Cryptocurrencies have become a global phenomenon and its trading volume has been increasing since 2017 Aloosh and Li (2019). However, cryptocurrencies have been accused of market manipulation in the past. Benford’s law is widely used for detecting probability of frauds and manipulation in various fields. This study applied Benford’s law on trade volume of cryptocurrencies. Chi- square statistics revealed that except for Cardano and USDT all the other cryptocurrencies did not conform to the distribution and reveals the dataset could have been manipulated. This method may be used as the pre-requisite before doing fine-grain screening such as machine learning and graph-based searching.

Peripheral intravenous catheters (PIVCs) are among the most common invasive devices used in hospitalized patients, with over 300 million sold in the United States each year. However, about one-fourth of PIVCs are left in situ with no prescriber orders for IV medications or solutions, "just in case" they might be needed. PIVC insertion can be painful, is often unnecessary, and may increase a patient's risk of developing a bloodstream infection. This article reviews the evidence for the appropriate use of short PIVCs in hospitalized patients, assesses the ongoing need for PIVCs, provides recommendations for alternative options, and argues for promptly removing a PIVC that is no longer in use.

We thank Dr. Biffi and colleagues for recognizing the robust methodology used to inform the Infusion Therapy Standards of Practice (the Standards).1 Biffi et al. critiqued the use of Committee Consensus (Table 1) to inform a clinical practice with significant influence on patient safety. The Committee Consensus rating provides expert opinion to guide practices of concern where higher levels of evidence are lacking. The foundation of the Standards is vascular preservation, irrespective of patient care settings, and sustained through careful evaluation and management of optimal vascular access by competent clinicians.

Microbes evolve rapidly by modifying their genome through mutations or acquisition of genetic elements. Antimicrobial resistance in Helicobacter pylori is increasingly prevalent in India. However, limited information is available about the genome of resistant H. pylori isolated from India. Our pan- and core-genome based analyses of 54 Indian H. pylori strains revealed plasticity of its genome. H. pylori is highly heterogenous both in terms of the genomic content and DNA sequence homology of ARGs and virulence factors. We observed that the H. pylori strains are clustered according to their geographical locations. The presence of point mutations in the ARGs and absence of acquired genetic elements linked with ARGs suggest target modifications are the primary mechanism of its antibiotic resistance. The findings of the present study would help in better understanding the emergence of drug-resistant H. pylori and controlling gastric disorders by advancing clinical guidance on selected treatment regimens.

Helicobacter pylori infection in stomach leads to gastric cancer, gastric ulcer, and duodenal ulcer. More than 1 million people die each year due to these diseases, but why most H. pylori-infected individuals remain asymptomatic while a certain proportion develops such severe gastric diseases remained an enigma. Several studies indicated that gastric and intestinal microbiota may play a critical role in the development of the H. pylori-associated diseases. However, no specific microbe in the gastric or intestinal microbiota has been clearly linked to H. pylori infection and related gastric diseases. Here, we studied H. pylori infection, its virulence genes, the intestinal microbiota, and the clinical status of Trivandrum residents (N = 375) in southwestern India by standard H. pylori culture, PCR genotype, Sanger sequencing, and microbiome analyses using Illumina Miseq and Nanopore GridION. Our analyses revealed that gastric colonization by virulent H. pylori strains (vacAs1i1m1cagA+) is necessary but not sufficient for developing these diseases. Conversely, distinct microbial pools exist in the lower gut of the H. pylori-infected vs. H. pylori-non-infected individuals. Bifidobacterium (belonging to the phylum Actinobacteria) and Bacteroides (belonging to the phylum Bacteroidetes) were present in lower relative abundance for the H. pylori+ group than the H. pylori- group (p < 0.05). On the contrary, for the H. pylori+ group, genus Dialister (bacteria belonging to the phylum Firmicutes) and genus Prevotella (bacteria belonging to the phylum Bacteroidetes) were present in higher abundance compared to the H. pylori- group (p < 0.05). Notably, those who carried H. pylori in the stomach and had developed aggressive gastric diseases also had extremely low relative abundance (p < 0.05) of several Bifidobacterium species (e.g., B. adolescentis, B. longum) in the lower gut suggesting a protective role of Bifidobacterium. Our results show the link between lower gastrointestinal microbes and upper gastrointestinal diseases. Moreover, the results are important for developing effective probiotic and early prognosis of severe gastric diseases.

This study examined the association between neighborhood characteristics, stigma related to mental illness reported by local community members, and measures of perceived stigma and community participation among individuals with psychiatric disabilities living in independent scattered-site housing or in congregate housing in three neighborhoods in the New York City metropolitan area.Neighborhood characteristics were drawn from the 2010 U.S. Census. Surveys focusing on attitudes and intended behavior toward people with mental illness were administered to 608 general community members, and clinical interviews were conducted with 343 persons with psychiatric disabilities.Of neighborhood characteristics, both greater socioeconomic disadvantage and more "suburban values" (lower housing density and greater political conservativism) predicted more perpetrated stigma reported by community members. There was no significant relationship between stigma reported by community members and perceived stigma among participants with psychiatric disabilities. Community stigma predicted vocational involvement and demonstrated interaction effects with housing, such that persons living in congregate housing demonstrated more community participation in communities with more stigma, whereas persons living in scattered-site housing demonstrated less participation in these communities. Perceived stigma was significantly negatively related to community participation.Findings suggest that effects of neighborhood characteristics and community stigma on people with psychiatric disabilities are complex and are partly conditioned by housing context.

Background A consensus on a central line-associated bloodstream infection (CLABSI) surveillance definition in home infusion is needed to standardize measurement and benchmark CLABSI to provide data to drive improvement initiatives Methods Experts across fields including home infusion therapy, infectious diseases, and healthcare epidemiology convened to perform a 3-step modified Delphi approach to obtain input and achieve consensus on a candidate home infusion CLABSI definition. Results The numerator criterion was identified by participants as involving one of the 2 following: (1) recognized pathogen isolated from blood culture and pathogen is not related to infection at another site, or (2) one of the following signs or symptoms: fever of 38°C (100.4°F), chills, or hypotension (systolic blood pressure ≤90 mm Hg), and one of the 2 following: (A) common skin contaminant isolated from 2 blood cultures drawn on separate occasions and organism is not related to infection at another site, or (B) common skin contaminant isolated from blood culture from patient with intravascular access device and provider institutes appropriate antimicrobial therapy. The criteria for a denominator included days from the day of admission with a central venous catheter to day of removal of central venous catheter. In addition, 11 inclusion criteria and 4 exclusion criteria were included. Discussion Home infusion therapy and healthcare epidemiology experts developed candidate criteria for a home infusion CLABSI surveillance definition. Conclusions Home care and home infusion agencies can use this definition to monitor their own CLABSI rates and implement preventative strategies.

Excavation undertaken at the Upper Severn valley round barrow cemetery at Four Crosses, Llandysilio, Powys, between 2004 and 2006 has increased the known barrows and ring ditches to some twenty-seven monuments within this complex, and revealed additional burials. Based on limited dating evidence, and the data from earlier excavations, the majority of the barrows are thought to be constructed in the Bronze Age. The barrows are considered part of a larger linear cemetery. The landscape setting and wider significance of this linear barrow cemetery are explored within this report. Dating suggests two barrows were later, Iron Age additions. The excavation also investigated Iron Age and undated pit alignments, Middle Iron Age copper working and a small Romano-British inhumation cemetery and field systems. Much of this evidence reflects the continuing importance of the site for ritual and funerary activity.

While specialized infusion clinical services remain the standard of care, widespread curtailing and disbanding of infusion teams as a cost-cutting measure has been documented in health care organizations for nearly 2 decades. Owing to this trend, as well as recent government interventions in medical error control, the authors engaged in an exploratory study of infusion administration practices in the US health care industry. This article presents the authors' exploratory findings, as well as their potential implications.
