5. Convective Excitation of Inertial Modes in Binary Neutron Star Mergers

We present the first very long-term simulations (extending up to ~140 ms after merger) of binary neutron star mergers with piecewise polytropic equations of state and in full general relativity. Our simulations reveal that at a time of 30-50 ms after merger, parts of the star become convectively unstable, which triggers the excitation of inertial modes. The excited inertial modes are sustained up to several tens of milliseconds and are potentially observable by the planned third-generation gravitational-wave detectors at frequencies of a few kilohertz. Since inertial modes depend on the rotation rate of the star and they are triggered by a convective instability in the postmerger remnant, their detection in gravitational waves will provide a unique opportunity to probe the rotational and thermal state of the merger remnant. 8. Spectral analysis of gravitational waves from binary neutron star merger remnants

In this work we analyze the gravitational wave signal from hypermassive neutron stars formed after the merger of binary neutron star systems, focusing on its spectral features. The gravitational wave signals are extracted from numerical relativity simulations of models already considered by De Pietri et al. [Phys. Rev. D 93, 064047 (2016)], Maione et al. [Classical Quantum Gravity 33, 175009 (2016)], and Feo et al. [Classical Quantum Gravity 34, 034001 (2017)], and allow us to study the effect of the total baryonic mass of such systems (from $2.4 M_{\odot}$ to $3 M_{\odot}$), the mass ratio (up to $q = 0.77$), and the neutron star equation of state, both in equal and highly unequal mass binaries. We use the peaks we find in the gravitational spectrum as an independent test of already published hypotheses of their physical origin and empirical relations linking them with the characteristics of the merging neutron stars. In particular, we highlight the effects of the mass ratio, which in the past was often neglected. We also analyze the temporal evolution of the emission frequencies. Finally, we introduce a modern variant of Prony's method to analyze the gravitational wave postmerger emission as a sum of complex exponentials, trying to overcome some drawbacks of both Fourier spectra and least-squares fitting. Overall, the spectral properties of the postmerger signal observed in our simulation are in agreement with those proposed by other groups. More specifically, we find that the analysis of Bauswein and Stergioulas [Phys. Rev. D 91, 124056 (2015)] is particularly effective for binaries with very low masses or with a small mass ratio and that the mechanical toy model of Takami et al. [Phys. Rev. D 91, 064001 (2015)] provides a comprehensive and accurate description of the early stages of the postmerger., Comment: 19 pages, 6 figures

The development of artificial intelligence-based tools is having a big impact on industry. In this context, the maintenance operations of important assets and industrial resources are changing, both from a theoretical and a practical perspective. Namely, conventional maintenance reacts to faults and breakdowns as they occur or schedules the necessary inspections of systems and their parts at fixed times by using statistics on component failures, but this can be improved by a predictive maintenance based on the real component's health status, which is inspected by appropriate sensors. In this way, maintenance time and costs are saved. Improvements can be achieved even in the marine industry, in which complex ship propulsion systems are produced for operation in many different scenarios. In more detail, data-driven models, through machine learning (ML) algorithms, generate the expected values of monitored variables for comparison with real measurements on the asset, for a diagnosis based on the difference between expectations and observations. The first step towards realization of predictive maintenance is choosing the ML algorithm. This selection is often not the consequence of an in-depth analysis of the different algorithms available in the literature. For that reason, here the authors propose a framework to support an initial implementation stage of predictive maintenance based on a benchmarking of the most suitable ML algorithms. The comparison is tested to predict failures of the oil circuit in a diesel marine engine as a case study. The algorithms are compared by considering not only the mean squared error between the algorithm predictions and the data, but also the response time, which is a crucial variable for maintenance. The results clearly indicate the framework well supports predictive maintenance and the prediction error and running time are appropriate variables to choose the most suitable ML algorithm for prediction. Moreover, the proposed framework can be used to test different algorithms, on the basis of more performance indexes, and to apply predictive maintenance to other engine components. We present a study of the merger of six different known galactic systems of binary neutron stars (BNS) of unequal mass with a mass ratio between $0.75$ and $0.99$. Specifically, these systems are J1756-2251, J0737-3039A, J1906+0746, B1534+12, J0453+1559 and B1913+16. We follow the dynamics of the merger from the late stage of the inspiral process up to $\sim$ 20 ms after the system has merged, either to form a hyper-massive neutron star (NS) or a rotating black hole (BH), using a semi-realistic equation of state (EOS), namely the seven-segment piece-wise polytropic SLy with a thermal component. For the most extreme of these systems ($q=0.75$, J0453+1559), we also investigate the effects of different EOSs: APR4, H4, and MS1. Our numerical simulations are performed using only publicly available open source code such as, the Einstein Toolkit code deployed for the dynamical evolution and the LORENE code for the generation of the initial models. We show results on the gravitational wave signals, spectrogram and frequencies of the BNS after the merger and the BH properties in the two cases in which the system collapse within the simulated time., Comment: 13 pages, 10 figures

16. Binary neutron star merger simulations with different initial orbital frequency and equation of state

We present results from three-dimensional general relativistic simulations of binary neutron star coalescences and mergers using public codes. We considered equal mass models where the baryon mass of the two Neutron Stars (NS) is $1.4M_{\odot}$, described by four different equations of state (EOS) for the cold nuclear matter (APR4, SLy, H4, and MS1; all parametrized as piecewise polytropes). We started the simulations from four different initial interbinary distances ($40, 44.3, 50$, and $60$ km), including up to the last 16 orbits before merger. That allows to show the effects on the gravitational wave phase evolution, radiated energy and angular momentum due to: the use of different EOSs, the orbital eccentricity present in the initial data and the initial separation (in the simulation) between the two stars. Our results show that eccentricity has a major role in the discrepancy between numerical and analytical waveforms until the very last few orbits, where "tidal" effects and missing high-order post-Newtonian coefficients also play a significant role. We test different methods for extrapolating the gravitational wave signal extracted at finite radii to null infinity. We show that an effective procedure for integrating the Newman-Penrose $\psi_4$ signal to obtain the gravitational wave strain $h$ is to apply a simple high-pass digital filter to $h$ after a time domain integration, where only the two physical motivated integration constants are introduced. That should be preferred to the more common procedures of introducing additional integration constants, integrating in the frequency domain or filtering $\psi_4$ before integration., Comment: 32 pages, 13 figures

ABSTRACTIntroductionPsoriatic arthritis (PsA) is a chronic immunoinflammatory disease of the enthesis and adjacent synovium, skin, and nail, which early diagnosis may be crucial for starting a prompt therapeutic intervention. Theoretically, early treatment offers the advantage of acting on the reduction of the articular damage progression since initial phases of the disease.Areas coveredThis review explores the challenges of clinical-diagnostic aspects and the underlying pathophysiology of early PsA phases, as well as the evidence evaluating the impact of early intervention on disease outcomes.Expert opinionMain instruments for early PsA diagnosis include recognizing synovial-entheseal inflammatory signs at onset, improving screening PsA high-risk subjects, and increasing disease knowledge of physicians and patients with psoriasis or familial history. PsA continues to significantly impact on the Quality of Life of patients affected by the disease, making necessary to deeply study clinical manifestations, risk factors, and underlying immunoinflammatory mechanisms, as well as to identify biomarkers for early identification. Additionally, it remains a need to increase more evidence on understanding how early treatment of PsA and of psoriasis might influence the course of the disease.

19. Modeling Equal and Unequal Mass Binary Neutron Star Mergers Using Public Codes

We present three-dimensional simulations of the dynamics of binary neutron star (BNS) mergers from the late stage of the inspiral process up to $\sim 20$ ms after the system has merged, either to form a hyper-massive neutron star (NS) or a rotating black hole (BH). We investigate five equal-mass models of total gravitational mass $2.207$, $2.373$, $2.537$, $2.697$ and $2.854 M_\odot$, respectively, and four unequal mass models with $M_{\mathrm{ADM}}\simeq 2.53\ M_\odot$ and $q\simeq 0.94$, $0.88$, $0.82$, and $0.77$ (where $q = M^{(1)}/M^{(2)}$ is the mass ratio). We use a semi-realistic equation of state (EOS) namely, the seven-segment piece-wise polytropic SLyPP with a thermal component given by $\Gamma_{th} = 1.8$. We have also compared the resulting dynamics (for one model) using both, the BSSN-NOK and CCZ4 methods for the evolution of the gravitational sector, and also different reconstruction methods for the matter sector, namely PPM, WENO and MP5. Our results show agreement and high resolution, but superiority of BSSN-NOK supplemented by WENO reconstruction at lower resolutions. One of the important characteristics of the present investigation is that, for the first time, this has been done using only publicly available open source software, in particular, the Einstein Toolkit code deployed for the dynamical evolution and the LORENE code for the generation of the initial models. All of the source code and parameters used for the simulations have been made publicly available. ABSTRACTIntroductionPsoriatic arthritis (PsA) is an immune-inflammatory disease that affects both joints and entheses, and with diverse extra-articular manifestations (psoriasis, inflammatory bowel disease (IBD), and uveitis). A wide range of comorbid conditions, including cardiovascular diseases, obesity, metabolic syndrome (MetS), nonalcoholic fatty liver disease (NAFLD), mental health disorders (depression/anxiety), and osteoporosis are highly prevalent in course of PsA.Biological DMARDs (bDMARD), including TNF-inhibitors (TNFi), Interleukin (IL-17i) and IL-23i represent the cornerstone of the management of active disease. The use of these therapies obviously requires considering comorbidities presence, safety aspects and contraindications.Areas coveredThe aim of this review is to describe the inflammatory mechanisms behind PsA comorbidities, and the role of bDMARDs in the prevention and treatment of these conditions in course of PsA.Expert opinionTailoring therapeutic strategies to the individual characteristics of each PsA patient can be an effective approach to manage comorbidities, maximizing the efficacy of bDMARDs, and reducing the incidence of AEs. Identifying targets within disease pathways can guide research into therapeutics that address both PsA and comorbidities simultaneously, but more studies are advocated for clarifying the potential prevention and management of bDMARDs used for PsA.

Aim: Several methods for assessing anastomotic integrity have been proposed, but the best is yet to be defined. The aim of this study was to compare the different methods to assess the integrity of colorectal anastomosis prior to ileostomy reversal. Method: A retrospective cohort analysis on patients between 1 January 2010 and 31 December 2020 with a defunctioning stoma for middle and low rectal anterior resection was performed. A propensity score matching comparison between patients who underwent proctoscopy alone and patients who underwent proctoscopy plus any other preoperative method to assess the integrity of colorectal anastomosis prior to ileostomy reversal (transanal water‐soluble contrast enema via conventional radiology, transanal water‐soluble contrast enema via CT, and magnetic resonance) was performed. Results: The analysis involved 1045 patients from 26 Italian referral colorectal centres. The comparison between proctoscopy alone versus proctoscopy plus any other preoperative tool showed no significant differences in terms of stenoses (p = 0.217) or leakages (p = 0.103) prior to ileostomy reversal, as well as no differences in terms of misdiagnosed stenoses (p = 0.302) or leakages (p = 0.509). Interestingly, in the group that underwent proctoscopy and transanal water‐soluble contrast enema the comparison between the two procedures demonstrated no significant differences in detecting stenoses (2 vs. 0, p = 0.98), while there was a significant difference in detecting leakages in favour of transanal water‐soluble contrast enema via CT (3 vs. 12, p = 0.03). Conclusions: We can confirm that proctoscopy alone should be considered sufficient prior to ileostomy reversal. However, in cases in which the results of proctoscopy are not completely clear or the surgeon remains suspicious of an anastomotic leakage, transanal water‐soluble contrast enema via CT could guarantee its detection. The increasing resistance of fungi to conventional antifungal drugs has prompted worldwide the search for new compounds. In this work, we investigated the antifungal properties of acylated Temporin L derivatives, Pent-1B and Dec-1B, against Candida albicans, including the multidrug-resistant strains. Acylated peptides resulted to be active both on reference and clinical strains with MIC values ranging from 6.5 to 26 µM, and they did not show cytotoxicity on human keratinocytes. In addition, we also observed a synergistic or additive effect with voriconazole for peptides Dec-1B and Pent-1B through the checkerboard assay on voriconazole-resistant Candida strains. Moreover, fluorescence-based assays, NMR spectroscopy, and confocal microscopy elucidated a potential membrane-active mechanism, consisting of an initial electrostatic interaction of acylated peptides with fungal membrane, followed by aggregation and insertion into the lipid bilayer and causing membrane perturbation probably through a carpeting effect. PsD has several extracutaneous manifestations, such as inflammatory articular and entheseal involvement, leading to psoriatic arthritis (PsA), and the less frequent intestinal and ocular manifestations with colitis/inflammatory bowel disease and uveitis, respectively. - Abstract
Italian Research Group for Gastric Cancer (GIRCG), during the 2013 annual Consensus Conference to gastric cancer, stated that laparoscopic or robotic approach should be limited only to early gastric cancer (EGC) and no further guidelines were currently available. However, accumulated evidences, mainly from eastern experiences, have supported the application of minimally invasive surgery also for locally advanced gastric cancer (AGC). The aim of our study is to give a snapshot of current surgical propensity of expert Italian upper gastrointestinal surgeons in performing minimally invasive techniques for the treatment of gastric cancer in order to answer to the question if clinical practice overcome the recommendation. Experts in the field among the Italian Research Group for Gastric Cancer (GIRCG) were invited to join a web 30-item survey through a formal e-mail from January 1st, 2020, to June 31st, 2020. Responses were collected from 46 participants out of 100 upper gastrointestinal surgeons. Percentage of surgeons choosing a minimally invasive approach to treat early and advanced gastric cancer was similar. Additionally analyzing data from the centers involved, we obtained that the percentage of minimally invasive total and partial gastrectomies in advanced cases augmented with the increase of surgical procedures performed per year (p=0.02 and p=0.04 respectively). It is reasonable to assume that there is a widening of indications given by the current national guideline into clinical practice. Propensity of expert Italian upper gastrointestinal surgeons was to perform minimally invasive surgery not only for early but also for advanced gastric cancer. Of interest volume activity correlated with the propensity of surgeons to select a minimally invasive approach.

ImportanceDelays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis.ObjectiveTo evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer.Design, Setting, and ParticipantsThis retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery.ExposuresAny type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections.Main Outcomes and MeasuresThe primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable.ResultsA total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95% CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95% CI, 1.15-1.53; P P = .03).Conclusions and RelevanceThis cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients.

Alzheimer's disease (AD) is one of the most prevalent forms of neurodegenerative disorders. Previously, we have shown that in vivo administration of an IL-17 neutralizing antibody (IL-17Ab) rescues amyloid-β-induced neuro-inflammation and memory impairment, demonstrating the pivotal role of IL-17 in AD-derived cognitive deficit. Recently, AD has been recognized as a more intriguing pathology affecting vascular network and platelet function. However, not much is known about peripheral vascular inflammation and how pro-inflammatory circulating cells/mediators could affect peripheral vessels' function. This study aimed to evaluate whether IL-17Ab treatment could also impact peripheral AD features, such as systemic inflammation, peripheral vascular dysfunction, and related pro-thrombotic state in a non-genetic mouse model of AD. Mice were injected intracerebroventricularly with Aβ1-42 peptide (3μg/3μl). To evaluate the systemic/peripheral protective profile of IL-17Ab, we used an intranasal administration of IL-17Ab (1μg/10μl) at 5, 12, and 19 days after Aβ1-42 injection. Circulating Th17/Treg cells and related cyto-chemokines, haematological parameters, vascular/endothelial reactivity, platelets and coagulation function in mice were evaluated. IL-17Ab treatment ameliorates the systemic/peripheral inflammation, immunological perturbance, vascular/endothelial impairment and pro-thrombotic state, suggesting a key role for this cytokine in fostering inflammatory processes that characterize the multifaced aspects of AD.
