International audience; Gliomas are the most frequent primary brain tumour. The proximity of organs at risk, the infiltrating nature, and the radioresistance of gliomas have to be taken into account in the choice of prescribed dose and technique of radiotherapy. The management of glioma patients is based on clinical factors (age, KPS) and tumour characteristics (histology, molecular biology, tumour location), and strongly depends on available and associated treatments, such as surgery, radiation therapy, and chemotherapy. The knowledge of molecular biomarkers is currently essential, they are increasingly evolving as additional factors that facilitate diagnostics and therapeutic decision-making. We present the update of the recommendations of the French society for radiation oncology on the indications and the technical procedures for performing radiation therapy in patients with gliomas.

Il existe aujourd’hui de nombreux matériaux et procédés de fabrication permettant au chirurgien-dentiste de réaliser une couronne périphérique tout céramique. Cependant il n’y a pas de système idéal pouvant être utilisé en toutes circonstances. Cette thèse d’exercice a pour objectif d’aider le praticien à choisir un système céramique en fonction de la situation clinique rencontrée. Dans un premier temps, ce document présente les systèmes céramiques principaux existants, les différents procédés de fabrication et les propriétés associées aux matériaux. La seconde partie propose une analyse de facteurs cliniques qui peuvent guider le praticien dans la sélection du matériau tout-céramique lors de la réalisation d’une couronne périphérique. On en distingue quatre grandes catégories : mécanique, esthétique, biologique et fonctionnelle. Enfin dans un dernier temps, plusieurs cas cliniques permettent d’illustrer certains propos détaillés précédemment. Connaître les propriétés physiques et esthétiques de ces matériaux, mais aussi les critères cliniques décisifs, permettront au praticien d’orienter son choix et de se conformer aux meilleures recommandations possibles.

L’avènement de l’adhésion en odontologie a considérablement modifié l’approche et la réalisation des soins plaçant de plus en plus la préservation tissulaire au cœur des préoccupations des praticiens. À l’heure actuelle les restaurations adhésives permettent de répondre à un large panel de situations cliniques. Il est communément admis qu’une restauration directe est plus adaptée pour les petites pertes de substance et qu’une indirecte est plus adaptée pour une perte de substance importante. Cependant cette distinction entre « petite » et « grande » n’a que peu de pertinence scientifiquement parlant si elle n’est pas précisément caractérisée car sujette à l’interprétation de chacun. À partir de quelles dimensions, considère-t-on qu’une perte de substance soit « petite » ou « grande » ? Et quid des pertes de substances « moyennes » ? Avant d’envisager la restauration d’une dent, il convient d’analyser les structures dentaires résiduelles. En effet la géométrie de la cavité à restaurer se doit d’être la plus stable possible afin de prolonger le plus possible la présence de la dent support sur l’arcade. Une fois la géométrie cavitaire optimale obtenue, la restauration de la dent peut commencer. Pour répondre à la problématique dimensionnelle nommée ci-dessus, cette thèse d’exercice a pour but d’explorer le fondement scientifique de ces indications et de voir si elles sont toujours d’actualité.

Coercion is a tool used in psychiatry when the psychic state of a patient is considered relevant to urgent care, the patient refuses care and that respecting the refusal would be more harmful for the patient than to impose this. that the doctor deems necessary. Behind this simple definition, in fact, there is a whole process leading to a decision to compel or not. This decision is made in a particular doctor-patient relationship where the patient's ability to decide for himself and the competition of non-reconcilable ethical values is assessed. To explore the justifications of psychiatrists in the decision to constrain and explain the ethical issues that accompany this decision-making process, we conducted a qualitative study using semi-structured interviews with psychiatrists who regularly perform restraint measures., La contrainte est un outil utilisé en psychiatrie lorsque que l’état psychique d’un patient est estimé comme relevant d’un soin urgent, que le malade refuse les soins et que de respecter le refus serait plus délétère pour le patient que d’imposer ce que le médecin juge nécessaire. Derrière cette définition simple se déroule en fait tout un processus amenant ou non à une décision de contraindre. Cette décision s’acte dans une relation médecin-patient particulière où s’évalue la capacité du patient à décider pour lui-même et la mise en compétition de valeurs éthiques non conciliables. Pour explorer les justifications des psychiatres dans la décision de contraindre et d’expliciter les enjeux éthiques qui accompagnent ce processus décisionnel, nous avons mené une étude qualitative à l’aide d’entretiens semi dirigés auprès de psychiatres réalisant régulièrement des mesures de contrainte.

Out-of-hospital cardiac arrest (OHCA) is a potential situation faced by all actors of the prehospital rescue chain. The prehospital emergency physician is the last link in this chain of survival and must decide to continue or stop initial resuscitation. The situation is further complex because the out-of-hospital environment does not provide physicians with the usual benchmarks of the in-hospital environment. Finally, the family or other bystanders are most often present and assist in the resuscitation attempts. OHCA is a pathology that requires invasive, sometimes aggressive medical gestures to restart the cardiac pump. The physician, first confronted with the choice of resuscitation, must then explain the situation to the family, and finally make the announcement of death to the relatives when it occurs. The objective of this study is to explore the physician’s initial choice to act or not to act medically upon arrival at the bedside of a patient with OHCA. Qualitative research involving semi-structured interviews with prehospital physicians was conducted in February 2020. A total of 13 military and civilian physicians were interviewed. Nine interviews were analyzed due to the unprecedented COVID-19 health crisis. Our preliminary results suggest that physicians perform “technical” gestures while waiting for objective information in terms of prognosis, in a personal impulse of hope in life, in an approach of protection toward the family, against a possible “medico-legal risk,” or for “oneself” in the face of the violence of a direct confrontation with death. The doctor legitimizes his or her actions for the principle of beneficence. However, the “technical” gesture does not always appear to be proportionate to the residual stakes and would seem to offer a reprieve to the practitioner to make a verbal announcement of the patient’s death to his or her relatives.; L’arrêt cardiaque en dehors d’un milieu hospitalier (Arrêt cardiaque extrahospitalier, ACEH) est une situation à laquelle est potentiellement confronté tout acteur de la chaîne de secours pré-hospitalière. Le médecin est le dernier maillon de cette chaîne de survie et doit décider de la poursuite ou de l’arrêt de la réanimation initialement entreprise. La situation est d’autant plus complexe que l’environnement extrahospitalier n’offre pas aux praticiens les repères habituels, notamment du milieu intrahospitalier. Enfin, la famille ou d’autres tiers sont le plus souvent présents et assistent aux manœuvres de réanimation. L’ACEH est une pathologie qui impose la réalisation de gestes médicaux invasifs, parfois agressifs dont le but est de relancer la pompe cardiaque. Le médecin, d’abord confronté au choix de la réanimation, doit ensuite expliquer la situation à la famille, et enfin faire l’annonce aux proches du décès quand il survient. L’objectif de ce travail est d’étudier le choix initial du médecin d’intervenir ou de ne pas intervenir médicalement à son arrivée au chevet d’un patient en ACEH. Une recherche qualitative, par entretiens semi-dirigés de médecins pré-hospitaliers a été menée en février 2020. Au total, 13 médecins militaires et civils ont pu être interrogés. Neuf entretiens ont pu être analysés suite à la crise sanitaire inédite COVID-19. Nos résultats préliminaires suggèrent que les médecins réalisent des gestes « techniques » dans l’attente d’un élément objectif informatif en termes de pronostic, dans un élan personnel d’espoir dans la vie, dans une approche de protection vis-à-vis de la famille, contre un éventuel « risque médico-légal » ou pour « soi-même » face à la violence d’une confrontation directe à la mort. Le médecin légitime ses gestes dans un souci de bienfaisance. Toutefois, le geste « technique » n’apparaît pas toujours proportionné à l’enjeu résiduel et semblerait offrir un sursis au praticien pour faire l’annonce verbale de la mort du patient à ses proches.

International audience; Adhesive small bowel obstruction (SBO) remains one of the leading causes of emergency room visits and is still associated with high morbidity and mortality rates. Because the management of adhesive SBO has shifted from immediate surgery to nonoperative treatment in the absence of ischemia, it is crucial to rapidly detect or predict strangulation, which requires emergent surgery. CT is now established as the best imaging technique for the initial assessment of patients suspected of having adhesive SBO. CT helps confirm the diagnosis of mechanical SBO, locate the site of obstruction, establish the cause, and detect complications. This article is a review of the role of imaging in answering specific questions to help predict the management needs of each individual patient. It includes (a) an update on the best CT signs for predicting ischemia and a need for bowel resection; (b) a discussion of the CT features that help differentiate open-loop from closed-loop obstruction and a single adhesive band from matted adhesions and how these differences can influence the management; and (c) a review of the main CT predictors of the success or failure of nonoperative management in adhesive SBO.

BackgroundThe best management of hypertension in frail oldest-old (≥80 years of age) remains unclear and we still lack guidelines that provide specific recommendations. Our study aims to investigate guideline use in general practitioners (GPs) and to examine if guideline use relates to different decisions when managing hypertension in frail oldest-old.Design/settingCross-sectional study among currently active GPs from 29 countries using a case-vignettes survey.MethodsGPs participated in a survey with case-vignettes of frail oldest-olds varying in systolic blood pressure (SBP) levels and cardiovascular disease (CVD). GPs from 26 European countries and from Brazil, Israel and New Zealand were invited. We compared the percentage of GPs reporting using guidelines per country and further stratified on the most frequently mentioned guidelines. To adjust for patient characteristics (SBP, CVD and GPs' sex, years of experience, prevalence of oldest-old and location of their practice), we used a mixed-effects regression model accounting for clustering within countries.ResultsOverall, 2,543 GPs from 29 countries were included. 59.4% of them reported to use guidelines. Higher guideline use was found in female (p = 0.031) and less-experienced GPs (pConclusionMany GPs reported using guidelines to manage hypertension in frail oldest-old patients, however guideline users did not decide differently from non-users concerning hypertension treatment decisions. Instead of focusing on the fact if GPs use guidelines or not, we as a scientific community should put an emphasis on what guidelines suggest in frail and oldest-old patients.

International audience; Cardiogenic shock complicating acute myocardial infarction is challenging, and continues to be associated with high rates of in-hospital and long-term mortality. Coronary revascularization is critical for improving prognosis in CS. Thus, a systematic protocol-driven approach to cardiogenic shock, the development of specialized cardiac care centres, technical advances in interventional cardiology enabling treatment of more complex and severe lesions, the availability of recent antithrombotic therapies and the evolution of new haemodynamic support devices are important considerations in current management of cardiogenic shock complicating acute ischaemic heart disease. Despite these potentially meaningful developments, several substantial gaps in knowledge still exist regarding optimal coronary revascularization of patients with cardiogenic shock. This review will describe current principles in the revascularization of these patients, with a focus on: the time to transfer and revascularize; the choice of vascular access site; the need for complete revascularization or only a culprit lesion strategy; the optimal antithrombotic therapy; the type, place and timing of haemodynamic support; and the medical care system network.; La survenue d’un choc cardiogénique après un infarctus du myocarde aigu reste une situation clinique fréquente et grave dont le pronostic est associé à une morbi-mortalité élevée. Purpose Genomic tests can identify ER-positive HER2-negative localized breast cancer patients who may not benefit from adjuvant chemotherapy. Such tests seem especially interesting in “intermediate” clinico-pathological risk categories. The psychological impact of the decision uncertainty in these women remains largely unexplored. We assessed the clinical and psychological impact of EndoPredict® (EpClin), a clinico-genomic test, in these patients. Methods This multicenter, single arm prospective study (NCT02773004) enrolled patients for which adjuvant chemotherapy was uncertain, based on predefined criteria. The primary endpoint was the proportion of change between initial adjuvant decision and final administration of chemotherapy. Secondary endpoints included post-test (Day 17) and 1-year patient reported outcomes. Results One third of 200 evaluable patients had a high EpClin score (≥3.32867; 10 years cumulative risk of distance failure ≥10%). The overall change rate of chemotherapy decision was 72/200 (35.8%, 95% CI 29.2–42.4). Chemotherapy was withdrawn in 57 cases (28.4% [22.2–34.8]) and added in 15 (7.5% [3.8–11.2]. 6 changes (8%) were based on patients’ decisions. Anxiety and distress levels increased at Day 17 when adding chemotherapy after the test result (p, Highlights • EndoPredict ® (EpClin) allowed a chemotherapy decision modification in 35% of the patients included in the Adendom trial. • Patient-physician concertation is important: 8% of treatment changes are based on patients’ will. • A single-step decision including the test appears preferable to limit anxiety and distress.

International audience; Introduction - Measures for controlling antimicrobial resistance are urgently required. We describe here AntibioGame®, a serious game for improving the training of medical students in antibiotic use in primary care. Objective - We aimed to design a serious game for antibiotics teaching and to evaluate its usability and playability by medical students. Methods - We used various gamification techniques (e.g. use of mascots, avatars, rewards, leader board) and cartoon graphics in the design of AntibioGame®. This game implements clinical case templates built from a list of learning goals defined by a medical team through an analysis of clinical practice guidelines. The game was evaluated by asking medical students to rate their satisfaction and the usability and playability of the game on an electronic form and through group discussions. The electronic form was derived from the MEEGA + scale, a five-point Likert scale including 32 items for assessing both usability and playability. Results - AntibioGame® is a case-based game in which students play the role of a doctor meeting patients in consultation and helping other health professionals to solve their problems, as in real life. The scenarios are realistic and cover situations frequently encountered in primary care. The 57 medical students enrolled found the game attractive, usable, fun, and appropriate for learning. Game quality was considered "good" (score = 60 on the MEEGA + scale). All the students said they would recommend the game, 96 % liked it and 81 % would use it for revision. Conclusion - AntibioGame® is a promising tool for improving knowledge in antibiotic prescription that could easily be included in multifaceted programs for training medical students.

ObjectivesTo investigate whether following a treat-to-target (T2T)-strategy in daily clinical practice leads to more patients with rheumatoid arthritis (RA) meeting the remission target.MethodsRA patients from 10 countries starting/changing conventional synthetic or biological disease-modifying anti-rheumatic drugs were assessed for disease activity every 3 months for 2 years (RA BIODAM (BIOmarkers of joint DAMage) cohort). Per visit was decided whether a patient was treated according to a T2T-strategy with 44-joint disease activity score (DAS44) remission (DAS44 ResultsIn total 4356 visits of 571 patients (mean (SD) age: 56 (13) years, 78% female) were included. Appropriate application of T2T was found in 59% of the visits. T2T (vs no T2T) did not yield a higher likelihood of DAS44 remission 3 months later (OR (95% CI): 1.03 (0.92 to 1.16)), but sustained T2T resulted in an increased likelihood of achieving DAS44 remission (OR: 1.19 (1.03 to 1.39)). Similar results were seen with DAS28-ESR remission. For more stringent definitions (CDAI, SDAI and ACR/EULAR Boolean remission), T2T was consistently positively associated with remission (OR range: 1.16 to 1.29), and sustained T2T had a more pronounced effect on remission (OR range: 1.49 to 1.52).ConclusionIn daily clinical practice, the correct application of a T2T-strategy (especially sustained T2T) in patients with RA leads to higher rates of remission.

International audience; Hormones were largely used for undescended testis (UDT) during the 1900s. However, in the early 2000s the Nordic consensus no longer recommended hormonal treatment and instead advocated surgery as first line management of UDT.1 Based on 3 meta-analyses of randomized trials, the Nordic group found that human chorionic gonadotropin and gonadotropin-releasing hormone (GnRH) provided poor results and were associated with frequent testis reascension to a suprascrotal position. Some studies also raised suspicion about possible side effects such as temporary inflammatory changes in the testes, germ cell apoptosis and reduction in the number of germ cells in adulthood, especially when the treatment was given in children 1 to 3 years old.2 The American Urological Association3 and the Canadian Urological Association4 did not recommend hormonal therapy. Most guidelines subsequently focused on the timing of surgery, recommending that it be performed before age 1 year as the best way to preserve fertility in these children, although there is no guarantee of normal fertility later in life.5 Thus, hormonal treatment has been ignored for more than 15 years and has been overlooked by pediatric urologists.

Contexte : la prise en charge de la bronchiolite en médecine générale est hétérogène et comporte fréquemment des prescriptions médicamenteuses. Pourtant, des recommandations de la HAS, datant de 2000 et réévaluées en 2012, soulignent l’intérêt d’un traitement uniquement symptomatique. Par ailleurs, le domaine de la psychologie cognitive reconnait depuis plusieurs décennies les biais et les heuristiques comme étant des facteurs déterminants dans la prise de décision. Des études récentes ont également souligné l’influence de biais cognitifs dans la prise de décision médicale. Objectif : notre étude vise à étudier le rôle des biais cognitifs dans l’hétérogénéité de la prise en charge de la bronchiolite. Méthode : des entretiens individuels et semi-directifs sont réalisés auprès de médecins généralistes installés en Essonne, dans le cadre d’une étude qualitative selon la méthode de la théorisation ancrée, menée jusqu’à saturation des données. Résultats : 21 biais cognitifs sont identifiés au total, et apparaissent chez neuf des treize médecins interviewés. Ces biais sont responsables de prescriptions médicamenteuses inappropriées, ainsi que de retard au recours hospitalier en cas de bronchiolite sévère. Le manque de connaissance et de formation continue, ainsi que l’ancienneté dans la pratique, constituent des facteurs favorisant l’apparition de biais cognitifs. Quatre praticiens ne présentaient pas de biais cognitifs et possédaient les facteurs communs suivants : âge plutôt jeune, sexe féminin, connaissance sur le sujet de la bronchiolite et pratique régulière de formations continues. Les médecins exprimant les émotions contrefactuelles, que sont le regret, le doute, et la curiosité, semblent être moins sujets aux biais cognitifs. Conclusion : les biais cognitifs ont une influence sur l’hétérogénéité de la prise en charge de la bronchiolite, observée sur un échantillon de treize médecins de l’Essonne. Des méthodes de sensibilisation aux biais cognitifs, ainsi que des stratégies de « debiaising » pourraient être mises en œuvre pour optimiser les prises de décision des médecins dans cette pathologie. Il serait intéressant de pouvoir étudier ces phénomènes dans d’autres champs de prises en charge de la médecine générale.

Introduction: long-term quality of life in children with craniopharyngioma has become the main challenge of our treatment strategies. The aim of this study was to confirm the superiority of hypothalamus sparing surgery (HSS) over extensive resection surgery (ERS) in these children regarding the hypothalamic syndrome and obesity. Methods: we compared an historical cohort (HC) of children with craniopharyngioma who underwent an ERS at our institution between January 1985 and December 2002 to a prospective cohort (PC) who underwent a risk-related treatment strategy combining HSS and proton radiation therapy in the same department between January 2003 and January 2017. We collected, compared and critically analyzed the anthropometric (BMI Z-score), oncologic, surgical and hormonal outcomes in both groups. Results: sixty consecutives patients were included in the HC and 127 consecutives patients in the PC. Follow-up, demographical and preoperative anthropometrical data were comparable in both groups. BMI Z-scores were significantly lower in the PC group than in the HC group at 6 months (p < 0.05), 12 months (p < 0.01) and at last follow-up (p < 0.01). In the HC group, postoperative BMI Z-score were significantly increasing throughout follow-up (p < 0,.001) whereas children in the PC group stopped gaining weight at 6 months after surgery and their BMI Z-score remained stable until last follow-up (p< 0.001). In the PC group, gross total resection (GTR) was possible in 17.2% of the cases and subtotal resection with radiation therapy was performed in 71.1% of the cases. There was no difference in the number of surgical interventions per patient in both groups. Conclusion: HSS provides better long-term outcomes over the hypothalamic syndrome and obesity with comparable surgical outcomes and recurrences for childhood craniopharyngioma.; Introduction : la qualité de vie à long-terme des enfants opérés d’un craniopharyngiome est devenu le défi principal de nos stratégies thérapeutiques. L’objectif de cette étude était de confirmer la supériorité de la chirurgie d’épargne hypothalamique sur la chirurgie d’exérèse extensive chez ces enfants concernant le syndrome hypothalamique et l’obésité. Matériel et méthodes : nous avons comparé une cohorte historique (CH) de patients qui ont bénéficié d’une chirurgie d’exérèse extensive de craniopharyngiome dans notre service entre janvier 1985 et décembre 2002 à une cohorte prospective (CP) de patients qui ont bénéficié d’une stratégie thérapeutique adaptée au risque hypothalamique combinant une chirurgie d’épargne hypothalamique et une protonthérapie dans le même service entre janvier 2003 et janvier 2017. Nous avons collecté, comparé et fait une analyse critique des résultats anthropométriques, oncologiques, chirurgicaux et hormonaux dans les deux groupes. Résultats : soixante patients consécutifs ont été inclus dans la CH et 127 patients consécutifs ont été inclus dans la CP. Le suivi médian, les données démographiques et anthropométriques préopératoires étaient comparables entre les deux groupes. Les IMC Z-scores étaient inférieurs dans la CP comparés à ceux de la CH à 6 mois (p < 0,05), 12 mois (p < 0,01) et au cours du dernier suivi (p < 0,01). Dans la CH, le IMC z-score augmentait de manière significative au cours du suivi (p < 0,001) tandis que les enfants de la CP arrêtaient de gagner du poids après 6 mois post-opératoires et leur IMC z-score restait stable jusqu’au dernier suivi (p < 0,001). Dans la CP, une exérèse complète n’a été possible que dans 17,2 % des cas et une résection incomplète suivie d’une protonthérapie a été réalisée dans 71,1 % des cas. Il n’y avait pas de différence dans le nombre d’intervention chirurgicale par patient entre les deux groupes. Conclusion : la chirurgie d’épargne hypothalamique entraîne de meilleurs résultats à long-terme sur le syndrome hypothalamique et l’obésité que la chirurgie d’exérèse extensive avec des résultats chirurgicaux et un contrôle tumoral local comparables pour le traitement des craniopharyngiomes pédiatriques.

International audience; BACKGROUND: Our previous survey on first-in-human trials (FIHT) of monoclonal antibodies (mAbs) showed that, due to their limited toxicity, the recommended phase II dose (RP2D) was only tentatively defined.METHODS: We identified, by MEDLINE search, articles on single-agent trials of mAbs with an FIHT included in our previous survey. For each mAb, we examined tested dose(s) and dose selection rationale in non-FIHTs (NFIHTs). We also assessed the correlation between doses tested in the registration trials (RTs) of all FDA-approved mAbs and the corresponding FIHT results.RESULTS: In the 37 dose-escalation NFIHTs, the RP2D indication was still poorly defined. In phase II-III NFIHTs (n=103 on 37 mAbs), the FIHT RP2D was the only dose tested for five mAbs. For 16 mAbs, only doses different from the FIHT RP2D or the maximum administered dose (MAD) were tested and the dose selection rationale infrequently indicated. In the 60 RTs on 27 FDA-approved mAbs with available FIHT, the FIHT RP2D was tested only for two mAbs, and RT doses were much lower than the FIHT MAD.CONCLUSIONS: The rationale beyond dose selection in phase II and III trials of mAbs is often unclear in published articles and not based on FIHT data.

Objective: Hy-Result® software is designed to help patients to comply with the home blood pressure measurement (HBPM) protocol and self-interpret their results. We compare in a daily routine care setting, the classification generated by Hy-Result® with the physician’s clinical evaluation. Design and method: the algorithm combines BP readings with patient’s characteristics. According to the ESH guidelines, BP readings and automatically generated text messages are made available to the patient in a report. The primary assessment criterion was whether classification of the BP status generated by the software concurred with the physician’s classification following a consultation (n=195 patients) (gold standard). Results: in the 58 untreated patients, the agreement between classification of the BP status generated by the software and the physician’s classification was 87.9%. In the 137 treated patients, the agreement was 91.9%. The kappa-test applied for all the patients was 0.81[95%CI:0.73–0.89]. After correction of errors identified in the algorithm during the study, agreement was 95.4% (? 0.9[95%CI:0.84–0.97]). For 100% of the patients with comorbidities (n=46), specific text messages were generated indicating that a physician might recommend a target BP lower than 135/85 mmHg. Specific text messages were also generated for 100% of the patients for whom global cardiovascular risks greatly exceeded norms relating to BMI, tobacco and/or alcohol consumption. Remaining discrepancies were more attributable to physician than software. Conclusion: classification by Hy-Result® software is at least as accurate as that of a specialist in current practice. Hy-Result® is the first validated software for self-interpretation of HBPM results taking into account both the recommended thresholds for normal values and patient characteristics.; Objectif : le logiciel Hy-Result® est destiné à améliorer la pratique de l’automesure tensionnelle (AMT) et l’auto-interprétation des résultats. Nous avons comparé en aveugle la classification du statut tensionnel du patient par le logiciel Hy-Result® par rapport à celle du médecin spécialiste de l’HTA au cours d’une consultation médicale. Méthode : après recueil des résultats d’automesure et des antécédents du patient, une courbe ainsi que des messages de prévention cardiovasculaire sont générés automatiquement par le logiciel. L’objectif principal de l’étude est la mesure de la concordance en aveugle entre la classification du statut tensionnel des 195 patients par le logiciel Hy-Result® versus la classification par le médecin en consultation (Gold-Standard). Résultats : chez les 58 patients non traités, la concordance entre la classification du statut tensionnel générée par le logiciel et la classification du médecin était de 87,9%. Chez les 137 patients traités, la concordance était de 91,9%. Le test kappa appliqué à tous les patients était de 0,81 [IC95%:0,73-0,89]. Après correction des erreurs identifiées dans l'algorithme au cours de l'étude, la concordance a augmenté à 95,4% (?=0,9 [IC 95%: 0,84-0,97]). Pour 100 % des patients atteints de comorbidités (n=46), des messages textes ont été générés indiquant qu'un médecin pourrait recommander une TA cible inférieure à 135/85 mmHg. Des messages textes ont également été générés pour 100% des patients pour lesquels les valeurs dépassaient largement les normes relatives à l'IMC, au tabac et/ou à la consommation d'alcool. Les divergences restantes étaient attribuables à des erreurs du médecin plutôt qu’au logiciel. Conclusion : la classification par le logiciel Hy-Result® est au moins aussi pertinente que celle d'un médecin spécialiste de l’HTA en pratique courante. Hy-Result® est le premier logiciel en accès libre, validé pour l'auto-interprétation des résultats d’automesure prenant compte à la fois des seuils cibles selon les recommandations européennes et des caractéristiques du patient.

International audience; The purpose of this study was to identify clinician characteristics associated with higher prescription rates of long-acting injectable (LAI) antipsychotics, as well as the sources that influence medical decision-making regarding the treatment of schizophrenia. We surveyed 202 psychiatrists during six regional French conferences (Bordeaux, Lyon, Marseille, Nice, Paris, and Strasbourg). Data on the characteristics of practice, prescription rates of antipsychotic, and information sources about their clinical decisions were collected. Most psychiatrists used second-generation antipsychotics (SGAs), and preferentially an oral formulation, in the treatment of schizophrenia. LAI SGAs were prescribed to 30.4% of schizophrenic patients. The duration and type of practice did not influence the class or formulation of antipsychotics used. The clinicians following the higher percentage of schizophrenic patients were associated with a higher use of LAI antipsychotics and a lower use of oral SGAs. Personal experience, government regulatory approval, and guidelines for the treatment of schizophrenia were the three main contributing factors guiding clinicians' decision-making regarding the treatment of schizophrenia. The more clinicians follow schizophrenic patients, the more they use LAI antipsychotics. The development of specialized programs with top specialists should lead to better use of LAI antipsychotics in the treatment of schizophrenia.

It analyses the effects of a reminder for completing the content of an order form. It shows that the CDSS impacted ordering practices. It could be seen as a system enabling to structure the information and improve the quality of orders.

International audience; Les incitations à la recherche et à l’évaluation se multiplient en soins palliatifs. Pourtant, faute de formation initiale, le manque d’une certaine culture méthodologique des professionnels est un frein à leur développement. L’introduction en 2009 d’une épreuve de lecture critique d’un article médical (LCA) à l’examen classant national témoigne du fait que savoir lire un article scientifique est désormais reconnu comme l’une des compétences indispensables à transmettre aux étudiants en médecine. Cet article décrit tout d’abord en quoi consiste un enseignement de LCA, son intérêt et les limites de sa mise en oeuvre, pour ensuite ouvrir une réflexion sur l’adaptation de la LCA à l’apprentissage et à l’exercice de la médecine palliative. L’enseignement de la LCA vise la maîtrise des outils de recherche documentaire, la méthode de lecture rapide et les bases méthodologiques nécessaires pour la compréhension des informations scientifiques et l’analyse de leur validité. Cet enseignement est utile et nécessaire, en soins palliatifs comme dans tout autres spécialités médicochirurgicales. Néanmoins, il est fondé sur un paradigme (la médecine fondée sur le niveau de preuve) qui a montré ses limites et ses dérives : l’utilisation des concepts qui lui sont sous-jacents doit donc être réfléchie et adaptée aux contraintes et aux principes fondateurs des soins palliatifs. Cela ne peut s’envisager qu’avec une motivation forte et une ouverture franche des praticiens de soins palliatifs et des méthodologistes, conditions nécessaires à une réelle collaboration.; The incentives for research and evaluation are increasing in palliative care. However, lack of methodological skills among professionals is an obstacle to their development. In 2009, a test for critical appraisal of literature (CAL) was introduced as a part of the national residency examination for medical students. Reading a scientific article is now recognized as one of the skills necessary to convey to future doctors. This paper first describes what CAL is and what it means to teach it. Second, it addresses the question whether or not CAL should be integrated as a part of a training program in palliative medicine and how this is feasible. CAL skills concern information retrieval and basic methodological knowledge necessary for the understanding of scientific information and analysis of its validity. Such skills are useful and necessary in palliative care as they are in all other medical or surgical specialties. However,CAL is based on a paradigm (evidence-based medicine), which has proved to be as powerful as potentially dangerous. Its underlying concepts must be used with caution and adapted to the constraints and founding principles of palliative care. This supposes strong motivation and open-minded collaboration between palliative care practitioners and methodologists.
