Introduction: multiple devices are available for non-invasive oxygenation support: oxygen mask, high flow nasal canula (HFNC), continuous positive airway pressure (CPAP), mask non-invasive ventilation (M-NIV) and helmet NIV (H-NIV). There are conflicted data about their respective effects during acute respiratory failure. Tidal volume is a determinant of both efficacy and safety during ventilatory support. We assessed whether the tidal volume (VT) was influenced by non-invasive oxygenation support device. Methods: this six devices were tested using the RespiSim® manikin connected to an ASL 5000® breathing simulator. Three different respiratory mechanics (normal, obstructive and restrictive) and three levels of respiratory effort (low, moderate and respiratory distress) were simulated. Results: as compared to the oxygen mask (VT=440 ± 352 mL), VT increased significantly with M-NIV (VT=690 ± 321 mL) and H-NIV (VT=652 ± 366 mL) and decreased significantly with CPAP (VT=377 ± 297 mL) and HFNC (VT=414 ± 333 mL) (p

L’extubation précoce est une préoccupation du réanimateur. L’hypoxémie est un critère de non-extubation. Le recours à la ventilation non invasive (VNI) ou à l’oxygénothérapie à haut débit nasal (OHDN) pourrait modifier la balance bénéfice/risque et plaider pour une extubation plus précoce. On se propose d’étudier le devenir de patients de chirurgie cardio-thoracique, extubés alors qu’ils présentent un rapport P/F inférieur à 150 et dont l’épreuve de ventilation spontanée (VS) est satisfaisante par ailleurs. Nous avons recueilli de façon rétrospective les données des patients dont le rapport P/F avant extubation était inférieur à 150 et dont l’épreuve de VS était satisfaisante en post-opératoire de chirurgie cardio-thoracique. Le critère de jugement principal était le taux de réintubation dans les 7 jours. Les données concernant les gaz du sang de J1 à J3, la durée et le type d’assistance respiratoire ainsi que les données cliniques au décours de l’extubation ont été recensées. Entre juin 2011 et décembre 2015, 98 patients ont été extubés alors qu’ils étaient hypoxémiques (PaO2/FiO2 = 134 ± 16). Sur ces 98 patients, 16 (16,3%) ont été réintubés au cours de séjour en réanimation. Les paramètres d’hématose s’amélioraient sous VNI et/ou OHDN. L’extubation de patients hypoxémiques, associée à des techniques d’assistance respiratoire non invasive, a un taux d’échec de 16,3 % qui semble acceptable, en postopératoire de chirurgie cardio-thoracique. Il serait intéressant d’étudier comparativement deux stratégies différentes (maintien de l’assistance invasive versus extubation précoce aidée par les techniques non-invasives), notamment en termes de complications de l’intubation.

International audience; BACKGROUND: Surgical resection is the best treatment for stage I and II non-small cell lung cancer. Despite an improvement in the perioperative management of cancer patients and specialization of surgical teams, morbidity and mortality remains significant. Non-invasive ventilation (NIV) is an effective therapeutic option in hypercapnic respiratory failure. It also improves functional and gasometric parameters when undertaken before surgery. The objective of the preOVNI study is to demonstrate that preoperative non-invasive ventilation for 7 days, at home, reduces the postoperative respiratory and cardiovascular complications of lung resection surgery, in a high-risk population. METHODS: A prospective, randomized, controlled open-labelled multicentric French study, under the supervision of the Groupe Français de Pneumocancérologie (GFPC), comparing 7 days of preoperative non-invasive ventilation with standard treatment. Inclusion criteria are: patients suitable for lobectomy or segmentectomy for primary bronchial carcinoma and presenting with obstructive or restrictive lung disease, obesity or chronic cardiac insufficiency. The primary criterion is a composite one, including all respiratory and cardiac complications. The number of patients is 150 in each treatment arm, 300 in total. EXPECTED RESULTS: We think that preoperative NIV will be able to reduce the rate of postoperative complications. If this objective is achieved, the management of these patients could be changed.
