Background Coronavirus disease 2019 (COVID-19)-related short-term mortality is high in dialysis patients, but longer-term outcomes are largely unknown. We therefore assessed patient recovery in a large cohort of dialysis patients 3 months after their COVID-19 diagnosis. Methods We analyzed data on dialysis patients diagnosed with COVID-19 from 1 February 2020 to 31 March 2021 from the European Renal Association COVID-19 Database (ERACODA). The outcomes studied were patient survival, residence and functional and mental health status (estimated by their treating physician) 3 months after COVID-19 diagnosis. Complete follow-up data were available for 854 surviving patients. Patient characteristics associated with recovery were analyzed using logistic regression. Results In 2449 hemodialysis patients (mean ± SD age 67.5 ± 14.4 years, 62% male), survival probabilities at 3 months after COVID-19 diagnosis were 90% for nonhospitalized patients (n = 1087), 73% for patients admitted to the hospital but not to an intensive care unit (ICU) (n = 1165) and 40% for those admitted to an ICU (n = 197). Patient survival hardly decreased between 28 days and 3 months after COVID-19 diagnosis. At 3 months, 87% functioned at their pre-existent functional and 94% at their pre-existent mental level. Only few of the surviving patients were still admitted to the hospital (0.8–6.3%) or a nursing home (∼5%). A higher age and frailty score at presentation and ICU admission were associated with worse functional outcome. Conclusions Mortality between 28 days and 3 months after COVID-19 diagnosis was low and the majority of patients who survived COVID-19 recovered to their pre-existent functional and mental health level at 3 months after diagnosis.

Background Patients on kidney replacement therapy (KRT) are at very high risk of coronavirus disease 2019 (COVID-19). The triage pathway for KRT patients presenting to hospitals with varying severity of COVID-19 illness remains ill-defined. We studied the clinical characteristics of patients at initial and subsequent hospital presentations and the impact on patient outcomes. Methods The European Renal Association COVID-19 Database (ERACODA) was analysed for clinical and laboratory features of 1423 KRT patients with COVID-19 either hospitalized or non-hospitalized at initial triage and those re-presenting a second time. Predictors of outcomes (hospitalization, 28-day mortality) were then determined for all those not hospitalized at initial triage. Results Among 1423 KRT patients with COVID-19 [haemodialysis (HD), n = 1017; transplant, n = 406), 25% (n = 355) were not hospitalized at first presentation due to mild illness (30% HD, 13% transplant). Of the non-hospitalized patients, only 10% (n = 36) re-presented a second time, with a 5-day median interval between the two presentations (interquartile range 2–7 days). Patients who re-presented had worsening respiratory symptoms, a decrease in oxygen saturation (97% versus 90%) and an increase in C-reactive protein (26 versus 73 mg/L) and were older (72 vs 63 years) compared with those who did not return a second time. The 28-day mortality between early admission (at first presentation) and deferred admission (at second presentation) was not significantly different (29% versus 25%; P = 0.6). Older age, prior smoking history, higher clinical frailty score and self-reported shortness of breath at first presentation were identified as risk predictors of mortality when re-presenting after discharge at initial triage. Conclusions This study provides evidence that KRT patients with COVID-19 and mild illness can be managed effectively with supported outpatient care and with vigilance of respiratory symptoms, especially in those with risk factors for poor outcomes. Our findings support a risk-stratified clinical approach to admissions and discharges of KRT patients presenting with COVID-19 to aid clinical triage and optimize resource utilization during the ongoing pandemic.

De transitie naar aardgasvrije wijken staat bij gemeenten hoog op de agenda met als doel de noodzakelijke CO2-reductie te realiseren. Het Klimaatakkoord schrijft voor dat de gebouwde omgeving in 2050 volledig aardgasvrij is. Daar is veel innovatie voor nodig op verschillende vlakken. Een transitie vereist niet alleen nieuwe technologieën, maar ook nieuwe manieren van werken, nieuwe gewoonten, rollen, investeringsmodellen en een op maat gemaakt ondersteunend instrumentarium. Vanuit het Ministerie van Binnenlandse Zaken en Koninkrijksrelaties (BZK) loopt het Programma Aardgasvrije Wijken, waarin 27 verschillende wijken experimenteren met nieuwe aanpakken en technieken om aardgasvrij te worden. De uitvraag voor een tweede ronde pilots is recent gestart. Ook gemeenten zijn volop aan de slag met het opstellen van Transitievisies Warmte en Wijkuitvoeringsplannen. De G4-gemeenten, TNO en Platform31 hebben een jaar lang samengewerkt aan nieuwe kennis en inzichten over besluitvorming voor aardgasvrije wijken, met de huidige gemeentelijke praktijk als belangrijk vertrekpunt. Deze publicatie bevat de belangrijkste kennis en inzichten die uit deze samenwerking naar voren zijn gekomen.

A regional energy transition (RET) implies a drastic transformation of the energy system and, hence, a lot of challenges. A RET calls for an integrative local approach. In this essay we describe and compare the current practice and the state of knowledge on this topic. We find that theory has not yet developed sufficiently to address the practical challenges. Part of the problem is that what has been developed has too little connection with local practice. We conclude that the development of theoretical knowledge must be better attuned to the needs of the practitioners. © 2018 Firenze University Press.

Vanaf 2009 is elektrisch vervoer door de steden Amsterdam, Rotterdam, Utrecht en Den Haag gestimuleerd om de luchtkwaliteit in met name stedelijke gebieden te verbeteren en om de uitstoot van broeikasgassen te verminderen. Dit gebeurde onder andere door het realiseren van publieke laadpunten. Het kabinet heeft in 2017 uitgesproken dat vanaf 2030 alle nieuw verkochte voertuigen emissieloos moeten zijn om de klimaatdoelstellingen te realiseren. Dit betekent dat het aantal elektrische voertuigen de komende jaren een enorme groei zal doormaken. Elektrische laadinfrastructuur is noodzakelijk om deze groei mogelijk te maken. Sommige scenario’s gaan uit van een vertienvoudiging van het aantal publieke laadpunten. De overheden in de provincies Noord-Holland, Flevoland en Utrecht werken op het gebied van elektrisch vervoer vanaf 2012 samen in het project MRA-elektrisch (MRA-E). Amsterdam, Rotterdam, Utrecht en Den Haag (G4) en MRA-E stemmen onderling aanpak en projecten af. Gezamenlijk hebben zij TNO gevraagd bij het ontwikkelen van beleidsopties en de rol voor decentrale overheden bij de realisatie van publieke laadinfrastructuur. Joint fact-finding has been advanced as a method for helping stakeholders grappling with technically intensive policy and planning challenges to collaboratively engage in research and arrive at shared sets of facts to inform their decision-making. This paper introduces joint fact-finding and considers its application in the context of infrastructure stakeholders aiming to assess and increase the resilience of their infrastructure systems to climate change. A set of evaluative criteria is introduced, which are proposed for assessing joint fact finding processes both procedurally and substantively in terms of the outcomes, considering them to be both arenas for collaborative governance and joint knowledge production efforts. These criteria are applied to a case in Rotterdam, the Netherlands. This case suggests that joint fact-finding can provide value, but also reveals some lessons. For the efforts themselves, these lessons relate to: The practical applicability of the outcomes; the inherently contingent nature of the outcomes when addressing wicked problems; questions of representation from stakeholder groups; and the importance of leadership and good process design. The following observations are made regarding the criteria: While they are typically interdependent, both process and outcomes should be evaluated; and more attention should be paid to the method and metrics of evaluation, while recognizing that there is no single formula or approach that can be applied, given the heterogeneity of the criteria. © 2016, Editorial Board EJTIR. BACKGROUND: Infectious disease burden estimates provided by a composite health measure give a balanced view of the true impact of a disease on a population, allowing the relative impact of diseases that differ in severity and mortality to be monitored over time. This article presents the first national disease burden estimates for a comprehensive set of 32 infectious diseases in the Netherlands. METHODS AND FINDINGS: The average annual disease burden was computed for the period 2007-2011 for selected infectious diseases in the Netherlands using the disability-adjusted life years (DALY) measure. The pathogen- and incidence-based approach was adopted to quantify the burden due to both morbidity and premature mortality associated with all short and long-term consequences of infection. Natural history models, disease progression probabilities, disability weights, and other parameters were adapted from previous research. Annual incidence was obtained from statutory notification and other surveillance systems, which was corrected for under-ascertainment and under-reporting. The highest average annual disease burden was estimated for invasive pneumococcal disease (9444 DALYs/year; 95% uncertainty interval [UI]: 8911-9961) and influenza (8670 DALYs/year; 95% UI: 8468-8874), which represents 16% and 15% of the total burden of all 32 diseases, respectively. The remaining 30 diseases ranked by number of DALYs/year from high to low were: HIV infection, legionellosis, toxoplasmosis, chlamydia, campylobacteriosis, pertussis, tuberculosis, hepatitis C infection, Q fever, norovirus infection, salmonellosis, gonorrhoea, invasive meningococcal disease, hepatitis B infection, invasive Haemophilus influenzae infection, shigellosis, listeriosis, giardiasis, hepatitis A infection, infection with STEC O157, measles, cryptosporidiosis, syphilis, rabies, variant Creutzfeldt-Jakob disease, tetanus, mumps, rubella, diphtheria, and poliomyelitis. The very low burden for the latter five diseases can be attributed to the National Immunisation Programme. The average disease burden per individual varied from 0.2 (95% UI: 0.1-0.4) DALYs per 100 infections for giardiasis, to 5081 and 3581 (95% UI: 3540-3611) DALYs per 100 infections for rabies and variant Creutzfeldt-Jakob disease, respectively. CONCLUSIONS: For guiding and supporting public health policy decisions regarding the prioritisation of interventions and preventive measures, estimates of disease burden and the comparison of burden between diseases can be informative. Although the collection of disease-specific parameters and estimation of incidence is a process subject to continuous improvement, the current study established a baseline for assessing the impact of future public health initiatives.

Het wereldwijde klimaat en ook het klimaat in Nederland verandert. De Nationale Adaptatiestrategie (NAS) vraagt om een breder inzicht in de risicofactoren voor Nederland als gevolg van de voorziene klimaatverandering. Onze energie-, transport-, informatie- en communicatietechnologie (ICT) - en overige (vitale) infrastructuren zijn mogelijk gevoelig voor de toename in frequentie en intensiteit van extreem weerfenomenen. Omdat er bovendien veel onderlinge afhankelijkheden bestaan tussen de energie-, transport- en ICT-infrastructuren, is het nodig de aard en omvang van die kwetsbaarheden en risicofactoren in onderling verband te analyseren. Deze afhankelijkheid van energie geldt ook voor andere sectoren in de Nederlandse samenleving. Dit deelrapport energie analyseert de risicofactoren die ontstaan door extreme weersomstandigheden als gevolg van de voorziene klimaatveranderingen tussen nu en 2050 en de mogelijke kansen die dit brengt voor de Nederlandse energie-infrastructuur. Omdat klimaatadaptatie met een complex speelveld te maken heeft van uiteenlopende actoren en belangen zijn ook de institutionele en governance risico’s en kansen in kaart gebracht.
