Abstract Background A multiseptate gallbladder is a very rare congenital malformation in which the lumen is divided into variously sized multiseptal compartments. The pathogenesis and natural history of this disease remain uncertain. We herein describe a pediatric case of a multiseptate gallbladder with pancreaticobiliary maljunction (PBM), which was treated by laparoscopic cholecystectomy. Case presentation A 5-year-old girl was referred to our hospital, because a multiseptate gallbladder had been incidentally detected on abdominal ultrasonography when she presented for transient abdominal pain. Ultrasonography showed hyperechoic septa throughout the lumen of the gallbladder, giving it a honeycomb appearance. The atrophied gallbladder had weak or no contractility. Magnetic resonance cholangiopancreatography performed to detect other coexisting biliary disorders revealed PBM without dilatation of the common bile duct. Although physical examination and laboratory tests revealed no abnormalities, we performed laparoscopic cholecystectomy to prevent cholecystitis and reduce the risk of cancer secondary to the PBM. Conclusions In recent pediatric case reports, the indication and timing of cholecystectomy has tended to be determined by the patient’s symptoms and the presence of biliary complications. In the present case, however, the combination of a multiseptate gallbladder and PBM may become problematic in the future. Surgical treatment without delay was appropriate even in this pediatric patient.

Introduction. Multiseptate gallbladder is a rare congenital malformation of the gallbladder. Septate gallbladder is a rare congenital anomaly. It is anatomical malformations affecting its shape, size, number, and position. Patients with usually admits to the emergency department with differential symptoms such as right upper abdominal pain, nause and vomiting, and abdominal complaints. The aim of this case report is to present a case diagnosed with multiseptated gallbladder. Case presentation. This case was diagnosed cause of the symptomatic clinic and with abdominal ultrasound such multiseptated gallbladder without stones. The case underwent surgical intervention for cholecystectomy. After the surgery intervention the material was sent for the histopathological diagnosis. Conclusions. We recommend cholecystectomy in such cases due to possible complications in the future. In our case we evidence that the clinical story of patients is determinant for the treatment.

Biliary dyskinesia due to a multiseptate gallbladder has been rarely described in literature. Septations within the lumen of the gallbladder can lead to bile resistance, causing symptoms of abdominal pain, nausea, and vomiting. The majority of cases are due to congenital malformation during the development of the gallbladder. Ultrasound (US) is usually sufficient to make the diagnosis, although other imaging techniques can be used for confirmation. We present the case of a 60-year-old female who presented to the emergency room (ER) with biliary pain due to septations in the lumen of her gallbladder.

Physical examination was normal; in laboratory tests, only liver enzymes (aspartate aminotransferase: 442 U/L, alanine aminotransferase: 270 U/L) and gamma glutamyl transpeptidase (73 U/L) were elevated. Ultrasonography (USG) showed a multiloculated gallbladder measuring approximately 41x33x46 mm in size with structures of septation with echogenic density contents. On magnetic resonance cholangiopancreatography (MRCP), a multiseptated cystic lesion approximately 90x50x80 mm in size was observed in the inferomedial aspect of the liver. Laparoscopic cholecystectomy was performed. Macroscopic evaluation demonstrated multiple septa dividing the gallbladder into compartments of different sizes. Multiseptate gallbladder is a rare congenital anomaly that may be silent or with symptoms. Herein, we report a case of 12-year-old male child with abdominal pain associated with nausea and vomiting. The diagnosis of multiseptate gallbladder was settled at 6 months after the first episode of abdominal pain by screening ultrasound and was confirmed by magnetic resonance cholangiopancreatography. We performed laparoscopic cholecystectomy with complete resolution of preoperative symptoms. The patient was discharged on postoperative day 1. Presentamos el caso de una paciente con dolor en hipocondrio derecho de varios años de evolución, a la que se le realizó una ecografía abdominal y posteriormente una resonancia magnética, identificando en ambas pruebas de imagen una vesícula biliar con múltiples septos finos dividiendo su luz.Multiseptate gallbladder is an extremely rare congenital anomaly characterized by the presence of multiple septa that divide the lumen of the gallbladder. Although some patients are asymptomatic and the anomaly is found incidentally, the most frequent clinical presentation is chronic pain in the right upper quadrant. Ultrasound is the imaging technique of choice for diagnosis, and cholangioMR is used mainly to rule out associated anomalies of the biliary tract. In this article we report the case of a patient with right upper quadrant pain of several years of evolution, where imaging through abdominal ultrasound and MRI allowed a confident diagnosis.

Multiseptate gallbladder is a very rare congenital malformation of the gallbladder. A 30-year-old man with multiseptate gallbladder, a very rare congenital anomaly, is presented. His presenting symptom was epigastric pain. A hypoplastic gallbladder with multiple septa was demonstrated by ultrasonography and endoscopic retrograde cholangiography. An injection of cerulein reproduced pain, and simultaneous biliary manometry and scintigraphy showed impairment of gallbladder filling and emptying. Laparoscopic cholecystectomy resulted in complete relief of the pain. Biliary manometry and scintigraphy are useful to determine the operative indication in a symptomatic patient with this entity. A case of multiseptate gallbladder is described, with a review of the literature. This is the 24th such case report. The patient complained of right upper quadrant pain, colicky in nature. Abdominal ultrasonography showed multiple fine echoes within the gallbladder. On endoscopic retrograde cholangiography, multiple radiolucent lines crossing the gallbladder in various directions were noted within the gallbladder. There are various diseases of the gallbladder and cystic duct, and imaging diagnosis is challenging for the rare among them. However, some rare diseases show characteristic imaging findings or patient history; therefore, familiarity with the imaging presentation of rare diseases may improve diagnostic accuracy and patient management. Multiseptate gallbladder is a rare congenital malformation. We present a case that emphasizes the importance of ultrasonography and magnetic resonance imaging. Ultrasound examination of the abdomen showed multiple linear internal echoes consistent with multiple septa crossing the entire gallbladder lumen, creating a honeycomb appearance. Multiseptate gallbladder (MSG) is a rare congenital anomaly. MSG may be accompanied with other abnormalities of the biliary tree. MSG can be asymptomatic and most cases are detected incidentally. Nevertheless, some patients present with biliary pain or colicky abdominal pain due to biliary stasis and calculi. We report a case of MSG in a girl who presented with recurrent abdominal pain and normal laboratory work-up recovered after laparoscopic cholecystectomy.
