11. Point of Care Ultrasound Training Needs for Primary Care Physicians: Practice Setting Matters

Abstract

Background: Point of care ultrasound (POCUS) is increasingly used in primary care. This study seeks to determine the educational needs of primary care physicians. Methods: All primary care practitioners in Calgary, Alberta, on the electronic departmental newsletter distribution list were invited to participate in an online survey. Interests in learning 25 POCUS applications and 9 procedures were assessed using a 5-point Likert scale. Results: Of the 1394 members on the distribution list, 96 responded. Of these, 88 completed the survey. The majority of the participants (n = 69, 78%) were office-based while 19 (21%) were hospital-based. The top applications for office-based participants included: (1) confirming fetal heart rate, (2) assessing fetal lie, and (3) confirming intrauterine pregnancy. For hospital-based participants, these were: (1) assessing soft tissue/superficial abscesses, (2) looking for ascites, and (3) confirming volume status of the patient. Of the 75 participants who perform procedures, both office- and hospital-based participants were most interested in learning incision and drainage for superficial abscesses and joint aspirations/injections; other procedural interests significantly differed between the two groups. Conclusions: Interests of office-based primary care practitioners in learning POCUS differed significantly from hospital-based practitioners. We recommend that separate office vs. hospital practice streams be offered to address their educational needs.

17. Cognitive Load Imposed by Ultrasound-Facilitated Teaching Does Not Adversely Affect Gross Anatomy Learning Outcomes

Abstract

Ultrasonography is increasingly used in medical education, but its impact on learning outcomes is unclear. Adding ultrasound may facilitate learning, but may also potentially overwhelm novice learners. Based upon the framework of cognitive load theory, this study seeks to evaluate the relationship between cognitive load associated with using ultrasound and learning outcomes. The use of ultrasound was hypothesized to facilitate learning in anatomy for 161 novice first-year medical students. Using linear regression analyses, the relationship between reported cognitive load on using ultrasound and learning outcomes as measured by anatomy laboratory examination scores four weeks after ultrasound-guided anatomy training was evaluated in consenting students. Second anatomy examination scores of students who were taught anatomy with ultrasound were compared with historical controls (those not taught with ultrasound). Ultrasound's perceived utility for learning was measured on a five-point scale. Cognitive load on using ultrasound was measured on a nine-point scale. Primary outcome was the laboratory examination score (60 questions). Learners found ultrasound useful for learning. Weighted factor score on "image interpretation" was negatively, but insignificantly, associated with examination scores [F (1,135) = 0.28, beta = -0.22; P = 0.61]. Weighted factor score on "basic knobology" was positively and insignificantly associated with scores; [F (1,138) = 0.27, beta = 0.42; P = 0.60]. Cohorts exposed to ultrasound had significantly higher scores than historical controls (82.4% ± SD 8.6% vs. 78.8% ± 8.5%, Cohen's d = 0.41, P < 0.001). Using ultrasound to teach anatomy does not negatively impact learning and may improve learning outcomes.

27. Comparing the Use of Global Rating Scale with Checklists for the Assessment of Central Venous Catheterization Skills Using Simulation

Abstract

The use of checklists is recommended for the assessment of competency in central venous catheterization (CVC) insertion. To explore the use of a global rating scale in the assessment of CVC skills, this study seeks to compare its use with two checklists, within the context of a formative examination using simulation. Video-recorded performances of CVC insertion by 34 first-year medical residents were reviewed by two independent, trained evaluators. Each evaluator used three assessment tools: a ten-item checklist, a 21-item checklist, and a nine-item global rating scale. Exploratory principal component analysis of the global rating scale revealed two factors, accounting for 84.1% of the variance: technical ability and safety. The two checklist scores correlated positively with the weighted factor score on technical ability (0.49 [95% CI 0.17-0.71] for the 10-item checklist; 0.43 [95% CI 0.10-0.67] for the 21-item checklist) and negatively with the weighted factor score on safety (-0.17 [95% CI -0.48-0.18] for the 10-item checklist; -0.13 [95% CI -0.45-0.22] for the 21-item checklist). A checklist score of less than 80% was strong indication of incompetence. However, a high checklist score did not preclude incompetence. Ratings using the global rating scale identified an additional 11 candidates (32%) who were deemed incompetent despite scoring greater than 80% on both checklists. All these candidates committed serious errors. In conclusion, the practice of universal adoption of checklists as the preferred method of assessment of procedural skills should be questioned. The inclusion of global rating scales should be considered.

Abstract

OBJECTIVES: The purpose of this study is to provide expert consensus recommendations to establish a global ultrasound curriculum for undergraduate medical students. METHODS: 64 multi-disciplinary ultrasound experts from 16 countries, 50 multi-disciplinary ultrasound consultants, and 21 medical students and residents contributed to these recommendations. A modified Delphi consensus method was used that included a systematic literature search, evaluation of the quality of literature by the GRADE system, and the RAND appropriateness method for panel judgment and consensus decisions. The process included four in-person international discussion sessions and two rounds of online voting. RESULTS: A total of 332 consensus conference statements in four curricular domains were considered: (1) curricular scope (4 statements), (2) curricular rationale (10 statements), (3) curricular characteristics (14 statements), and (4) curricular content (304 statements). Of these 332 statements, 145 were recommended, 126 were strongly recommended, and 61 were not recommended. Important aspects of an undergraduate ultrasound curriculum identified include curricular integration across the basic and clinical sciences and a competency and entrustable professional activity-based model. The curriculum should form the foundation of a life-long continuum of ultrasound education that prepares students for advanced training and patient care. In addition, the curriculum should complement and support the medical school curriculum as a whole with enhanced understanding of anatomy, physiology, pathophysiological processes and clinical practice without displacing other important undergraduate learning. The content of the curriculum should be appropriate for the medical student level of training, evidence and expert opinion based, and include ongoing collaborative research and development to ensure optimum educational value and patient care. Abstract

Objectives: Discrete B‐lines have clear definitions, but confluent B‐lines, consolidations, and pleural line abnormalities are less well defined. We proposed definitions for these and determined their reproducibility using COVID‐19 patient images obtained with phased array probes. Methods: Two raters collaborated to refine definitions, analyzing disagreements on 107 derivation scans from 10 patients. Refined definitions were used by those raters and an independent rater on 1260 validation scans from 105 patients. Reliability was evaluated using intraclass correlation coefficients (ICC) or Cohen's kappa. Results: The agreement was excellent between collaborating raters for B‐line abnormalities, ICC = 0.97 (95% confidence interval [CI] 0.97–0.98) and pleural line to consolidation abnormalities, ICC = 0.90 (95% CI 0.87–0.92). The independent rater's agreement for B‐line abnormalities was excellent, ICC = 0.97 (95% CI 0.96–0.97) and for pleural line to consolidation was good, ICC = 0.88 (95% CI 0.84–0.91). Agreement just on pleural line abnormalities was weak (collaborators, κ = 0.54, 95% CI 0.48–0.60; independent, κ = 0.54, 95% CI 0.49–0.59). Conclusion: With proposed definitions or via collaboration, overall agreement on confluent B‐lines and pleural line to consolidation abnormalities was robust. Pleural line abnormality agreement itself was persistently weak and caution should be used interpreting pleural line abnormalities with only a phased array probe. [ABSTRACT FROM AUTHOR]

Additional file 7. PoCUS-empowered triage in respiratory failure during COVID-19 Pandemic. Conceptual framework of point-of-care ultrasound (PoCUS) use for the triage of dyspneic and/or hypoxemic patients, during the SARS-CoV-2 pandemic: the diagram does not represent an algorithm but rather a framework for potentially developing protocols according to local/institutional clinical practices, policies and regulations. It does not either provide a list of conclusive diagnosis or specific treatments, but suggests how to integrate at best PoCUS in the workflow of this specific setting
