Abstract Background Post-intensive care syndrome could be responsible for inability to receive proper cancer treatment after ICU stay in patients with solid tumors (ST). Our purpose was to determine the factors associated with cancer treatment resumption and the impact of cancer treatment on the outcome of patients with ST after ICU stay. Methods We conducted a retrospective study including all patients with ST admitted to the ICU between 2014 and 2019 in a French University-affiliated Hospital. Results A total of 219 patients were included. Median SAPS II at ICU admission was 44.0 [IQR 32.8, 66.3]. Among the 136 patients who survived the ICU stay, 81 (59.6%) received cancer treatment after ICU discharge. There was an important increase in patients with poor performance status (PS) of 3 or 4 after ICU stay (16.2% at admission vs. 44.5% of patients who survived), with significant PS decline following the ICU stay (median difference − 1.5, 95% confidence interval [-1.5-1.0], p

Chenopodium ambrosioides aerial parts have been historically employed in traditional medicine for addressing various ailments such as headaches, abdominal discomfort, joint issues, and respiratory disorders, alongside treatments for lice and warts. This study aimed to conduct a comprehensive phytochemical analysis of C. ambrosioides and assess the acute and subacute toxicity of oral treatments using fractions in preclinical trials. Spectrophotometric analysis via LC–MS/MS was used to characterize the plant’s chemical composition. Acute toxicity evaluation followed Organisation for Economic Co-operation and Development code 42 guidelines, conducted on adult male and female Wistar strain mice. Subsequently, Swiss mice were divided into six groups for the subacute toxicity study, receiving oral doses of 200 mg/kg extracts and fractions for 28 days. Daily observations and biochemical analyses were performed, with LC–MS/MS revealing a diverse array of compounds including organic acids, flavonoids, phenolic acids, rutin, hesperidin, nicotiflorine, and fumaric acid. Results indicated no lethality or alterations in body weight in treated groups, though some organ weight changes were noted. Biochemical analyses demonstrated values within the normal range for all groups, suggesting that the treatments did not induce adverse effects. Acute and subacute treatments with fractions did not result in lethality or toxic alterations at therapeutic doses, implying the safety of the product at appropriate levels. This study underscores the potential of C. ambrosioides as a safe therapeutic option warranting further exploration.

Abstract Background Sepsis is a leading cause of death and serious illness that requires early recognition and therapeutic management to improve survival. The quick-SOFA score helps in its recognition, but its diagnostic performance is insufficient. To develop a score that can rapidly identify a community acquired septic situation at risk of clinical complications in patients consulting the emergency department (ED). Methods We conducted a monocentric, prospective cohort study in the emergency department of a university hospital between March 2016 and August 2018 (NCT03280992). All patients admitted to the emergency department for a suspicion of a community-acquired infection were included. Predictor variables of progression to septic shock or death within the first 90 days were selected using backward stepwise multivariable logistic regression to develop a clinical score. Receiver operating characteristic (ROC) curves were constructed to determine the discriminating power of the area under the curve (AUC). We also determined the threshold of our score that optimized the performance required for a sepsis-worsening score. We have compared our score with the NEWS-2 and qSOFA scores. Results Among the 21,826 patients admitted to the ED, 796 patients were suspected of having community-acquired infection and 461 met the sepsis criteria; therefore, these patients were included in the analysis. The median [interquartile range] age was 72 [54–84] years, 248 (54%) were males, and 244 (53%) had respiratory symptoms. The clinical score ranged from 0 to 90 and included 8 variables with an area under the ROC curve of 0.85 (confidence interval [CI] 95% 0.81–0.89). A cut-off of 26 yields a sensitivity of 88% (CI 95% 0.79–0.93), a specificity of 62% (CI 95% 57–67), and a negative predictive value of 95% (CI 95% 91–97). The area under the ROC curve for our score was 0.85 (95% CI, 0.81–0.89) versus 0.73 (95% CI, 0.68–0.78) for qSOFA and 0.66 (95% CI, 0.60–0.72) for NEWS-2. Conclusions Our study provides an accurate clinical score for identifying septic patients consulting the ED early at risk of worsening disease. This score could be implemented at admission.

Bacteria of the genus Brucella are facultative intracellular parasites that cause brucellosis, a severe animal and human disease. Recently, a group of taxonomists merged the brucellae with the primarily free-living, phylogenetically related Ochrobactrum spp. in the genus Brucella. This change, founded only on global genomic analysis and the fortuitous isolation of some opportunistic Ochrobactrum spp. from medically compromised patients, has been automatically included in culture collections and databases. We argue that clinical and environmental microbiologists should not accept this nomenclature, and we advise against its use because (i) it was presented without in-depth phylogenetic analyses and did not consider alternative taxonomic solutions; (ii) it was launched without the input of experts in brucellosis or Ochrobactrum; (iii) it applies a non-consensus genus concept that disregards taxonomically relevant differences in structure, physiology, population structure, core-pangenome assemblies, genome structure, genomic traits, clinical features, treatment, prevention, diagnosis, genus description rules, and, above all, pathogenicity; and (iv) placing these two bacterial groups in the same genus creates risks for veterinarians, medical doctors, clinical laboratories, health authorities, and legislators who deal with brucellosis, a disease that is particularly relevant in low- and middle-income countries. Based on all this information, we urge microbiologists, bacterial collections, genomic databases, journals, and public health boards to keep the Brucella and Ochrobactrum genera separate to avoid further bewilderment and harm.

Abstract:As an established concept in modern environmental philosophy, ecofeminism emphasises the need to change the unfair systems that capitalism uses to dominate women and the environment. It raises concerns about the continued exploitation of the environment, which puts humans and other non-human species in danger of going extinct. It is necessary to adopt a new economy in order to prevent this—one that prioritises opportunities for future generations as well as a prosperous life within the limits of the planet, going beyond satisfying the needs of wealthy consumers. Here, "living economies," as promoted by Vandana Shiva, become a way to guarantee life on Earth and build equitable, sustainable economies for all members of the Earth family, where nobody is denied access to the necessities of life. They are alternative economies to the free market economy, which is characterised by depletion and pollution of the Earth's vital resources. Abstract Backgrounds and Aims Daily lifestyle plays a vital role in modifying the risk for cardiovascular diseases (CVDs). Our daily life isn't inseparable from nutrition intake. As such, tea and coffee are often regarded as the most consumed beverages worldwide. There have been a lot of debates on the adverse effects and benefits of consuming these popular beverages. This comprehensive review explores the different types of tea and coffee and their mechanism of action. It delves deeper into their roles in reducing CVD risk, aiding CVD recovery, lowering CVD mortality, and their varying effects across populations and regions. Methods An extensive literature search was conducted on PubMed. Relevant articles were identified through cross‐referencing and manual searches. Excluded from the study were commentaries, case reports, clinical vignettes, and non‐English articles. Results Tea and coffee contain varying levels of caffeine and other bioactive compounds with cardioprotective effects against oxidative stress, inflammation, and more. Genetic factors further modulate their effects. Tea flavonoids benefit cholesterol, blood pressure, and endothelial function, while coffee constituents impact oxidative stress, metabolism, insulin sensitivity, and gut flora. Moderate consumption of both beverages may offer cardiovascular benefits, but outcomes vary depending on populations and conditions. Tea and coffee consumption may influence CVD recovery by reducing mortality and improving survival, however, it must be noted that it has the potential to be harmful to some individuals. Conclusion Evidence suggests that moderate consumption of these beverages may be linked to reduced cardiovascular mortality, although individual characteristics and pre‐existing conditions can influence outcomes. Excessive caffeine consumption, found in both beverages, may pose risks such as arrhythmias, hypertension, and cardiovascular mortality in CVD patients, with a dose‐dependent nature. Future research should delve into mechanisms, genetic factors, and diverse cultural impacts of its use. Health care providers should consider individual characteristics when advising on tea and coffee consumption in the context of cardiovascular health.

Electric car charging stations are in high demand as a result of the development of the e-mobility sector and the adoption of electric vehicles in transportation. This study aims to construct and analyze a stand-alone solar PV-powered electric car charging station to fulfil electric vehicle load demand and make recommendations for optimizing its operation. The goal is to achieve 3D’s i.e., Decarbonization, Digitalization and Decentralization in both the transport and power supply (electricity supply). Advancing towards attaining 3D’s goal, an off-grid solar PV-powered EV charging station was built at the University of Sharjah to meet the load demand. The EV charging station includes PV panels, inverters, energy storage devices and EV charging outlets. A solar PV system of 7.4 kWp with an energy storage capacity of 34.56 kWh is installed. A battery inverter of 4.6 kW is incorporated to meet the load demand of three electrical vehicles simultaneously. The performance of the system is monitored over a year. The EV charging station is in off-grid mode; thus, it is following the load demand for PV energy production. The findings reveal that the total energy production by the PV system is 2971.59 kWh from which the direct consumption is 383.77 kWh (12.91 %), and 2587.83 kWh (87.09 %) is stored in the energy storage devices. The total energy consumption by electric vehicles is 2379.01 kWh of which 1995.24 kWh (83.87 %) is taken from the battery bank. The PV system is working at 24.58 % of its full capacity. By elevating the daily load demand to 33 kWh, the full utilization of the solar resource can be achieved. To evaluate the adverse effect of dust, natural dust is allowed to settle down on the surface of PV panels continuously for 322 days. A linear degradation was recorded in the performance ratio from 77.92 % to 27.30 % in 218 days. Thereafter a plateau was attained and insignificant variation (min. 26.36 % and max. 33.78 %) was observed in the next 104 days. It is concluded that once a thick layer of dust forms on the surface of the PV panels, further dust accumulation becomes minimal.

Background: History of coronary artery disease (CAD) and/or atrial fibrillation (AF) and/or valvular replacement (VR) are prevalent among patients admitted to intensive care units (ICUs). The impact of these conditions on outcomes in patients with acute respiratory distress syndrome (ARDS) remains insufficiently explored. Methods: We performed a retrospective study on prospectively collected data from patients with ARDS and a PaO2/FiO2 ratio ≤150 mmHg. Patients were admitted between January 2006 and March 2022. We used multivariable logistic regression analysis. The primary outcome was 1-year mortality from admission to the ICU; secondary outcomes included mortality at 28 days and 90 days. Results: Among 1.033 patients, 181 (17.5 %) had a history of CAD and/or AF and/or VR. Opaque surfaces, such as walls, are well-known for their significant contributions to heat loss and energy demands in buildings. However, transparent surfaces, such as windows, are equally critical to a building's energy performance. The design of these transparent elements requires a careful balance of various factors, including window size, glazing type, and orientation, each of which plays a pivotal role in enhancing energy efficiency. This study explores the optimization of these factors during the design process, emphasizing their impact on the overall building performance.This research evaluates the potential energy savings in a building archetype representative of the Algerian building stock. Utilizing the EnergyPlus simulation tool, the study conducted 1152 simulations on a baseline model to generate a comprehensive dataset detailing the building's energy demands for heating and cooling across various climatic conditions. The findings reveal that annual energy savings for this type of housing essentially depend on its climatic zone and can range from 6.92 % for a hot semi-arid climate (Bsh) to reach a maximum of 9.75 % in a cold semi-arid climate (Bsk), a window-to-wall ratio (WWR) of 60 % typically maximizes energy efficiency, low-E glazing proved most effective in most cases, although regions needing significant solar protection favored alternative glazing types. Optimal window orientation generally trends Eastward, except in regions where southern exposure better supports solar management, highlighting the complex relationship between architectural design choices and energy efficiency.

Background: Food poisoning caused by bacterial agents is a worldwide problem, usually accompanied by unpleasant symptoms and may be sever leading to death. Natural compounds from marine algae namely flavonoids may play a role in the remedy of this condition. Aim: This research aims to assess the potency of flavonoids extracted from Enteromorpha intestinalis and Caulerpa prolifera as antibacterial agents. Methods: E. intestinalis was collected from Western Libyan Coast and C. prolifera was collected from Farwa Island. The antimicrobial activity and determination of minimum inhibitory concentration of algal flavonoid containing extracts was performed in vitro against some positive and negative Gram bacteria. Results: Crude extract containing flavonoids from E. intestinalis were more effective than C. prolifera extract against Staphylococcus aureus with antimicrobial essay (25-28+1 and 14.5-37.5+0.5-1.5), MIC (50 and 50-250 µg/mL), MBC (75 and 75-250 µg/mL). In Bacillus cereus, the antimicrobial assay (19-24.5+0.5-1.5: 24+1), MIC (50-250 + 100 µg/mL) and MBC (250 and 125 µg/mL). On the other hand, flavonoids containing extract from C. prolifera were more effective than E. intestinalis against Enterohemorrhagic Escherichia coli O157 EHEC O157 (25-28+1: 14-18.5+0.5-1.5), MIC (100-250:100-500 µg/mL) and MBC (150-250 and 250-500 µg/mL). Salmonella enterica qualitatively combat by flavonoid from E. intestinalis (13.5-14+0.5-1: 10.5-13.5+0.5-1.5), MIC (100-250: 250 µg/mL) and MBC (100-250: 250 µg/mL). Flavonoids from C. prolifera (4 strains: 2 strains) were effective against S. enterica. Crude flavonoids from both algae were not effective against Bacillus pumilus. Conclusion: Data from this study could conclude that flavonoid extracts from E. intestinalis and C. prolifera could be used against foodborne bacterial agents. [Open Vet J 2024; 14(3.000): 769-778]

Abstract Methionine (Met) rich diet inducing oxidative stress is reported to alter many organs. Proteasome as a regulator of oxidative stress can be targeted. This study was performed to investigate if excessive methionine supplementation causes hepatotoxicity related to proteasome dysfunction under endogenous oxidative stress in rats. Male Wistar albino rats (n = 16) were divided into controls and treated groups. The treated rats (n = 08) received orally l-methionine (1 g/kg/day) for 21 days. Total homocysteine (tHcy), total oxidant status (TOS), total antioxidant status (TAS), hepatic enzymes levels: aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LDH), alkaline phosphatase (ALP), with total bilirubin (TBil), albumin (Alb), and C-reactive protein (CRP) were determined in plasma by biochemical assays. Liver supernatants were used for malondialdehyde (MDA), protein carbonyls (PC), glutathione (GSH), catalase (CAT), superoxide dismutase (SOD), glutathione peroxidase (GPx), 20S proteasome activities and their subunits expression, tumor necrosis factor-α (TNF-α), and interleukin 6 (IL-6) evaluation by appropriate methods and light microscopy for liver histological examination. Methionine treatment increased homocysteine, TOS, oxidative stress index (OSI), MDA and PC but decreased TAS, GSH, CAT, SOD, GPx with the 20S proteasome activities and their β subunits expression. Liver proteins: AST, ALT, LDH, ALP, TBil and CRP were increased but Alb was decreased. Liver histology was also altered. An increase in liver TNF-α and IL-6 levels were observed. These findings indicated that methionine supplementation associated oxidative stress and proteasome dysfunction, caused hepatotoxicity and inflammation in rat. Further investigations should be to better understand the relation between methionine, oxidative stress, proteasome, and liver injuries.

Precision Medicine is being increasingly used in the developed world to improve health care. While several Precision Medicine (PM) initiatives have been launched worldwide, their implementations have proven to be more challenging particularly in low- and middle-income countries. To address this issue, the “Personalized Medicine in North Africa” initiative (PerMediNA) was launched in three North African countries namely Tunisia, Algeria and Morocco. PerMediNA is coordinated by Institut Pasteur de Tunis together with the French Ministry for Europe and Foreign Affairs, with the support of Institut Pasteur in France. The project is carried out along with Institut Pasteur d’Algérie and Institut Pasteur du Maroc in collaboration with national and international leading institutions in the field of PM including Institut Gustave Roussy in Paris. PerMediNA aims to assess the readiness level of PM implementation in North Africa, to strengthen PM infrastructure, to provide workforce training, to generate genomic data on North African populations, to implement cost effective, affordable and sustainable genetic testing for cancer patients and to inform policy makers on how to translate research knowledge into health products and services. Gender equity and involvement of young scientists in this implementation process are other key goals of the PerMediNA project.In this paper, we are describing PerMediNA as the first PM implementation initiative in North Africa. Such initiatives contribute significantly in shortening existing health disparities and inequities between developed and developing countries and accelerate access to innovative treatments for global health.

Background: Intake of potassium iodide (KI) reduces the accumulation of radioactive iodine in the thyroid gland in the event of possible contamination by radioactive iodine released from a nuclear facility. The World Health Organization (WHO) has stated the need for research for optimal timing, appropriate dosing regimen, and safety for repetitive iodine thyroid blocking (ITB). The French PRIODAC project, addressed all these issues, involving prolonged or repeated releases of radioactive iodine. Preclinical studies established an effective dose through pharmacokinetic modeling, demonstrating the safety of repetitive KI treatment without toxicity. Summary: Recent preclinical studies have determined an optimal effective dose for repetitive administration, associated with pharmacokinetic modeling. The results show the safety and absence of toxicity of repetitive treatment with KI. Good laboratory practice level preclinical studies corresponding to individuals >12 years have shown a safety margin established between animal doses without toxic effect. After approval from the French health authorities, the market authorization of the two tablets of KI, 65 mg/day, was defined with a new dosing scheme of a daily repetitive intake of the treatment up to 7 days unless otherwise instructed by the competent authorities for all categories of population except pregnant women and children under the age of 12 years. Conclusion: This new marketed authorization resulting from scientific-based evidence obtained as part of the PRIODAC project may serve as an example to further harmonize the application of KI for repetitive ITB in situations of prolonged radioactive release at the European and international levels, under the umbrella of the WHO.
