Abstract

Individuals with minor ischaemic stroke and intracranial occlusion are at increased risk of poor outcomes. Intravenous thrombolysis with tenecteplase might improve outcomes in this population. We aimed to test the superiority of intravenous tenecteplase over non-thrombolytic standard of care in patients with minor ischaemic stroke and intracranial occlusion or focal perfusion abnormality.

Abstract

Female participation rates in ultra-running is considerably lower than that of males, but the reasons for this are not fully understood. In this study, online questionnaire responses were collected from ultra-runners taking part in the Scottish Jedburgh 3 Peaks ultra and Highland Fling race. One-to-one-semi-structured phone interviews were conducted with a subset of runners to further explore themes from questionnaire responses. Similar experiences among male and female ultra-runners were identified. Enablers were: wanting a challenge, nature/outdoors, and the friendliness of the ultra-community. The main barrier was training time. Qualitative responses suggested negotiation-efficacy was important for factoring in training around work and family commitments, and although a greater proportion of males had dependants compared to females, female participation was considerably lower than males. This may be due to the historical influences of traditional gender roles however this effect may be diminishing as female participation in Scottish ultra-races appears to be increasing. Abstract

Inclusive and modified sport promotes opportunities for participation usually for older people or people with disabilities, physical and or mental health issues. Walking sports, a form of modified sport, is often designed to support engagement in physical and social activity amongst people over 50 years. This qualitative study explored how walking football sessions were designed and delivered for, and experienced by people living with dementia and their family carers. We thematically analysed 18 qualitative interviews and identified three main themes (1) social impact, (2) embodied memories and (3) flexible carer respite. The findings increase our understanding of how walking sports such as football can be viewed as a psychosocial intervention and be modified to promote the inclusion and agency of people living with dementia and respite for family carers. The study offers best practice recommendations for those organising modified sports activities for older people living with dementia. [ABSTRACT FROM AUTHOR]

Byline: E. Michaela Macrae, James Kehoe Abstract: Rabbits in the nictitating membrane preparation received training in a conditional discrimination (A [right arrow] X+/B [right arrow] X-). Specifically, one conditional cue (A) signaled reinforced presentations of a second stimulus (X+), and another conditional cue (B) signaled nonreinforced presentations of the same, second stimulus (X-). Experiments 1 and 2 demonstrated that rabbits could use 'tonic' stimuli (41-82 s) as conditional cues (A, B) for differential responding to a shared 'phasic' stimulus (X; 100 ms). Experiment 3 investigated whether initial training with the tonic stimuli in the conditional discrimination yielded any transfer to subsequent training in which those conditional cues were used as phasic stimuli in either a consistent, discrete discrimination (A+/B-) or a reversed, discrete discrimination (A-/B+). There was positive transfer, more so to the consistent discrimination than to the reversed discrimination. However, Experiment 4 revealed that the role of a stimulus as a conditional cue could be readily dissociated from its role in a discrete discrimination. Following the initial acquisition of the conditional discrimination, the conditional task was continued, and it was interspersed with phasic presentations of A and B. As in Experiment 3, these presentations were either consistent (A+/B-) or reversed (A-/B+) to their role in the conditional discrimination. Responding acquired in the discrete discriminations transferred to the onset of the tonic stimuli. In the case of the reversed, discrete discrimination, this transfer did not interfere with the conditional responding to X. These results are discussed with reference to alternative accounts of conditional discriminations. Abstract

Background: This multicentre randomised trial is examining the effect of single flexible sigmoidoscopy (FS) screening on colorectal cancer (CRC) incidence and mortality. Previous analyses of this and other trials have only reported follow-up after FS for a maximum of 12 years. The objective of this analysis was to examine the long-term effects of FS after 17 years of follow-up. Methods: Between 1994 and 1999, 170,432 eligible men and women, who had indicated on a previous questionnaire that they would probably attend for screening if invited, were randomised to an intervention group (offered FS screening) or a control group (not contacted) in a 1:2 ratio. Randomisation was performed centrally in blocks of 12, and stratified by trial centre, general practice and household type. The primary outcomes were incidence and mortality of CRC. Hazard ratios (HR) and 95% confidence intervals (CI) for CRC incidence and mortality were estimated for intention-to-treat and per-protocol analyses. The trial is registered, number ISRCTN28352761. Findings: This analysis included 170,034 people (57,098 in the intervention and 112,936 in the control group). During screening and a median of 17.1 years’ follow-up, CRC was diagnosed in 1,230 individuals in the intervention and 3,253 in the control group, and 353 vs. 996 respectively died from CRC. In intention-to-treat analyses, CRC incidence was reduced by 26% (HR 0.74, 95% CI 0.70-0.80) and CRC mortality by 30% (HR 0.70, 95% CI 0.62-0.79). In per-protocol analyses, adjusted for non-compliance, CRC incidence and mortality were 35% (HR 0.65, 95% CI 0.59-0.71) and 41% (HR 0.59, 95% CI 0.49-0.70) lower in attenders. Interpretation: A single FS continues to provide substantial protection from CRC diagnosis and death, with protection lasting at least 17 years.

Abstract

Background: The prevalence and the origin of HIV-1 subtype B, the most prevalent circulating clade among the long-term residents in Europe, have been studied extensively. However the spatial diffusion of the epidemic from the perspective of the virus has not previously been traced. Results: In the current study we inferred the migration history of HIV-1 subtype B by way of a phylogeography of viral sequences sampled from 16 European countries and Israel. Migration events were inferred from viral phylogenies by character reconstruction using parsimony. With regard to the spatial dispersal of the HIV subtype B sequences across viral phylogenies, in most of the countries in Europe the epidemic was introduced by multiple sources and subsequently spread within local networks. Poland provides an exception where most of the infections were the result of a single point introduction. According to the significant migratory pathways, we show that there are considerable differences across Europe. Specifically, Greece, Portugal, Serbia and Spain, provide sources shedding HIV-1; Austria, Belgium and Luxembourg, on the other hand, are migratory targets, while for Denmark, Germany, Italy, Israel, Norway, the Netherlands, Sweden, Switzerland and the UK we inferred significant bidirectional migration. For Poland no significant migratory pathways were inferred. Conclusion: Subtype B phylogeographies provide a new insight about the geographical distribution of viral lineages, as well as the significant pathways of virus dispersal across Europe, suggesting that intervention strategies should also address tourists, travellers and migrants. © 2009 Paraskevis et al; licensee BioMed Central Ltd.

Because of high variability between subtypes, particular HIV-1 subtypes could have different genetic barriers for drug resistance substitutions. This study compared the genetic barrier between subtypes using some 2000 HIV-1 sequences (9600 of non-B subtype) isolated from antiretroviral- naive patients in Europe. Methods: The genetic barrier was calculated as the sum of transitions (scored as 1) and/or transversions (2.5) required for evolution to any major drug resistance substitution. In addition, the number of minor protease substitutions was determined for every subtype. Results: Few dissimilarities were found. An increased genetic barrier was calculated for I82A (subtypes C and G), V108I (subtype G), V118I (subtype G), Q151M (subtypes D and F), L210W (subtypes C, F, G, and CRF02_AG), and P225H (subtype A) (P G 0.001 compared with subtype B). A decreased genetic barrier was found for I82T (subtypes C and G) and V106M (subtype C) (P G 0.001 vs subtype B). Conversely, minor protease substitutions differed extensively between subtypes. Conclusions: Based on the calculated genetic barrier, the rate of drug resistance development may be similar for different HIV-1 subtypes. Because of differences in minor protease substitutions, protease inhibitor resistance could be enhanced in particular subtypes once the relevant major substitutions are selected. Abstract

Background. Infection with drug-resistant human immunodeficiency virus type 1 (HIV-1) can impair the response to combination therapy. Widespread transmission of drug-resistant variants has the disturbing potential of limiting future therapy options and affecting the efficacy of postexposure prophylaxis. Methods. We determined the baseline rate of drug resistance in 2208 therapy-naive patients recently and chronically infected with HIV-1 from 19 European countries during 1996-2002. Results. In Europe, 1 of 10 antiretroviral-naive patients carried viruses with >= 1 drug-resistance mutation. Recently infected patients harbored resistant variants more often than did chronically infected patients (13.5% vs. 8.7%; P = .006). Background: The prevalence and the origin of HIV-1 subtype B, the most prevalent circulating clade among the long-term residents in Europe, have been studied extensively. However the spatial diffusion of the epidemic from the perspective of the virus has not previously been traced. Results: In the current study we inferred the migration history of HIV-1 subtype B by way of a phylogeography of viral sequences sampled from 16 European countries and Israel. Migration events were inferred from viral phylogenies by character reconstruction using parsimony. With regard to the spatial dispersal of the HIV subtype B sequences across viral phylogenies, in most of the countries in Europe the epidemic was introduced by multiple sources and subsequently spread within local networks. Poland provides an exception where most of the infections were the result of a single point introduction. According to the significant migratory pathways, we show that there are considerable differences across Europe. Specifically, Greece, Portugal, Serbia and Spain, provide sources shedding HIV-1; Austria, Belgium and Luxembourg, on the other hand, are migratory targets, while for Denmark, Germany, Italy, Israel, Norway, the Netherlands, Sweden, Switzerland and the UK we inferred significant bidirectional migration.

Abstract

When assessing a reservoir's Hydrocarbon Initially In Place (HIIP) volumes there are often many uncertain input factors. If they are discretized into low, mid and high case scenarios, this can result in tens of thousands of possible combinations of factors and it is often necessary to reduce the amount of uncertain factors to a manageable number. This can reduce the range of model outcomes, and the confidence in the predicted volumes will be overestimated. Experimental design allows a reduced representative set of models to be constructed for uncertainty analysis when it would take too long to construct all possible realizations. This paper uses a case study to demonstrate a refined application of experimental design, whereby uncertain factors were first identified and experts were consulted to specify high, mid and low values with associated probabilities. The analysis was completed in two parts: a screening phase, whereby non-significant factors were eliminated; and an optimization phase, whereby a limited number of model runs were used to train a proxy model for use in a Monte Carlo procedure to generate a probability distribution of HIIP.
