Introduction and importance: Secondary endolymphatic hydrops (SEH) is a pathologic condition of the inner ear that usually manifests as episodic vertigo and fluctuating hearing loss, overlapping with other temporal bone pathologies and inner ear diseases. Methods: We searched Pubmed and the Cochrane database for English-language studies published through July 2024. Results: Fifty-four relevant studies and reviews were included in this review on secondary endolymphatic hydrops. This review presents a range of the underlying pathologies in endolymphatic hydrops, along with their corresponding radiological findings, while discussing the associated pathophysiological mechanisms. Secondary endolymphatic hydrops may result from cerebellopontine angle tumors, longstanding inner ear conditions, inner ear malformations, intracranial hypotension and recent investigations have highlighted the role of trauma and inflammation as key factors in SEH development. Discussion: Despite the diverse etiologies of SEH, the findings suggest that many of these conditions share a common final pathway in disrupting endolymphatic fluid balance. This review provides a better understanding of the pathophysiology and etiologies of this intricate disease process, thereby facilitating the diagnosis and treatment of the affected patients. Background: Meniere disease (MD) is characterized by recurrent episodes of vertigo, accompanied by tinnitus and hearing loss. Although MD has attracted increasing research interest, the intellectual structure of MD research is not clear when combined with scientific indicators. Objectives: This study aims to examine the structure of MD research through scientometric indicators and social network analysis (SNA). Materials & Methods: Documents related to MD were searched in the topic search (TS) format indexed in the Scopus database from 1970 until 2023. Then, co-word, co-authorship analysis and SNA were employed to create a platform for visualizing the MD research network. Results: The annual growth rate of MD documents is 2.92% and the average citations per doc is 16.97. The trend of the publication year was relatively upward. Otolaryngologica journal obtained the first rank in MD publications with more productivity (879 docs). The United States was the most influential country in producing and collaborating documents. Paparella MM, Nakashima T and Kitahara M were ranked 1 to 3 with 86, 83 and 81 documents, respectively. Nevertheless, Strupp M, Nakashima T and Naganawa S. received 4064, 3906 and 3870 citations in co-authorship analysis, respectively. Applying co-word analysis led to the formation of 6 clusters and “related comorbidities on the severity of MD” was well developed, had the highest occurrence of keywords, and held the most significant position in impact. Conclusion: Tracing the research landscape in MD can help researchers identify more specific research focus within this field and provide a reference for the study of MD research in otolaryngology and head and neck surgery.

BackgroundMénière's disease (MD) is a disorder of the inner ear that causes episodic bouts of severe dizziness, roaring tinnitus, and fluctuating hearing loss. To date, no targeted therapy exists. As such, we have undertaken a large whole genome sequencing study on carefully phenotyped unilateral MD patients with the goal of gene/pathway discovery and a move towards targeted intervention. This study was a retrospective review of patients with a history of Ménière's disease. Genomic DNA, acquired from saliva samples, was purified and subjected to whole genome sequencing.ResultsStringent variant calling, performed on 511 samples passing quality checks, followed by gene-based filtering by recurrence and proximity in molecular interaction networks, led to 481 high priority MD genes. These high priority genes, including MPHOSPH8, MYO18A, TRIOBP, OTOGL, TNC, and MYO6, were previously implicated in hearing loss, balance, and cochlear function, and were significantly enriched in common variant studies of hearing loss. Validation in an independent MD cohort confirmed 82 recurrent genes. Pathway analysis pointed to cell-cell adhesion, extracellular matrix, and cellular energy maintenance as key mediators of MD. Furthermore, the MD-prioritized genes were highly expressed in human inner ear hair cells and dark/vestibular cells, and were differentially expressed in a mouse model of hearing loss.ConclusionBy enabling the development of model systems that may lead to targeted therapies and MD screening panels, the genes and variants identified in this study will inform diagnosis and treatment of MD.

Introduction: The most common histopathological finding in Ménière's disease (MD) is endolymphatic hydrops (EH), which involves the dilation of the membranous labyrinth. The direct relationship between EH and MD is debated, although EH plays a crucial role in auditory and vestibular functional tests. MRI sequences such as 3D-FLAIR and 3D-real-IR are used to study EH, with the latter being more effective. This study aimed to examine whether the severity of EH detected by MRI is always more pronounced in the cochlea than in the vestibule, indicating a cochleocentric progression of the condition. Methods: A retrospective longitudinal study was conducted at a tertiary care medical center from 2019 to 2023, involving patients diagnosed with unilateral Ménière's disease. All patients underwent MRI hydrops assessments (3D-REAL-IR sequences) using 3 Tesla magnets and gadobutrol contrast agent. EH was graded qualitatively and quantitatively for both ears using scales for cochlear endolymphatic hydrops (cEH) and vestibular endolymphatic hydrops (vEH). Volumetric measurements of the vestibule and endolymph were performed, and the vestibular endolymphatic ratio (vELR) was calculated. The degree of perilymphatic enhancement (PE) and endolymphatic herniation was also assessed. Patient data, including demographics, disease features, comorbidities, hearing loss, and vestibular function, were collected from medical records. Statistical analysis involved various tests to compare groups and evaluate correlations, using a significance level of p < 0.05. The study aimed to classify the patients into cochleocentric (CC) or non-cochleocentric (NCC) groups based on the difference in the severity of EH in both compartments. Results: We included 137 patients, of whom 55 (40.15%) were classified as CC, and the remaining 82 (59.85%) were classified as NCC. The degree of vestibular EH (vEH) was more severe in the NCC group (p < 0.001), while cochlear EH (cEH) showed a moderate correlation with vEH. The mean vestibular endolymphatic ratio (vELR) was higher in the NCC group (80.5% ± 38%) compared to the CC group (55% ± 49.5%) (p < 0.0001). Vestibular herniation was more common in the NCC group, while vestibular perilymphatic enhancement was more prevalent in the CC group. Cardiovascular risk was associated with the CC group, while the NCC group reported more vestibular symptoms. Delayed Ménière's disease was linked to the CC group. The hearing loss and vestibular function tests did not show significant differences between the groups. Discussion: In conclusion, our study found that endolymphatic hydrops (EH) was more severe in the vestibule than in the cochlea in nearly 60% of the cases, with a clinical correlation to the initial symptoms. However, no significant differences were observed in the auditory or vestibular function tests during the follow-up.V Previous studies have indicated that vestibular EH occurs early in Ménière's disease (MD) and subsequently progresses to the cochlea, a finding that challenges the traditional cochleocentric progression theory supported by experimental and clinical otopathology. MRI techniques have enhanced the detection of EH, revealing that the relative amount of endolymph is slightly higher in the vestibule than in the cochlea, thereby supporting the study's findings. We considered the important technical limitations in the MRI visualization of EH and suggested that advanced imaging techniques and volumetric quantification could enhance the classification of cochleocentric and non-cochleocentric groups. The clinical findings revealed that cardiovascular risk factors and delayed MD phenotypes were more common in the cochleocentric group, while the non-cochleocentric group exhibited poorer vestibular MRI results and a higher incidence of endolymph herniation into the semicircular canals. Migraine (MI) is the most common neurological disease, affecting with 20% of the world population. A subset of 25% of MI patients showcase concurrent vestibular symptoms, which may classify as vestibular migraine (VM). Meniere's disease (MD) is a complex inner ear disorder defined by episodes of vertigo associated with tinnitus and sensorineural hearing loss with a significant autoimmune/autoinflammatory contribution, which symptoms overlap with VM. Blood samples from 18 patients with MI (5), VM (5) and MD (8) and 6 controls were collected and compared in a case–control study. Droplet‐isolated nuclei from mononuclear cells used to generate scRNAseq and scATACseq data sets from MI, VM and MD. MI and VM have no differences in their immune transcriptome; therefore, they were considered as a single cluster for further analyses. Natural Killer (NK) cells transcriptomic data support a polarisation triggered by Type 1 innate immune cells via the release of interleukin (IL)‐12, IL‐15 and IL‐18. According to the monocyte scRNAseq data, there were two MD clusters, one inactive and one driven by monocytes. The unique pathways of the MI + VM cluster were cellular responses to metal ions, whereas MD monocyte‐driven cluster pathways showed responses to biotic stimuli. MI and MD have different immune responses. Introduction: 3 T-MRI of the inner ear has been used to identify the endolymphatic hydrops (EH) phenomenon, and less frequently 1.5 T-MRI. The aim of this study was to assess whether there was agreement between findings of EH at 1.5 T MRI and those obtained at 3.0 T MRI in patients clinically diagnosed with definite Meniere disease (MD). Methods: Cross-sectional, blinded study was conducted in a tertiary neurotology ambulatory practice. Thirty patients with clinical diagnosis of unilateral definite MD was included. Two MRI exams (1.5 T and 3.0 T) were performed for each patient and were evaluated by two examiners (E1, E2) who were blinded to the symptomatic ear. An analysis of intra-and inter-examiner agreement was performed. It was determined whether there was an association between MRI findings and disease duration, symptom severity, and MD clinical stage. Results: E1 found EH at 3 T-MRI in 26 (86.66%) patients and at 1.5 T-MRI in 25 (83.33%). E2 found EH in 25 (83.33%) patients in 3 T-MRI and in 22 (73.33%) at 1.5 T-MRI. The agreement between the examiners' assessments in relation to the EH was high (0.844) for the 3 T MRI and substantial for the 1.5 T, both statistically significant. There was no statistically significant relationship between EH imaging findings and clinical disease severity and course. Discussion: 1.5 T and 3.0 T MRI images agreed regarding the findings of absence or presence of cochlear hydrops (CH) and vestibular hydrops (VH). Objective: To investigate the impact of sound exposure, with the resultant windows vibration on perilymphatic concentrations following intratympanic (IT) dexamethasone and gentamicin in an animal model. Study Design: Animal model blinded study. Setting: Animal facility of a tertiary medical center. Methods: Bilateral IT dexamethasone or gentamicin was applied to 15 tested rats. Following injections, each rat was exposed for 3 minutes to free field 30 dB sound pressure level (SPL), 512 vHz noise, with 1 external auditory canal plugged (contralateral control). Following noise exposure, perilymph was obtained from both ears. Drug concentrations were measured using ultrahigh‐performance liquid chromatography‐mass spectrometer. Results: For dexamethasone, the average (±SD) perilymphatic steroidal concentration was 0.417 µg/mL (±0.549) in the control ears versus 0.487 µg/mL (±0.636) in the sound‐exposed ears (P =.008). The average (±SD) gentamicin perilymphatic concentration was 8.628 µg/mL (±2.549) in the sound‐exposed ears, compared to 4.930 µg/mL (±0.668) in the contralateral control (nonsound exposed) ears. Sound exposure promoted steroidal and gentamicin diffusion to the inner ear by an averaged (±SD) factor of 1.431 and 1.730 (±0.291 and 0.339), respectively. Conclusion: Low‐intensity noise (30 dB SPL) was found to enhance dexamethasone phosphate and gentamicin diffusion to the inner ear (by an averaged factor of ∼1.4 and 1.7, respectively) in a murine model. HYPOTHESIS: Analysis of human temporal bone specimens of patients with Menières disease (MD) may demonstrate altered expression of gene products related to barrier formation and ionic homeostasis within cochlear structures compared with control specimens. BACKGROUND: MD represents a challenging otologic disorder for investigation. Despite attempts to define the pathogenesis of MD, there remain many gaps in our understanding, including differences in protein expression within the inner ear. Understanding these changes may facilitate the identification of more targeted therapies for MD. METHODS: Human temporal bones from patients with MD (n = 8) and age-matched control patients (n = 8) were processed with immunohistochemistry stains to detect known protein expression related to ionic homeostasis and barrier function in the cochlea, including CLDN11, CLU, KCNJ10, and SLC12A2. Immunofluorescence intensity analysis was performed to quantify protein expression in the stria vascularis, organ of Corti, and spiral ganglion neuron (SGN). RESULTS: Expression of KCNJ10 was significantly reduced in all cochlear regions, including the stria vascularis (9.23 vs 17.52, p = 0.011), OC (14.93 vs 29.16, p = 0.014), and SGN (7.69 vs 18.85, p = 0.0048) in human temporal bone specimens from patients with MD compared with control, respectively. CLDN11 (7.40 vs 10.88, p = 0.049) and CLU (7.80 vs 17.51, p = 0.0051) expression was significantly reduced in the SGN. CONCLUSION: The results of this study support that there may be differences in the expression of proteins related to ionic homeostasis and barrier function within the cochlea, potentially supporting the role of targeted therapies to treat MD.

While pathognomonic for MD, endolymphatic hydrops has also been identified in patients with vestibular migraine. Treatment with the integrative neurosensory rehabilitation approach (diet and lifestyle changes, magnesium and riboflavin supplementation, and when needed, prophylactic medication) to treat the underlying migraine process has been highly effective in patients with MD.SummaryMD can be understood as a manifestation of migraine such that patients with MD can be effectively treated with migraine therapies.

ObjectivesThe objective of this study is to understand whether the hearing loss laterality in Menière's disease (MD) correlates with migraine symptoms laterality such as headache, neck stiffness, and otalgia.MethodsWe performed a retrospective review of prospectively obtained data on patients presenting between September 2015 and October 2021 with definite or probable MD. A custom-designed, comprehensive questionnaire was used to identify patients' migraine-related symptoms. The clinical and audiometric data were used to diagnose patients with definite or probable MD using criteria set by the American Academy of Otolaryngology-Head and Neck Surgery.ResultsIn total, 113 patients with definite or probable MD were included in the study. The mean age of the patients was 60 ± 15 years with no gender predominance (49.6% male and 50.4% female). A total of 57 (50%) patients were presented with headaches. Among the migraine headache cohort, headache and otalgia were on the same side as the MD ear affected by hearing loss. In addition, in patients who present with otalgia as the primary feature of headache, otalgia was more likely to be on the same side as the ear affected by the hearing loss.ConclusionsThe high prevalence of migraine symptoms on the same side of the ear affected by MD among this cohort could suggest a shared pathophysiology in both MD and migraine, possibly involving migraine-related changes in both the cochlea and vestibule.

Abstract Background Vertigo is a common presenting symptom in clinical practice, predominantly of vestibular etiology, and often medicated with betahistine, despite the lack of knowledge concerning its mechanism of action. This paper’s objective was to review the scientific evidence regarding the efficacy of betahistine on peripheral vertigo. Methods A systematic search of articles written in English, published within the past 10 years, was conducted in April 2024, on online evidence-based medicine databases, using the MeSH terms “betahistine,” “ménière disease,” “acute vestibular neuritis,” “paroxysmal vertigo,” “acute peripheral vestibulopathy,” and “labyrinthitis.” The Strength of Recommendation Taxonomy of the American Academy of Family Physicians was used to evaluate the level of evidence and strength of recommendation. Results This review included 12 articles for evidence analysis. Concerning Menière’s disease, despite contradictory results, three of the articles analyzed showed a positive effect of betahistine, which is in line with the recommendations of the European Academy of Otology and Neurotology and NICE guidelines, while the American Academy of Otorhinolaryngology guidelines suggest offering betahistine as maintenance therapy. For benign paroxysmal positional vertigo, the established treatment is repositioning maneuvers, and three of the five studies analyzed proved the addition of betahistine to be beneficial. In regard to peripheral vertigo from other causes, two out of three articles revealed a positive recommendation for the use of betahistine. Conclusion There is a potential benefit to the utilization of betahistine for various etiologies of peripheral vertigo. However, additional studies are required to establish more robust evidence.

Its diagnosis relies on clinical criteria that are subjective and pure-tone audiometry results that are not specific. Its pathological substrate is endolymphatic hydrops. Its imaging was recently made possible by the late-enhanced 3D FLAIR sequence. This technique was primarily tested on 3 T. Our objective was to prove its feasibility using a 1.5 T magnet. Methods We conducted a prospective study including 30 patients who fulfilled the Bárány society criteria for Ménière’s disease. We performed the late-enhanced 3D FLAIR sequence on all patients. We used it to look for and grade endolymphatic hydrops in the utricle, the saccule and the cochlear canal using the Kahn method. Results We found endolymphatic hydrops in all of the 30 patients who fulfilled the diagnostic criteria for Ménière’s disease. We had no false positives and only one false negative with a patient presenting with bilateral disease clinically but having endolymphatic hydrops only on one side. Thus, our correspondence rate between clinical and imaging findings was 97%. Conclusions It is possible to diagnose endolymphatic hydrops with the late-enhanced 3D FLAIR sequence using a 1.5 T MRI machine. Since Ménière’s disease diagnosis is sometimes tricky, imaging endolymphatic hydrops can aid in the diagnosis when the clinical picture is incomplete. It also helps guide invasive treatment plans. Feasibility at 1.5 T ensures broader access to the late-enhanced 3D FLAIR sequence. Beyond the scope of Ménière’s disease, this sequence offers the possibility to better understand pressure-related inner ear diseases.

HYPOTHESIS: Na + , K + -ATPase (Na/K-ATPase) α1 subunit expression in the saccule of patients diagnosed with otologic disease is different compared with normal controls. BACKGROUND: We have recently characterized changes in the expression of Na/K-ATPase α1 subunit in the normal and pathological cochlea; however, no studies have determined the distribution Na/K-ATPase α1 subunit in the human saccule. The present study uses archival temporal bones to study the expression Na/K-ATPase α1 subunit in the human saccule. METHODS: Archival celloidin formalin fixed 20-micron thick sections of the vestibule from patients diagnosed with Menières disease (n = 5), otosclerosis (n = 5), sensorineural hearing loss, and normal hearing and balance (n = 5) were analyzed. Sections containing the saccular macula were immunoreacted with mouse monoclonal antibodies against Na/K-ATPase α1 subunit. Micrographs were acquired using a high-resolution digital camera coupled to a light inverted microscope. RESULTS: In the normal human saccule vestibular sensory epithelium, Na/K-ATPase α1 immunoreactivity (IR) was present in nerve fibers and calyces that surround type I vestibular hair cells and nerve terminals. The transition epithelium cells were also Na/K-ATPase α1 immunoreactive. Comparison between normal and pathological specimens showed that there was a significant reduction of Na/K-ATPase α1 IR in the saccule vestibular sensory epithelium from patients with Menières disease, otosclerosis, and sensorineural hearing loss. CONCLUSIONS: The decrease of Na/K-ATPase-IR α1 in the saccule vestibular sensory epithelium from patients with otopathologies suggests its critical role in inner ear homeostasis and pathology.

Introduction3 T-MRI of the inner ear has been used to identify the endolymphatic hydrops (EH) phenomenon, and less frequently 1.5 T-MRI. The aim of this study was to assess whether there was agreement between findings of EH at 1.5 T MRI and those obtained at 3.0 T MRI in patients clinically diagnosed with definite Meniere disease (MD).MethodsCross-sectional, blinded study was conducted in a tertiary neurotology ambulatory practice. Thirty patients with clinical diagnosis of unilateral definite MD was included. Two MRI exams (1.5 T and 3.0 T) were performed for each patient and were evaluated by two examiners (E1, E2) who were blinded to the symptomatic ear. An analysis of intra-and inter-examiner agreement was performed. It was determined whether there was an association between MRI findings and disease duration, symptom severity, and MD clinical stage.ResultsE1 found EH at 3 T-MRI in 26 (86.66%) patients and at 1.5 T-MRI in 25 (83.33%). E2 found EH in 25 (83.33%) patients in 3 T-MRI and in 22 (73.33%) at 1.5 T-MRI. The agreement between the examiners’ assessments in relation to the EH was high (0.844) for the 3 T MRI and substantial for the 1.5 T, both statistically significant. There was no statistically significant relationship between EH imaging findings and clinical disease severity and course.Discussion1.5 T and 3.0 T MRI images agreed regarding the findings of absence or presence of cochlear hydrops (CH) and vestibular hydrops (VH). The degrees of CH and VH found at 3.0 T MRI in symptomatic ear were not associated with clinical aspects and the stage of disease.
