Abstract

International audience; OBJECTIVES: To test the hypothesis that behavioral predictors of serious road traffic crashes (RTC) are correlated with unfavorable attitudes towards traffic safety. DESIGN: Prospective and cross-sectional cohort study. SETTING: France PARTICIPANTS: 13,447 of the 19,894 living members of the GAZEL cohort, workers and recent retirees of a French national utility company followed up since 1989. MAIN OUTCOMES MEASURES: Driving behavior and attitudes towards traffic safety in 2001 by questionnaire. Serious RTCs were recorded over the subsequent 3 years using the cohort annual questionnaire. Behavioral predictors of serious RTCs were assessed using generalized linear Poisson regression models with time-dependent covariates. Factor scores extracted from the first four attitudinal factors of principal component analysis were saved and then regressed on behavioral predictors as independent variables. RESULTS: After controlling for potential confounders, the best predictors of serious RTCs were: "exceeding speed limits on rural roads", "risky use of cellular phone", and "sleepy driving". The adjusted rate ratio ranged from 1.47 to 2.16. Predictors of contravention of the highway code (the first two predictors) were found to be strongly associated with negative attitudes towards "enforcement" and "speed limitations" with an adjusted odds ratio ranging from 1.31 to 2.02. CONCLUSION: Our study supports the view that individuals with a high propensity for driving behaviors associated with an increased risk of RTCs were more likely to have negative attitudes towards traffic safety. Changing drivers' negative or distorted opinions of traffic "enforcement" as well as "speed limitations" and "alcohol prohibition on roads" could improve their compliance with road traffic rules.

Abstract

Is dependence on mobile phones or smartphones for professional use a risk for health and safety at work? The mobile phone and smartphone penetration rate in the population has increased rapidly in recent years. Previous studies in adolescents and young adults show that the use of these tools is responsible for health and safety risks. The objective of the present study is to evaluate the problem of dependence on these tools (technodependence) in the workplace and observe, among other things, if the most dependent workers have an increased risk of accidents. This survey was conducted among 478 workers between March and June 2014. Technodependence was measured using the questionnaire MPPUS (Mobile Phone Problem Use Scale). This Australian 27-item scale was used in several studies and adapted in Spanish. A total of 296 (62%) employees used a phone (simple or smart) as part of their work and 121 (25%) used their personal phone for work purposes. Multivariate analyses showed that the level of technodependance is significantly associated with the occurrence of accidents (excluding commuting) (p = 0,031), neck pain (p = 0,002) and stress (p < 0,000). Therefore, it is necessary to implement specific preventive measures in the workplace. It is also important that multidisciplinary occupational health teams explore this issue, both in workstation studies and during medical examinations. A larger-scale study should be conducted to validate or invalidate these results.; L’augmentation du taux d’équipement en téléphones portables et smartphones dans la population a été très rapide ces dernières années. Les données de la littérature chez les adolescents et les jeunes adultes montrent que l’utilisation de ces outils est responsable de risques pour la santé et la sécurité. L’objectif de ce travail est d’évaluer le problème de la dépendance à ces outils (technodépendance) en milieu professionnel et d’observer, entre autres, si les salariés les plus dépendants ont un risque accru d’accidents du travail. Une enquête a été menée auprès de 478 travailleurs d’Ile de France entre mars et juin 2014. La technodépendance a été mesurée à l’aide du questionnaire MPPUS (Mobile Phone Problem Usage Scale). Cette échelle australienne composée de 27 items a été utilisée dans plusieurs études et adaptée en espagnol Au total, 296 (62%) salariés utilisent un téléphone (simple ou intelligent) dans le cadre de leur travail et 121 (25%) se servent de leur téléphone privé à des fins professionnelles. Les analyses multivariées montrent que le degré de technodépendance est significativement associé à la survenue d’accidents du travail (hors trajet) (p = 0,031), de douleurs cervicales (p = 0,002) et de stress (p < 0,000). Il est nécessaire de mettre en place des mesures de prévention spécifiques en milieu professionnel. De même, il est important que les équipes pluridisciplinaires de santé au travail explorent cette problématique, tant dans les études de postes que lors des examens médicaux. Une étude de plus grande ampleur devra être menée afin de confirmer ou infirmer ces résultats.

Abstract

BackgroundThere is no functional pharmacovigilance system in Cambodia to our knowledge. Mobile phone–based tools, such as short message service (SMS) text messages, are increasingly used for surveillance purposes. ObjectiveTo pilot-test the FrontlineSMS mobile phone–based tool for notification of adverse events, using Cambodia’s only International Vaccination Center at the Institut Pasteur du Cambodge as a field site. MethodsPeople receiving vaccinations, aged over 18 years, and who owned a cell phone were recruited in the study following informed consent. The names and mobile phone numbers of the participants interviewed were entered each day into the FrontlineSMS software. Two days after being vaccinated, participants received an automatically generated SMS text message asking whether any adverse events had occurred. Their SMS reply was number-coded and exported from the software daily to an Excel spreadsheet and examined before being saved. If the participant replied with a code for a severe adverse event (8 or 9), they were automatically advised to consult the nearest doctor. ResultsThe active surveillance study was conducted over 72 days in the spring of 2012. Patients agreed to be asked by SMS text message whether unwanted events had occurred after vaccination. Of 1331 persons aged over 18 years referred to the vaccination unit, 184 (13.8%) were asked and agreed to participate. When texted for clinical status 48 hours after vaccination, 52 (28.3%) participants did not reply, 101 (54.9%) sent an immediate SMS reply, and 31 (16.8%) sent an SMS reply after additional prompting. Of the initial 184 participants, 132 (71.7%) replied. These 132 participants received 135 vaccine doses and 109 (82.6%) reported no adverse events, whereas 23 (17.4%) reported adverse events, all benign. ConclusionsNotification using an SMS-based text message system is already used in Cambodia for syndromic surveillance in health centers and reporting by health care workers. Our results show that such tools can also be useful for notification by patients or health users in Cambodia, especially in an urban setting.

Abstract

Background: The rapid increase in mobile telephone use has generated concern about possible health risks of radiofrequency electromagnetic fields from these devices. Methods: A case-control study of 1105 patients with newly diagnosed acoustic neuroma (vestibular schwannoma) and 2145 controls was conducted in 13 countries using a common protocol. Past mobile phone use was assessed by personal interview. In the primary analysis, exposure time was censored at one year before the reference date (date of diagnosis for cases and date of diagnosis of the matched case for controls); analyses censoring exposure at five years before the reference date were also done to allow for a possible longer latent period. Results: The odds ratio (OR) of acoustic neuroma with ever having been a regular mobile phone user was 0.85 (95% confidence interval 0.69-1.04). The OR for >= 10 years after first regular mobile phone use was 0.76 (0.52-1.11). There was no trend of increasing ORs with increasing cumulative call time or cumulative number of calls, with the lowest OR (0.48 (0.30-0.78)) observed in the 9th decile of cumulative call time. In the 10th decile (>= 1640 h) of cumulative call time, the OR was 1.32 (0.88-1.97); there were, however, implausible values of reported use in those with >= 1640 h of accumulated mobile phone use. With censoring at 5 years before the reference date the OR for >= 10 years after first regular mobile phone use was 0.83 (0.58-1.19) and for >= 1640 h of cumulative call time it was 2.79(1.51-5.16). but again with no trend in the lower nine deciles and with the lowest OR in the 9th decile. In general, ORs were not greater in subjects who reported usual phone use on the same side of the head as their tumour than in those who reported it on the opposite side, but it was greater in those in the 10th decile of cumulative hours of use. Conclusions: There was no increase in risk of acoustic neuroma with ever regular use of a mobile phone or for users who began regular use 10 years or more before the reference date. Elevated odds ratios observed at the highest level of cumulative call time could be due to chance, reporting bias or a causal effect. As acoustic neuroma is usually a slowly growing tumour, the interval between introduction of mobile phones and occurrence of the tumour might have been too short to observe an effect, if there is one. Abstract

International audience; OBJECTIVES: The objective of this study was to examine the associations of brain tumours with radio frequency (RF) fields from mobile phones. METHODS: Patients with brain tumour from the Australian, Canadian, French, Israeli and New Zealand components of the Interphone Study, whose tumours were localised by neuroradiologists, were analysed. Controls were matched on age, sex and region and allocated the 'tumour location' of their matched case. Analyses included 553 glioma and 676 meningioma cases and 1762 and 1911 controls, respectively. RF dose was estimated as total cumulative specific energy (TCSE; J/kg) absorbed at the tumour's estimated centre taking into account multiple RF exposure determinants. RESULTS: ORs with ever having been a regular mobile phone user were 0.93 (95% CI 0.73 to 1.18) for glioma and 0.80 (95% CI 0.66 to 0.96) for meningioma. ORs for glioma were below 1 in the first four quintiles of TCSE but above 1 in the highest quintile, 1.35 (95% CI 0.96 to 1.90). The OR increased with increasing TCSE 7+ years before diagnosis (p-trend 0.01; OR 1.91, 95% CI 1.05 to 3.47 in the highest quintile). A complementary analysis in which 44 glioma and 135 meningioma cases in the most exposed area of the brain were compared with gliomas and meningiomas located elsewhere in the brain showed increased ORs for tumours in the most exposed part of the brain in those with 10+ years of mobile phone use (OR 2.80, 95% CI 1.13 to 6.94 for glioma). Patterns for meningioma were similar, but ORs were lower, many below 1.0. CONCLUSIONS: There were suggestions of an increased risk of glioma in long-term mobile phone users with high RF exposure and of similar, but apparently much smaller, increases in meningioma risk. The uncertainty of these results requires that they be replicated before a causal interpretation can be made.

Abstract

AIM: Conventional follow-up of type 1 diabetic patients treated with continuous subcutaneous insulin infusion (CSII) was compared with intensive coaching using the Web and the cellular phone network for retrospective data transmission and short message service (SMS). METHODS: Thirty poorly controlled patients (HbA1c 7.5-10%) were enrolled in a bicenter, open-label, randomized, 12-month, two-period, crossover study. After a 1-month run-in period, 15 patients were randomly assigned to receive weekly medical support through SMS based upon weekly review of glucose values, while 15 patients continued to download self-monitored blood glucose (SMBG) values on a weekly basis without receiving SMS. After 6 months, patients crossed over to the alternate sequence for 6 additional months. Visits at the clinic were maintained every 3 months.
