Lung cancer (LC) is a malignant tumor with the highest incidence in the world, and its specific pathogenesis is still unclear. Circular RNAs (circRNAs) are a group of non-coding RNAs that play a key role in the development and progression of various cancers. The expression pattern and function of circRNAs in LC are still not completely distinct. In this study, it was aimed to study the expression and potential mechanism of circ-UBR1 in LC cells. Then it was found that circ-UBR1 was up-regulated in LC cells, and had microRNA (miR)-545-5p binding sites. Meanwhile, it was confirmed by dual-luciferase reporter assay that circ-UBR1 directly bound to miR-545-5p and then repressed its expression. MiR-545-5p was down-regulated in LC cells and refrained its expression by binding to the downstream target gene SSFA2. Knockdown circ-UBR1 or enhancive miR-545-5p repressed A549 cell proliferation, migration, and invasion, but accelerated apoptosis. After transfection with circ-UBR1 low expression vector, upregulation of SSFA2 apparently reversed the depression of reduced circ-UBR1 on cell proliferation, migration, and invasion, and the promotion of cell apoptosis. Further tumor xenograft experiments in nude mice also confirmed that knockdown of circ-UBR1 could increase the expression of miR-545-5p, but decrease the expression of SSFA2, thus alleviating the progression of LC in vivo. Therefore, these results fully indicate that circ-UBR1 promotes LC cell proliferation, migration, and invasion, but represses apoptosis via the circ-UBR1 axis, which may be a closely related marker and therapeutic target of LC.

The high-pressure, low-temperature environment prevailing in offshore gas production and transportation is prone to forming methane hydrates inside the tubings or pipelines, resulting in reduced production and flow assurance problems. Conventional hydrate prevention relies on the continuous injection of chemicals to alter the chemical potential of the mixture stream. Such a method has the disadvantages of high cost, high toxicity, and high environmental impact. Effective hydrate prevention methods are urgently needed in offshore petroleum engineering. When fluid in nozzles or near turbine blades experiences an abrupt pressure drop, cavitation bubbles form and accumulate in the fluid. Bubbles collapse as they move downstream along the flow. As the bubble collapse, extremely high temperature and high pressure are generated, and the ambient fluid around the bubble is heated. In this paper, the thermal effect of cavitation is introduced into methane hydrate prevention. A numerical simulation of cavitation inside an injector was performed, an experimental setup was established, and the influences of various working parameters such as injection pressure, injection frequency, and fluid temperature on the thermal effect of cavitation were analyzed. Computational fluid dynamics studies have revealed the bubble collapse process. The evolution of pressure and temperature inside and outside the bubble has been analyzed and validated by previous experimental observations. Different impact chambers have been tested for their cavitation performance, and the one with a cone shape shows superior performance over the other two. In the experimental observations, an increase in the injection pressure leads to an increase in the fluid temperature. An increase in injection frequency and chamber pressure facilitates the increase in ambient fluid temperature, while a further increase in fluid temperature hinders the cavitation heating process. A tubing configuration with a cavitation method is also proposed in this paper.

Backgrounds: To explore the value of umbilical artery cord blood glucose (UACBG) in predicting hypoglycemia in gestational diabetes mellitus (GDM) and other at-risk newborns, and to provide a cut-off UACBG value for predicting hypoglycemia occurrence. Methods: In this prospective study, we enrolled at-risk infants delivered vaginally, including neonates born to mothers with GDM, premature, macrosomia, and low birth weight. We separated the infants into GDM group and other at-risk group. All subjects underwent UACBG measurement during delivery. Neonatal peripheral blood glucose measurement was performed at 0.5 hand 2 h after birth. The predictive performance of UACBGfor neonatal hypoglycemia was assessed using receiver operating characteristic curve (ROC), area under curve (AUC), sensitivity and specificity. Results: 916 newborns were included, 538 in GDM group and 378 in other at-risk group. For hypoglycemia prediction within 2 h, the best cut-off of UACBG was 4.150 mmol/L, yielding an AUC of 0.688 (95% CI 0.625-0.751). UACBG had a better performance at 0.5 h (AUC 0.757; 95% CI 0.690-0.824). In detail, the AUC was 0.703 in GDM group (95% CI 0.594-0.811), with the optimal cut-off of 4.550 mmol/L. In other at-risk group, the AUC was 0.797（95% CI 0.716-0.877), and the best threshold was 4.350 mmol/L. No significant differences were observed between GDM group and other at-risk group in AUC at 0.5 h, 2 h and within 2 h. Conclusions: UACBG may be an efficient, non-invasive approach to predicting neonatal hypoglycemia within 2 h after birth.

In this study, the distributed optical fiber is used to monitor the overburden changes during the coal mining process, and the average frequency shift change degree of optical fiber is introduced as the index to judge the periodic pressure. The ARMA-SVR model composed of auto-regressive moving average (ARMA) and support vector machine regression (SVR) is established to predict the rock pressure behavior through the prediction of the frequency shift change of optical fiber. Firstly, a similar material model is built to obtain the data of the frequency shift of the optical fiber during the excavation of the working face; Then, the frequency shift change sequence of optical fiber is converted into a time series. Finally, a combined model is built. The linear auto-correlation part of the frequency shift change sequence is predicted by ARMA model, and the nonlinear residual part is predicted by SVR model to jointly complete the prediction of the frequency shift change of optical fiber. The experimental results show that the combined model proposed in this paper is superior to the single auto-regressive moving average model in different data sets, and the ARMA-SVR combined model can effectively predict the change of optical fiber frequency shift and provide a new idea for the prediction of rock pressure in the mining process.

The review is to explore the connection between gestational hypertension diseases (GHD) and small for gestational age (SGA) in twin pregnancies.According to the recommendations of PRISMA, relevant studies were systematically searched through PubMed, Web of Science, Cochrane Library, Embase from inception until January 16, 2022. Subgroup analysis was performed according to chorionicity and diagnostic criteria of SGA. Odds ratios (OR) were assessed to judge the link between GHD and SGA in twin pregnant women. A random-effect model was used to estimate the pooled hazard ratio when there was significant heterogeneity (ISeven articles containing 470 589 twin pregnant women were included. The increased risk of SGA was connected to the twin pregnancies complicated with GHD (OR = 1.57, 95% confidence interval [CI] = 1.10-2.24, p = 0.01). After subgroup analysis, the connection between SGA and GHD had no statistical significance (OR = 1.17, 95% CI = 0.95-1.44, p = 0.14) when the enrolled studies using the SGA diagnosis referred to singleton birth weight, but significant (OR = 2.14, 95% CI = 1.77-2.60, p0.001) in the group using the SGA diagnosis referred to twin birth weight. Stratified by chorionicity, SGA was relevant to GHD in the dichorionic (DC) group (OR = 1.68, 95% CI = 1.17-2.42, p = 0.005), while not in the monochorionic (MC) group (OR = 1.68, 95% CI = 0.93-3.03, p = 0.09). More future articles are warranted to confirm these outcomes.Our review demonstrated that GHD in DC twin pregnancies was related to an enlarged risk of SGA. Two SGA diagnosis references led to different results. Twin pregnancies complicated with GHD were at significantly higher risk of SGA when twin birth weight reference was used.

The mechanical properties and microstructures, in particular precipitation in the AISI H13 steel, quenched and tempered from 773 K to 973 K for different periods, were systematically investigated by scanning electron microscopy, electron backscatter diffraction (EBSD) and transmission electron microscopy (TEM). The results indicate a sharp decrease in hardness at temperatures > 863 K during the first 2 h of tempering. Ultimate tensile strength and yield strength decrease, while elongation and impact energy increase with the increase of tempering temperature. The volume fraction of static recrystallization increases from the EBSD result. Regarding precipitates, the coarsening rate of M23C6 was much faster than that of MC and M2C and was verified by using the Ostwald ripening model. In addition, kinetic modeling of the softening of H13 during tempering from 863 K to 973 K was performed. This model was also applied successfully to predict the hardness of double-tempered H13 steel samples.

378 Background: Neoadjuvant PD-1 inhibitor combined with chemotherapy has been reported to improve the pathologic response of locally advanced esophageal squamous cell carcinoma (ESCC). However, systematic report of efficacy and safety data in elderly Chinese patients is lacking. This study we report the efficacy and safety outcomes of neoadjuvant sintilimab combined with chemotherapy in elderly patients with locally advanced ESCC. Methods: Elderly patients (70 years or older) with locally advanced ESCC received neoadjuvant sintilimab combined with albumin-bound paclitaxel and nedaplatin in our institution between July 2019 and September 2022 were reviewed. Efficacy including pathologic response, short-term survival outcomes, and safety profile retrospectively were evaluated. Results: Thirty-one eligible patients were included in the analysis, with median age 72 years (range 70-78 years), male 18 (58.1%), Upper/middle/lower tumors 2(6.5%)/15(48.4%) /14(45.2%), ECOG PS 0/1-2 15 (48.4%)/16 (51.6%), clinical stage II/III 14 (45.2%)/17 (54.8%). The median number of neoadjuvant cycles was 3 (range 2-4). All 31 patients achieved R0 resection. Postoperative pathology showed pCR rate was 45.2% (95CI 27.3%-64.0%), MPR rate was 58.1% (95CI 39.1%-75.5%), tumor down-staging rate was 90.3% (95CI 74.2%-98.0%). With a median follow-up of 15.9 months (range 3.0-32.3), 1 (4.2%) patient had disease recurrence and died. The median DFS and median OS had not been reached yet, 2-year DFS rate was 95.0% (95%CI 85.9%-100.0%). Grade 3-4 Surgical complications occurred in 1 (4.2%) patient, with a grade 3 pulmonary infection. The median operation time was 332 min (range, 142–470 min), and the median intraoperative blood loss was 150 ml (range, 80–300 ml). The median hospital stay was 12.5 days (range 7-22). There was no perioperative death. Grade 3-4 TRAEs were Neutropenia (9.7%), leukopenia (9.7%), abnormal liver function (6.5%), anemia (6.5%), sensory neuropathy (3.2%), pneumonia (3.2%). Conclusions: Our results preliminarily confirms that Chinese elderly patients with locally advanced ESCC can benefit from neoadjuvant sintilimab combined with chemotherapy, similarly with this combination treatment in all age patients.

Background: Cervical anastomotic leakage (CAL) is one of the most common complications that occur minimally invasive esophagectomy (MIE). It is associated with high postoperative mortality. Some risk factors still remained controversial and so accurate prediction of risk groups for CAL remained very difficult. This study aimed to identify the risk factors of CAL after McKeown MIE to predict the accuracy of the technique as early as possible. Material and Methods: A total of 129 patients with esophageal cancer who underwent McKeown MIE at the Department of Thoracic Surgery, the Fourth Hospital of Hebei Medical University, between January 2018 and June 2019 were retrospectively reviewed. Multivariate logistic regression analysis was used to identify the risk factors for CAL and receiver operating characteristic (ROC) curve analysis was used to predict the accuracy for each quantitative data variable and determine the cutoff value. Results: There were statistically significant differences between Group CAL and Group NCAL in FEV1 (p = 0.031), neoadjuvant chemotherapy (p = 0.001), intraoperative minimum PaCO2 (p = 0.002), and hospital stays (p 45.5 mmHg (p = 0.002) demonstrated good accuracy. Conclusion: FEV1, neoadjuvant chemotherapy, and intraoperative minimum PaCO2 in arterial blood gas (ABG) were considered as risk factors of CAL after McKeown MIE for esophageal cancer. Preoperative FEV1 45.5 mmHg in ABG showed good accuracy in predicting risk factors for CAL.

Aiming at the multi-objective polarity design of Mixed-polarity Reed-Muller (MPRM) circuit, such as small area and low power consumption, an integrated polarity optimization scheme based on improved Multi-objective particle swarm optimization (MOPSO) is proposed. In the Improved MOPSO (IMOPSO) algorithm, particles in the external archive can be actively evolved through self-learning operations to find better circuit polarity. The particles in the population achieve selflearning fractals by comparing the differences between their own states and individuals in external archive to enhance the evolutionary level of the population. A multiobjective decision model of area and power consumption is established according to the characteristics of MPRM circuit. The tabular technique and the IMPOPO algorithm are combined to obtain the Pareto optimal polarity set of the MPRM circuit for area and power consumption. The MCNC Benchmark circuit is used to test the performance of the algorithm. The results verify the effectiveness of the proposed algorithm.

Background To investigate the value of contrast-enhanced CT (CECT)-derived imaging features in predicting lymphovascular invasion (LVI) status in esophageal squamous cell carcinoma (ESCC) patients. Methods One hundred and ninety-seven patients with postoperative pathologically confirmed esophageal squamous cell carcinoma treated in our hospital between January 2017 and January 2019 were enrolled in our study, including fifty-nine patients with LVI and one hundred and thirty-eight patients without LVI. The CECT-derived imaging features of all patients were analyzed. The CECT-derived imaging features were divided into quantitative features and qualitative features. The quantitative features consisted of the CT attenuation value of the tumor (CTVTumor), the CT attenuation value of the normal esophageal wall (CTVNormal), the CT attenuation value ratio of the tumor-to-normal esophageal wall (TNR), the CT attenuation value difference between the tumor and normal esophageal wall (ΔTN), the maximum thickness of the tumor measured by CECT (Thickness), the maximum length of the tumor measured by CECT (Length), and the gross tumor volume measured by CECT (GTV). The qualitative features consisted of an enhancement pattern, tumor margin, enlarged blood supply or drainage vessels to the tumor (EVFDT), and tumor necrosis. For the clinicopathological characteristics and CECT-derived imaging feature analysis, the chi-squared test was used for categorical variables, the Mann–Whitney U test was used for continuous variables with a nonnormal distribution, and the independent sample t-test was used for the continuous variables with a normal distribution. The trend test was used for ordinal variables. The association between LVI status and CECT-derived imaging features was analyzed by univariable logistic analysis, followed by multivariable logistic regression and receiver operating characteristic (ROC) curve analysis. Results The CTVTumor, TNR, ΔTN, Thickness, Length, and GTV in the group with LVI were higher than those in the group without LVI (P P Tumor, TNR, ΔTN and enhancement pattern, Thickness, Length, GTV, tumor margin, EVFDT, and tumor necrosis were associated with LVI status (P Conclusion The CECT-derived imaging features, including TNR, Thickness, tumor margin, and their combination, can be used as predictors of LVI status for patients with ESCC.

Osteoporosis is a metabolic bone illness characterized by low bone density and a high risk of fracture. It is estimated that there are >60 million individuals in China suffering from this disease, which highlights an urgent requirement for the development of novel and safe drugs for the long-term treatment of osteoporosis. MicroRNAs (miRNAs/miRs) have previously been identified as critical regulators in the progression of osteoporosis. As an intronic miRNA, miR-27b enhances the osteoblastic differentiation of stem cells from the bone marrow and the maxillary sinus membrane. However, the mechanism underlying miR-27b in osteoporosis remains to be elucidated. In the present study, MC3T3-E1 pre-osteoblasts were treated with dexamethasone (DEX) to establish an in vitro model of osteoporosis. The results of the present study demonstrated that DEX treatment markedly inhibited the viability of MC3T3-E1 cells, and downregulated the expression level of miR-27b. The results of reverse transcription-quantitative PCR, western blotting and dual-luciferase assays revealed that miR-27b directly regulated and suppressed the expression of peroxisome proliferator-activated receptor γ2 (PPARγ2) in MC3T3-E1 cells. Furthermore, overexpression of miR-27b by transfection of cells with miR-27b mimic attenuated DEX-mediated inhibition of cell viability, alkaline phosphatase (ALP) activity and the expression levels of bone morphogenetic protein-2 (BMP2), runt-related protein 2 (Runx2) and osteocalcin (OCN). The results of the present study indicated that miR-27b alleviated DEX-inhibited proliferation and osteoblastic differentiation. Moreover, miR-27b knockdown repressed MC3T3-E1 cell viability, ALP activity and protein levels of BMP2, Runx2 and OCN. However, these effects were abrogated by small interfering RNA-mediated PPARγ2 silencing. In conclusion, the results of the present study demonstrated that miR-27b attenuated DEX-inhibited proliferation and osteoblastic differentiation in MC3T3-E1 pre-osteoblasts by targeting PPARγ2.

The study focused on the risk factors of postoperative arrhythmia and lung infection and the preventive effects of targeted low-molecular-weight heparin (LMWH) on the occurrence of deep venous thrombosis (DVT) in patients with esophageal/cardia cancer. In this article, 82 patients who were pathologically diagnosed with esophageal/cardia cancer and underwent surgical treatment were selected as the research subjects. According to the different preoperative treatment methods, the patients were divided into the control group (without anticoagulant drugs before the operation, 44 cases) and the anticoagulation group (anticoagulant drugs were administered before the operation, 38 cases), and they were compared for basic clinical indicators and disease history. Logistic regression analysis was performed to analyze the risk factors of adverse events, and the Wells and Autar scale scores were calculated. Different groups were compared for the operation time, blood loss, and postoperative drainage volume during the operation. D-dimer was detected on the first 1, 3, 5, and 7 days after the operation, and the lower extremity venous color Doppler ultrasound was performed on the 1st and 7th days after the operation. The results showed that age ≥65 years, abnormal preoperative ECG, preoperative coronary heart disease (CHD), preoperative chronic obstructive pulmonary disease (COPD), operative time ≥4 h, and preoperative blood sodium P < 0.01 . It suggested that age, preoperative disease history, hospital stay, and operation time were risk factors for postoperative adverse events in patients with esophageal/cardia cancer. The targeted anticoagulant LMWH has a significant preventive effect on the occurrence of lower extremity DVT in patients with esophageal/cardia cancer, providing an effective reference for the prognosis and prevention of esophageal/cardia cancer.

The sensitivity and specificity of endoscopic ultrasound (EUS) for esophageal cancer are variable. The aim of the present study was to determine the accuracy of EUS for the T staging of esophageal cancer and to explore the factors that affect the accuracy.This was a retrospective study of patients with esophageal cancer who underwent EUS between January 2018 and September 2019 at the author's hospital. All patients underwent EUS, surgery, and pathological examination. The diagnostic value of ultrasound-based T (uT) staging was evaluated using the pathological T (pT) staging as the gold standard.Finally, 169 patients were included. Among the 169 patients, 37 were overstaged by EUS, 33 were understaged, and 99 were correctly staged. The overall accuracy of EUS was 58.6%. Sensitivity was low, at 0% to 70.8% depending upon the pT stage, but specificity was higher, at 71.0% to 100.0%, also depending upon the pT stage. The multivariable analysis revealed that highly differentiated tumors (odds ratio = 9.167, P = .041) and pT stage ≥T2 (odds ratio = 2.932, P = .004) were independent factors of accurate uT stage.The staging of esophageal cancer using EUS has low sensitivity but high specificity. Highly differentiated tumors and pT stage ≥2 tumors were associated with the accuracy of uT staging.

Emerging studies have indicated that microRNAs (miRNAs/miRs) are involved in regulating non-small cell lung cancer (NSCLC)-associated processes. The present study aimed to evaluate the biological roles of miR-374a-5p in NSCLC. Using reverse transcription-quantitative PCR, the expression levels of miR-374a-5p were determined in NSCLC cells and a normal cell line. Functional experiments were performed to investigate the functions of miR-374a-5p in NSCLC. A luciferase activity reporter assay and rescue experiments were performed to validate NCK adaptor protein 1 (NCK1) as a functional target of miR-374a-5p. It was demonstrated that miR-374a-5p levels were decreased in NSCLC cell lines compared with those in a normal cell line. Furthermore, overexpression of miR-374a-5p inhibited NSCLC cell proliferation and migration in vitro. Of note, NCK1 overexpression reversed the effects of miR-375a-5p on NSCLC cell proliferation and migration. The present results confirmed the tumor suppressor role of miR-374a-5p via targeting NCK1 in NSCLC, indicating the importance of the miR-374a-5p/NCK1 axis in NSCLC.

ObjectivesTo develop a radiomics model based on contrast-enhanced CT (CECT) to predict the lymphovascular invasion (LVI) in esophageal squamous cell carcinoma (ESCC) and provide decision-making support for clinicians.Patients and MethodsThis retrospective study enrolled 334 patients with surgically resected and pathologically confirmed ESCC, including 96 patients with LVI and 238 patients without LVI. All enrolled patients were randomly divided into a training cohort and a testing cohort at a ratio of 7:3, with the training cohort containing 234 patients (68 patients with LVI and 166 without LVI) and the testing cohort containing 100 patients (28 patients with LVI and 72 without LVI). All patients underwent preoperative CECT scans within 2 weeks before operation. Quantitative radiomics features were extracted from CECT images, and the least absolute shrinkage and selection operator (LASSO) method was applied to select radiomics features. Logistic regression (Logistic), support vector machine (SVM), and decision tree (Tree) methods were separately used to establish radiomics models to predict the LVI status in ESCC, and the best model was selected to calculate Radscore, which combined with two clinical CT predictors to build a combined model. The clinical model was also developed by using logistic regression. The receiver characteristic curve (ROC) and decision curve (DCA) analysis were used to evaluate the model performance in predicting the LVI status in ESCC.ResultsIn the radiomics model, Sphericity and gray-level non-uniformity (GLNU) were the most significant radiomics features for predicting LVI. In the clinical model, the maximum tumor thickness based on CECT (cThick) in patients with LVI was significantly greater than that in patients without LVI (PPConclusionsThe combined model incorporating radiomics features and clinical CT predictors may potentially predict the LVI status in ESCC and provide support for clinical treatment decisions.

Background: Liver transplant is frequently performed at night due to the unpredictability of organ procurement and reduction of Cold Ischemia Time (CIT). Previous study reported a doubled mortality Hazard Ratio (HR) of early death and increased postoperative complications in nighttime liver transplant. This study aims to evaluate the impact of nighttime operation on patients’ survival and postoperative complications by using meta-analysis. Methods: We performed a systematic review of the PubMed database and identified five eligible studies. Three time points (30 days, 90 days and 1 year) were explored in patients’ survival by using pooled HR. Four types of postoperative complications (vascular, biliary tract, wound and primary graft non-function) were investigated by using pooled Odds Ratio (OR). Publication bias was also performed. Results: Our study results were contradicting with the previous report and yielded no significant difference with a HR=0.98 (95% CI=0.89-1.06) on 30 days, HR=1.12 (95% CI=0.89-1.35) on 90 days and HR=1.07 (95% CI=0.95-1.18) on 1 year in nighttime procedure. Consistent with the result of patients’ survival, no significant result was found in postoperative complications evaluation as well. None of the four complications demonstrated significant result. And we failed to detect any significant publication bias. Conclusions: Based on current evidence, nighttime liver transplantations do not degrade patients’ survival or increase postoperative complications risk compared with daytime operations.
