This paper reports preliminary yet compelling kinematical inferences for N ~ 600 carbon-rich dwarf stars that demonstrate around 30% to 60% are members of the Galactic halo. The study uses a spectroscopically and non-kinematically selected sample of stars from the SDSS, and cross-correlates these data with three proper motion catalogs based on Gaia DR1 astrometry to generate estimates of their 3-D space velocities. The fraction of stars with halo-like kinematics is roughly 30% for distances based on a limited number of parallax measurements, with the remainder dominated by the thick disk, but close to 60% of the sample lie below an old, metal-poor disk isochrone in reduced proper motion. An ancient population is consistent with an extrinsic origin for C/O >1 in cool dwarfs, where a fixed mass of carbon pollution more readily surmounts lower oxygen abundances, and with a lack of detectable ultraviolet-blue flux from younger white dwarf companions. For an initial stellar mass function that favors low-mass stars as in the Galactic disk, the dC stars are likely to be the dominant source of carbon-enhanced, metal-poor stars in the Galaxy., Comment: 7 pages, 1 table, and 3 figures. No contexto de expansão urbana, do desenvolvimento sustentável e da discussão do planejamento do município de São Paulo, este artigo propõe apresentar a relação entre o Marco Regulatório Urbanístico da cidade de São Paulo, desenvolvimento entre 2014 e 2016, e o desenvolvimento urbano sustentável, orientado pelos Objetivos de Desenvolvimento Sustentável da ONU, frente às questões climáticas. Após 8 anos de monitoramento do Marco Regulatório, é possível identificar quais elementos induziram e foram protagonistas em implantar os conceitos de um desenvolvimento urbano sustentável, em termos de políticas públicas urbanas, projetos e planos, ações e iniciativas isoladas, bem como os próprios instrumentos urbanos. Como iniciativa do governo municipal em prol do desenvolvimento alinhado com as preocupações globais relacionadas ao clima, como o Acordo de Paris, o plano municipal de Ação Climática, o PlanClima SP, é apresentado e representa a sinergia quanto aos conceitos de desenvolvimento sustentável no planejamento urbano.

- Abstract
Studies of horse evolution arose during the middle of the 19th century, and several hypotheses have been proposed for their taxonomy, paleobiogeography, paleoecology and evolution. The present contribution represents a collaboration of 19 multinational experts with the goal of providing an updated summary of Pliocene and Pleistocene North, Central and South American, Eurasian and African horses. At the present time, we recognize 114 valid species across these continents, plus 4 North African species in need of further investigation. Our biochronology and biogeography sections integrate Equinae taxonomic records with their chronologic and geographic ranges recognizing regional biochronologic frameworks. The paleoecology section provides insights into paleobotany and diet utilizing both the mesowear and light microscopic methods, along with calculation of body masses. We provide a temporal sequence of maps that render paleoclimatic conditions across these continents integrated with Equinae occurrences. These records reveal a succession of extinctions of primitive lineages and the rise and diversification of more modern taxa. Two recent morphological-based cladistic analyses are presented here as competing hypotheses, with reference to molecular-based phylogenies. Our contribution represents a state-of-the art understanding of Plio-Pleistocene Equus evolution, their biochronologic and biogeographic background and paleoecological and paleoclimatic contexts.

- Abstract
In the summer of 2021, European governments removed most NPIs after experiencing prolonged second and third waves of the COVID-19 pandemic. Most countries failed to achieve immunization rates high enough to avoid resurgence of the virus. Public health strategies for autumn and winter 2021 have ranged from countries aiming at low incidence by re-introducing NPIs to accepting high incidence levels. However, such high incidence strategies almost certainly lead to the very consequences that they seek to avoid: restrictions that harm people and economies. At high incidence, the important pandemic containment measure ‘test-trace-isolate-support’ becomes inefficient. At that point, the spread of SARS-CoV-2 and its numerous harmful consequences can likely only be controlled through restrictions. We argue that all European countries need to pursue a low incidence strategy in a coordinated manner. Such an endeavour can only be successful if it is built on open communication and trust. © 2021 The Authors

28. A Successful Case of an Acquired Methemoglobinemia Treated with Methylene Blue

Methylene blue has been advocated for multiple indications, but data reporting its use on intoxication is rare. Acquired methemoglobinemia can be caused by a wide variety of chemicals and toxins and on severe cases, with high methemoglobin levels (>20%), methylene blue is the first line treatment. We describe a case report of a 45-year-old man with acute methemoglobinemia due to involuntary exposure to n-propyl nitrate. The patient came into the emergency department conscious, with profound peripheral and central cyanosis and oxygen saturation of 88% on air, with no response to oxygen supplementation. Arterial blood gas showed methemoglobin of 22.9%. Methylene blue was administered intravenously and symptoms improved dramatically, with control arterial blood gas with 2.6% of methemoglobin. Despite the rarity of occurrence of methemoglobinemia in occupational set, and since methylene blue is highly effective, a high degree of suspicious and good training aiming for prompt diagnosis is crucial in treating this emergency situation.

- Abstract
Background: Results from retrospective studies suggest that use of neuromuscular blocking agents during general anaesthesia might be linked to postoperative pulmonary complications. We therefore aimed to assess whether the use of neuromuscular blocking agents is associated with postoperative pulmonary complications. Methods: We did a multicentre, prospective observational cohort study. Patients were recruited from 211 hospitals in 28 European countries. We included patients (aged ≥18 years) who received general anaesthesia for any in-hospital procedure except cardiac surgery. Patient characteristics, surgical and anaesthetic details, and chart review at discharge were prospectively collected over 2 weeks. Additionally, each patient underwent postoperative physical examination within 3 days of surgery to check for adverse pulmonary events. The study outcome was the incidence of postoperative pulmonary complications from the end of surgery up to postoperative day 28. Logistic regression analyses were adjusted for surgical factors and patients' preoperative physical status, providing adjusted odds ratios (ORadj) and adjusted absolute risk reduction (ARRadj). This study is registered with ClinicalTrials.gov, number NCT01865513. Findings: Between June 16, 2014, and April 29, 2015, data from 22 803 patients were collected. The use of neuromuscular blocking agents was associated with an increased incidence of postoperative pulmonary complications in patients who had undergone general anaesthesia (1658 [7·6%] of 21 694); ORadj 1·86, 95% CI 1·53–2·26; ARRadj −4·4%, 95% CI −5·5 to −3·2). Only 2·3% of high-risk surgical patients and those with adverse respiratory profiles were anaesthetised without neuromuscular blocking agents. The use of neuromuscular monitoring (ORadj 1·31, 95% CI 1·15–1·49; ARRadj −2·6%, 95% CI −3·9 to −1·4) and the administration of reversal agents (1·23, 1·07–1·41; −1·9%, −3·2 to −0·7) were not associated with a decreased risk of postoperative pulmonary

How will the coronavirus disease 2019 (COVID-19) pandemic develop in the coming months and years? Based on an expert survey, we examine key aspects that are likely to influence the COVID-19 pandemic in Europe. The challenges and developments will strongly depend on the progress of national and global vaccination programs, the emergence and spread of variants of concern (VOCs), and public responses to non-pharmaceutical interventions (NPIs). In the short term, many people remain unvaccinated, VOCs continue to emerge and spread, and mobility and population mixing are expected to increase. Therefore, lifting restrictions too much and too early risk another damaging wave. This challenge remains despite the reduced opportunities for transmission given vaccination progress and reduced indoor mixing in summer 2021. In autumn 2021, increased indoor activity might accelerate the spread again, whilst a necessary reintroduction of NPIs might be too slow. The incidence may strongly rise again, possibly filling intensive care units, if vaccination levels are not high enough. A moderate, adaptive level of NPIs will thus remain necessary. These epidemiological aspects combined with economic, social, and health-related consequences provide a more holistic perspective on the future of the COVID-19 pandemic.

Plantios florestais homogêneos são comumente atacados por agentes bióticos devido à menor diversidade biológica. Este tipo de plantio, especialmente de eucalipto, se intensificou no sudeste do estado do Pará, Brasil, a partir dos anos 1990. Desta forma, o objetivo deste estudo foi prospectar a diversidade da entomofauna aérea, avaliar suas respostas à sazonalidade e detectar a ocorrência de possíveis insetos praga importantes para monocultivos de Eucalyptus spp. Foram utilizadas armadilhas amarelas adesivas para a amostragem em plantios de E. urophylla e do híbrido E. grandis x E. urophylla. No total, foram realizadas 12 coletas, com 27 armadilhas cada, divididas em estação seca e chuvosa, durante dois anos em três municípios do Sudeste do Pará. Foram calculados os índices de diversidade de Shannon-Wiener e Pielou. As famílias foram classificadas quanto à diversidade faunística, através dos índices de dominância, frequência, abundância e constância. O teste de análise Fatorial 2 x 2 foi realizado para comparar o efeito da sazonalidade entre os eucaliptos. Foram coletados 10.280 indivíduos, distribuídos em oito ordens e 68 famílias. As ordens mais abundantes foram Hemiptera, Coleoptera e Diptera, onde Hemiptera foi a mais abundante na estação seca, e Diptera na estação chuvosa. As famílias Aphalaridae e Thaumastocoridae foram as mais importantes. - Abstract
In various countries throughout the world, the bureaucratic development of the modern states has been accompanied by the creation of identification systems whose purpose is to collect, store and manage personal and biometric data about its citizens. In this chapter we analyse the establishment of a national DNA database for criminal and civil forensic identification and the intention of implementing CCTV (closed-circuit television) in open areas on a national scale. The comprehensive analysis of these processes in the Portuguese context is especially relevant due mainly to the fact that, on the one hand, we are considering a country with a long history of a political dictatorship in the twentieth century (1928- 1974) characterized by political and police repression and censorship and, on the other hand, a newly democratic state divided between the quest for modernization and uniformity by following the paths of surveillance implemented in other European countries (considered to be more advanced) while at the same time struggling with its own cultural and social specificities marked by scarce economic resources and low criminality rates. From our point of view, it is intriguing that Portugal has a long and social history of citizens' apparently passive compliance with the state's requirements of collecting diverse sorts of personal identification data and, at the same time, both national and international studies suggest that public confidence in the state, the police and the justice system is weak in European terms (Cabral et al. 2003). - Abstract
In the summer of 2021, European governments removed most NPIs after experiencing prolonged second and third waves of the COVID-19 pandemic. Most countries failed to achieve immunization rates high enough to avoid resurgence of the virus. Public health strategies for autumn and winter 2021 have ranged from countries aiming at low incidence by re-introducing NPIs to accepting high incidence levels. However, such high incidence strategies almost certainly lead to the very consequences that they seek to avoid: restrictions that harm people and economies. At high incidence, the important pandemic containment measure ‘test-trace-isolate-support’ becomes inefficient. At that point, the spread of SARS-CoV-2 and its numerous harmful consequences can likely only be controlled through restrictions. We argue that all European countries need to pursue a low incidence strategy in a coordinated manner. Such an endeavour can only be successful if it is built on open communication and trust.

The incidence may strongly rise again, possibly filling intensive care units, if vaccination levels are not high enough. A moderate, adaptive level of NPIs will thus remain necessary. These epidemiological aspects combined with economic, social, and health-related consequences provide a more holistic perspective on the future of the COVID-19 pandemic.

- Abstract
Genetic diversity of Mycobacterium tuberculosis affects immune responses and clinical outcomes of tuberculosis (TB). However, how bacterial diversity orchestrates immune responses to direct distinct TB severities is unknown. Here we study 681 patients with pulmonary TB and show that M. tuberculosis isolates from cases with mild disease consistently induce robust cytokine responses in macrophages across multiple donors. By contrast, bacteria from patients with severe TB do not do so. Secretion of IL-1 beta is a good surrogate of the differences observed, and thus to classify strains as probable drivers of different TB severities. Furthermore, we demonstrate that M. tuberculosis isolates that induce low levels of IL-1 beta production can evade macrophage cytosolic surveillance systems, including cGAS and the inflammasome. Isolates exhibiting this evasion strategy carry candidate mutations, generating sigA recognition boxes or affecting components of the ESX-1 secretion system. Therefore, we provide evidence that M. tuberculosis strains manipulate host-pathogen interactions to drive variable TB severities. Some strains of Mycobacterium tuberculosis seem to be able to avoid host defense systems. Here the authors stratify patients by severity of tuberculosis and find correlations with the level of IL-1 beta production by macrophages exposed to these isolates.

Introduction: Patients experience preoperative anxiety. The objective of this study was to assess patients’ fears concerning anaesthesia and evaluate possible associations with sociodemographic background and previous anaesthesia. Methods: A survey was filled preoperatively, for 6 months, including anaesthesia-related fears, sociodemographic data, previous anaesthesia and surgery, current surgical and anaesthetic procedure, and preferred anaesthesia technique. Descriptive statistics, principal component analysis, Chi-square test, t-test and ANOVA were used for statistical analysis. Results: 153 questionnaires were collected. The mean age was 58 years, 61.4% were female and 59.5% employed. 47.1% had an anaesthesia consultation before. 49.7% were ASA II. 78.4% had been submitted to surgery before, 81.7% to anaesthesia and 14.5% had regional anaesthesia. In 73.5% general anaesthesia preferred. Half of patients with previous regional anaesthesia favoured this technique. The most relevant fears were “being paralysed because of anaesthesia”, “not waking up” and “waking up during surgery”. The least relevant were “fasting incapacity”, “having nightmares” and “speaking of personal matters”. A statistically significant relation was observed between the fear of “feeling pain after surgery” and having a previous anaesthetic experience. Female and unemployed patients were more concerned with general complications, while male patients gave more importance to awareness and paralysis. Conclusion: Paralysis after anaesthesia, intraoperative awareness and death were the main concerns of our inquiries.
