This study examines the findings from a 6-month follow-up of our previous results in which 30 patients participated. Materials and methods: The patient’s cognition was examined by conducting the tests of 14 questions and letter-writing ability over a period of 19 days, and it was repeated after 6 months. The difference between these two cognitive measures (PQ1 before–PQ2 before), tested previously and later the writing test, was designated DΔ before. The test was repeated after 6 months, and PQ1 after–PQ2 after was designated DΔ after. Results: Several markedly strong relationships between dysgraphia and other measures of cognitive performance in AD patients were observed. The most aged patients (over 86 years), despite less frequency, maintain the cognitive capacity manifested in the graphic expressions. A document, written by an AD patient presents an honest expression of the patient’s intention if that document is legible, clear, and comprehensive. Conclusion: The identification of impairment/deficits in writing and cognition during different phases of AD may facilitate the understanding of disease progression and identify the occasions during which the patient may be considered sufficiently lucid to make decisions. The degree of writing disability and personal, spatial, and temporal orientation was compared in these two groups. Design and methods: Fourteen simple questions, designed in a form that could be utilized by any general practitioner in order to document the level of cognitive functioning of each patient, were presented to 30 AD patients and 26 DLB patients. The initial cognition test was designated PQ1. The patients were examined on tests of letter-writing ability. Directly after the letter-writing, the list of 14 questions presented in PQ1 was presented again in a repeated procedure that was designated PQ2. The difference between these two measures (PQ1 – PQ2) was designated DΔ. This test of letter-writing ability and cognitive performance was administered over 19 days.Results: Several markedly strong relationships between dysgraphia and several measures of cognitive performance in AD patients and DLB patients were observed, but the deterioration of performance from PQ1 to PQ2 over all test days were markedly significant in AD patients and not significant in DLB patients. It is possible that in graphic expression even by patients diagnosed with moderate to relatively severe AD and DLB there remains some residual capacity for understanding and intention that may be expressed. Furthermore, the deterioration in performance and the differences noted in AD and DLB patients may be due to the different speed at which the process of the protein degradation occurs for functional modification of synapses.Conclusion: Our method can be used as part of neuropsychological tests to differentiate the diagnosis between AD and DLB. 5. Biomedical radar system for real-time contactless fall detection and indoor localization

Abstract

Fall incidents represent a major public health problem among elderly people. This resulted in a significant increase of the number of investigated systems aiming at detecting falls promptly. In this respect, in this work, a biomedical radar system is proposed for remote real-time fall detection and indoor localization. The system, consisting of a sensor and a base station, combines radar and wireless communication techniques, and uses a data processing technique to distinguish between fall events and normal movements. The classification, based on a Least-Square Support Vector Machine (LS -SVM), combined with the sliding window principle allows to perform fall detection in real-time. Moreover, it is capable to localize the subjects when the fall incident has been detected. The in-vivo validation showed a high success rate in detecting fall events, with a maximum delay of 340 ms. Moreover, a maximum mean absolute errors (MAE) of 3.8 cm and a maximum root-mean-square error (RMSE) of 7.5 cm were reported in measuring the subject’s absolute distance.

Abstract

La guerra in Ucraina non ha zittito le armi in Nord Africa e nel Medio Oriente, un'area segnata da conflitti irrisolti, guerre per procura e rivolte che si estendono fino ai confini dell'Asia centrale. Il Mediterraneo è una polveriera pronta a esplodere. Le recenti proteste in Iran, la crisi che sta vivendo l'Afghanistan dopo il ritiro delle truppe americane, le ambizioni egemoniche turche, l'instabilità libica, il revanscismo jihadista in Nord Africa e la futura traiettoria di paesi "in bilico" come l'Algeria, l'Arabia Saudita e la Siria rappresentano alcune delle maggiori incognite per il futuro. Gli effetti di queste "bombe a orologeria" potrebbero riverberarsi sugli Stati vicini e sull'intero sistema internazionale, con esiti che potrebbero essere devastanti.

Abstract

Foreward: Nurses' attitudes towards COVID-19 vaccination is a relevant issue, for the protection of the vulnerable people they care for, and the key role they play in promoting health behaviors that encourage trust and adherence to vaccination among population. This study aimed to validate the Italian version of the Vaccination Attitudes Examination (VAX) scale and to describe nurses' attitudes towards COVID-19 vaccination.Design: A cross-sectional study was carried out from May to June 2021. Descriptive statistics, Explorative and Confirmatory Factor Analyses have been performed.Methods: An online survey was carried out in Italy. The VAX scale referring to the COVID-19 vaccine was used.Findings: 430 nurses participated in the study, mainly female (73.2). Mean age was 40.2 years. VAX scale revealed an optimal reliability; Exploratory Factor Analysis and Confirmatory Analysis supported a 4-factors model. VAX scale mean scores showed low mistrust about vaccine's benefit (2.03±1.07), concerns about commercial profiteering (2.33±1.39) and preference for natural immunity (2.90±1.37). More worries concerning unexpected future effects were found (4.46±1.36). Gender, taking care of a frail person in family, having children or working in a COVID-19 setting are no significantly related to vaccination attitude. Participants from northern Italy expressed greater confidence in vaccine's benefits, the younger had significant lower scores about commercial profiteering.Conclusions: The Italian version of the VAX scale resulted a reliable tool to assess the nurses' attitudes towards COVID-19 vaccination. An overall positive nurses' attitude towards the COVID-19 vaccination was highlighted. The concern about unforeseen future effects suggested the need to increase the information on this issue.Clinical relevance: The results provided a valid and reliable tool to measure vaccination attitudes in the Italian context. This study could strengthen the health policies with educational interventions of healthcare workers through specific vaccination pathways. The healthcare professionals' vaccination attitudes play the key role also in promoting vaccination uptake in the population.

Abstract

Exploiting redox activity in supramolecular frameworks such as coordination polymers, metal-organic frameworks and related nanostructures is of paramount importance both at the molecular level and for their technological applications, since the modulation of their redox states is an emerging strategy to enhance their physical properties. In the plethora of organic linkers, quinone derivatives are excellent redox-active ligands, widely used for various applications such as electrode materials, flow batteries, pseudocapacitors, etc. Benzoquinones undergo a one-electron reversible reduction to form a semiquinone radical species that can be further reduced to form hydroquinone. Remarkably, the quinoid ring can be functionalized with various functional groups making these systems excellent linkers to construct supramolecular frameworks as well as challenging platforms to tune the redox potential and therefore the stability of radical anions and electrochemical performances of the obtained materials. The recent advances on benzoquinone-based 2D CPs/MOFs and related nanostructures are reported, highlighting the extreme versatility of this class of redox-active linkers in tailoring the physical properties of the obtained materials. Abstract

The continuous wavelet transform (CWT) has been used in radar-based vital signs detection to identify and to remove the motion artifacts from the received radar signals. Since the CWT algorithm is computationally heavy, the processing of this algorithm typically results in long processing time and complex hardware implementation. The algorithm in its standard form typically uses software processing tools and is unable to support high-performance data processing. The aim of this research is to design an optimized CWT algorithm architecture to implement it on Field Programmable Gate Array (FPGA) in order to identify the unwanted movement introduced in the retrieved vital signs signals. The optimization approaches in the new implementation structure are based on utilizing the frequency domain processing, optimizing the required number of operations and implementing parallel processing of independent operations. Our design achieves significant processing speed and logic utilization optimization. It is found that processing the algorithm using our proposed hardware architecture is 48 times faster than processing it using MATLAB. It also achieves an improvement of 58% in speed performance compared to alternative solutions reported in literature. Moreover, efficient resources utilization is achieved and reported. This advanced performance of the proposed design is due to consciously implementing comprehensive approaches of multiple optimization techniques that results in multidimensional improvements. As a result, our achieved design is suitable for utilization in high-performance data processing applications.

Abstract

Importance: High-quality evidence on how to improve palliative care in nursing homes is lacking. Objective: To investigate the effect of the Palliative Care for Older People (PACE) Steps to Success Program on resident and staff outcomes. Design, Setting, and Participants: A cluster-randomized clinical trial (2015-2017) in 78 nursing homes in 7 countries comparing PACE Steps to Success Program (intervention) with usual care (control). Randomization was stratified by country and median number of beds in each country in a 1:1 ratio. Interventions: The PACE Steps to Success Program is a multicomponent intervention to integrate basic nonspecialist palliative care in nursing homes. Using a train-the-trainer approach, an external trainer supports staff in nursing homes to introduce a palliative care approach over the course of 1 year following a 6-steps program. The steps are (1) advance care planning with residents and family, (2) assessment, care planning, and review of needs and problems, (3) coordination of care via monthly multidisciplinary review meetings, (4) delivery of high-quality care focusing on pain and depression, (5) care in the last days of life, and (6) care after death. Main Outcomes and Measures: The primary resident outcome was comfort in the last week of life measured after death by staff using the End-of-Life in Dementia Scale Comfort Assessment While Dying (EOLD-CAD; range, 14-42). The primary staff outcome was knowledge of palliative care reported by staff using the Palliative Care Survey (PCS; range, 0-1). Results: Concerning deceased residents, we collected 551 of 610 questionnaires from staff at baseline and 984 of 1178 postintervention in 37 intervention and 36 control homes. Mean (SD) age at time of death ranged between 85.22 (9.13) and 85.91 (8.57) years, and between 60.6% (160/264) and 70.6% (190/269) of residents were women across the different groups. Residents' comfort in the last week of life did not differ between intervention and control groups (baseline-adjusted mean difference, -0.55; 95% CI, -1.71 to 0.61; P =.35). Concerning staff, we collected 2680 of 3638 questionnaires at baseline and 2437 of 3510 postintervention in 37 intervention and 38 control homes. Mean (SD) age of staff ranged between 42.3 (12.1) and 44.1 (11.7) years, and between 87.2% (1092/1253) and 89% (1224/1375) of staff were women across the different groups. Abstract

Background: By 2030, 30% of the European population will be aged 60 or over and those aged 80 and above will be the fastest growing cohort. An increasing number of people will die at an advanced age with multiple chronic diseases. In Europe at present, between 12 and 38% of the oldest people die in a long-term care facility. The lack of nationally representative empirical data, either demographic or clinical, about people who die in long-term care facilities makes appropriate policy responses more difficult. Additionally, there is a lack of comparable cross-country data; the opportunity to compare and contrast data internationally would allow for a better understanding of both common issues and country-specific challenges and could help generate hypotheses about different options regarding policy, health care organization and provision. The objectives of this study are to describe the demographic, facility stay and clinical characteristics of residents dying in long-term care facilities and the differences between countries.Methods: Epidemiological study (2015) in a proportionally stratified random sample of 322 facilities in Belgium, Finland, Italy, the Netherlands, Poland and England. The final sample included 1384 deceased residents. The sampled facilities received a letter introducing the project and asking for voluntary participation. Facility manager, nursing staff member and treating physician completed structured questionnaires for all deaths in the preceding 3 months.Results: Of 1384 residents the average age at death ranged from 81 (Poland) to 87 (Belgium, England) (p p Conclusions: The population dying in long-term care facilities is complex, displaying multiple diseases with cognitive and functional impairment and high levels of dementia. We recommend future policy should include integration of high-quality palliative and dementia care.

Abstract

OBJECTIVES This study sought to describe worldwide variations in the use of myocardial perfusion imaging hardware, software, and imaging protocols and their impact on radiation effective dose (ED). BACKGROUND Concerns about long-term effects of ionizing radiation have prompted efforts to identify strategies for dose optimization in myocardial perfusion scintigraphy. Studies have increasingly shown opportunities for dose reduction using newer technologies and optimized protocols. METHODS Data were submitted voluntarily to the INCAPS (International Atomic Energy Agency Nuclear Cardiology Protocols Study) registry, a multinational, cross-sectional study comprising 7,911 imaging studies from 308 labs in 65 countries. The study compared regional use of camera technologies, advanced post-processing software, and protocol characteristics and analyzed the influence of each factor on ED. RESULTS Cadmium-zinc-telluride and positron emission tomography (PET) cameras were used in 10% (regional range 0% to 26%) and 6% (regional range 0% to 17%) of studies worldwide. Attenuation correction was used in 26% of cases (range 10% to 57%), and advanced post-processing software was used in 38% of cases (range 26% to 64%). Stress-first single-photon emission computed tomography (SPECT) imaging comprised nearly 20% of cases from all world regions, except North America, where it was used in just 7% of cases. Background: ‘PACE Steps to Success’ is a multicomponent training program aiming to integrate generalist and non-disease-specific palliative care in nursing homes. This program did not improve residents’ comfort in the last week of life, but it appeared to improve quality of care and dying in their last month of life. Because this program included only three dementia-specific elements, its effects might differ depending on the presence or stage of dementia. We aimed to investigate whether the program effects differ between residents with advanced, non-advanced, and no dementia. Methods: Pre-planned subgroup analysis of the PACE cluster-randomized controlled trial in 78 nursing homes in seven European countries. Participants included residents who died in the previous 4 months. The nursing home staff or general practitioner assessed the presence of dementia; severity was determined using two highly-discriminatory staff-reported instruments. Using after-death questionnaires, staff assessed comfort in the last week of life (Comfort Assessment in Dying–End-of-Life in Dementia-scale; primary outcome) and quality of care and dying in the last month of life (Quality of Dying in Long-Term Care scale; secondary outcome). Results: At baseline, we included 177 residents with advanced dementia, 126 with non-advanced dementia and 156 without dementia. Post-intervention, respectively in the control and the intervention group, we included 136 and 104 residents with advanced dementia, 167 and 110 with non-advanced dementia and 157 and 137 without dementia. Using after-death questionnaires, staff assessed comfort in the last week of life (Comfort Assessment in Dying–End-of-Life in Dementia-scale; primary outcome) and quality of care and dying in the last month of life (Quality of Dying in Long-Term Care scale; secondary outcome). Results: At baseline, we included 177 residents with advanced dementia, 126 with non-advanced dementia and 156 without dementia. Post-intervention, respectively in the control and the intervention group, we included 136 and 104 residents with advanced dementia, 167 and 110 with non-advanced dementia and 157 and 137 without dementia.
