Abstract

BACKGROUND Lamin A/C (LMNA) cardiomyopathy is a genetic disease with a proclivity for ventricular arrhythmias. We describe the multicenter experience with percutaneous catheter ablation of sustained monomorphic ventricular tachycardia (VT) in LMNA cardiomyopathy. METHODS AND RESULTS Twenty-five consecutive LMNA mutation patients from 4 centers were included (mean age, 55±9 years; ejection fraction, 34±12%; VT storm in 36%). Complete atrioventricular block was present in 11 patients; 3 patients were on mechanical circulatory support for severe heart failure. A median of 3 VTs were inducible per patient; in 82%, mapping was consistent with origin from scar in the basal left ventricle, particularly the septum, but also basal inferior wall and subaortic mitral continuity. After multiple procedures (median 2/patient; transcoronary alcohol in 6 and surgical cryoablation in 2 patients), acute success (noninducibility of any VT) was achieved in only 25% of patients. Partial success (inducibility of a nonclinical VT only: 50%) and failure (persistent inducibility of clinical VT: 12.5%) was attributed to intramural septal substrate in 13 of 18 patients (72%). Complications occurred in 25% of patients. After a median follow-up of 7 months after the last procedure, 91% experienced ≥1 VT recurrence, 44% received or were awaiting mechanical circulatory support or transplant for end-stage heart failure, and 26% died. CONCLUSIONS Catheter ablation of VT associated with LMNA cardiomyopathy is associated with poor outcomes including high rate of arrhythmia recurrence, progression to end-stage heart failure, and high mortality. Basal septal scar and intramural VT origin makes VT ablation challenging in this population.

Abstract

Introduction:Infusion-needle catheter ablation of VT within deep intramural substrate requires accurate identification of appropriate targets. We have developed an automated method for localizing VTs in real time using 12-lead ECG and CT, and have developed CT-based 3D virtual-heart technology incorporating intramyocardial fat to predict ablation targets.Hypothesis:We hypothesized that 2 new technologies, automated VT localization and CT-based virtual-heart can both help localize intramural ablation targets in patients with infarct-related VT.Methods:In a post-infarction patient with prior unsuccessful VT ablation, intramural needle ablation was performed. VT termination sites were compared to exits predicted by automated VT localization and to the VT circuits predicted by simulations with CT-based virtual-heart. VT-exit localization was conducted during the procedure; the virtual heart simulations were conducted retrospectively.Results:Of 4 VTs induced in the patient, 3 were successfully terminated with the needle procedure. VTs A and C terminated with ablation in the mid-inferoseptal region, and VT B at the mid posterior scar margin (Fig1. a, d, g). The automated VT localization method predicted VT A would exit at the mid inferoseptal region within 6.3mm of the site of termination with needle ablation (Fig1. c); VTs B & C were terminated within the middle of the scar at sites proximal to the scar exit site. The predicted exit sites were close to the termination sites (Fig1. f, i) (accuracy could not be precisely quantitated because VT was terminated at a mid-diastolic site). The CT-based virtual heart predicted 3 VT circuits (Fig1. b, e, h), one primarily endocardial (VT B) and two intramural circuits (VTs A & C), which also closely matched clinical ablation targets.Conclusions:Both methodologies reach a consensus in localizing intramural ablation targets and may prove valuable in directing deep myocardial ablation procedures.

30. Abstract 15241: Implantable Cardioverter Defibrillator Monotherapy and Congenital Long QT Syndrome: A Single Center?s Experience

Abstract

Background:In contrast to symptomatic Brugada syndrome where an implantable cardioverter defibrillator (ICD) as monotherapy is a class I recommendation, most patients with congenital long QT syndrome (LQTS) do not need and should not receive an ICD. However, with shared decision making, there may be situations where an ICD, either in combination with other LQTS-directed therapies or potentially all by itself (monotherapy), may make sense.Objective:To examine the prevalence, phenotype, and outcome of patients with LQTS who opted for ICD monotherapy.Methods:A retrospective analysis of 1306 patients (756 females; average QTc of 469 ? 42 ms) diagnosed with LQTS was performed to identify the subset of LQTS patients where ICD therapy was continued or initiated following evaluation in our LQTS clinic between 07/1999 and 06/2019. After exclusion of those individuals who received an ICD in combination with other LQTS-directed therapies, the genotype, phenotype, and clinical outcomes for the subset receiving ICD monotherapy was examined.Results:Overall, 267/1306 (20%) patients received an ICD as part of their LQTS-directed treatment program and 21/267 (7.9%; 6 with LQT1, 12 with LQT2, and 3 with LQT3; average QTc of 470 ? 34 ms) had the ICD as their only treatment modality. Of note, 11/21 (52%) were asymptomatic at the time of diagnosis and all had been trialed previously on other LQTS-directed therapies. The average age at ICD monotherapy implementation was 27 ? 13 years and the primary reason for ICD monotherapy was beta-blocker intolerance and/or non-compliance (18/21; 86%). None of these patients have received an appropriate therapy in over 200 follow-up years. In contrast, 7/21 (33%) have experienced a serious adverse ICD event including a total of 15 inappropriate ICD therapies, secondary to either T-wave over-sensing or lead fracture, in 3 patients. Two of these 3 patients subsequently underwent ICD explantation.Conclusion:ICD monotherapy is seldom pursued in LQTS. Among the <2% of patients who chose this therapeutic approach, the ICD-associated co-morbidities far exceeded its therapeutic necessity. This information should be incorporated into the shared decision making process and overall, ICD monotherapy for LQTS should be discouraged.
