Abstract

We studied rotation of a disk propelled by a number of camphor pills symmetrically distributed at its edge. The disk was put on a water surface so that it could rotate around a vertical axis located at the disk center. In such a system, the driving torque originates from surface tension difference resulting from inhomogeneous surface concentration of camphor molecules released from the pills. Here we investigated the dependence of the stationary angular velocity on the disk radius and on the number of pills. The work extends our previous study on a linear rotor propelled by two camphor pills [Phys. Rev. E, 96, 012609 (2017)]. It was observed that the angular velocity dropped to zero after a critical number of pills was exceeded. Such behavior was confirmed by a numerical model of time evolution of the rotor. The model predicts that, for a fixed friction coefficient, the speed of pills can be accurately represented by a function of the linear number density of pills. Abstract

We consider a rotor made of two camphor disks glued below the ends of a plastic stripe. The disks are floating on a water surface and the plastic stripe does not touch the surface. The system can rotate around a vertical axis located at the center of the stripe. The disks dissipate camphor molecules. The driving momentum comes from the nonuniformity of surface tension resulting from inhomogeneous surface concentration of camphor molecules around the disks. We investigate the stationary angular velocity as a function of rotor radius $\ell$. For large $\ell$ the angular velocity decreases for increasing $\ell$. At a specific value of $\ell$ the angular velocity reaches its maximum and, for short $\ell$ it rapidly decreases. Such behaviour is confirmed by a simple numerical model. Abstract

Chronic kidney disease (CKD) affects approximately 13% of people globally, including 20%–48% with type 2 diabetes (T2D), resulting in significant morbidity, mortality, and healthcare costs. There is an urgent need to increase early screening and intervention for CKD. We are experts in diabetology and nephrology in Central Europe and Israel. Herein, we review evidence supporting the use of sodium‐glucose cotransporter‐2 (SGLT2) inhibitors for kidney protection and discuss barriers to early CKD diagnosis and treatment, including in our respective countries. SGLT2 inhibitors exert cardiorenal protective effects, demonstrated in the renal outcomes trials (EMPA‐KIDNEY, DAPA‐CKD, CREDENCE) of empagliflozin, dapagliflozin, and canagliflozin in patients with CKD. EMPA‐KIDNEY demonstrated cardiorenal efficacy across the broadest renal range, regardless of T2D status. Renoprotective evidence also comes from large real‐world studies. International guidelines recommend first‐line SGLT2 inhibitors for patients with T2D and estimated glomerular filtration rate (eGFR) ≥20 mL/min/1.73 m2, and that glucagon‐like peptide‐1 receptor agonists may also be administered if required for additional glucose control. Although these guidelines recommend at least annual eGFR and urine albumin‐to‐creatinine ratio screening for patients with T2D, observational studies suggest that only half are screened. Diagnosis is hampered by asymptomatic early CKD and under‐recognition among patients with T2D and clinicians, including limited knowledge/use of guidelines and resources. Based on our experience and on the literature, we recommend robust screening programmes, potentially with albuminuria self‐testing, and SGLT2 inhibitor reimbursement at general practitioner (GP) and specialist levels. High‐tech tools (artificial intelligence, smartphone apps, etc.) are providing exciting opportunities to identify high‐risk individuals, self‐screen, detect abnormalities in images, and assist with prescribing and treatment adherence. Better education is also needed, alongside provision of concise guidelines, enabling GPs to identify who would benefit from early initiation of renoprotective therapy; although, regardless of current renal function, cardiorenal protection is provided by SGLT2 inhibitor therapy. Abstract

Recombinant AAV (rAAV) vectors are increasingly favored for gene therapy due to their useful features of vectorology, such as transfection of dividing and nondividing cells, the presence of tissue-specific serotypes, and biosafety considerations. This study investigates the impact of commonly used therapeutic drugs—acetaminophen, budesonide, and simvastatin—on rAAV transduction efficiency in HEK-293 cells. Cells were transduced with an AAV mosaic vector under the control of a cytomegalovirus (CMV) promoter encoding green fluorescent protein (GFP). Transduction efficiency was assessed by qPCR and fluorescent microscopy. Analysis of functional interactions between genes potentially involved in rAAV transduction in drug-exposed cells was also performed. This study showed a clear effect of drugs on rAAV transmission. Notably, acetaminophen enhanced transduction efficiency by 9-fold, while budesonide and simvastatin showed 2-fold and 3-fold increases, respectively. The gene analysis illustrates the possible involvement of genes related to cell membranes in the potentiation of rAAV transduction induced by the drugs under investigation. Attention should be paid to S100A8, which is a common drug-modified gene for drugs showing anti-inflammatory effects (budesonide and simvastatin), demonstrating an interaction with the gene encoding the receptor for AAV (HGFR). This study underscores the significance of assessing rAAV pharmacokinetics/pharmacodynamics (PKs/PDs) and drug–gene therapy interactions in optimizing gene therapy protocols. [ABSTRACT FROM AUTHOR]

Abstract

The Central Nervous System (CNS) is vulnerable to a range of diseases, including neurodegenerative and oncological conditions, which present significant treatment challenges. The blood–brain barrier (BBB) restricts molecule penetration, complicating the achievement of therapeutic concentrations in the CNS following systemic administration. Gene therapy using recombinant adeno-associated virus (rAAV) vectors emerges as a promising strategy for treating CNS diseases, demonstrated by the registration of six gene therapy products in the past six years and 87 ongoing clinical trials. This review explores the implementation of rAAV vectors in CNS disease treatment, emphasizing AAV biology and vector engineering. Various administration methods—such as intravenous, intrathecal, and intraparenchymal routes—and experimental approaches like intranasal and intramuscular administration are evaluated, discussing their advantages and limitations in different CNS contexts. Additionally, the review underscores the importance of optimizing therapeutic efficacy through the pharmacokinetics (PK) and pharmacodynamics (PD) of rAAV vectors. A comprehensive analysis of clinical trials reveals successes and challenges, including barriers to commercialization. This review provides insights into therapeutic strategies using rAAV vectors in neurological diseases and identifies areas requiring further research, particularly in optimizing rAAV PK/PD. 44. Appliance of continuous glucose monitoring system as an educational tool in patients with type 2 diabetes

Abstract

Type 2 diabetes mellitus is an economical, medical and social problem. Patients with type 2 diabetes live shorter compared to peers without diabetes, mainly because of cardiovascular diseases. The basis of the treatment of these illness is multifactor therapy focused on cardiovascular risk factors, including hyperglycemia. Continuous glucose monitoring is a small device, which measures glucose level in tissue fluid every few minutes. It allows to observe glucose trends on the graph. One of the most popular system is Free Style Libre, it is based on scanning. Patient should fix sensor every 2 week on the back of the arm. To check glycemia, patient should bring the reader closer to sensor. The reader is a mobile phone or special device from producer, which is equipped with NFC. The measurement is non-invasive, without repeated finger pricks. Recent years have provided convincing evidence that the continuous, long-term use of registered continuous monitoring systems (CGMS) can be treated as a therapeutic method in patients with type 1 diabetes and type 2 diabetes treated intensively with insulin. Compared to people using glucometers, patients using CGM systems, including FreeStyle Libre, achieve better diabetes control, have fewer hypoglycaemic episodes, and their quality of life improves. Much less data concerns patients with type 2 diabetes remaining on less intensive therapeutic models, for example oral drugs or GLP-1 receptor analogues. Several publications from recent years show that they can also benefit from better metabolic control. The hypothesis that the temporary, educational use of the CGM system may have clinical significance in patients at the early stage of type 2 diabetes requires verification. The aim of the study is to evaluate the effectiveness of an educational intervention involving the periodic use of the Freestyle Libre 2 CGM system in patients at an early stage of type 2 diabetes.
