La leucemia linfoblastica acuta del sistema nervoso centrale (LLA-SNC) rappresenta un importante problema a livello clinico. La profilassi è neurotossica ed un terzo delle recidive coinvolge il SNC. Inoltre, è importante sviluppare nuovi modelli preclinici per studiare l'interazione tra cellule leucemiche e microambiente cerebrale. In questo studio abbiamo sviluppato un nuovo modello ex vivo di fettine organotipiche corticali cerebrali microiniettate con la linea cellulare leucemica NALM6, che esprime la proteina iRFP670, per studiare le modifiche del microambiente cerebrale indotte dalle cellule leucemiche. Abbiamo osservato un aumento esponenziale delle cellule NALM6-iRFP670 fino a 7 giorni dopo la microiniezione nel tessuto cerebrale. In più, il pretrattamento con Attivina di tipo A ha aumentato significativamente la proliferazione delle cellule NALM6 dopo 5 e 7 giorni rispetto alle cellule non trattate. Il pretrattamento con Attivina di tipo A è stato associato anche ad una maggiore mortalità delle fettine corticali dopo a 24 e 72 ore dopo la microiniezione. Utilizzando fettine corticali CX3CR1+/GFP, abbiamo osservato un aumento dell'attivazione della microglia nelle fettine corticali microiniettate con NALM6 pretrattate con Attivina di tipo A a 72 ore, 5 e 7 giorni dopo la microiniezione. L'analisi morfologica della microglia ha mostrato che, 72 ore dopo la microiniezione, le cellule NALM6 pretrattate con Attivina di tipo A hanno indotto un leggero aumento della microglia di forma rotonda rispetto ai controlli (CTRL) e alle fettine microiniettate con cellule NALM6 non trattate. Tuttavia, 7 giorni dopo la microiniezione, la microglia delle fettine corticali microiniettate con NALM6 pretrattate con Attivina di tipo A ha mostrato un aumento dell'area e dei perimetri cellulari, una ridotta circolarità e solidità, una polarizzazione e un diametro di Feret più elevati. Queste modifiche sono state associate a cambiamenti significativi nell'espressione genica, caratterizzati da una down-regolazione dei geni pro-infiammatori della microglia (iNOS, PAI-1) e da una up-regolazione di quelli anti-infiammatori (Arginase-1, CD206). Inoltre, nelle fettine microiniettate con cellule NALM6 pretrattate con Attivina di tipo A, abbiamo trovato livelli significativamente ridotti del GFAP a 72h dopo la microiniezione rispetto ai CTRL. Inoltre, la morfologia degli astrociti è risultata essere protoplasmica nelle fettine corticali microiniettate con cellule NALM6 stimolate e non con Attivina di tipo A rispetto alla forma fibrosa riscontrata nelle fettine CTRL. I risultati preliminari in vivo hanno indicato che la leucemia è inizialmente una malattia delle leptomeningi e che le cellule NALM6 inducono la produzione di Attivina di tipo A nel SNC. Concludendo, abbiamo messo a punto in modo efficiente un modello ex vivo per studiare le interazioni delle cellule leucemiche con il tessuto cerebrale. I nostri dati indicano che l'Attivina di tipo A può essere considerata una nuova molecola candidata nei cambiamenti microambientali del SNC. Inoltre, il pretrattamento con Attivina di tipo A promuove una maggiore proliferazione delle cellule NALM6 nelle fettine corticali rispetto alle cellule non trattate e, modifica l'ambiente cerebrale a favore della sopravvivenza e della proliferazione delle cellule leucemiche. Il nostro modello può aiutare a rispondere a importanti domande sui cambiamenti microambientali cerebrali dopo l'infiltrazione di cellule leucemiche e potrebbe essere utilizzato come strumento per lo screening di farmaci antitumorali. Central nervous system (CNS) acute lymphoblastic leukemia is a major clinical problem. Prophylactic therapy is neurotoxic, and a third of the relapses involve the CNS. In addition, new preclinical models are needed to study the interaction between leukemia cells and brain microenvironment. In this study, we established a new ex vivo model of cortical cerebral organotypic slices microinjected with leukemic cell line (NALM6-iRFP670) to study central nervous system microenvironment modifications by leukemia cells. We observed an exponential increase of iRFP670+ NALM-6 cells in the cortical slices up to 7d post- injection. Of note, pretreatment with Activin A significantly increased NALM6 proliferation after 5 and 7d compared to non-stimulated cells. Cortical slices injected with Activin A pretreated NALM6 cells (NALM6 Activin A) showed higher mortality 24h and 72h compared to cortical slices injected with non-stimulated NALM6 (NALM6 NS) and compared to non-injected slices (CTRL). By using CX3CR1+/GFP cortical slices, we observed an increased microglia activation in NALM6 Activin A slices at 72h, 5d and 7d post-injection compared to CTRL. Microglia morphological analysis showed a slight increase of round-shape in NALM6 Activin A slices compared to CTRL and to NALM6 NS slices at 72h. However, at 7d NALM6 Activin A slices showed an increased microglia cell area and perimeters, a reduced circularity and solidity and an increased aspect ratio and feret’s diameter. These modifications were associated with significant changes in gene expression, characterized by a down-regulation of microglial pro-inflammatory genes (iNOS, PAI-1) and an up-regulation of the anti-inflammatory ones (Arginase-1, CD206). At last, NALM6 Activin A slices showed significant reduction of GFAP gene expression at 72h compared to CTRL. Furthermore, the astrocyte morphology was found to be protoplasmic in both NALM6 NS and NALM6 Activin A slices as opposed to the fibrous one found in CTRL. Preliminary in vivo results indicated that leukemia is initially a leptomeningeal disease that induces Activin A production in the central nervous system. Overall, we efficiently set up an ex vivo model to study leukemia cell interactions with brain tissue. In addition, our data candidate Activin A as a key molecule involved in CNS microenvironmental modifications. In conclusion, Activin A pretreatment promotes higher NALM6 proliferation in organotypic cortical brain slices, modifying the cerebral milieu in favor of leukemic cell survival and proliferation. Our model can aid in answering important questions about brain changes after leukemic cell infiltration and could be used as useful tool for anti-cancer drug screening.

Inotuzumab Ozogamicin is a CD22-directed antibody conjugated to calicheamicin, approved in adults with relapsed or refractory (R/R) B cell acute lymphoblastic leukemia (BCP-ALL). Patients aged 1–18 years, with R/R CD22 + BCP-ALL were treated at the RP2D of 1.8 mg/m2. Using a single-stage design, with an overall response rate (ORR) ≤ 30% defined as not promissing and ORR > 55% as expected, 25 patients needed to be recruited to achieve 80% power at 0.05 significance level. Thirty-two patients were enrolled, 28 were treated, 27 were evaluable for response. The estimated ORR was 81.5% (95%CI: 61.9–93.7%), and 81.8% (18/22) of the responding subjects were minimal residual disease (MRD) negative. The study met its primary endpoint. Median follow up of survivors was 16 months (IQR: 14.49–20.07). One year Event Free Survival was 36.7% (95% CI: 22.2–60.4%), and Overall Survival was 55.1% (95% CI: 39.1−77.7%). Eighteen patients received consolidation (with HSCT and/or CAR T-cells therapy). Sinusoidal obstructive syndrome (SOS) occurred in seven patients. MRD negativity seemed correlated to calicheamicin sensitivity in vitro, but not to CD22 surface expression, saturation, or internalization. InO was effective in this population. The most relevant risk was the occurrence of SOS, particularly when InO treatment was followed by HSCT.

Il trapianto di cellule ematopoietiche (HCT) rappresenta una terapia cardine per il trattamento delle neoplasie ematologiche altrimenti incurabili. Tuttavia, la procedura di trapianto può essere gravata dalla recidiva della malattia, dalla malattia del trapianto contro l'ospite (GVHD) e dalle infezioni. Le cellule T e NK che ricostituiscono dopo l'HCT proteggono da infezioni e recidive, ma sono anche coinvolte nella patogenesi della GVHD. Gli obiettivi del mio progetto di dottorato erano di migliorare la comprensione della ricostituzione delle cellule T e NK, utilizzando campioni di donatori sani e pazienti dopo il trapianto e diversi approcci tecnici (citometria a flusso, citometria di massa, sequenziamento dell'RNA e test funzionale ex vivo) e sviluppare nuove strategie immunoterapiche basate sui linfociti T e NK dopo HCT. In primo luogo, abbiamo dimostrato che un ritardo nel recupero dei linfociti T, un rapporto Treg/Tcon più elevato, un'aumentata espressione di PD-1 sui linfociti T di memoria e un arricchimento di cellule NK a fenotipo immaturo sono stati osservati dopo HCT aploidentico (aplo-HCT) con l’utilizzo di ciclofosfamide post-trapianto. Inoltre, la funzione delle cellule NK CD56brightCD16+ immature funzionalmente alterate dopo aplo-HCT può essere migliorata con l’utilizzo dell'interleuchina-15 in vitro. In secondo luogo, abbiamo avviato uno studio di fase I sulle cellule cytokine-induced memory-like (CIML) NK infuse da donatore haploidentico in pazienti con neoplasie mieloidi che hanno avuto una recidiva dopo aplo-HCT. Nei primi 6 pazienti arruolati, l'infusione di cellule CIML-NK ha portato a una rapida espansione in vivo da 10 a 50 volte, che è stata mantenuta per mesi. L'infusione è stata ben tollerata, con febbre e pancitopenia come eventi avversi più comuni. Sulla base di questi dati preliminari, le cellule CIML-NK possono fungere da piattaforma per il trattamento della recidiva post-trapianto delle patologie mieloidi. Infine, ci siamo concentrati sul bilanciamento della risposta dei linfociti T per controllare l’incidenza di GVHD. CD6, un recettore co-stimolatorio dei linfociti T, che aiuta a stabilizzare la sinapsi immunologica tra la cellula T e l'APC, dopo legame con il suo ligando, la molecola di adesione delle cellule leucocitarie attivate (ALCAM). In questo contesto, il legame CD6-ALCAM promuove l'attivazione, la proliferazione e la maturazione delle cellule T. Abbiamo dimostrato che le cellule T CD6 ricostituivano subito dopo il trapianto, con le cellule Treg che esprimono livelli inferiori di CD6 rispetto alle cellule Tcon e cellule T CD8+. Dopo l'insorgenza della aGVHD, l'espressione sia di CD6 che di ALCAM è stata mantenuta. Itolizumab ha inibito l'attivazione e la proliferazione delle cellule T CD4+ e CD8+ nell'ambito di aGVHD in esperimenti ex vivo, senza mediare l'attività citolitica diretta o la citotossicità anticorpo-dipendente. I nostri risultati identificano la via di CD6-ALCAM come potenziale bersaglio per il controllo dell'aGVHD. Uno studio di fase I/II che utilizza itolizumab come trattamento di prima linea in combinazione con steroidi per i pazienti con aGVHD è attualmente in corso. In conclusione, questi risultati evidenziano la necessità di bilanciare le proprietà effettrici e tolerogeniche del sistema immunitario che si ricostituisce dopo HCT e suggeriscono differenti strategie per promuovere o moderare le funzioni delle cellule T e NK. Hematopoietic cell transplantation (HCT) represents a cardinal therapy for hematological malignancy otherwise incurable. However, HCT can be complicated by disease recurrence, graft versus host disease (GVHD) and infections. After HCT, reconstituting T and NK cells protect against infection and relapse, but they are also involved in the pathogenesis of GVHD. The aims of my PhD project were to improve the understanding of T and NK-cell reconstitution, using samples from both healthy donor and patients after transplant and different technical approaches (flow cytometry, mass cytometry, RNA sequencing, and ex vivo functional assay) and to develop post-transplant T and NK cell-based immunotherapeutic strategies. First, we showed that delayed early T-cell recovery, a higher Treg/ Tcon ratio, an increased PD-1 expression on memory T cells, and an enriched immature NK phenotype were observed after haploidentical HCT (haplo-HCT) with post-transplant cyclophosphamide. In addition, the expansion of functionally impaired immature CD56brightCD16+ NK cells after haplo-HCT can be enhanced with in vitro interleukin-15 priming. Second, we initiated a phase I trial of adoptively transferred cytokine-induced memory-like (CIML) NK cells in patients with myeloid malignancies who relapsed after haplo-HCT. In the first 6 enrolled patients, infusion of CIML NK cells led to a rapid 10- to 50-fold in vivo expansion that was sustained over months. The infusion was well tolerated, with fever and pancytopenia as the most common adverse events. Based on these preliminary data, CIML NK cells may serve as a promising platform for the treatment of posttransplant relapse of myeloid disease. Finally, we focused on the balancing of T cell response to control GVHD occurrence. CD6, a pan-T cell co-stimulatory receptor, helps to stabilize the immunological synapse between the T cell and the APC, upon ligation, with its ligand, activated leukocyte cell adhesion molecule (ALCAM). In this context, CD6-ALCAM binding promotes T cell activation, proliferation, maturation. We showed that CD6 T cells reconstituted early after transplant with Treg expressing lower levels of CD6 compared to Tcon and CD8+ T cells. After onset of aGVHD, both CD6 and ALCAM expression was maintained. Itolizumab inhibited CD4+ and CD8+ T cell activation and proliferation in the setting of aGVHD in ex vivo experiments, without mediate direct cytolytic activity or antibody-dependent cytotoxicity. Our results identify the CD6-ALCAM pathway as a potential target for aGVHD control. A phase I/II study using itolizumab as first line treatment in combination with steroids for patients with aGVHD is currently ongoing. In conclusion, these results highlight the need of balancing the effector and tolerogenic properties of the immune system reconstituting after HCT and suggest different strategies to enhance or moderate the T and NK cells functions.

The incidence of hypersensitivity reactions (HSRs) to PEG-asparaginase (PEG-ASNase) was evaluated in 6136 children with ALL enrolled in the AIEOP-BFM ALL 2009 study. Patients with B-cell precursor-acute lymphoblastic leukemia (BCP-ALL) were stratified as standard-risk/medium-risk (MR)/high-risk (HR) and those with T-ALL as non-High/HR. PEG-ASNase was administered intravenously at 2500 IU/sqm/dose. All patients received 2 PEG-ASNase doses in induction; thereafter non-HR versus HR patients received 1 versus 6 PEG-ASNase doses, respectively. After the single regular dose of PEG-ASNase at the beginning of delayed intensification, BCP-ALL-MR patients were randomized to receive 9 additional PEG-ASNase doses every 2 weeks (experimental arm [EA]) versus none (standard arm [SA]); HR patients were randomized to receive, in consolidation, 4 weekly PEG-ASNase doses (EA) versus none (SA). The HSR cumulative incidence (CI) was estimated adjusting for competing risks. An HSR occurred in 472 of 6136 (7.7%) patients. T-non- HR/BCP-Standard-Risk, BCP-MR-SA, BCP-MR-EA, HR-SA and HR-EA patients had 1-year-CI-HSR (±SE) rates of 5.2% (0.5), 5.2% (0.5), 4.0% (0.8), 20.2% (1.2), and 6.4% (1.3), respectively. The randomized intensification of PEG-ASNase did not significantly impact on HSR incidence in BCP-MR patients (1-y-CI-HSR 3.8% [0.8] versus 3.2% [0.6] in MR-EA versus MR-SA; P = 0.55), while impacted significantly in HR patients (1-y-CI-HSR 6.4% [1.3] versus 17.9% [1.8] in HR-EA and HR-SA, respectively; P < 0.001). The CI-HSR was comparable among non-HR groups and was not increased by a substantial intensification of PEG-ASNase in the BCP-MR-EA group whilst it was markedly higher in HR-SA than in HR-EA patients, suggesting that, in such a chemotherapy context, a continuous exposure to PEG-ASNase reduces the risk of developing an HSR.

Background: The process of receiving a communication of positivity for metabolic diseases at expanded newborn screening (ENBS) is extremely articulated, involves a variety of actors (parents, maternal and child departments, clinical centres and laboratories) and is open to a variety of outcomes from false positive to true positive cases. Receiving communication of positivity can be highly stressful for parents and requires an adequate communication process to give clear and reliable information without causing excessive worry. This qualitative study describes the parents' experience of receiving a communication of positivity to metabolic diseases at ENBS, and their assessment of the quality of the communication process and steps, with the main aim to identify the process' strengths and weaknesses and to advance tailored recommendations to improve the communication process. Method: Fourteen in-depth, semi-structured phone interviews were conducted with parents whose children resulted positive to the ENBS. As part of the ENBS communication process, parents received a first phone call communication of positivity and a second in-person communication at metabolic clinical centres (MCC). The framework analysis method was used to organize the data and identify emerging themes. Results: Parents were largely dissatisfied with the quality and depth of the information received and with the way the healthcare staff delivered the first communication phone call, which failed to create a caring, empathic and safe setting. Many parents tried to reduce the uncertainty by searching online information or consulting with other providers. Nevertheless, the majority of parents described the in-person visit at MCC as clear, welcoming and reassuring. Conclusion: More efforts are needed to improve the quality of the communication process of the ENBS. Guidelines, recommendations and standard scripts to communicate positivity are needed along with programmes and educational resources to train tailored communication skills.

Background and Objectives: Besides allergic reactions, antibodies against polyethylene glycol (PEG) have been associated with reduced PEG-asparaginase (PEG-ASNase) activity. Population pharmacokinetics (popPK) allow for an in-depth investigation of the influence of anti-PEG antibodies on PEG-ASNase pharmacokinetics. Methods: PEG-ASNase activity (6261 samples) and anti-PEG antibodies (2082/6412 samples prior to/post administration) in 1444 children with acute lymphoblastic leukaemia treated in the AIEOP-BFM ALL 2009 trial were evaluated. Patients received two doses of PEG-ASNase during induction (2500U/m2, intravenous, biweekly) and a third dose during reinduction treatment. Anti-PEG IgG and IgM measured prior to and post administration were explored for their influence on the initial clearance of PEG-ASNase using a previously established popPK model. Categorical and continuous antibody data, including each isotype individually as well as in combination, were assessed. Results: High pre-existing levels of anti-PEG antibodies increase the initial drug clearance. Analysed separately, both anti-PEG IgGprior and IgMprior were significant covariates; the stronger effect was observed for anti-PEG IgMprior. Hockey stick models best described the data. For anti-PEG IgMprior, each additional log unit above the estimated cut point was related to a 41.4% increase in initial clearance after the first dose in induction. Antibody levels below the cut point were not associated with an effect on clearance. The combination of both isotypes did not provide additional information compared to anti-PEG IgMprior alone. Antibody levels post administration were not associated with an effect on clearance. Conclusion: Pre-existing antibodies against PEG-ASNase significantly increased the initial clearance in a subgroup of patients showing high antibody levels. GnRH-independent precocious puberty (GIPP) can be the presenting clinical picture experienced by patients with secreting germ cell tumor (GCT). Indeed, as luteinizing hormone (LH) and human chorionic gonadotropin (hCG) share identical α-subunits and similar β-subunits, an increased secretion of β-hCG may result in a precocious activation of Leydig cells. Though the co-occurrence of raised β-hCG levels and signs of precocious virilization usually prompts a complete oncological work-up, the diagnostic and therapeutic management of GCT-induced GIPP may be challenging. We report the case of a 6.2 year-old boy presenting with clinical and biochemical findings consistent with GIPP (discrepancy between overt virilization and pre-pubertal testicular volume, suppressed gonadotropins and remarkably raised testosterone). Brain imaging detected a bilobed cyst of the pineal gland, while serum and cerebrospinal baseline assessment initially ruled out raised alpha-fetoprotein or β-hCG levels. Nevertheless, a strict biochemical follow-up highlighted a fluctuant trend of tumor markers, with a more aggressive behavior and recurrent erections occurring as a result of unpredictable phases of raised testosterone and serum/cerebrospinal β-hCG, followed by sudden spontaneous decrease. Accordingly, a secreting pineal GCT was suspected. Given the fluctuating trend of tumor markers, surgery was initially kept on hold and a combined treatment with bicalutamide (androgen receptor blocker) and anastrozole (aromatase inhibitor) was undertaken in order to prevent the patient from experiencing further virilization and excessive bone age maturation. Subsequently, a progression in the size of the pineal tumor prompted surgical resection and a diagnosis of secreting GCT was histologically confirmed. Accordingly, the patient was started on adjuvant chemo- and radiotherapy. Antineoplastic treatment was followed by persistent and remarkable decrease of tumor markers and by a complete pubertal arrest. We reported the challenging diagnosis of a secreting pineal GCT in a patient with GIPP and a fluctuating trend of tumor markers, testosterone levels and associated clinical signs, hence prompting the indication for a systematic assessment and a strict monitoring whenever a patient with GnRH-independent precocious puberty shows clinical or radiological markers potentially consistent with a GCT.

La leucemia mieloide acuta (LMA) è la neoplasia ematologica maggiormente diagnosticata nei pazienti adulti (25%) e mentre rappresenta il 15-20% dei casi nei pazienti pediatrici. La chemioterapia convenzionale, che impiega antraciclina e citarabina, rappresenta il trattamento standard per l’LMA, con tassi di remissione completa dal 60% all'80% nei bambini e dal 40% al 60% negli adulti (>60 anni). Sfortunatamente, la ricaduta dopo tale terapia è comune e la sopravvivenza dei pazienti stimata a 5 anni è ancora inferiore al 30%. Risulta quindi di primaria importanza trovare alternative terapeutiche per i pazienti recidivanti e refrattari. Il recente successo clinico, ottenuto nelle leucemie di tipo B, dell'immunoterapia con cellule CAR (chimeric antigen receptor) T ha portato allo sviluppo di nuove strategie terapeutiche nell’ambito dell’LMA. Tuttavia, lo sviluppo del trattamento con cellule CAR T nel contesto dell'LMA è ancora agli albori a causa dell'eterogeneità della malattia, della mancanza di un antigene bersaglio adatto e del ruolo protettivo del microambiente tumorale (TME). Infatti, non esiste ancora un protocollo clinico approvato per il trattamento della leucemia mieloide. Per creare le cellule CAR T abbiamo scelto di utilizzare la piattaforma non virale Sleeping-Beauty (SB) per ingegnerizzare le cellule CIK (cytokine-induced killer). In primo luogo, abbiamo scelto di utilizzare come potenziale strumento per il targeting del TME le cellule CIK ingegnerizzate con anti-CD146.CAR. Di conseguenza, abbiamo ottimizzato 6 diverse molecole CAR aventi un design differente, ottenendo un'espressione ottimale di CD146 nella variante VLVH Long. Abbiamo quindi testato le cellule CD146.CAR-CIK in vitro, ottenendo l’attivazione specifica delle funzioni effettrici (in termini di capacità di killing, produzione di citochine e proliferazione) contro cellule target CD146+. In seguito, abbiamo progettato un Tandem CAR bispecifico (CD33xCD146.CAR-CIKs) che ha mostrato una significativa attività antileucemica in vitro. È stato ampiamente dimostrato che la nicchia midollare contribuisce al supporto e alla protezione delle cellule staminali leucemiche (CSLs). Quindi, per mimare al meglio l’azione del CAR nella nicchia midollare umana, abbiamo testato le cellule CD33xCD146.CAR-CIK contro le linee cellulari stromali CD146+ e le cellule mesenchimali (MSC) primarie sane (HD-) e di derivazione mieloide (LMA-). I dati mostrano una inibizione delle funzioni effettrici delle cellule CAR-CIK e una drastica diminuzione della produzione di citochine e della proliferazione. Inoltre, l'equilibrio tra citochine pro e antinfiammatorie è risultato alterato, infatti la produzione di citochine Th1/Tc1 da parte delle cellule CD146.CAR-CIK è stata inibita dalla co-coltura con cellule stromali, mentre è stato rilevato un aumento delle citochine Th2/Tc2. Questi risultati suggeriscono un potenziale ruolo immunosoppressivo del compartimento stromale nei confronti delle cellule CAR-CIK. Sulla base dell’ effetto immunomodulatorio delle MSC sui linfociti T, abbiamo ipotizzato che la nicchia midollare possa influenzare le funzioni effettrici delle cellule CAR T. Di conseguenza, il targeting del CD146 rappresenta una "proof-of-principles" del fatto che aggredire il microambiente leucemico possa migliorare la terapia CAR T nell’ambito dell’LMA. Per ridurre al minimo la tossicità "off-target ", stiamo cercando di selezionare un antigene bersaglio specifico ed overespresso sulle cellule stromali dell'LMA, che abbia un'espressione minima nello stroma sano e che sia coinvolto nelle interazioni leucemia/nicchia. Il nuovo marker di interesse sarà accoppiato al CD33.CAR nella creazione di un CAR bispecifico, che verrà confrontato con il costrutto CD33xCD146.CAR, valutandone i profili di efficacia e sicurezza sia in vitro che in vivo. Acute myeloid leukemia (AML) is the most frequently diagnosed leukemia in adults (25%) and accounts for 15-20% cases in pediatric patients. Conventional chemotherapy employing anthracycline and cytarabine represents the gold standard treatment for AML, with rates of complete remission from 60% to 80% in children and from 40% to 60% in adults (>60 years). Despite these high rates, relapse after conventional therapy is common and the estimated five-year survival of AML patients is still below 30%. Indeed, there is an urgency to find alternative therapeutic strategies for relapsed and refractory patients. The recent clinical success of chimeric antigen receptor (CAR) T cell immunotherapy in the context of B-cell malignancies has opened a new route of investigation also towards AML. However, the development of CAR T cell therapy in the context of AML is still in its infancy due to heterogeneity of the disease, the lack of a suitable target antigen and the leukemia protective role of the tumor microenvironment (TME) and no approved CAR T cells study exists for AML treatment yet. Non-viral Sleeping-Beauty (SB) transposon platform was employed to redirect cytokine-induce killer (CIK) cell. In this scenario, we firstly characterize non-viral SB engineered CIK cells with anti-CD146.CAR as a potential tool for the targeting of the bone marrow (BM) microenvironment. We optimized the CAR design structure by testing 6 different CAR molecules, achieving a specific and efficient CD146 expression in the VLVH Long variant. CD146.CAR-CIK cells were subsequently tested in vitro, showing an optimal activation of effector functions (in terms of killing activity, cytokines production and proliferation) when they were engaged against CD146+ target cells. Consequently, we developed a bispecific Tandem CAR (CD33xCD146.CAR-CIKs), which displayed anti-leukemic activity in vitro. It has been extensively proven that BM niche contribute to establish a sanctuary in which leukemic stem cells (LSCs) are able to acquire drug-resistant phenotype, therefore, to better mimicking the human BM niche we tested CD33xCD146.CAR-CIK cells against CD146+ stromal cell lines (HS-27A and HS-5) and primary derived healthy (HD-) and patient-derived (AML-) mesenchymal stromal cells (MSCs). Results showed inhibition of the redirected CAR-CIK cells effector functions, resulting in a drastic decrease of cytokines production and proliferation. The balance between pro- and anti- inflammatory cytokines showed that Th1/Tc1 cytokines production by CD146.CAR-CIK cells was inhibited by the co-culture with stromal cells, while increase Th2/Tc2 cytokines was detected when CD146.CAR-CIK cells were co-cultured with stromal target cells. These results suggest a potential immunosuppressive role of the stromal compartment against CAR-CIK cells. According to these results, we hypothesized that BM stromal cells can potentially exert an immunomodulatory effect on T cells, suggesting that the niche microenvironment may be involved in the regulation of CAR T cells therapy effectiveness. Indeed, the targeting of CD146 on stroma represents a “proof-of-principle” that stromal components of leukemic microenvironment may be attractive targets for CAR T based immunotherapy. To minimize “off-tumor” toxicity, we are looking for a specific surface target antigen selectively overexpressed on AML stromal cells, with minimal expression in healthy stroma and possibly involved in leukemia/niche interactions. The newly marker of interest will be coupled to the CD33.CAR and this bispecific CAR will be compared with CD33xCD146.CAR construct, evaluating their efficacy and safety profiles both in vitro and in vivo.

In questo progetto di dottorato, sono stati presi in considerazione alcuni punti cruciali relativi alla malattia Mucopolisaccaridosi di tipo I (MPSI) e ai limiti delle attuali terapie, per migliorarle concentrandosi su approcci in epoca neonatale, a livello di combinazione terapeutiche (HSCT e ERT) e di terapia genica, e su riduzione delle tossicità associate ai regimi di condizionamento. Complessivamente, le problematiche più significative riguardo MPSI rimangono la necessità di un intervento precoce e rapido, l’incompleta correzione della malattia in seguito agli attuali approcci terapeutici e gli effetti collaterali dovuti al regime di precondizionamento. La prima parte del progetto si è concentrata sul testare un approccio terapeutico che combinasse i trattamenti canonici per MPSI. È stata testata l'efficacia della combinazione di HSCT ed ERT nel modello murino di MPSI come intervento neonatale, per valutare i benefici aggiuntivi della terapia enzimatica effettuata in modo continuativo a seguito del trapianto di cellule da donatore. Tre opzioni di trattamento sono state confrontate dalla nascita, considerando la mancante attività enzimatica (IDUA), l’accumulo di GAG e di vacuoli nei principali organi viscerali, la risposta nei confronti di IDUA ricombinante e i miglioramenti di tipo scheletrico e cerebrale. Pertanto, un approccio combinato di HSCT ed ERT nel periodo neonatale potrebbe essere applicato per migliorare alcune manifestazioni cliniche di MPSI, soprattutto evitando danni irreversibili. La seconda parte del progetto di dottorato è stata svolta in collaborazione con TIGET-SR e il Prof. Alessandro Aiuti. Si è concentrato sulla sperimentazione di un approccio di terapia genica neonatale in un modello murino di MPSI, considerando l'importanza di correggere precocemente la malattia. In particolare, abbiamo valutato se questo trattamento terapeutico potesse essere applicato nei neonati MPSI e potesse essere una strategia efficace per superare i principali problemi clinici che permangono dopo il trattamento canonico. Abbiamo valutato l'effetto della terapia genica somministrata nei neonati affetti da MPSI, monitorando i valori di IDUA e VCN nel sangue periferico e considerando infine la mancante attività enzimatica (IDUA), l’accumulo di GAG e di vacuoli nei principali organi viscerali, la risposta nei confronti di IDUA ricombinante e i miglioramenti di tipo scheletrico e cerebrale. Contemporaneamente, abbiamo cercato di ridurre gli effetti collaterali causati dal regime di condizionamento nel contesto delle terapie neonatali per MPSI, come progetto parallelo. L’obiettivo principale è stato trasporre l'applicazione di ADC, molecole congiunte di anticorpo-farmaco, capaci di agire specificatamente sulla cellula, come condizionamento per il trattamento neonatale di MPSI, in cui un intervento precoce è fondamentale. Poiché nessuna delle opzioni testate è stata in grado di indurre un sufficiente attecchimento di cellule del donatore da essere rilevante per il trattamento precoce dell'MPSI, ne abbiamo ricercato le cause, dimostrando la necessità di ulteriori studi prima dell'applicazione degli ADC nel modello studiato, in quelli umanizzati e nei cuccioli di MPSI NSG. Analisi preliminari sono state effettuate relativamente all’aumento dei livelli di citochine dopo CD117-SAP nei topi NSG adulti, confrontandoli agli altri regimi di condizionamento, per valutare una possibile applicazione di ADC con farmaci antinfiammatori prima della terapia precoce nei cuccioli MPSI. The present PhD project has taken into account critical issues around Mucopolysaccharidosis type I (MPSI) and limitations of current therapies to further improve them, by generally focusing on neonatal therapeutic approaches, both in terms of combined HSCT and ERT and of gene therapy, and on trying to reduce the overall toxicities associated with pre-conditioning settings. Overall, the most important open issues regarding this rare life-threatening disorder are the need for a precocious and rapid intervention, the lack of complete disease correction after current therapeutic approaches and side effects due to pre-conditioning regiment. My PhD project partially focused on testing a combined approach of the current standard-of-cares for treating MPSI. HSCT and ERT combination efficacy was tested in a mouse model of MPSI as neonatal intervention, for evaluating additional benefits of continuous enzyme therapy after transplant of donor’s cells. We compared three treatment options starting from MPSI pups’ birth, considering IDUA deficient activity, GAGs storage and vacuoles in visceral organs, the immune response against the recombinant IDUA and skeletal and CNS ameliorations. Therefore, performing a combined approach of HSCT and ERT in the neonatal period could help improving some hard-to-treat MPSI manifestations. The second part of this PhD project was carried out in collaboration with TIGET-SR and Prof. Alessandro Aiuti. It was focused on testing a neonatal gene therapy approach in a mouse model of MPSI, considering the importance of an early phenotype correction. In particular, we evaluated if this early treatment could be applied in MPSI neonates and could be a successful strategy for overcoming the main clinical issues that still remain after current canonical HSCT treatment. We monitored peripheral blood of treated mice for 8 months in terms of enzymatic activity and VCN, and we evaluated the effect of GT performed in MPSI pups at endpoint, considering IDUA deficient activity, vector copies/genome, GAGs storage and vacuoles in visceral organs, the immune response against the recombinant IDUA and skeletal and CNS ameliorations. The last part of this PhD project was centered on trying to reduce the high morbidity and mortality due to the severe conditioning regimen in the context of neonatal MPSI therapies, as a side project. The main objective was to translate the application of hematopoietic cell–specific antibody-drug conjugates (ADCs) as conditioning for early MPSI treatment, in which a precocious intervention is crucial. Since none of the tested setting was able to induce enough engraftment of donor’s cells to be relevant for MPSI early treatment, we tried to understand what could interfere, but we demonstrated the need for further studies prior to ADCs application in humanised models and in MPSI NSG pups. Preliminary results on increased cytokines levels after CD117-SAP in adult NSG compared to other conditioning settings were performed to evaluate impairment of inflammation and possible ADC application with anti-inflammatory drugs prior to early therapy in MPSI pups.

La sindrome Shawachman-Diamond (SDS) è una malattia multi-organo caratterizzata da disfunzioni midollari ed insufficienza pancreatica. I pazienti SDS sono inoltre soggetti a sviluppo di anomalie ematologiche gravi, quali neutropenia, SMD e/o LMA. Nella prima parte di questo lavoro ci siano focalizzati sullo studio dell’alterata capacità angiogenica in vitro delle MSCs derivate da pazienti SDS. L’angiogenesi non coinvolge solo la patogenesi dei tumori solidi, ma anche lo sviluppo delle malattie ematologiche. Le MSCs sono in grado di supportare l’angiogenesi attraverso il differenziamento cellulare, l’interazione cellula-cellula e mediante meccanismi autocrini o paracrini. Grazie a modelli in vivo ed in vitro, il nostro gruppo di ricerca ha recentemente dimostrato come le SDS-MSCs siano caratterizzate da un alterato potenziale angiogenico. Qui, abbiamo confermato l’anomala capacità angiogenica in vitro delle cellule mesenchimali SDS dopo stimolazione angiogenica. Abbiamo dimostrato come questa alterazione sia associata a cambiamenti nel pathway di segnalazione TGFβ1/VEGFA. Infatti, l’espressione di diversi fattori di crescita in grado di stimolare il rilascio endogeno di VEGFA ed in grado di essere indotti da TGFβ1 è down-regolata nelle SDS- vs HD-MSCs. Inoltre, la somministrazione esogena di TGFβ1 o VEGFA permette la reversione del fenotipo angiogenico solo nelle cellule mesenchimali derivanti dai pazienti gravemente neutropenici. In fine, abbiamo dimostrano che a seguito di stimolo angiogenico i livelli proteici di P53 sono raddoppiati nelle SDS-MSCs vs HD-MSCs, analogamente al numero di cellule in apoptosi precoce e tardiva. Complessivamente, i nostri dati indicano un forte collegamento tra TGFβ1 e VEGFA nella modulazione dell’alterata capacità in vitro delle SDS-MSCs. Inoltre, forniscono un razionale per futuri studi mirati alla comprensione della correlazione tra angiogenesi e grado di neutropenia dei pazienti. Una migliore comprensione dei meccanismi molecolari alla base della regolazione del numero e della funzionalità dei neutrofili potrebbe portare a nuove strategie terapeutiche atte alla gestione delle infezioni ricorrenti dei pazienti SDS. La seconda parte del lavoro, invece, si è focalizzata sull’analisi dei meccanismi molecolari e dei pathways di segnalazione responsabili della neutropenia e dell’evoluzione SMD o LMA dei pazienti SDS. STAT3 è un regolatore di diversi processi cellulari, quali granulogenesi dei neutrofili, leucemia e trasformazione maligna del linfoma. Inizialmente riconosciuto come un fattore di trascrizione attivato da IL6, oggi è anche considerato un substrato diretto di mTOR. Recentemente, è stato dimostrato che mTOR e STAT3 sono costitutivamente up-regolati in leucociti primari e linee cellulari linfoblastoidi derivati da pazienti SDS. In questo lavoro, dimostriamo che la via di segnalazione mTOR-STAT3 è up-regolata anche in altre tipologie cellulari appartenenti alla linea linfoide dei pazienti SDS. Inoltre, i nostri dati rivelano elevati livelli di IL6 sia in surnatanti cellulari derivanti da linfoblasti, cellule mononucleate di midollo osseo e mesenchimali, sia in campioni di plasma ottenuti da una coorte di 10 pazienti SDS. Da notare che, l’inibizione di mTOR mediata da everolimus riporta a livelli basali la fosforilazione di STAT3. In ultimo, l’inibizione di mTOR-STAT3 porta alla normalizzazione dei livelli di espressione di IL6. Complessivamente, i nostri dati rafforzano l’ipotesi che la sindrome SDS interessa sia il compartimento linfoide che mieloide e suggerisce everolimus come potenziale agente terapeutico per ridurre l’eccessiva attivazione del pathway mTOR-STAT3 [Vella A., et al. 2020]. La scoperta di nuove alterazioni nei pathway molecolari che regolano la sindrome SDS potrebbe permettere l’individuazione di target terapeutici mirati al miglioramento delle alterazioni ematologiche ed all’evoluzione leucemica di questi pazienti. Shwachman-Diamond Syndrome (SDS, OMIM 260400) is a multi-organ disorder mainly characterized by bone marrow (BM) dysfunctions and exocrine pancreatic insufficiency. SDS patients present also severe haematologic abnormalities, with neutropenia as the most common deficiency. Of note, SDS patients have an increased risk for myelodysplastic syndrome (MDS) and malignant transformation to acute myeloid leukaemia (AML). In the first part of this work, we focused our attention on the in vitro angiogenic capability of SDS-mesenchymal stromal cells (MSCs). Angiogenesis is not only involved in the pathogenesis of solid tumours, but also in haematological malignancies. MSCs can potentiate angiogenesis via direct cell differentiation, cell-cell interaction, and autocrine or paracrine effects. Using both in vitro and in vivo models, our research group recently demonstrated that SDS-MSCs display a marked impairment in their angiogenic potential. Here, we confirm that SDS-derived cells obtained from a cohort of 10 patients show altered angiogenic properties in response to angiogenic stimuli and that the defective in vitro tube formation is associated with TGFβ1/VEGFA signalling abnormalities. Indeed, we show that the expression of several growth factors able to increase the endogenous release of VEGFA and to be induced by TGFβ1 is down-regulated in SDS- vs HD-MSCs. Moreover, by providing the exogenous administration of VEGFA or TGFβ1, we demonstrate that only SDS-MSCs from severely neutropenic patients can restore their angiogenic properties. Finally, our data also show that under angiogenic stimulation, P53 protein levels are 2-fold increase in SDS- vs HD-MSCs, as well as the number of early/late apoptotic cells. Collectively, our results suggest a strong link between TGFβ1 and VEGFA in dictating the altered in vitro angiogenic capability of SDS-MSCs. Moreover, we provide a rational to investigate whether the defective angiogenesis driven by SDS-MSCs could be related to neutropenia. The better comprehension of the molecular mechanisms regulating neutrophil number and functionality may lead to novel strategies for the management of recurrent SDS infections. The second part of our study was focused on the analysis of the molecular mechanisms and signalling pathways responsible of SDS patients neutropenia, and evolution to MDS or AML. Signal transducer and activator of transcription 3 (STAT3) is a key regulator of several cellular processes including neutrophil granulogenesis, leukaemia, and lymphoma malignant transformation. Firstly recognised as an interleukin-6 (IL6)-activated transcription factor, nowadays STAT3 is also considered a direct substrate for the mammalian target of rapamycin (mTOR). Recently, it has been demonstrated that both mTOR and STAT3 pathways are constitutively up-regulated in primary leukocytes and lymphoblastoid cell lines derived from SDS patients. Here, we show that mTOR-STAT3 signalling is markedly up-regulated in several cell subsets belonging to the lymphoid compartment of SDS patients. Furthermore, our data reveal elevated IL6 levels in cellular supernatants obtained from lymphoblasts, bone marrow mononuclear and mesenchymal stromal cells, and plasma samples obtained from a cohort of 10 patients. Of note, everolimus-mediated inhibition of mTOR signalling was associated with the basal state of phosphorylated STAT3. Finally, inhibition of mTOR-STAT3 pathway leads to normalization of IL6 expression in SDS cells. Altogether, our data strengthen the hypothesis that SDS affects both lymphoid and myeloid blood compartment and suggest everolimus as a potential therapeutic agent to reduce excessive mTOR-STAT3 activation in SDS [Vella A., et al. 2020]. The discovery of new altered molecular pathways underlying SDS pathophysiology could lead to the identification of new therapeutic targets for the correction of SDS-related haematological defects and the prevention of leukemic evolution.

La Leucemia Linfoblastica Acuta (LLA) è il tumore più frequente in età pediatrica. Nonostante l’alto successo terapeutico raggiunto, una percentuale di pazienti non risponde alle terapie convenzionali. Numerosi studi hanno mostrato come il microambiente midollare giochi un importante ruolo nella progressione tumorale. Il nostro scopo è stato quello di identificare i pahways cruciali coinvolti nel cross-talk tra le cellule leucemiche e il microambiente stromale e che possano essere dei potenziali target terapeutici. Abbiamo studiato AttivinaA, molecola della superfamiglia del TGF-β, all’interno della nicchia midollare leucemica. Questa molecola è nota nel contesto dei tumori solidi per la sua capacità di promuovere la progressione tumorale attraverso la regolazione della motilità e invasività cellulare. Abbiamo qui definito per la prima volta AttivinaA come un fattore associato alla leucemia: all’esordio di leucemia, questa molecola è altamente espressa nel midollo leucemico e risulta essere prodotta a più alti livelli dalle cellule stromali mesenchimali (MSC) in seguito a co-coltura con le cellule leucemiche. Anche nel contesto della B-LLA, abbiamo dimostrato che AttivinaA promuove la motilità ed invasività cellulare in presenza o meno del fattore chemiotattico CXCL12, fondamentale nella localizzazione delle cellule staminali ematopoietiche (HSC) e delle cellule leucemiche all’interno della nicchia midollare. In particolare, il meccanismo d’azione di AttivinaA dipende dall’incremento dei livelli di calcio intracellulare e della polimerizzazione dell’actina che sono importanti regolatori della riorganizzazione del citoscheletro e del movimento cellulare. Interessante, AttivinaA modula in modo specifico le proprietà biologiche delle cellule leucemiche in quanto svolge un effetto contrario sulle HSC, favorendo quindi le cellule leucemiche nella competizione per la nicchia. Come già riportato in letteratura, abbiamo trovato livelli ridotti di CXCL12 nel midollo leucemico e abbiamo osservato che AttivinaA è responsabile almeno in parte di questa riduzione a causa di un suo effetto diretto sulla secrezione della chemochina da parte delle MSC. Tuttavia, essendo AttivinaA capace di aumentare la migrazione cellulare anche verso concentrazioni subottimali di CXCL12, questi dati suggeriscono un possibile meccanismo tramite il quale le cellule leucemiche persistono all’interno della nicchia midollare distruggendo l’ematopoiesi sana. I nostri dati in vitro circa il ruolo pro-migratorio e pro-invasivo di AttivinaA sono stati confermati anche in vivo. I processi di regolazione dei flussi di calcio e della riorganizzazione del citoscheletro da parte di AttivinaA sono importanti per stimolare anche la vescicolazione da parte delle cellule. Abbiamo dimostrato che AttivinaA è in grado di aumentare il rilascio di vescicole extracellulari (VE) da parte delle cellule di B-LLA. Tali vescicole trasportano al loro interno l’oncogene t(1;19), tipico delle cellule dalle quali esse originano. Sia AttivinaA sia le VE da essa indotte conferiscono chemioresistenza alle cellule leucemiche, diminuendo in maniera significativa la loro sensibilità al farmaco Asparaginasi, che viene poi ripristinata bloccando la via di segnalazione di AttivinaA. Il meccanismo alla base della chemioprotezione esercitata dalle VE può essere spiegato dalla presenza al loro interno di microRNA differenzialmente espressi in seguito al trattamento con AttivinaA, tra cui il miRNA-491-5p precedentemente associato a chemioresistenza all’Asparaginasi nella leucemia pediatrica. Complessivamente, i nostri dati suggeriscono che AttivinaA è una molecola chiave della nicchia leucemica, che conferisce un vantaggio migratorio alle cellule leucemiche e le protegge dai farmaci convenzionali attraverso la produzione di VE. Il nostro lavoro potrebbe portare dunque allo sviluppo di nuovi farmaci in grado di agire sul cross-talk stroma-leucemia. Acute Lymphoblastic Leukemia (ALL) is the most common type of cancer in children. About 80% of the cases arises from precursor B cells (BCP-ALL), which abnormally accumulate as a consequence of genetic alterations associated to differentiation inhibition and abnormal expansion. Despite the 85% survival rate, a total of 10-15% of patients retains leukemic stem cells and their progenitors in the bone marrow (BM), thereby relapsing following treatment cessation. The importance of BM microenvironment for cancer progression has been widely recognized in recent years. In this study, we aimed to identify the crucial pathways involved in the bi-directional leukemia-stroma cross-talk that could be an attractive target for future antileukemic therapy. We focused our attention on the characterization of ActivinA, a TGF-β family member, within the BM leukemic niche. Here, we identified ActivinA as a new crucial factor exploited by leukemic cells to create a self-reinforcing niche: indeed, this molecule was highly expressed in the BM plasma of leukemic patients. Furthermore, we reported that BCP-ALL cells, along with the highly pro-inflammatory environment of leukemic BM, induced a strong increase in the molecule secretion by Mesenchymal Stromal Cells (MSCs). In accordance with its protumoral role in solid tumors, ActivinA strongly induced both random and CXCL12-driven migration of cells also in the context of BCP-ALL. We observed that ActivinA selectively stimulated these leukemic cell biological properties with a calcium- and actin polymerization-mediated mechanism as this molecule showed an opposite effect on Hematopoietic stem cells (HSCs). According to the literature, we found reduced CXCL12 levels in the leukemic BM, but ActivinA enhanced cell migration also towards suboptimal CXCL12 concentrations, suggesting a possible mechanism by which leukemic cells could persist in the BM niche, displacing healthy hematopoiesis. Our in vitro data about the pro-migratory and pro-invasive role of ActivinA were confirmed also in vivo. By using a xenograft mouse model of human BCP-ALL, we demonstrated the ability of ActivinA to enhance both BM engraftment and metastatic potential intro extra-medullary sites of leukemic cells. Notably, the regulation of calcium influx and cytoskeleton organization by ActivinA is an important process to stimulate also cell vesiculation. Recent studies have shown that cancer extracellular vesicles (EVs) can mediate cell-cell communication and potentially contribute to tumor progression. Therefore, we investigated whether ActivinA was able to influence vesiculation by leukemic cells. We demonstrated that ActivinA increased the production of both exosomes and MVs by BCP-ALL cells. We found that EVs transport the t(1;19) fusion transcript, typical of cells from which they originate. We then studied the biological effects by which ActivinA-induced leukemia EVs can actively promote BCP-ALL disease, focusing our attention on resistance to therapy. Firstly, we demonstrated that ActivinA significantly decreased the sensitivity of leukemic cells to the anti-leukemic drug Asparaginase (ASNase) which was re-stored by blocking ActivinA signaling. Interestingly, also ActivinA-induced leukemia EVs conferred resistance to leukemic cells. To understand the mechanism underlying EV chemoprotection, we explored their miRNA cargo and identified differentially expressed miRNAs induced by ActivinA treatment. Of these, miR-491-5p has been previously reported to be associated with ASNase chemoresistance in childhood leukemia. The discovery of ActivinA signaling between BCP-ALL cells and MSCs adds significant insights into the mechanisms of communication in the leukemic niche. Moreover, ActivinA-induced leukemia EVs seem to play a crucial role in sustaining leukemic cells, by conferring them drug resistance. Our data provide a new concept to develop alternative therapeutic strategies that include targeting of the leukemic niche in BCP-ALL.

Dopo che il progetto sul genoma umano è stato completato nell'aprile del 2003, il flusso continuo di nuovi database e dati di sequenziamento ha iniziato a trasformare il campo della genomica in scienza basata sui dati. La bioinformatica analizza i dati sperimentali grezzi con l'obiettivo di ottenere informazioni che descrivono le condizioni biologiche misurate, fornendo così un potente strumento per studiare specifici meccanismi molecolari e genetici. Questa conoscenza deve essere combinata con la genomica per decifrare le interrelazioni tra geni, elementi regolatori, vie metaboliche e interazioni proteiche. L'apprendimento profondo, conosciuto come Deep Learning, e’ una sottodisciplina dell'apprendimento automatico, è stato recentemente applicato al campo della genomica, portando a risultati notevoli. I due obiettivi principali di questo lavoro sono: lo sviluppo e le applicazioni di strumenti bioinformatici che consentano lo studio delle basi genetiche della leucemia linfoblastica acuta e l'uso di tecniche di apprendimento profondo per l'identificazione di piccoli elementi di RNA non codificanti del genoma umano. Questa tesi fornisce al lettore una panoramica completa della recente evoluzione della genomica come campo interdisciplinare di ricerca strettamente connesso con l'informatica e l'analisi dei dati. After the completion of the human genome project in April 2003, the continuous flow of sequencing data and the development of new databases began to transform the field of genomics into data-driven science. Bioinformatics analyses raw experimental data with the aim to obtain information describing biological processes, thus providing a powerful tool to investigate specific molecular and genetic mechanisms. This domain knowledge in combination with genomics allows to decipher the interrelationships between genes, regulatory elements, metabolic pathways, and protein interactions. Deep learning, a subdiscipline of machine learning, has been recently applied to the field of genomics, leading to remarkable results. The two main objectives of this study were: the development and application of bioinformatic tools for the study of the genetic basis of acute lymphoblastic leukaemia, and the usage of deep learning techniques for the identification of small non-coding RNA elements in the human genome. This dissertation provides a comprehensive overview of the recent evolution of genomics as an interdisciplinary field of research strongly associated with computer science and data analysis.

Although most children with acute lymphoblastic leukemia (ALL) receive fractionated total body irradiation (FTBI) as myeloablative conditioning (MAC) for allogeneic hematopoietic stem cell transplantation (allo-HSCT), it is an important matter of debate if chemotherapy can effectively replace FTBI. To compare outcomes after FTBI versus chemotherapy-based conditioning (CC), we performed a retrospective EBMT registry study. Children aged 2–18 years after MAC for first allo-HSCT of bone marrow (BM) or peripheral blood stem cells (PBSC) from matched-related (MRD) or unrelated donors (UD) in first (CR1) or second remission (CR2) between 2000 and 2012 were included. Propensity score weighting was used to control pretreatment imbalances of the observed variables. 3.054 patients were analyzed. CR1 (1.498): median follow-up (FU) after FTBI (1.285) and CC (213) was 6.8 and 6.1 years. Survivals were not significantly different. CR2 (1.556): median FU after FTBI (1.345) and CC (211) was 6.2 years. Outcomes after FTBI were superior as compared with CC with regard to overall survival (OS), leukemia-free survival (LFS), relapse incidence (RI), and nonrelapse mortality (NRM). However, we must emphasize the preliminary character of the results of this retrospective “real-world-practice” study. These findings will be prospectively assessed in the ALL SCTped 2012 FORUM trial.

Monociti e macrofagi sono elementi fondamentali all’interno di una nicchia ematopoietica sia fisiologica che patologica. Infatti, tali cellule possono regolare il mantenimento e il differenziamento delle cellule staminali ematopoietiche e contribuire all’aumento della resistenza delle cellule staminali leucemiche in seguito a trattamento chemioterapeutico. Nella prima parte del progetto, abbiamo testato un nuovo regime di condizionamento in un modello murino poco permissivo (Scid-Beige), ricreando un sistema efficace per lo studio dell’ematopoiesi umana fisiologica e patologica. Combinando l’irradiamento con la Fludarabina, un farmaco immunosoppressivo normalmente utilizzato come sistema di condizionamento in clinica, siamo stati in grado di aumentare l’attecchimento delle cellule umane in topi SCID-beige rispetto al medesimo modello murino condizionato con solo irradiamento. Probabilmente, tale effetto è dovuto alla capacità della Fludarabina di ridurre l’attività immunitaria dei linfociti. Attraverso l’utilizzo di questo modello murino, è stato possibile osservare come, la presenza di monociti, possa aumentare notevolmente l’attecchimento delle cellule staminali ematopoietiche. Infatti, i monociti, in vitro, aumentano la sopravvivenza delle cellule CD34+ e permettono il mantenimento della sottopopolazione CD34+CD38-, sia tramite contatto diretto che attraverso il rilascio di fattori solubili. I monociti classici (CD14+CD16-) sembrano essere i principali responsabili di questo effetto: la presenza di monociti classici in co-coltura con CD34+ permette il mantenimento dello stato indifferenziato e garantisce un corretto attecchimento delle cellule staminali umana in topi NSG. Inoltre, sono stati valutati anche possibili meccanismi d’azione (Notch e COX-2/PGE2) che possono essere implicati in tale processo di mantenimento. In aggiunta, nel presente progetto di tesi, è stata ampiamente investigata l’efficacia del farmaco Asparaginasi (ASNase) nel trattamento di leucemie mieloidi acute (LMA) utilizzando materiale da pazienti alla diagnosi. In primo luogo, è stato considerato l’effetto antileucemico di tale farmaco nei blasti LMA e in particolare nella sottopopolazione dei progenitori (CD34+CD38+ CD34+CD38-), dimostrando che sono intrinsecamente suscettibili al farmaco. Successivamente, abbiamo valutato la tossicità di tale farmaco contro le cellule LMA in relazione ad altri componenti cellulari che possono regolare la sensibilità delle cellule staminali leucemiche al trattamento chemioterapico. Tali cellule includono le mesenchimali stromali e i monociti/macrofagi che sembrano produrre rispettivamente asparaginasi (ASNS) e catepsina B (CTSB), quest’ultima in grado di inattivare ASNase, aumentando la resistenza dei blasti a tale trattamento. Monocytes/macrophages are crucial component of the normal and malignant niche. In fact, they can regulate HSC self-renewal and differentiation and contribute to increase resistance of leukemic cells to chemotherapy. In the first part of the project, we tested the use of a new conditioning regimen in a poor permissive mouse (SCID-beige) to generate an effective xenograft model of human normal and, above all, malignant haematopoiesis. Combining irradiation and Fludarabine, an immunosuppressive drug used in conditioning regimen in clinic, we were able to augment the human engraftment level in SCID-beige mice as compared to irradiation only, probably due to Fludarabine effect on murine leaky lymphocytes. Using this xenograft model, we have observed that when HSC are transplanted in combination with monocytes, there is a massive increment in human haemopoietic engraftment. Moreover, we demonstrated that monocytes increase in vitro CD34+ cell survival and retain CD34+CD38- multipotent stem cells subpopulation, by both contact- and soluble factors-dependent mechanisms. The classical monocyte subset (CD14+CD16-) is primary responsible to maintain HSC survival and prevent their differentiation. In fact, HSC co-cultured with classical monocytes retain their CFC potential and haematopoietic repopulation activity following intravenous injection in NSG mice. Finally, we observed that Notch and COX-2/PGE2 pathways seems to be involved in this crosstalk. In addition, we deeply investigated ASNase effectiveness against AML cells derived from newly diagnosed patients. Firstly, we considered the anti-leukaemic effects on the bulk population and, most importantly, on AML progenitors (clonogenic cells and, especially, CD34+CD38+ CD34+CD38- LSC), showing that they are intrinsically susceptible to ASNase. Secondly, we evaluated the ASNase toxicity against AML in relation with other BM niche cellular components that can regulate the LSC sensitivity to chemotherapy. These cells include mesenchymal stromal cells and monocytes/macrophages that seem to produce respectively ASNS and CTSB, which inactivate ASNase, increasing the resistance to ASNase treatment.

La sindrome nefrosica idiopatica (INS) in età pediatrica è caratterizzata da proteinuria massiva, secondaria alla alterazione della struttura della membrana basale glomerulare (GBM). I pazienti pediatrici affetti da sindrome nefrosica cortico-resistente (SRNS) sono ad elevato rischio di insufficienza renale terminale (ESRD). Sebbene evidenze suggeriscano la presenza di un fattore circolante di permeabilità correlato al sistema immunitario, tutti i precedenti tentativi di comprensione degli esatti meccanismi alla base della patologia sono stati inconclusivi. L'identificazione dei meccanismi alla base della INS e della cortico-resistenza potrebbe favorire lo sviluppo di terapie mirate, in grado di arrestare la progressione verso del danno renale in pazienti con SRNS, nei quali l'ESRD è un evento comune ed in cui è necessario ricorrere a dialisi o trapianto di rene, con elevato rischio di recidiva su rene trapiantato. Questo progetto mira a identificare i meccanismi responsabili della INS in età pediatrica. Durante i 3 anni di progetto, abbiamo a tal fine applicato un approccio clinico e di laboratorio. Abbiamo effettuato uno studio retrospettivo, multicentrico di corte nazionale per identificare la prognosi renale, i fattori di rischio per la recidiva e quelli predittivi di risposta alla terapia post-recidiva, in tutti i pazienti pediatrici sottoposti a trapianto di rene per SRNS in Italia dal 2005 al 2017. Sono stati identificati 101 pazienti. In base ai risultati ottenuti, l'assenza di mutazioni causative per SRNS rappresenta il più importante fattore di rischio per la recidiva nei bambini con SRNS. Il trapianto è invece curativo per le SRNS genetiche. Prolungare il tempo di dialisi prima del trapianto non ha un effetto protettivo sul rischio di recidiva ed è una strategia da non incoraggiare. Il ritrapianto è possibile dopo la perdita di un primo rene trapiantato. Contemporaneamente è stato eseguito uno studio di laboratorio identificando pazienti con differenti forme di SRNS, raccogliendone i sieri e testandoli su un nuovo modello di laboratorio in grado di determinare la permeabilità della albumina sierica bovina (BSA) attraverso un sistema a 3 strati (3LD). I 3 strati comprendono: 1. Podociti umani immortalizzati (HCiPodo), 2. una membrana rivestita di collagene tipo IV e 3. cellule endoteliali glomerulari umane (HCiGEnC). I sieri di 7 pazienti con SRNS recidivata su trapianto (rSRNS), 5 SRNS non recidivate (nrSRNS), e 5 SRNS genetiche (gSRNS) sono stati testati sul sistema. E' stato possibile dimostrare che i sieri di tutte i pazienti con rSRNS aumentano la permeabilità all'albumina nel modello, mentre i sieri dei pazienti con nrSRNS e gSRNS non ne determinano alterazioni significative. Inoltre, la plasmaferesi elimina la capacita dei sieri di incrementare la permeabilità all'albumina nei pazienti rSRNS. Questi risultati confermano l'ipotesi di un fattore circolante di permeabilità responsabile della SRNS nei pazienti recidivati dopo trapianto, senza mutazioni genetiche causative. Se questi risultati verranno confermati su casistiche più ampie, il modello di GBM potrà essere utilizzato per predire la probabilità di recidiva su trapianto, monitorare la terapia ed escludere la presenza di mutazioni genetiche alla diagnosi di INS. Una analisi preliminare di proteomica dei sieri ha evidenziato differenti profili proteici nelle diverse forme con differente attività sul modello di GBM. Idiopathic Nephrotic Syndrome (INS) in children is characterized by protein loss in urine, related to the disruption of the integrity of glomerular filtration barrier (GBM). Children with steroid-resistant INS (SRNS) have a great risk of end-stage renal disease (ESRD). Although evidence suggests the presence of an immune-related circulating factor responsible for glomerular barrier dysfunction, any previous attempt to unravel these mechanisms has been inconclusive. The ability to identify the underlying mechanisms of the disease and/or steroid-resistance would lay the foundations for a potential, much-needed target therapy, since 50% of drug-resistant cases progress to ESRD, requiring renal replacement therapy and may suffer from relapse after renal transplantation. This project aim is to clarify the mechanisms responsible of INS in children. During the 3 years of this PhD project we have applied both a clinical and laboratory approach. We performed a retrospective, multicentre, national cohort study to address the long-term prognosis of renal graft, the risk factors for recurrence and the predictors for response to therapy following recurrence in paediatric patients undergoing renal transplantation because of a SRNS. We identified all children with SRNS who underwent renal transplantation in Italy, between 2005 and 2017. 101 patients were identified. According to our results, the absence of a causative mutation represents the major risk factor for post-transplant recurrence in children with SRNS, while transplant can be curative in genetic SRNS. A prolonged time spent on dialysis before transplantation has no protective effect on the risk of relapse and should not be encouraged. Retransplantation represents a second chance after graft loss for recurrence. At the bench side, we identify patients with different forms of SRNS, collected their sera and tested the samples by means of a novel method which assess the permeability to bovine serum albumin (BSA) through a three-layer device (3LD). The 3 layers comprise: 1. conditionally immortalized human podocytes (HCiPodo), 2. collagen type IV coated porous membrane and 3. human glomerular endothelial cells (HCiGEnC). Seven recurrent SRNS (rSRNS), 5 non-recurrent SRNS (nrSRNS), and 5 genetic SRNS (gSRNS) were collected. Using this model, we were able to demonstrate that sera from all rSRNS patients recurred after transplantation increased albumin permeability, while sera from nrSRNS and gSRNS did not. Furthermore, we also found that the plasmapheresis eliminated the increasing BSA permeability effect in sera from rSRNS patients. These preliminary results confirm the hypothesis that a circulating permeability factors is responsible for SRNS in patients recurring after renal transplantation, without underlying causative gene mutations. If these results will be confirmed on larger cohorts, this GBM model could be a useful tool to predict the probability of disease relapse on transplantation, monitor the therapy or exclude a genetic diagnosis at the INS onset. A preliminary proteomic analysis of sera tested on the GBM model showed different profiles for each category.

La mucopolisaccaridosi di tipo I (MPS-I) è una malattia da accumulo lisosomiale, dovuta alla carenza dell'enzima alfa-L-iduronidasi, determinando il progressivo accumulo di glicosaminoglicani (GAGs) negli organi. Il trapianto di cellule staminali ematopoietiche (HSCT) e la terapia di sostituzione enzimatica (ERT) sono le terapie attualmente disponibili per i pazienti affetti da MPS-I. Nonostante la loro efficacia nel correggere la maggior parte delle manifestazioni cliniche, le alterazioni muscolo-scheletriche e i difetti neurocognitivi influenzano ancora la qualità della vita dei pazienti anche dopo la terapia. Oggigiorno è ampiamente riconosciuto che un intervento precoce è di cruciale importanza per ottenere un migliore outcome clinico. Sulla base di queste osservazioni abbiamo testato l'efficacia terapeutica di HSCT e ERT, da soli o in combinazione, somministrati alla nascita, in un modello murino di MPS-I. Abbiamo dimostrato che tutti e tre i trattamenti sono in grado migliorare i parametri biochimici, ripristinando l’attività enzimatica e riducendo l’accumulo dei GAGs negli organi e nel plasma; in particolare, la terapia combinata ha dimostrato una maggiore efficacia negli organi più difficili da correggere, come i reni e il cuore. Abbiamo osservato solamente una moderata riduzione dell’infiammazione, senza una evidente correzione metabolica nel cervello. Per quanto riguarda invece il fenotipo scheletrico abbiamo riscontrato un effetto differenziale nella riduzione della patologia ossea, con una migliore correzione nei topi trapiantati. I nostri risultati dimostrano che la maggior parte delle manifestazioni patologiche possono essere prevenute, agendo in epoca neonatale, con un trattamento basato sulla combinazione di HSCT e ERT, promuovendo l'idea di un intervento precoce supportato dallo sviluppo di programmi di screening neonatali. Mucopolysaccharidosis type I (MPS-I) is a lysosomal storage disorder due to deficiency in the alpha-L-iduronidase enzyme, resulting in the progressive accumulation of glycosaminoglycans (GAGs) in multiple organs and the consequent multiorgan dysfunction. Hematopoietic stem cell transplantation (HSCT) and enzyme replacement therapy (ERT) are the currently available therapies for MPS-I patients. Despite their effectiveness in correcting most clinical manifestations, muscoloskeletal abnormalities and neurocognitive defects still impact quality of life of treated patients. It is widely accepted that an early intervention may be crucial to obtain a better outcome. On the basis of these observations, we tested the therapeutic efficacy of HSCT and ERT, alone or in combination, administered at birth, in a MPS-I mouse model. We demonstrated that all three treatments were able to prevent biochemical and pathological manifestations of the disease in visceral organs, and, in particular, the combination therapy had a better outcome in difficult-to-treat organs, such as kidney and heart. Of note, transplant procedure can halt the production of antibodies specific for the recombinant enzyme that could reduce ERT efficacy. Regarding the severe skeletal disease, we observed a differential effect in reduction of bone disease, with a significant better correction in transplanted mice, in the presence of ERT or not. Otherwise in the brain, all the treatments provided a moderate decrease of inflammation without an evident metabolic correction. Our findings demonstrate that most manifestations of MPS-I can be prevented by a neonatal treatment based on a combination of HSCT and ERT, promoting the idea of an early intervention supported by the development of newborn screening programs.

La nicchia staminale ematopoietica è formata da molteplici tipi cellulari che contribuiscono alla regolazione dell’ematopoiesi all’interno del midollo osseo. In alcuni disordini ematopoietici il microambiente midollare risulta alterato, sebbene il suo contributo alla patogenesi non sia ancora chiaro, fornisce supporto alle cellule staminali leucemiche (LSC), a spese delle normali cellule staminali ematopoietiche, e protezione contro i chemioterapici. Dunque, per eradicare le LSC, le strategie terapeutiche devono considerare l’effetto protettivo contro i chemioterapici esercitato dal microambiente midollare oltre alla refrattarietà intrinseca delle LSC alle convenzionali terapie. Nella prima parte di questo progetto di dottorato, abbiamo valutato il ruolo delle cellule stromali mesenchimali (MSC) nella patogenesi dell’anemia aplastica (AA). Le MSC derivate dai pazienti (AA-MSC) sono state caratterizzate in vitro e, soprattutto in vivo, utilizzando il modello di nicchia midollare in vivo recentemente sviluppato dal nostro gruppo di ricerca, basato sull’impianto sottocutaneo di pellet cartilaginei derivati dalle MSC (Serafini et al., Stem Cell Research, 2014). In vitro, le AA-MSC mostravano solo un ridotto potenziale clonogenico senza difetti morfologici, fenotipici, proliferativi e differenziativi. Inoltre, le AA-MSC erano in grado di ricreare una nicchia midollare funzionale in vivo dimostrando un’inalterata capacità di supportare l’ematopoiesi e l’assenza di un loro coinvolgimento nella patogenesi della AA. Nella seconda parte di questo dottorato, abbiamo analizzato gli effetti dell’asparaginasi (ASNase) sulle cellule derivate da pazienti affetti da leucemia mieloide acuta (AML), soprattutto sulle LSC, ed il contributo del microambiente alla resistenza ai chemioterapici. L’ASNase era egualmente efficace contro il bulk delle cellule AML e contro le LSC (CD34+CD38- e CD34+CD38+), mentre l’effetto nei confronti delle cellule ematopoietiche sane era trascurabile. L’azione del farmaco contro le cellule primitive di AML è stato ulteriormente confermato in saggi clonogenici ed in esperimenti effettuati in condizioni di coltura specifiche per il mantenimento delle LSC in vitro. Tuttavia, una nicchia midollare protettiva formata dalle MSC, che esprimono asparagina sintetasi, dai monociti/macrofagi e dai blasti leucemici stessi, che esprimono catepsina B, potrebbe ridurre il potenziale anti-neoplastico del farmaco. Infine, abbiamo testato un nuovo regime di condizionamento (somministrazione di fludarabina in aggiunta all’irraggiamento) in topi SCID-beige, un ceppo poco permissivo per l’engraftment umano. Tale strategia di condizionamento ha incrementato i livelli di engraftment della linea cellulare AML, KG-1, rispetto al solo irraggiamento, e ha permesso di ottenere l’engraftment nel 50% dei blasti AML primari trapiantati. In futuro, vorremmo combinare questo nuovo modello di xenograft così ottenuto con il trapianto di pellet cartilaginei per generare un modello di nicchia AML da poter usare per studiare il microambiente midollare leucemico e per testare promettenti agenti terapeutici contro l’AML, come l’ASNase. The haematopoietic stem cell (HSC) niche is formed by several cell types which contribute to the regulation of the haematopoiesis within the bone marrow (BM). In pathological conditions, alterations of the BM microenvironment have been reported, but it is still debated whether they are cause or consequence of the disease. Support to leukaemic cells, especially to leukaemic stem cells (LSC), at the expense of normal HSC and protection against chemotherapeutic agents characterise the malignant BM niche. Thus, in order to eradicate LSC, promising therapeutic strategies may consider the chemoprotection exerted by the BM microenvironment in addition to LSC intrinsic features of refractoriness to conventional therapies. In the first part of this PhD project, we investigated the role of mesenchymal stromal cells (MSC) in the pathogenesis of aplastic anaemia (AA). Patient-derived MSC (AA-MSC) were characterised in vitro and, especially, in vivo taking advantage of our recently described in vivo BM niche model, based on the subcutaneous implantation of cartilaginous pellets generated from MSC (Serafini et al., Stem Cell Research, 2014). AA-MSC did not exhibit morphological, phenotypical, proliferative and differentiation defects in vitro. Only a reduced clonogenic potential was observed. Most importantly, they were able to recreate a functional and complete BM niche in vivo, proving their unaltered ability to support haematopoiesis and excluding their involvement in AA pathogenesis. In the second part of this PhD project, we analysed the effects of L-asparaginase (ASNase) on acute myeloid leukaemia (AML) cells, especially on LSC, and the contribution of the microenvironment to chemoresistance. ASNase was equally effective against unfractionated AML cells and against CD34+CD38- and CD34+CD38+ LSC, while slightly affecting healthy haematopoietic cells. The action of the drug against AML primitive cells was confirmed by clonogenic assays and by experiments performed in LSC supportive culture conditions. However, the anti-leukaemic potential of ASNase could be in part counteracted by MSC, expressing asparagine synthetase, and by monocytes/macrophages and blasts themselves, expressing cathepsin B, generating a protective niche. Lastly, we tested a new conditioning regimen based on the addition of fludarabine to irradiation in SCID-beige mice, a poorly permissive strain for human engraftment. The administration of fludarabine after irradiation increased the levels of engraftment of a AML cell line, KG-1, as compared to irradiation only, and it allowed the engraftment in 50% of primary AML blasts transplanted, reproducing human leukaemic infiltration. Thus, we generated a novel AML xenograft model in which we would like to test promising therapeutic agents, as ASNase, and to study different features of the malignant BM niche.

La Leucemia Mieloide Acuta (LMA) è ancora associata ad alti tassi di ricaduta in seguito al trattamento con le terapie convenzionali che comprendono la chemioterapia e il trapianto di cellule staminali ematopoietiche. Da qui la necessità di identificare nuove strategie terapeutiche. L’immunoterapia con linfociti T ingegnerizzati per esprimere recettori chimerici artificiali (CARs) in grado di riconoscere specifici antigeni tumorali ha mostrato risultati sorprendenti in particolare nel contesto leucemie linfoblastiche di tipo B, suscitando un vivo interesse nell'estendere questo approccio anche ad altre neoplasie ematologiche come la LMA. Tra le molecole di superficie identificate nell’ambito della LMA, il CD33 e CD123 (la subunità α del recettore dell’IL-3), essendo ampiamente espressi sia dai blasti leucemici che dalle cellule staminali leucemiche, rappresentano dei buoni antigeni bersaglio per lo sviluppo di terapie CAR mediate. Il mio progetto di dottorato è stato dunque incentrato sulla caratterizzazione di cellule killer indotte da citochine (CIK) geneticamente modificate attraverso il sistema non virale “Sleeping Beauty” per esprimere CARs specifici per il CD123 e il CD33 quale strategia terapeutica per il trattamento della LMA. Nel contesto del targeting dell’antigene CD123, abbiamo focalizzato la nostra attenzione sull’ effetto di diverse variabili coinvolte nel CAR design, e note per la loro capacità di modulare i profili funzionali delle cellule CAR T, come l'affinità di legame e l'espressione del CAR in relazione alla densità dell'antigene bersaglio. Infatti, mentre l'effetto "off-target" associato al targeting del CD33 è per lo più limitato al compartimento ematologico, il targeting del CD123 richiede un livello più alto di cautela, a causa del potenziale riconoscimento da parte del CD123.CAR del tessuto endoteliale che esprime il CD123 a bassi livelli. Utilizzando il nostro modello in vitro siamo stati in grado di definire sia soglie di antigene "litiche" che di "attivazione" dimostrando che, mentre l'attività citotossica precoce non è influenzata né dall'espressione del CAR né dalla modifica dell’affinità del CAR, l’ espressione del CAR rappresenta la variabile principale che incide sulle funzioni effettrici. Complessivamente, la completa caratterizzazione di tutte queste variabili ha fornito ulteriori informazioni per la corretta progettazione di un CAR anti-CD123 per il trattamento della LMA che possa garantire un corretto equilibrio tra i profili di efficacia e sicurezza. In parallelo, una corretta valutazione preclinica di queste nuove terapie richiede anche un'accurata valutazione dell'efficacia, in particolare considerando le complessità intrinseca alla LMA, come l'eterogeneità e il microambiente mieloide immunosoppressivo. Pertanto, per quanto riguarda il targeting del CD33, abbiamo studiato i profili di efficacia di cellule CD33.CAR CIK da sole e in combinazione con agenti chemioterapici convenzionali, sfruttando un modello di xenotrapianto abbinato a chemioterapia per testare le cellule CD33.CAR-CIK nei confronti di cellule di LMA resistenti / residue alla chemioterapia. Abbiamo scoperto che le cellule CD33.CAR CIK da sole mostrano una potente attività anti-leucemica in vitro e in vivo, riducendo in modo significativo lo sviluppo della LMA quando somministrate come approccio di trattamento precoce, e ritardando la progressione di LMA in topi con una malattia conclamata. Inoltre, dati preliminari hanno mostrato che le cellule CD33.CAR-CIK sono ancora in grado di eliminare le cellule di LMA nei topi con recidive dopo la chemioterapia. Acute Myeloid Leukemia (AML) is still associated with high relapse rates when treated with conventional chemotherapeutic and hematopoietic transplantation regimens. Thus, new treatment options are urgently needed. Immunotherapy adopting T cells engineered to express tumor-directed Chimeric Antigen Receptors (CARs) has shown striking results particularly in the context of B-cell malignancies, sparking a keen interest in extending this approach also to other hematological malignancies such as AML. Among the surface molecules identified, the CD33 and CD123 (IL-3 receptor α subunit) molecules have emerged as the main validated targets in AML and, being broadly expressed on both AML blasts and leukemic stem cells (LSCs), represent suitable antigens to be targeted with CAR-T cells. My PhD project has been focused on the characterization of non-viral Sleeping-Beauty engineered Cytokine-Induced Killer (CIK) cells with both anti-CD123 and CD33 CARs as a potential tool for the treatment of AML. In the context of CD123 targeting, we focused our attention on dissecting the effect of several variables involved in the CAR design, known to modulate CAR T-cell functional profiles, such as CAR binding affinity and expression in relation to the target antigen density. Indeed, while the “on target-off tumor” effect associated to the CD33 targeting is mostly limited to the hematological compartment, the CD123 targeting demands a higher level of caution, due to the potential recognition of low CD123-positive endothelial tissue. By using our model we were able to define both “lytic” and “activation” antigen thresholds showing that, while the early cytotoxic activity is not affected either by CAR expression or by CAR affinity tuning, the CAR expression represents the main variable impacting on later effector functions. Overall, the full dissection of all these variables offers additional knowledge for the proper design of a suitable anti-CD123 CAR for the treatment of AML which can grant a proper balance between efficacy and safety profiles. In parallel, a proper preclinical assessment of novel therapies also demands for accurate efficacy evaluation, particularly considering the AML disease complexity, such as the heterogeneity and the immunosuppressive myeloid microenvironment. Thus, regarding the CD33 targeting, we investigated the efficacy profiles of CD33.CAR CIK cells alone and in combination with conventional chemotherapeutic agents by exploiting a xenograft chemotherapy model to examine the CD33.CAR-CIK cell contribute in the elimination of the chemotherapy resistant/residual AML cells. We found that CD33.CAR CIK cells alone exhibited a potent anti-leukemic activity in vitro and in vivo, significantly reducing AML development when administered as an early treatment approach and delaying the progression of established disease in mice. Moreover, preliminary data showed that CD33.CAR-CIK cells were still capable to target chemotherapy resistant/residual AML cells in mice experiencing disease recurrence after chemotherapy.

La traslocazione t(12;21) è il riarrangiamento cromosomico più frequente nei tumori pediatrici e si associa esclusivamente alla leucemia linfoblastica acuta (LLA) a precursori B. La traslocazione avviene in utero nelle cellule staminali-progenitrici ematopoietiche ma è insufficiente per la leuchemogenesi, poiché il gene di fusione ETV6-RUNX1 (E/R) che ne deriva genera un clone pre-leucemico clinicamente silente; mutazioni secondarie sono quindi necessarie per completare la trasformazione. Queste ultime avvengono nel periodo post-natale verosimilmente in seguito ad una risposta immunitaria disregolata a infezioni /infiammazioni comuni. Le recidive E/R+ non sono molto frequenti ma è plausibile che siano determinate dall’accumulo di nuove mutazioni nel clone pre-leucemico chemioresistente. In passato abbiamo dimostrato che TGFβ, una citochina prodotta durante l’infiammazione, limita la proliferazione delle cellule normali pro-B mentre favorisce il clone pre-leucemico che è insensibile al suo effetto; in più, TGFβ seleziona cellule pre-leucemiche staminali in progenitori CD34+ derivati da sangue cordonale umano. Abbiamo anche precedentemente dimostrato che cellule pro-B murine E/R+ mostrano alterazioni in molecole di adesione e nella migrazione verso CXCL12, suggerendo un loro possibile comportamento anomalo all’interno della nicchia midollare. Le cellule mesenchimali stromali (MSC) sono regolatori chiave sia delle cellule ematopoietiche nella nicchia che dell’infiammazione. È stato inoltre dimostrato che alterazioni delle MSC attivano pathways infiammatori nelle cellule staminali/progenitrici del sangue promuovendo la loro trasformazione a leucemia mieloide acuta in sindromi genetiche predisponenti. Grazie a due modelli cellulari esprimenti E/R (la linea cellulare murina proB Ba/F3 e cellule CD34+ di sangue cordonale umano), il presente studio dimostra che le cellule pre-leucemiche sono avvantaggiate dalla copresenza di MSC e infiammazione in termini di migrazione, sopravvivenza e potenziale progressione. Ba/F3 E/R+ mostrano un profilo di espressione genica pro-infiammatorio, con una particolare signature migratoria e pro-mieloide. Inoltre, sia Ba/F3 che CD34+ esprimenti E/R migrano di più verso i surnatanti di MSC infiammate rispetto alle cellule controllo; nel primo caso, la migrazione dipende dal recettore CXCR2. Molto importante, Ba/F3 E/R+ sono favorite rispetto al controllo quando coltivate su MSC e infiammazione, poiché questa condizione diminuisce molto proliferazione e sopravvivenza delle Ba/F3 normali mentre ha effetti minori o assenti sulle pre-leucemiche. Il vantaggio è mediato da fattori solubili, ma né TGFβ né CXCR2 sono implicati. In aggiunta, MSC e infiammazione aumentano il danno genotossico sia nelle Ba/F3 controllo che E/R+, come indicato dagli aumentati livelli di fosforilazione dell’istone H2AX e di espressione dell’enzima AID, il quale è stato dimostrato favorire la transizione da pre-leucemia a leucemia E/R+. Tuttavia, mentre le Ba/F controllo vanno incontro ad apoptosi, le pre-leucemiche resistono accumulando danni genetici che possono favorirne la trasformazione. Infine, infiammazione e MSC cooperano nel far emergere il compartimento CD34+IL7R+ all’interno della popolazione di CD34+ pre-leucemiche, mentre sfavoriscono quello della popolazione normale. Questa osservazione è particolarmente importante alla luce del fatto che tale compartimento rappresenta lo stadio dell’ematopoiesi fetale che è suscettibile all’azione pre-leucemica di E/R. Concludendo, il presente lavoro dimostra che cellule mesenchimali stromali di midollo osseo e infiammazione cooperano nel favorire la persistenza e la possibile progressione del clone pre-leucemico ETV6-RUNX1+. L’elucidazione dei meccanismi che sottendono tale azione favorente potrebbe fornire strategie innovative per l’eradicazione del clone pre-leucemico chemioresistente. Translocation t(12;21) is the most frequent chromosomal rearrangement in pediatric cancers, exclusively leading to B-cell Precursors Acute Lymphoblastic Leukemia (BCP-ALL). Translocation occurs in utero in stem-progenitor cells (HSPC) but it is insufficient for leukemogenesis, since the consequent ETV6-RUNX1 (E/R) fusion gene only generates a silent B-progenitor pre-leukemic clone; additional mutations are thus required for transformation. These latter occur in the post-natal period likely due to dysregulated immune response to common infections/inflammation. Our published data demonstrated that TGFβ, a cytokine produced during inflammation, limited the proliferation of normal pro-B and cells while favoring the insensitive E/R+ clone; moreover, TGFβ selected putative pre-leukemic stem cells (preLSC) in umbilical cord blood (UCB) CD34+ progenitors transduced with the oncogene. On the other hand, we previously showed that ETV6-RUNX1+ murine B-progenitors were altered in adhesion molecules expression and CXCL12-directed migration, suggesting possible dysregulated interactions within the bone marrow (BM) niche. Mesenchymal Stromal Cells (MSC) are key regulators of both HSPC and inflammation in the niche. Importantly, it has been shown that mesenchymal inflammation promotes secondary myeloid leukemia in predisposing syndromes by increasing DNA damage in HSPC, while MSC/BCP-ALL blasts cross-talk profoundly modifies cytokine and chemokine signalling within the niche excluding normal hematopoiesis in favor of leukemia. Taking advantages from two ETV6-RUNX1-expressing cell models (Ba/F3, a murine pro-B cell line, and ETV6-RUNX1-expressing human UCB-CD34+ progenitors) the present PhD study demonstrates that ETV6-RUNX1-expressing cells take advantage from mesenchymal inflammation in terms of migration, persistence and potential progression. In particular, we have found that pre-leukemic Ba/F3 show a peculiar pro-inflammatory gene expression profile characterized by a marked migratory and myeloid signature. Concordantly, ETV6-RUNX1+ Ba/F3 and CD34+ cells preferentially migrate toward inflamed compared to unstimulated BM-MSC supernatants; in case of the first, migration is CXCR2-dependent. Moreover, ETV6-RUNX1+ Ba/F3 are favored compared to controls in presence of BM-MSC and inflammatory cytokines, as they decrease normal cells proliferation and survival while minimally affecting pre-leukemic cells. The effect is mediated by soluble factors, but neither TGFβ nor CXCR2 axis are implicated. Importantly, the inflamed mesenchymal niche increases genotoxic stress in both control and E/R+ Ba/F3, as indicated by high levels of H2AX phoshorilation, as well as transcription of the activation-induced cytidine deaminase (AID) enzyme (which is implicated in ETV6-RUNX1+ pre-leukemia to leukemia transition). However, while control cells go through apoptosis, pre-leukemic Ba/F3 are resistant to this fail-safe mechanism, increasing chance to accumulate secondary mutations and malignantly transform. Finally, an inflamed MSC favor the emergence of CD34+ILR7+ compartment within ETV6-RUNX1+ UCB-CD34+ population while decreasing its frequency in the normal counterpart; of note, such differential effect doesn’t occur in case of unstimulated MSC. This observation is particularly important as the CD34+ILR+ compartment seems to represent the critical developmental stage during early fetal hematopoiesis for ETV6-RUNX1 pre-leukemic activity. Concluding, our work demonstrated that BM-MSC and inflammation cooperate in favoring the persistence and transformation of ETV6-RUNX1+ pre-leukemic clone. Elucidating mechanisms that underlay such promoting action could provide novel strategies for the pre-leukemic clone eradication.

La Leucemia Linfoblastica Acuta (LLA) è il più comune tumore in età pediatrica. La percentuale di successo terapeutico è del 90%, tuttavia la recidiva si può presentare in quasi il 20% dei casi per l’insorgenza di meccanismi di resistenza farmacologica. Lo studio della “nicchia leucemica" ha permesso di individuare potenziali bersagli molecolari grazie allo studio delle modalità con cui le cellule stromali mesenchimali (MSC), riprogrammate dalle cellule leucemiche, conferiscono supporto e chemio resistenza. In questo lavoro ci siamo focalizzati sul ruolo di Attivina A, molecola della superfamiglia del TGF-β, nell’interazione tra cellule leucemiche e stroma midollare. Recentemente Attivina A è stata identificata in diversi tipi di tumori solidi come molecola capace di promuovere migrazione e invasione, le quali possono favorire la progressione tumorale. Il presente lavoro definisce per la prima volta il ruolo di Attivina A come molecola chiave nell’interazione tra cellule leucemiche e MSC capace di contribuire alla generazione di un microambiente in grado di sostenere la progressione leucemica. Attivina A è altamente espressa nel midollo dei pazienti LLA-B e, nello specifico, tale molecola è prodotta all’interno del midollo leucemico nelle aree infiltrate dai blasti leucemici. Inoltre, la stimolazione da parte di blasti leucemici e di molecole pro-infiammatorie, tipiche del microambiente midollare leucemico, è in grado di aumentare la produzione di Attivina A nelle cellule mesenchimali stromali. Tale molecola agisce principalmente regolando i meccanismi di motilità cellulare e di omeostasi dello ione calcio. Sia l’analisi dell’espressione genica sia i saggi funzionali hanno messo in luce la capacità di Attivina A di promuovere nelle cellule leucemiche i processi di migrazione e di invasività, in presenza o meno di un fattore chemotattico, quale CXCL12. Tale molecola è fondamentale nella localizzazione delle cellule staminali ematopoietiche e delle cellule leucemiche all’interno della nicchia midollare. Pertanto, la ridotta espressione di CXCL12 nel midollo leucemico insieme all’aumentata espressione di Attivina A, associata sia ad un aumento della sensibilità delle cellule leucemiche a CXCL12 sia a una riduzione della risposta delle cellule staminali sane a CXCL12, possono essere considerati come meccanismo di persistenza delle cellule leucemiche all’interno della nicchia, a discapito dell’ematopoiesi sana. Il meccanismo di azione di Attivina A non dipende dalla regolazione dell’espressione dei recettori di CXCL12, CXCR4 e CXCR7, ma è mediato dall’incremento dei livelli di calcio intracellulare, il quale ha un ruolo chiave nella riorganizzazione citoscheletrica e nel movimento cellulare. Inoltre, è possibile identificare un effetto a lungo termine determinato dalla regolazione di geni associati ad assi molecolari, quali Ras e PI3K/AKT, e a meccanismi di regolazione del calcio, i quali possono spiegare l’effetto pro-migratorio di Attivina A. Infine, il modello in vivo di malattia ha permesso di confermare il ruolo pro-invasivo di Attivina A. Complessivamente, questo lavoro definisce il meccanismo con cui Attivina A è in grado di conferire un vantaggio alle cellule leucemiche, tale da renderle in grado di generare una nicchia maligna a discapito dell’ematopoiesi sana. Pertanto, in futuro, tale meccanismo potrebbe essere bersagliato con nuovi approcci terapeutici per il trattamento della LLA-B. Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy. About 80% of the cases originates from precursor B cells (BCP-ALL), which abnormally accumulate as a consequence of genetic alterations associated to differentiation inhibition and abnormal expansion. Despite cure rates exceeding 90% in children, relapse still occurs in about 20% of patients because of the emergence of therapy resistance. In the last years, an increasing number of new molecular targets is emerging from studies on the “leukemic niche” where stromal cells and, in particular, mesenchymal stromal cells (MSCs), could confer support and chemoprotection to leukemic cells. This pro-tumoral effect is the consequence of the corruption of the bone marrow (BM) stroma operated by leukemic cells. In present work, we focused our attention on stroma-derived Activin A, a member of the TGF-β superfamily, well known for its role as a regulator of immune system but also for its tumor-promoting role in several type of solid cancers. Here we found that Activin A could be considered a new crucial factor exploited by leukemic cells to create a self-reinforcing microenvironment: indeed, Activin A was highly expressed in BM plasma of BCP-ALL patients, compared to healthy donors, and its expression was specifically confined to leukemic infiltrated area. In particular, we demonstrated that the engagement of bone marrow stromal cells by leukemic blasts, along with a pro-inflammatory milieu, that characterized the leukemic microenvironment, enhanced Activin A secretion. The main effect exerted by this molecules was the regulation of cell motility associated pathways: indeed, both gene expression analysis and functional assays for evaluation of cell motility, chemotaxis and invasion revealed the ability of Activin A to promote basal and CXCL12-driven migration and invasion of leukemic cells, while it impaired CXCL12-induced migration of healthy CD34+. We found out, according to previous published data, a strong reduction in CXCL12 levels in BM plasma of BCP-ALL patients. It is worth to notice that Activin A was able to increase the responsiveness of leukemic cells even to very low levels of CXCL12. Overall these data suggest a possible mechanism by which leukemic cells could persist in the BM niche, concomitantly displacing healthy heamatopoiesis. Activin A did not alter the expression of CXCL12 receptors, CXCR4 and CXCR7, however it was able to increase both the intracellular calcium content and actin polymerization in leukemic cells. Since Ca2+ signaling is intrinsically related to actin cytoskeletal reorganization and effective cell movement, we hypothesized that Activin A enhanced migration could be ascribed to a calcium-dependent mechanism. Moreover, long-lasting effect of Activin A could be mediated by the regulation of genes associated with Ras and PI3K/AKT pathways as well as calcium homeostatic mechanisms. These molecules may account for the pro-migratory and pro-invasive effects of Activin A. Finally, in vivo model further supported in vitro data about Activin A pro-migratory and pro-invasive role. Overall, our data suggest that Activin A could confer a migratory advantage to leukemic cells to generate a malignant BM niche at the detriment of the healthy hematopoiesis. Therefore, our work will pave the way for the design of new therapeutically approaches for BCP-ALL treatment aimed to target the interplay between leukemic cells and BM niche.

Lo scopo di questo progetto è stato lo sviluppo preclinico di una nuova terapia per la Mucopolisaccaridosi di tipo I (MPS-I): il trapianto di cellule staminali da sangue del cordone ombelicale, eseguito in epoca neonatale. L’MPS-I è una rara malattia lisosomiale dovuta a mutazioni nel gene IDUA, che codifica per l’enzima lisosomiale α-L-iduronidasi (IDUA). L’assenza di attività iduronidasica provoca l’accumulo di glicosaminoglicani nei tessuti, causando una disfunzione multi-organo progressiva e, in particolare, gravi anomalie scheletriche. La terapia d’elezione per la MPS-I è il trapianto di cellule staminali ematopoietiche (HSCT) da donatore sano, che riduce l’accumulo dei substrati e attenua molte manifestazioni cliniche, ma non è del tutto efficace sulle anomalie ossee. L’obiettivo di questa tesi è stato quello di testare nel modello murino di MPS-I l’efficacia di una nuova strategia trapiantologica, che combinava l’intervento precoce (neonatale) e l’uso del sangue del cordone ombelicale come fonte. Infatti, è stato deciso di trattare gli animali nel periodo peri-natale, per prevenirne le manifestazioni fenotipiche, e di impiegare il sangue cordonale come fonte di cellule da trapiantare, perché il trapianto cordonale in clinica dà diversi vantaggi rispetto al trapianto di midollo osseo, in particolare in questi pazienti. Il primo risultato ottenuto in questo lavoro è stata la caratterizzazione delle proprietà fenotipiche e funzionali delle cellule cordonali murine. E’ stato poi eseguito il trapianto di tali cellule in topi adulti wild type (WT) C57BL/6: si è ottenuto un buon attecchimento a lungo termine ed è stata verificata la presenza di cellule di origine del donatore in tutti i lineages ematopoietici. Infine, è stato valutato l’esito del trapianto neonatale di cellule cordonali nei topi MPS-I: si è ottenuto un aumento di attività IDUA negli organi periferici dei topi MPS-I con attecchimento elevato (>50%), e una conseguente riduzione dei livelli di glicosaminoglicani. Il fenotipo scheletrico dei topi a 20 settimane d’età è stato dettagliatamente descritto tramite indagini radiografiche, microCT e istologiche. Le radiografie hanno rivelato che, mentre nei topi MPS-I non trattati si aveva un generale ispessimento osseo rispetto ai WT, nei topi affetti con elevato attecchimento questa anomalia non si riscontrava. Le analisi microCT e istologiche hanno dimostrato che la porzione corticale del femore dei topi MPS-I presentava irregolarità che erano invece ridotte nei topi trattati con alto attecchimento. Questi dati confermano che il trapianto neonatale di cellule del cordone ombelicale risulta una terapia efficace nel modello murino di MPS-I. In collaborazione con il Prof. Aiuti presso l’Ospedale San Raffaele-Tiget, è stato intrapreso un nuovo progetto di ricerca con lo scopo di correggere geneticamente le cellule di cordone ombelicale per ottenere livelli sovra-fisiologici di espressione dell’enzima IDUA. L’obiettivo è quello di trasdurre cellule di cordone murino MPS-I con un vettore lentivirale PGK-IDUA e di trapiantare le cellule così corrette in topi MPS-I neonati. Verificheremo se sarà possibile ottenere nei riceventi livelli di attività enzimatica più alti che nel trapianto di cellule WT, e se l’esito della terapia genica sarà quindi ancora migliore. Finora, abbiamo sviluppato una procedura per isolare dal sangue cordonale murino le cellule ematopoietiche staminali e progenitrici, che sono state in grado di ripopolare topi C57BL/6 adulti e neonati. A questo punto verranno effettuate le prime prove di infezione con vettori lentivirali GFP e IDUA su tali cellule, per trovare le migliori condizioni per il loro trapianto nei neonati MPS-I. Questi risultati potrebbero aprire la strada verso lo sviluppo di un approccio di terapia genica neonatale con cellule di cordone ombelicale geneticamente corrette nei pazienti MPS-I. The aim of this PhD project was the preclinical testing of a new possible therapeutic option for Mucopolysaccharidosis type I (MPS-I): the transplantation of umbilical cord blood (UCB) in neonatal age. MPS-I is a rare lysosomal disease due to mutations in the IDUA gene, which encodes for the lysosomal enzyme α-L-iduronidase (IDUA). The absence of IDUA activity leads to the accumulation of glycosaminoglycans (GAGs) in patients’ tissues, which causes a progressive multi-organ dysfunction, with a wide spectrum of skeletal anomalies. The first-choice therapy for MPS-I is hematopoietic stem cell transplantation (HSCT) from healthy donor, because it reduces the accumulation of substrates and it solves many clinical symptoms, but this treatment is not very effective on the skeletal defects. The aim of this thesis was to test in the murine model a novel transplantation strategy for MPS-I, combining early (neonatal) intervention and the use of murine UCB as a source. Indeed, we decided to treat our mouse model at early age, in order to prevent the anomalies, and to employ UCB cells (UCBCs) as a source for transplantation, because UCB transplantation (UCBT) has shown advantages over bone marrow transplantation in patients suffering from inherited metabolic disorders. The first result of this work was the characterization of the phenotypical and functional properties of murine hematopoietic UCBCs compared to adult bone marrow cells. Then, adult wild-type (WT) C57BL/6 mice were transplanted with UCBCs, and they reached good long-term engraftment levels, with the presence of cells of donor origin among all the hematopoietic lineages. Finally, the outcome of UCBT on neonatally-transplanted MPS-I mice was evaluated: an increase of IDUA activity was evident in the peripheral organs of high-engrafted MPS-I mice (engraftment >50%), and, consequently, GAG levels reduced. An extensive characterization of the skeletal phenotype was performed by radiographs, microCT, and histology. Radiographic images showed that MPS-I untreated mice had increased radio-opacity and diameter of the bones at 20 weeks of age, compared to WT, and these parameters were reduced in high-engrafted mice. MicroCT scans and histomorphometry revealed that the cortical region of MPS-I femurs appeared irregular, and returned to normal in treated mice with high-engraftment, confirming the benefit of neonatal UCBT on MPS-I mice. In collaboration with Alessandro Aiuti’s group at Ospedale San Raffaele-Tiget (Telethon Institute for gene therapy), a new project started with the aim of gene-correcting murine UCBCs to obtain a supra-physiological expression of IDUA enzyme. The goal is to transduce MPS-I murine UCBCs with a PGK-IDUA lentiviral vector, and to transplant gene-corrected cells into MPS-I newborns. We will verify if we can obtain in the recipients higher levels of IDUA activity than transplanting WT UCBCs, and if the outcome of gene therapy could be better than the one obtained with normal UCBCs. A method was developed to isolate hematopoietic stem and progenitor cells (HSPCs) from murine UCB and to culture them for lentiviral infection. WT untransduced UCB-HSPCs engrafted WT adult and newborn mice at high levels. We are now in the process of testing the GFP and IDUA vector on MPS-I UCBCs, to set the best conditions for their transplantation in MPS-I neonates. These results will hopefully pave the way for developing a neonatal gene therapy approach with lentivirally-corrected UCB cells in MPS-I babies.
