Abstract

Community-acquired Burkholderia cepacia pneumonia is rare. We report a 29-year-old female who suffered pulmonary tuberculosis and developed community-acquired Burkholderia cepacia pneumonia, which was confirmed by the culture of the pulmonary tissue. The patient received antitubercular therapy. Meanwhile, she was treated with meropenem and minocycline. The patient was followed up for 6 months, and she achieved complete absorption of lung lesions.

Abstract

Background. Lumbar disk degeneration (LDD) has been related to heavy physical loading. However, the quantification of the exposure has been controversial, and the dose-response relationship with the LDD has not been established. Objective. The purpose of this study was to investigate the dose-response relationship between lifetime cumulative lifting load and LDD. Design. This was a cross-sectional study. Methods. Every participant received assessments with a questionnaire, magnetic resonance imaging (MRI) of the lumbar spine, and estimation of lumbar disk compression load. The MRI assessments included assessment of disk dehydration, annulus tear, disk height narrowing, bulging, protrusion, extrusion, sequestration, degenerative and spondylolytic spondylolisthesis, foramina narrowing, and nerve root compression on each lumbar disk level. The compression load was predicted using a biomechanical software system. Results. A total of 553 participants were recruited in this study and categorized into tertiles by cumulative lifting load (ie, Limitations. There is no 'gold standard' assessment tool for measuring the lumbar compression load. Conclusions. Abstract

Abstract Background The aims of this study were: (1) to show the feasibility of using adipose-derived stromal vascular fraction (SVF) as an alternative to bone marrow mono nuclear cell (BM-MNC) for cell transplantation into chronic ischemic myocardium; and (2) to explore underlying mechanisms with focus on anti-inflammation role of engrafted SVF and BM-MNC post chronic myocardial infarction (MI) against left ventricular (LV) remodelling and cardiac dysfunction. Methods Four weeks after left anterior descending coronary artery ligation, 32 Male Lewis rats with moderate MI were divided into 3 groups. SVF group (n = 12) had SVF cell transplantation (6 × 106 cells). BM-MNC group (n = 12) received BM-MNCs (6 × 106) and the control (n = 10) had culture medium. At 4 weeks, after the final echocardiography, histological sections were stained with Styrus red and immunohistochemical staining was performed for α-smooth muscle actin, von Willebrand factor, CD3, CD8 and CD20. Results At 4 weeks, in SVF and BM-MNC groups, LV diastolic dimension and LV systolic dimension were smaller and fractional shortening was increased in echocardiography, compared to control group. Histology revealed highest vascular density, CD3+ and CD20+ cells in SVF transplanted group. SVF transplantation decreased myocardial mRNA expression of inflammatory cytokines TNF-α, IL-6, MMP-1, TIMP-1 and inhibited collagen deposition. Conclusions Transplantation of adipose derived SVF cells might be a useful therapeutic option for angiogenesis in chronic ischemic heart disease. Anti-inflammation role for SVF and BM transplantation might partly benefit for the cardioprotective effect for chronic ischemic myocardium.

Abstract

Distribution of antibiotic resistance genes (ARGs) in environment has become an increasing concern. To seek solutions to mitigate the spreading of ARGs in environment, identification and quantification are the prior tasks to be resolved. Metagenomics sequencing can effectively detect wide profile of ARGs in environmental samples; however, the detection and subsequent classification of ARG-like sequences are time consuming and not comprehensively evaluated for current similarity based analyzing methods. In this study, we sought to accelerate quantification of ARGs in metagenomic data and optimize the method performance by simulation datasets. A structured ARG reference database (SARG) was constructed with a hierarchical structure (type-subtype-reference sequence) to facilitate classification (assigning ARG-like sequence to type, subtype and reference sequence) of sequences identified through similarity search. The SARG database was constructed by integrating ARDB and CARD, the two most commonly used databases. SARG was curated to remove redundant sequences and optimized to facilitate query sequence identification by similarity. Utilizing the SARG and a previously proposed hybrid functional gene annotation pipeline, we developed a gateway of online platform called ARGs-OAP for fast annotation and classification of ARG-like sequences from metagenomic data. We also evaluated and proposed a set of criteria important for efficiently conducting metagenomic analysis of ARGs using the ARGs-OAP.
