Abstract

HIV-1 infection cannot be cured due to long-lived viral reservoirs formed by latently infected CD4+ T cells. "Shock and Kill" strategy has been considered to eliminate the viral reservoir and achieve a functional cure but the stimulation of cytotoxic immunity is necessary. Ponatinib is a tyrosine kinase inhibitor (TKI) clinically used against chronic myeloid leukemia (CML) that has demonstrated to be effective against HIV-1 infection in vitro. Several TKIs may induce a potent cytotoxic response against cancer cells that makes possible to discontinue treatment in people with CML who present long-term deep molecular response. In this longitudinal study, we analyzed the capacity of ponatinib to induce an antiviral response against HIV-1 infection in peripheral blood mononuclear cells (PBMCs) obtained from people with CML previously treated with imatinib for a median of 10 years who changed to ponatinib for 12 months to boost the anticancer response before discontinuing any TKI as part of the clinical trial NCT04043676. Participants were followed-up for an additional 12 months in the absence of treatment. PBMCs were obtained at different time points and then infected in vitro with HIV-1. The rate of infection was determined by quantifying the intracellular levels of p24-gag in CD4+ T cells. The levels of p24-gag+ CD4+ T−cells were lower when these cells were obtained during and after treatment with ponatinib in comparison with those obtained during treatment with imatinib. Cytotoxicity of PBMCs against HIV-infected target cells was significantly higher during treatment with ponatinib than during treatment with imatinib, and it was maintained at least 12 months after discontinuation. There was a significant negative correlation between the lower levels of p24-gag+ CD4+ T−cells and the higher cytotoxicity induced by PBMCs when cells were obtained during and after treatment with ponatinib. This cytotoxic immunity was mostly based on higher levels of Natural Killer and Tγδ cells seemingly boosted by ponatinib. In conclusion, transient treatment with immunomodulators like ponatinib along with ART could be explored to boost the antiviral activity of cytotoxic cells and contribute to the elimination of HIV-1 reservoir. Abstract

Introduction: In the past few years, the use of artificial intelligence in healthcare has grown exponentially. Prescription of antibiotics is not exempt from its rapid diffusion, and various machine learning (ML) techniques, from logistic regression to deep neural networks and large language models, have been explored in the literature to support decisions regarding antibiotic prescription. Areas covered: In this narrative review, we discuss promises and challenges of the application of ML-based clinical decision support systems (ML-CDSSs) for antibiotic prescription. A search was conducted in PubMed up to April 2024. Expert opinion: Prescribing antibiotics is a complex process involving various dynamic phases. In each of these phases, the support of ML-CDSSs has shown the potential, and also the actual ability in some studies, to favorably impacting relevant clinical outcomes. Nonetheless, before widely exploiting this massive potential, there are still crucial challenges ahead that are being intensively investigated, pertaining to the transparency of training data, the definition of the sufficient degree of prediction explanations when predictions are obtained through black box models, and the legal and ethical framework for decision responsibility whenever an antibiotic prescription is supported by ML-CDSSs. Abstract

The lack of relevant annotated datasets represents one key limitation in the application of Natural Language Pro- cessing techniques in a broad number of tasks, among them Protected Health Information (PHI) identification in Norwegian clinical text. In this work, the possibility of exploiting resources from Swedish, a very closely related language, to Norwegian is explored. The Swedish dataset is annotated with PHI information. Different processing and text augmentation techniques are evaluated, along with their impact in the final performance of the model. The augmentation techniques, such as injection and generation of both Norwegian and Scandinavian Named Entities into the Swedish training corpus, showed to increase the performance in the de-identification task for both Danish and Norwegian text. This trend was also confirmed by the evaluation of model performance on a sample Norwegian gastro surgical clinical text.

Abstract

Ethical considerations regarding our treatment of animals have gained strength, leading to legislation and a societal focus across various disciplines. This is a subject of study within curricula related to agri-food sciences. The aim was to determine the perceptions of agronomy university students concerning animal welfare in livestock production systems. A survey was conducted to encompass various aspects, from participants’ sociodemographic attributes to their attitudes and behaviors regarding animal welfare and the consumption of animal products. Statistical analysis, performed using R software, delved into the associations between participants’ characteristics and their perspectives on the ethical, bioethical, and legal dimensions of animal welfare. Associations between demographic factors and ethical viewpoints among students were identified. Gender differences emerged in animal treatment perceptions, while rural and urban environments impacted perspectives on various animals. Bioethical considerations revealed distinctive disparities based on gender and education in concerns regarding animal welfare, value perceptions, evaluations of animal behaviors, and opinions on animal research. It is crucial to distinguish between animal welfare and the ethical considerations arising from coexisting with sentient beings capable of experiencing suffering. Ethical theories provide a lens through which we perceive our obligations toward animals. The responsibility to ensure animal welfare is firmly rooted in recognizing that animals, like humans, experience pain and physical suffering. Consequently, actions causing unjustified suffering or mistreatment, particularly for entertainment purposes, are considered morally unacceptable.

Abstract

Communication and cooperation are fundamental for the correct deployment of P5 medicine, and this can be achieved only by correct comprehension of semantics so that it can aspire to medical knowledge sharing. There is a hierarchy in the operations that need to be performed to achieve this goal that brings to the forefront the complete understanding of the real-world business system by domain experts using Domain Ontologies, and only in the last instance acknowledges the specific transformation at the pure information and communication technology level. A specific feature that should be maintained during such types of transformations is versioning that aims to record the evolution of meanings in time as well as the management of their historical evolution. The main tool used to represent ontology in computing environments is the Ontology Web Language (OWL), but it was not created for managing the evolution of meanings in time. Therefore, we tried, in this paper, to find a way to use the specific features of Common Terminology Service—Release 2 (CTS2) to perform consistent and validated transformations of ontologies written in OWL. The specific use case managed in the paper is the Alzheimer's Disease Ontology (ADO). We were able to consider all of the elements of ADO and map them with CTS2 terminological resources, except for a subset of elements such as the equivalent class derived from restrictions on other classes. Abstract

In this narrative review, we discuss studies assessing the use of machine learning (ML) models for the early diagnosis of candidemia, focusing on employed models and the related implications. There are currently few studies evaluating ML techniques for the early diagnosis of candidemia as a prediction task based on clinical and laboratory features. The use of ML tools holds promise to provide highly accurate and real-time support to clinicians for relevant therapeutic decisions at the bedside of patients with suspected candidemia. However, further research is needed in terms of sample size, data quality, recognition of biases and interpretation of model outputs by clinicians to better understand if and how these techniques could be safely adopted in daily clinical practice.

Abstract

Although safety footwear has an important role to play in ensuring the safety, comfort and health of workers in work environments that require the use of personal protective equipment (PPE), until now there has been no such thing as safety footwear designed specifically for women. In collaboration with the Instituto de Biomecánica (IBV), the company DIADORA has developed Athena, a new safety shoe for women. The design of Athena has involved a study of the needs of working women, provided by working women themselves, a morphological study of their feet, and an analysis of their functional requirements. The result is a shoe that provides an exceptional fit thanks to components designed to adapt perfectly to the user’s tread, providing optimum functionality at every step., El calzado de seguridad es importante para garantizar la seguridad, la comodidad y la salud de las personas trabajadoras en entornos laborales que requieren del uso de equipos de protección individual (EPIs). Sin embargo, no existe disponibilidad de calzado de seguridad específico para mujeres. La empresa DIADORA ha contado con el Instituto de Biomecánica (IBV) para desarrollar Athena, un nuevo calzado de seguridad destinado a la mujer. Athena se fundamenta en el estudio de las necesidades de las mujeres trabajadoras, aportadas por ellas mismas, en el estudio morfológico de sus pies y en el análisis de sus requerimientos funcionales. El resultado es un calzado que aporta un ajuste excepcional a través del diseño de unos componentes que optimizan la adaptación a la pisada aportando una funcionalidad óptima en cada momento del paso.

Abstract

La publicación en 2021, por parte de FIFA, del Programa de Calidad y del manual de ensayos para campos de hierba natural, dentro del programa FIFA Quality, estableció un nuevo marco para la calidad en superficies de juego de césped natural destinadas a la práctica del fútbol. Este marco se completó en mayo de 2022 con la publicación de los valores de referencia que deben cumplir los campos para los diferentes parámetros establecidos para su evaluación. El Instituto de Biomecánica (IBV) ha participado en este nuevo marco de actuación. En septiembre de 2022 FIFA organizó el primer Round Robin para la acreditación de laboratorios y el IBV consiguió su primera acreditación como centro de ensayos para hierba natural. Acreditación renovada recientemente en Granada, y que posiciona al centro entre los punteros a nivel mundial en hierba natural. Así, el IBV es, en estos momentos, el único centro español acreditado para certificar campos de césped natural, de acuerdo con el programa de calidad FIFA Quality. Recién lanzados los largos preparativos para el mundial 2030 en los que la superficie de juego será el “escenario clave”, el IBV está preparado para acreditar que el verde esté en óptimas condiciones.
