Abstract

Pravilna prehrana iznimno je važna u prevenciji šećerne bolesti i regulaciji glikemije. Šećerna bolest ubraja se u kategoriju bolesti koje se uspješno mogu prevenirati, dobro liječiti te im se može spriječiti ili odgoditi razvoj kroničnih komplikacija. Edukacija o pravilnoj prehrani treba biti individualno prilagođena, pri čemu treba obratiti pozornost na dob bolesnika, način života, socioekonomski status, tjelesnu aktivnost i komplikacije vezane uz šećernu bolest. Kod šećerne bolesti promjena životnih navika, što uključuje pravilnu prehranu, redovitu tjelesnu aktivnost i regulaciju tjelesne mase, može uvelike pridonijeti regulaciji glikemije, a u tipu 2 u nekim slučajevima može dovesti i do njene remisije. Smjernice su rezultat suradnje zdravstvenih stručnjaka koji sudjeluju u liječenju i edukaciji osoba koje boluju od šećerne bolesti. Utemeljene su na dokazima, prema metodologiji GRADE (engl. grading of recommendations, assessment, development and evaluation) koja uz snagu dokaza opisuje i razinu preporuke. Temeljni zaključci ovih smjernica odnose se na procjenu nutritivnih potreba te primjenu medicinske nutritivne terapije, individualno prilagođene osobama sa šećernom bolešću kao i onima koji imaju i neke od vezanih komorbiditeta., Adequate nutrition is extremely important in diabetes prevention and regulation of glycemia. Diabetes counts into the category of diseases that can be successfully prevented and treated with possible delay of chronic complications development. Nutrition education should be individually tailored, by paying attention to the patient’s age, lifestyle, socio-economic status, physical activity and complications related to diabetes. Change in lifestyle habits, which includes proper diet, regular physical activity and weight management, can greatly contribute to the regulation of glycemia and in some cases of type 2 diabetes can lead to its remission. The guidelines were made as a result of collaboration of health professionals who participate in the treatment and education of individuals with diabetes. They are evidence-based, according to the GRADE methodology (Grading of Recommendations, Assessment, Development and Evaluation), which describes the level of recommendation in addition to the strength of the evidence. The fundamental conclusions refer to the assessment of nutritional needs and the implementation of medical nutrition therapy, individually adapted to individuals with diabetes and those with related comorbidities.

2. ISTRAŽIVANJE KVALITETE ŽIVLJENJA, KOGNITIVNE I FUNKCIONALNE SPOSOBNOSTI KORISNIKA U TRIMA ZAGREBAČKIM DOMOVIMA ZA STARIJE

Abstract

Na kvalitetu života osoba starijih od 65 godina nerijetko utječu promjene koje nastaju preseljenjem u dom za starije. Provedeno je analitičko presječno istraživanje o kvaliteti života koje je obuhvatilo 150 ispitanika u trima decentraliziranim domovima za starije u gradu Zagrebu. Sasvim pokretnima procijenjeno je 53% ispitanika, ograničeno pokretnim 47%, dok se nitko nije izjasnio kao nepokretan. Prema Gerijatrijskoj skali depresije 64% ispitanika bilo je bez znakova depresivnosti dok ih je 6% imalo depresivnost visokog intenziteta. Medijan Mini Mental State Exam – standardne verzije skale bio je 26, minimum 15, maksimum 30. Ispitanici su postigli visoke rezultate u svim samoprocijenjenim domenama skale kvalitete življenja. Najviša kvaliteta života zabilježena je u domenama smislenih aktivnosti, dostojanstva, autonomije i individualnosti. Najniži rezultati zabilježeni su u domeni osjećaja sigurnosti. Zaključno, ovo istraživanje provedeno na korisnicima doma za starije koji imaju održanu relativno visoku funkcionalnu i kognitivnu sposobnost, ukazuje na zadovoljavajuću samoprocjenu kvalitete njihova življenja.

4. THE HEALTH FUNCTION OF PEOPLE WITH DIABETES MELLITUS DUE TO THEIR DIET.

Abstract

Diabetes mellitus is defined as a chronic metabolic disorder due to absolute or relative insulin deficiency resulting in intermittent or permanent hyperglycemia. The key to treating diabetes is proper nutrition, physical activity and pharmacotherapy. Numerous studies have proven a positive correlation between diet and optimal blood glucose levels. The aim of this study was to determine what type of diet is most commonly eaten, how body mass index (BMI) and average blood glucose are linked, whether laughter has an effect on blood sugar, what the health function of patients is, and how often they are engaged in physical activity. The study involved 98 subjects (20 of the Type 1 and 78 of Type 2) from Međimurje, Croatia. Participants filled in the survey in writing form in their Diabetes associations, hospital or family medicine practice. The results were processed using descriptive statistics and were divided by type of diabetes, eating habits and years of illness. Considering the diet, the lowest average of blood glucose levels are shown by subjects who feed on the American diet association (ADA) model. Health functioning is, on average, appropriate for this population with rare variations. In this study, we have proved that the lower BMI can be associated with lower blood glucose levels. In subjects with normal BMI the incidence of glucose level is 4.8 mmol/L, those who are overweight it is 7.63 mmol/ L. In patients with type 1 diabetes, a positive correlation was found between longer laughing and lower blood glucose. All respondents stated that they know that physical activity and exercise have a positive effect on health, but only about half of them exercise. In this study we see that the lowest average of blood glucose levels have people who are fed by ADA model. It is found the positive correlation between the lower BMI and the lower blood glucose levels. Abstract

EXERCISE AND BRAIN Goran Sporiš Faculty of Kinesiology, University of Zagreb It is well known that regular exercising cause numerous benefits on physical abilities, health of cardiovascular system, muscular and skeletal system, protect the body from the chronic diseases such as diabetes, hypertension, osteoporosis, some types of cancer etc. However, relatively new field of research which occupied scientist in last few decades is the influence of exercise on structures and functions of brain. Exercise can reduce the risk of various neurological diseases and protect the brain from detrimental factors of aging and cognitive decline (Bherer, 2015). Although for decades it has been thought that nerve cells can be produced exclusively during the embryonic period and early childhood it is still unclear that it is true, even there are numerous research in last few decades that support the hypothesis that neurogenesis is possible even in the adulthood (Cameron & McKay, 2001). This process is known as neurogenesis and according to some studies this process produce thousands of new neurons every day (Cameron & McKay, 2001). Most of this new neural cells are produced in the hippocampal formation which is highly responsive to the physical training and exercise (Curlik & Shors, 2013). The hippocampus is the region of brain with a high degree of endogenous neuroplasticity (Bavelier & Neville, 2002). The function of hippocampus is highly related to the process of learning and memory. Therefore, applying exercise, especially aerobic, will be strong stimulus for hippocampal neuroplasticity and consequently improvement of learning and memory, and overall cognitive function. The use of exercise as a brain protective tool can be particularly important for the patients with diabetes. Diabetes is the disease that has the high correlation with loss of total brain volume, smaller brain volumes in gray matter and increase of ischemic lesion volumes throughout the brain. Another possible influence of exercise on brain is related to the increased concentrations of the brain derived neurotrophic factor (BDNF). This mechanism is also very important in a prevention and therapy of diabetes because low levels of BDNF accompany impaired glucose metabolism. Decreased BDNF may be a pathogenetic factor involved not only in dementia and depression, but also in type 2 diabetes. Key words: physical activity, neurogenesis, neuroplasticity, diabetes TJELESNA AKTIVNOST I MOZAK Goran Sporiš Kineziološki fakultet, Sveučilište u Zagrebu Poznato je da redovito vježbanje uzrokuje brojne koristi na fizičke sposobnosti, zdravlje kardiovaskularnog sustava, mišićnog i koštanog sustava, štiti tijelo od kroničnih bolesti kao što su dijabetes, hipertenzija, osteoporoza, neke vrste raka itd. Međutim, relativno novo područje istraživanja koja su u posljednjih nekoliko desetljeća okupirala znanstvenike je utjecaj tjelesnog vježbanja na strukture i funkcije mozga. Vježbanje može smanjiti rizik od različitih neuroloških bolesti i zaštititi mozak od štetnih čimbenika starenja i kognitivnog propadanja (Bherer, 2015). Iako se već desetljećima smatralo da se živčane stanice mogu proizvoditi isključivo tijekom embrionalnog razdoblja i ranog djetinjstva, još uvijek nije jasno da li je to istina, čak i tjekom posljednjih nekoliko desetljeća postoje brojna istraživanja koja podupiru hipotezu da je neurogeneza moguća čak i u odraslo doba (Cameron i McKay, 2001). Ovaj proces je poznat kao neurogeneza i prema nekim istraživanjima ovaj proces proizvodi tisuće novih neurona svaki dan (Cameron i McKay, 2001). Većina novih neuronskih stanica proizvodi se u formaciji hipokampusa koja je vrlo osjetljiva na fizički trening i vježbanje (Curlik & Shors, 2013). Hipokampus je područje mozga s visokim stupnjem endogene neuroplastičnosti (Bavelier & Neville, 2002). Funkcija hipokampusa vrlo je povezana s procesom učenja i pamćenja. Stoga će primjena vježbanja, osobito aerobnih, biti snažan poticaj za neuroplastičnost hipokampusa i posljedično poboljšanje učenja i pamćenja te ukupne kognitivne funkcije. Korištenje vježbe kao zaštitnog alata za mozak može biti posebno važno za pacijente s dijabetesom. Dijabetes je bolest koja ima visoku korelaciju s gubitkom ukupnog volumena mozga, manjim volumenom mozga u sivoj tvari i povećanjem volumena ishemijskih lezija u mozgu. Drugi mogući utjecaj vježbanja na mozak povezan je s povećanim koncentracijama neurotrofnog faktora koji mozak sam proizvodi (BDNF). Ovaj mehanizam je također vrlo važan u prevenciji i terapiji dijabetesa jer niske razine BDNF-a prate poremećeni metabolizam glukoze. Smanjeni BDNF može biti patogenetski čimbenik koji nije uključen samo u razvoj demencije i depresije, već i dijabetesa tipa 2.
