti-rheumatic drugs. The authors present the recommendations for systemic treatment of IAU, based on their own clinical experience, and systematic review of the literature.

15. Isoflavone composition, polyphenols content and antioxidative activity of soybean seeds during tempeh fermentation

The content of polyphenols and the antioxidative properties were determined in soybean tempeh fermented by four strains of Rhizopus oligosporus. The highest concentration of polyphenols was observed in soybean fermented for 4 days by R. oligosporus strain NRRL 5905 (5.307 mg/g). The strength of antioxidative activity increased from the first day of fermentation to the fourth day, and on the fourth and fifth fermentation day it was near 12 times higher than in unfermented material. Isoflavone content and composition were analysed. The content of isoflavone aglycones (daidzein and genistein) increased up to the third day of fermentation. Fermentation time and the kind of used R. oligosporus strain are factors that enable creation of prohealthy quality of tempeh.

l-3,4-dihydroxyphenylalanine (l-DOPA) is a medically relevant compound in Parkinson’s disease therapy. Several extraction methods of l-DOPA from beans, including velvet and faba beans, have been described in the literature. However, these methods require the use of strong acids, long extraction times, or complex downstream processing, which makes the extraction of l-DOPA expensive and energy-demanding, limiting its industrial application. In addition, the stability of l-DOPA during the extraction process is critical, further complicating the extraction of adequate amounts of this amino acid. This work is the first report on a simple, rapid, greener, and robust extraction method of l-DOPA. The developed method consists of a quick homogenization step followed by a double extraction with 0.2% v/v acetic acid for 20 min and was applied to faba bean at a ratio of 1:25 with respect to the extracting solvent. This study also investigated the stability of l-DOPA during extraction and thermal treatment. The proposed method demonstrated to be robust and extraordinarily efficient for numerous cultivars of faba bean, velvet bean, and food products containing faba beans.
