A Leishmaniose Visceral (LV) e uma parasitose com sintomatologia grave, que pode ser fatal, causada por protozoarios do genero Leishmania . Nesse relato, apresentamos um caso suspeito de LV de uma paciente residente em area endemica para a doenca. O quadro de hepatoesplenomegalia, anemia, dor abdominal e plaquetopenia gerou suspeita de LV, apesar de os exames laboratoriais apresentarem resultados negativos. A paciente foi tratada empiricamente com anfotericina B desoxicolato e obteve regressao dos sintomas.

Introducao: A toxoplasmose e causada pelo protozoario Toxoplasma gondii sendo geralmente assintomatica. Quando apresenta manifestacoes clinicas, geralmente cursa com dano ocular, linfadenopatia (frequentemente com unico nodulo cervical aumentado) e astenia sem febre, sintomas estes que nao persistem por muito tempo (LEAO et.al, 2013). A febre de origem indeterminada classica (FOI) e definida em pacientes com temperatura axilar superior a 37,8oC, pelo tempo minimo de tres semanas, cuja etiologia permanece desconhecida apos uma semana de investigacao. A maior causa de FOI e de origem infecciosa sendo de dificil diagnostico, principalmente quando apresentada de forma isolada (LAMBERTUCCI et.al, 2005). Objetivo: Relatar caso raro de paciente com toxoplasmose tendo hipertermia como unica manifestacao clinica e sugerir inclusao dessa zoonose como hipotese diagnostica de FOI. Metodo: Revisao bibliografica de artigos da plataforma EBSCO, livros academicos de parasitologia e pediatria ambulatorial e relato de caso. Resultados: Esse artigo traz o relato de caso de paciente com picos febris, duas a tres vezes ao dia, sem outros sintomas ou historia pregressa que indicasse infeccao parasitaria, por periodo superior a um mes. Durante internacao foi diagnosticado com toxoplasmose apos realizacao de pesquisa para TORCHS (Toxoplasmose, Rubeola, Citomegalovirus, virus Herpes simples e Sifilis). Devido a persistencia da febre, realizou-se tratamento com Sulfadiazina + Pirimetamina + Acido Folinico. Conclusao: A toxoplasmose, apesar de nao ter a hipertermia isolada como manifestacao classica, deve ser investigada em casos de FOI visando proporcionar menores desgastes para o paciente e sua familia, uma vez que a febre e queixa preocupante e comum a inumeras patologias.

Introdução: As infecções hospitalares se prolongaram por muitas décadas e séculos, mas, felizmente, houve a criação das Comissões de Controle e Prevenção de Infecção Hospitalar (CCIH), que têm como princípio estabelecer e adotar medidas preventivas contra as infecções. Entretanto, as infecções hospitalares, agora denominadas infecções relacionadas à assistência em saúde, continuam presentes na atualidade, exigindo um maior cuidado e inovações de métodos para sua erradicação e controle. Objetivo: Avaliar os métodos de prevenção das infecções hospitalares a fim de diminuir o impacto aos pacientes, visitantes e profissionais da saúde. Métodos: Foram analisados artigos científicos da base Scielo, nacionais e internacionais, entre os anos de 2012 e 2018, acessados em março e abril de 2018, além da visita realizada ao hospital universitário Ciências Médicas. Resultados: Após as análises efetivadas, os dados apontados e as visitas realizadas, constata-se que, por meio da literatura dos artigos internacionais e nacionais, avistou-se a possibilidade de utilizar métodos efetivos praticados em países como o Japão, além da adoção de medidas preventivas colocadas em análise. Conclusão: Há muito a se discutir sobre as infecções e sua prevalência nos ambientes hospitalares, mas as medidas já abordadas e exemplificadas neste artigo mostram também a possibilidade e possivelmente o melhor direcionamento no controle dessas infecções.

BACKGROUND: Remission rates achieved after the initial treatment of acute lymphoblastic leukemia may be similar in both developed and developing countries, but relapse rates are much higher in the latter. Thus, other reasons are needed, in addition to biological characteristics of the leukemic cells themselves, to explain the unfavorable evolution of patients living in unfavorable socioeconomic and cultural conditions. OBJECTIVE: The aim of this study was to retrospectively evaluate compliance to an acute lymphoblastic leukemia treatment protocol. METHODS: Main abstracted data were: total duration and reasons for interruption of chemotherapy, prescribed doses of 6-mercaptopurine, and median white blood cell and neutrophil counts during the maintenance phase. Interruptions of chemotherapy were considered inappropriate if they did not follow predetermined criteria established in the protocol. RESULTS: Fourteen of 73 patients (19.2%) unduly interrupted chemotherapy by determination of their physicians. The median white blood cell count was higher when compared with the protocol recommendations; the median 6-MP dose was lower than the standard recommended dose. The estimated probability of event-free survival was higher for patients with lower median leukocyte counts and close to those predetermined by the protocol. Event-free survival was also higher for children with a higher percentage of days without chemotherapy due to bone marrow or liver toxicity excluding undue interruptions. In multivariate analysis, both factors remained statistically significant. These results suggest that the intensity of maintenance chemotherapy may not have been enough in some children, to achieve adequate myelosuppression, hence the observation of higher leukocyte counts and none or rare episodes of therapy interruption. CONCLUSIONS: Compliance to the therapeutic protocol by both doctors and patients should always be considered in the evaluation of therapeutic failure in acute lymphoblastic leukemia; strict adherence to treatment protocols contributes to better treatment results in acute lymphoblastic leukemia children.

Summary Introduction: Several hemoglobin variants have electrophoretic behavior similar to hemoglobin S, which may lead to false diagnosis for sickle-cell disorders in newborn screening programs. A homozygous hemoglobin with S mobility was detected in two unrelated babies in Brazil. Methods: Isoelectric focusing and high-performance liquid chromatography assays, gene sequencing, and restriction fragment length polymorphism with AfeI were used to characterize the hemoglobin. Results: Hb Stanleyville-II and −α3.7/−α3.7 type I deletion in the α-globin gene was diagnosed. Parents were heterozygous for both Hb Stanleyville-II and α-thalassemia. Hypochromia and microcytosis were probably due to the homozygous α-thalassemia. Conclusion: Stanleyville-II gene mutation is HBA2:c.237C>A, or C>G, and this information on the Globin Gene Server should be updated; AfeI test is a fast and accurate method to detect it; NBS programs should consider the possibility of Hb Stanleyville-II whenever IEF shows one band in the HbS position, and another one between S and C.

The frequency of allele variants of gene TPMT*2, *3A, *3B, and *3C was estimated in a population of 116 Brazilian children with acute lymphoblastic leukemia. The association between genotype and clinical and laboratory data obtained during chemotherapy maintenance phase and the correlation of intraerythrocyte concentration of 6-mercaptopurine metabolites (6-tioguanine nucleotide nucleotides and methylmercaptopurine) were analyzed. A multiplex amplification refractory mutation system-polymerase chain reaction (ARMS-PCR) was used in DNA amplification. Twelve patients presented TPMT gene mutation, all in heterozygous form. The most frequent allele variation was TPMT*3A (3.9%), followed by *3C (0.9%), *2 (0.4%), and *3B (0%). There was no significant association between clinical and laboratory data and the presence of mutation in TPMT gene. Of the 36 patients who were monitored for 6-mercaptopurine metabolite levels, only 1 had the mutation. In this patient, high 6-tioguanine nucleotide and low methylmercaptopurine concentrations were found. Event-free survival (EFS) for the whole group was 73.4%. There was no significant difference in event-free survival in the comparison between the groups with and without mutation (P = 0.06).

OBJECTIVE: To characterize alpha-chain variant hemoglobins with electric mobility similar to that of hemoglobin S in a newborn screening program. METHODS: βS allele and alpha-thalassemia deletions were investigated in 14 children who had undefined hemoglobin at birth and an electrophoretic profile similar to that of hemoglobin S when they were six months old. Gene sequencing and restriction enzymes (DdeI, BsaJI, NlaIV, Bsu36I and TaqI) were used to identify hemoglobins. Clinical and hematological data were obtained from children who attended scheduled medical visits. RESULTS: The following alpha chain variants were found: seven children with hemoglobin Hasharon [alpha2 47(CE5) Asp>His, HbA2:c.142G>C], all associated with alpha-thalassemia, five with hemoglobin Ottawa [alpha1 15(A13) Gly>Arg, HBA1:c.46G>C], one with hemoglobin St Luke's [alpha1 95(G2) Pro>Arg, HBA1:c.287C>G] and another one with hemoglobin Etobicoke [alpha212 84(F5) Ser>Arg, HBA212:c.255C>G]. Two associations with hemoglobin S were found: one with hemoglobin Ottawa and one with hemoglobin St Luke's. The mutation underlying hemoglobin Etobicoke was located in a hybrid α212 allele in one child. There was no evidence of clinically relevant hemoglobins detected in this study. CONCLUSION: Apparently these are the first cases of hemoglobin Ottawa, St Luke's, Etobicoke and the α212 gene described in Brazil. The hemoglobins detected in this study may lead to false diagnosis of sickle cell trait or sickle cell disease when only isoelectric focusing is used in neonatal screening. Additional tests are necessary for the correct identification of hemoglobin variants.

Almost 3 million babies were tested in a newborn screening program in Minas Gerais, Brazil (1998-2008); 128 who have S-like hemoglobins (Hbs) were tested for the β(S) allele and 112 were identified through polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) or sequencing. Hb Stanleyville-II [α78(EF7)Asn→Lys (α2); HbA2: c.237CA] was present in 96 children (85.7%), two in a homozygous state and 94 in a heterozygous state. Its estimated prevalence was 1:11,500. Hbs Hasharon [α47(CE5)Asp→His, GACCAC (α2)], Ottawa [α15(A13)Gly→Arg (GGTCGT) (α2 or α1)], G-Ferrara [β57(E1)Asn→Lys (AACAAA or AAG)], St. Luke's [α95(G2)Pro→Arg, C CGC GG (α1)], Maputo [β47(CD6)Asp→Tyr (GATTAT)] and Etobicoke [α84(F5)Ser→Arg (AG CAG G or CGC or AGA) (α2 or α1)] were also identified. Many children with Hbs Stanleyville-II and Hasharon also co-inherited the -α(3.7) thalassemia gene. African ancestry was recognized by parents of all 31 children with Hb Stanleyville-II who were interviewed. Mean corpuscular volume (MCV) and mean corpuscular Hb (MCH) values were significantly lower in children with α-thalassemia (α-thal). We came to the conclusion that Hb Stanleyville-II is not so uncommon in Brazil and seems to have originated from the African slave trade. This study reinforces the importance of an accurate diagnosis of variants that have electrophoretic mobility similar to Hb S [β6(A3)Glu→Val, GAGGTG] so that false diagnoses are avoided.

Algumas hemoglobinas (Hb) variantes apresentam mobilidade eletroforética semelhante à da Hb S, o que pode levar ao falso diagnóstico de traço ou doença falciforme. A maioria dos indivíduos possuidores das Hbs variantes é assintomática, mas em vários casos aHb está associada à alfa-talassemia, o que pode resultar em alterações hematológicas. A associação de Hb variante com a Hb S ou Hb C pode determinar quadros clínicos e/ou laboratoriais complexos. Objetivo e Métodos: O objetivo deste estudo foi determinar a frequência das variantes de hemoglobina detectadas no Programa de Triagem Neonatal de Minas Gerais com mobilidade elétrica similar à da Hb S. O DNA de 118 crianças foi utilizado para verificar a presença do alelo bS. O diagnóstico de alfa-talassemia do tipo 3.7 ou 4.2 foi realizado por meio da Reação em Cadeia da Polimerase (PCR) multiplex do tipo gap (gap- PCR multiplex). Os produtos dessa PCR foram então digeridos com endonucleases de restrição para detectar as mutações correspondentes à Stanleyville-II e Hasharon, já descritasno Brasil. Nos casos negativos para essas Hbs fez-se sequenciamento dos genes HBA ou HBB. Dados clínicos foram obtidos das crianças que compareceram à consulta clínica na Fundação Hemominas. Resultados: Das 118, apenas 6 crianças apresentaram a Hb S. Dentre as 112 foram encontradas duas crianças, (1,8%) com Hb Stanleyville-II em homozigose (-3.7;Stanleyville/-3.7;Stanleyville) e outras 94 (84%) com Stanleyville-II em heterozigose (84 têm genótipo /- 3.7;Stanleyville, cinco -a3.7/-a3.7;Stanleyville e cinco aStanleyvillea/aa) Dentre os 112 casos, seis apresentavam Hb Hasharon em heterozigose (todas aa/-a3.7Hasharon-). Detectaramse ainda cinco casos com Hb Ottawa, um com Hb St Lukes e um com Hb Etobicoke, todasvariantes de cadeia , em heterozigose e genótipo /. Hb G-Ferrara foi encontrada em duas crianças (uma /-a3.7 e outra aa/aa) e Hb Maputo (/-a3.7)em uma, ambas variantes b e em heterozigose. Dentre as hemoglobinas citadas acima, foram encontradas cincoassociações com a Hb S: em três crianças com Hb Stanleyville-II, uma Ottawa e uma St Luke's. A Hb Etobicoke foi encontrada em uma criança que possui o alelo híbrido 212. Não foi observada nenhuma alteração clínica decorrente dessas associações. Em várias crianças com Hb Stanleyville-II, todas com ancestralidade africana, foram observadas microcitose e hipocromia, devido a associação com a alfa talassemia do tipo 3.7. Foi estabelecida PCRRFLP (Polimorfismo de Fragmento de Restrição) para a identificação de todas as hemoglobinas, com exceção da hemoglobina G-Ferrara. Conclusões: 1. Hemoglobina Stanleyville-II é relativamente comum em Minas Gerais (incidência de 1:11.500); 2. Apresença das Hbs Stanleyville-II, Ottawa, Maputo, G-Ferrara, Hasharon, Etobicoke e St Luke's pode levar a falso diagnóstico de traço ou doença falciforme, necessitando-se de testes adicionais para o diagnóstico diferencial correto; 3. A presença dessas Hbs parece não possuirrelevância clínica, mesmo nos casos encontrados neste estudo quando se detectou associação das Hbs variantes St Luke's, Stanleyville-II e Ottawa com a Hb S; 4. Hipocromia e microcitose, observadas em várias crianças, são devidas à co-herança de -talassemia e não à presença da Hb Stanleyville-II; 5. Foram encontrados casos assintomáticos de Hb Maputo e G- Ferrara em heterozigose com a Hb A. Os pais dessas crianças receberam orientação genética sobre a possibilidade das crianças terem filhos com doença falciforme se houver coherançacom a Hb S; 6. A PCR-RFLP pode ser utilizada como método diagnóstico para todas as Hbs variantes identificadas neste estudo, com exceção da Hb G-Ferrara. Some hemoglobin (Hb) variants have similar electrophoretic mobility to that of Hb S (S-like), which can lead to false diagnosis of sickle cell trait or disease. Most individuals with Hb variants are asymptomatic, but in several cases the Hb is associated with alpha-thalassemia and may result in hematological changes. The association of a Hb variantwith the Hb S or Hb C may result in complex clinical and/or laboratory features. Objective and Methods: The aim of this study was to determine the frequency of S-like Hb variants detected in the Newborn Screening Program in Minas Gerais and to assess the clinical relevance of these Hbs. The DNA from 118 children was tested for the bS allele. Alphathalassemia types 3.7 or 4.2 were tested in 112 children in 56 cities in Minas Gerais by mutiplex gap-PCR. The products of this PCR were then digested with restriction endonucleases to detect the mutations corresponding to Hb Stanleyville-II and Hb Hasharon, both already reported in Brazil. In the remaining cases sequencing of HBB or HBA genes was necessary to identify the Hb variant. Clinical data were obtained from children who attended medical consultations at Hemominas Foundation. Results: Of the 118 children, only six werepositive for the bS allele without any other variant. Among the remaining 112 children there were two children (1.8%) with homozygous Hb Stanleyville-II (-3.7; Stanleyville/-3.7; Stanleyville), 94 (84%) with heterozygous Stanleyville-II (genotypes: 84 / - a3.7; Stanleyville, five - a3.7 / - a3.7; Stanleyville and five aStanleyvillea/aa), and six heterozygous Hb Hasharon (all aa / -a 3.7; Hasharon). Five cases with Hb Ottawa, one with Hb St. Luke's and one with Hb Etobicoke, all - chain variants in heterozygosis and genotype /, were also identified. Hb G-Ferrara was identified in two children (one /-3.7 and the other aa/aa) and Maputo Hb ( / -a3.7) in one, both b variants in heterozygosis. Among the above mentioned Hbs, there were five associations with the bS allele: three children with Hb Stanleyville-II, one with Hb Ottawa and one with Hb St Luke's. Hb Etobicoke was found in a child who has a hybrid 212 gene. There was no clinical change due to these associations. In several children with Hb Stanleyville-II, all from African ancestry, microcytosis and hypochromia were observed because of the association with a 3.7 alpha thalassemia gene. PCR-RFLP (Restriction Fragment LengthPolymorphism) with specific endonucleases confirmed all mutations underlying the variant Hbs, with the exception of Hb G-Ferrara, for which no diagnostic endonuclease was found. Conclusions: 1. Hemoglobin Stanleyville-II is relatively common in Minas Gerais (incidenceof 1:11,500); 2. Hbs Stanleyville-II, Ottawa, Maputo, G-Ferrara, Hasharon, Etobicoke and St. Luke's can lead to false diagnosis of sickle cell trait or disease in newborn screening, unless additional tests for a correct diagnosis are used; 3. These Hbs seem to have no clinical relevance, even when found in combination with the bS allele, as happened in this study with the Hb St Luke's, Stanleyville-II, and Ottawa; 4. Hypochromia and microcytosis detected in several children are due to the co-inheritance of -thalassemia and not to the presence of HbStanleyville-II; 5. Asymptomatic cases of heterozygous Hb Maputo and Hb G-Ferrara were identified. Genetic counseling about the possibility of having children or grandchildren with sickle cell disease if the bS allele is co-inherited was given to these families; 6. Specific PCRRFLPmay be used as a diagnostic tool for all variant Hbs identified in the present study, except for Hb G-Ferrara.

Hemoglobin Rush is an unstable variant generated by a mutation of the β-globin gene which causes amino acid replacement Glu>Gln in the central cavity of hemoglobin (G3). Many members of a Brazilian family of Italian descent have hemoglobin Rush. This is the second report in world literature. Clinical and laboratory features were retrieved and gene mutation was characterized. Hemoglobin electrophoresis, gene sequencing, and restriction fragment length polymorphism with Hpy188I were used to characterize it. In 13 affected members, hemoglobin ranged from 9.3 to 13.0 g/dL and reticulocyte count up to 12.8%. The intensity of hemolysis appeared to be linked to increased stress. The mutation was proved to be HBB:c.304G>C, beta 101(G3) Glu>Gln. Heterozygous hemoglobin Rush should be suspected when alkaline electrophoresis shows three bands, whereas isoelectric focusing and acid electrophoresis show only two. Adequate genetic counseling to avoid intermarriage should be provided because homozygous hemoglobin Rush is predicted to be clinically severe.

Introdução: A 6-mercaptopurina (6-MP) é o principal componente da maioria dos esquemas de quimioterapia na fase de manutenção do tratamento da leucemia linfocítica aguda (LLA). A tiopurinametiltransferase (TPMT) é uma das enzimas responsáveis pelo metabolismo da 6-MP. Diversos estudos demonstram um polimorfismo genético de caráter autossômico co-dominante para a TPMT em humanos. O metabolismo da 6-MP é intensamente afetado por este polimorfismo. Objetivos: Estimar a freqüência das quatro principais variantes alélicas do gene TPMT: *2 (238G>C), *3A (460G>A e 719A>G) *3B(460G>A) e *3C (719A>G) em uma população de crianças e adolescentes com LLA tratados no Serviço de Hematologia do Hospital das Clínicas UFMG. Correlacionar o genótipo com dados clínicos e laboratoriais obtidos durante a fase de manutenção da quimioterapia e com a concentração intra-eritrocitária dos metabólitos da 6-MP (6-TGN e MMP). Métodos: Amostras de DNA de 116 pacientes foram extraídas sendo 95 a partir de sangue total, 13 a partir de concentrado de hemácias e 8 utilizando esfregaço de medula óssea. A PCR alelo específica foi otimizada separadamente para cada locus, utilizando a estratégia do sistema de amplificação refratário a mutações (ARMS). Posteriormente, a PCR foi realizada em multiplex. Esta PCR foi possível somente com preparado de DNA de sangue total. Os dados clínicos e laboratoriais foram coletados, retrospectivamente, em prontuários médicos. Resultados: Doze dos 116 pacientes (10,34%) apresentaram mutação. Cento e quatro pacientes (89,66%) não apresentaram mutação no gene TPMT. Todas as mutações foram encontradas em heterozigose. O alelo mais freqüente foi o TPMT*3A (9/232 alelos; 3,87%), seguido pelo *3C (2/232; 0,86%), *2 (1/232; 0,43%) e *3B (nenhum alelo; 0%). Não houve diferenças significativas nas medianas da contagem de leucócitos e neutrófilos, quando os grupos de pacientes com e sem mutação foram comparados. Também o tempo de interrupção da quimioterapia de manutenção foi semelhante nos dois grupos. A média das doses prescritas de 6-MP situou-se abaixo da dose preconizada pelo protocolo em ambos os grupos e a mediana da contagem de leucócitos e neutrófilos durante o tratamento de manutenção ficou acima da preconizada pelos protocolos de tratamento. Dos 36 pacientes em que foi realizada a dosagem de metabólitos apenas um apresentou mutação. Neste paciente foi encontrado altas concentrações intra-eritrocitárias de 6-TGN e baixas concentrações de MMP. Portanto, não foi possível a realização de análises estatísticas para avaliação da relação entre o genótipo e a concentração de metabólitos da 6-MP. A sobrevida livre de eventos aos 8,5 anos para todo o grupo foi de 75,1% (IC 95%: 60,8 a 84,8%). Não foi observada diferença na sobrevida livre de eventos quando comparados os grupos com e sem mutação (p = 0,06). Conclusões: Os nossos resultados em pacientes brasileiros coincidem com a literatura, em que o alelo TPMT*3A é o mais freqüente na maioria das populações. Vários indícios encontrados no presente estudo sugerem que a intensidade da quimioterapia pode não ter sido suficiente para se alcançar uma mielossupressão adequada, principalmente no grupo de pacientes com mutação do gene TPMT. Isso explicaria porque não se encontraram diferenças significativas no prognóstico dos grupos com e sem mutação do gene da TPMT. A incorporação da genotipagem à rotina de acompanhamento dos pacientes tratados no Serviço de Hematologia do Hospital das Clínicas da UFMG poderia ser uma ferramenta auxiliar na otimização das doses de 6-MP, minimizando riscos de toxicidade e contribuindo para uma melhor abordagem diagnóstica e terapêutica.
