Abstract

In the medical era of early detection of diseases and tailored therapies, an accurate characterization and staging of the disease is pivotal for treatment planning. The widespread use of computed tomography (CT)-often with the use of contrast material (CM)-probably represents the most important advance in diagnostic radiology. The result is a marked increase in radiation exposure of the population for medical purposes, with its intrinsic carcinogenic potential, and CM affecting kidney function. The radiologists should aim to minimize patient's risk by reducing radiation exposure and CM amount, while maintaining the highest image quality. To achieve this goal, it is necessary to perform "patient-centric imaging". The purpose of this review is to provide radiologists with "tips and tricks" to control radiation dose at CT, summarizing technical artifices in order to reduce image noise and increase image contrast. Also chest CT tailored protocols are supplied, with particular attention to three most common thoracic CT protocols: aortic/cardiac CT angiography (CTA), pulmonary CTA, and routine chest CT.

Abstract

The aim of this study is to determine the diagnostic performance of Magnetic Resonance Arthrography (MRA) in evaluating lesions of the glenoid labrum, in young active patients with chronic unstable shoulder, compared to shoulder arthroscopy. We retrospectively considered 65 MRA examinations, performed between December 2011 and January 2018. Among them, thirty-five patients (31 men, 4 women; mean age, 27.3 years; range, 16-53 years; 4 patients with a previous arthroscopy of the same shoulder) underwent shoulder arthroscopy after MRA. Arthroscopic reports were collected and analyzed for the correlation with MRA results.

Abstract

We aim to present clinical features, imaging findings, treatment aspects of the elastofibroma dorsi (ED), which is a benign tumor arising from connective tissue at the scapular region, and long-term outcomes after surgical resection.We evaluated retrospectively 82 patients (55 females, 27 males; mean age, 60 years; age range, 23-78 years) with ED who underwent surgery between January 1994 and May 2014; subsequently all patients were invited for follow-up, which consisted of physical and US examinations.Subscapular location was almost constant (79/82 patients). Right, left and bilateral location was noted in 39, 28 and 15 cases, respectively. 52/82 patients were symptomatic. The diagnosis was made on physical examination and imaging studies: 49 ultrasound, 43 computed tomography and 54 magnetic resonance examinations were performed overall. Surgical treatment consisted in marginal excision; in all cases diagnosis was confirmed by histological examination. The mean hospitalization was 3 days, with minor complications. Out of the 82 patients, only 25 gave their consent to follow-up; mean time passed after surgery was 64.7 months; 1 case of local recurrence was suspected by ultrasound and, then, confirmed by magnetic resonance imaging.In our series, clinical features and imaging findings of ED are consistent with current evidence; however, results of our follow-up group marks a difference from the literature, according to which there is no evidence of local recurrence after complete resection. Diagnosis of ED is based on clinical and imaging features; treatment is surgical, especially in symptomatic cases. Prolonging the clinical and US follow-up period may be useful in identifying local recurrence.

Abstract

Malignant spinal bone marrow disorders are one of the major causes of significant morbidity and reduction in quality of life in oncological patients. Thus, the characterization of these conditions is of crucial importance in the management of these patients. Magnetic resonance (MR) imaging plays a vital role in differentiation between benign and malignant spinal bone marrow disorders. However, morphological imaging features, based on T1 and T2 relaxation properties, might fail in differentiating between these conditions because signal characteristics may overlap. Quantitative MR imaging based on diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) values has been proved to help in defining the nature of the lesion. The aims of this paper were: to review basic principles of DWI technique and ADC maps, to describe DWI and ADC maps appearances of normal vertebral bone marrow briefly, to discuss the DWI and ADC maps characteristics in vertebral malignant lesions, to provide indications for differential diagnosis between malignant and benign lesions.

Abstract

Purpose: An accurate evaluation of left ventricular volumes, mass, and ejection fraction (EF) and an early exclusion or detection of significant coronary artery disease or cardiac allograft vasculopathy are mandatory for clinical management and prognosis assessment of heart-transplanted patients (HTP). The purpose of this article was to evaluate the role of dual-step prospective electrocardiographytriggered Dual-Source CT (pECGdual-step-DSCT) in HTP for the assessment of left-ventricular function, in comparison with echocardiography (echo) and cardiac magnetic resonance (CMR) performed on the same day, and of the coronary arteries as well. Materials and Methods: Left-ventricular EF, end-diastolic volume, end-systolic volume, stroke volume, cardiac output (CO), and mass were assessed in 11 HTP by pECGdual-step-DSCT in comparison with CMR and echo performed on the same day. During all the examinations, the heart rate was recorded. CT coronary artery assessment was also performed. Results: Heart rate was lower during DSCT (75.6±7.8 bpm; P<0.001). EF resulted slightly lower for DSCT (55.7%±5.0%; P≥0.05) in comparison with CMR (57.8%±5.3%; P≥ 0.05) and echo (59.2%±5.6%; P≥0.05). DSCT showed statistically significant higher end-diastolic volume (153.7±24.2 mL), end-systolic volume (67.8±11.5 mL), and stroke volume (85.9±17.6 mL) (P< 0.01 up to 0.001) than CMR, but with a high correlation (P< 0.001). Cardiac output was almost similar for DSCT versus CMR, with a very high correlation coefficient (r=0.914; P<0.001). DSCT showed higher mass values than CMR (P<0.001), but with a high correlation (r=0.866; P<0.001). DSCT versus echo results were less correlated. No significant coronary artery disease was detected. Conclusion: pECGdual-step-DSCT allows reliable assessment of leftventricular function in HTP, with good agreement and correlation with CMR, within a global diagnostic approach including coronary artery evaluation in one single-volume acquisition.

Abstract

Mucopolysaccharidoses (MPS) represent a group of inheritable lysosomal storage diseases caused by mutations in the genes coding for enzymes involved in catabolism of different glycosaminoglycans (GAGs). They are clinically heterogeneous multisystemic diseases, often involving the spine. Bony abnormalities of the spine included in the so-called dysostosis multiplex and GAG deposits in the dura mater and supporting ligaments can result in spinal cord compression, which can lead to compressive myelopathy. Spinal involvement is a major cause of morbidity and mortality in some MPS (e.g., MPS IVA, VI, and I), and early radiological diagnosis is critical in preventing or arresting neurological deterioration and loss of function.

Abstract

The synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome includes a group of chronic, relapsing, inflammatory musculoskeletal disorders with similar manifestations, in particular synovitis, hyperostosis, and osteitis, which may or may not be associated with neutrophilic skin eruptions such as palmoplantar pustulosis and acne conglobata. The syndrome occurs at any age, can involve any skeletal site, and its imaging appearances are variable, depending on the stage/age of the lesion and imaging method. The diagnosis is difficult if there is no skin disease. Awareness of the imaging appearances, especially in the spine, may help the radiologist in avoiding misdiagnosis (e.g., infection, tumor) and unnecessary invasive procedures, while facilitating early diagnosis and selection of an effective treatment. In this article, we provide an overview of the radiological appearances of SAPHO syndrome, focusing on the magnetic resonance imaging findings of vertebral involvement, and present relevant clinical and pathological features that assist early diagnosis.

Abstract

Benign neurogenic tumors at the sciatic notch that are purely intrapelvic have rarely been reported. Because of this tumor's particular position, a transabdominal or combined transabdominal-gluteal approach is usually used to achieve total resection. However, the transabdominal approach carries a remarkable surgical risk because of the manipulation of intraperitoneal organs. Here, the authors describe a 59-year-old woman harboring a purely intrapelvic sciatic notch schwannoma extrinsic to the sciatic nerve, which was totally removed via the infragluteal approach preserving sciatic function. The postoperative course was uneventful. The infragluteal approach can be safely used for the effective resection of intrapelvic benign neurogenic tumors at the sciatic notch that are extrinsic to the sciatic nerve, avoiding the more invasive and risky transabdominal approach. © 2013 AANS.

Abstract

Background: Spinal infections (pyogenic or non-pyogenic) are increasing in incidence and are a common cause of morbidity in high-risk patients (elderly, immunocompromised patients, diabetic patients, drug addicts, and patients with sickle-cell disease). Aim: To provide an overview of the radiological features of spinal infections, focusing on magnetic resonance (MR) imaging, and to illustrate the differential diagnosis. Materials and Methods: We reviewed the spine imaging of 118 patients with spinal infections from our files. All patients underwent radiography and MR imaging examinations. computed tomography (CT) was performed in 96 patients. Results: MR imaging has greatly contributed to prompt diagnosis, thus allowing implementation of timely appropriate treatment. Conclusions: Prompt diagnosis and treatment are essential to prevent serious bone and joint destruction, and severe neurologic sequelae. [ABSTRACT FROM AUTHOR]

Abstract

A series of 8 cases of elastofibroma is reported, and the clinical, pathological and imaging features and different therapeutic modalities are reviewed. On this basis, we suggest an algorithm for the diagnosis and treatment of elastofibroma. Abstract

The purpose of this study was to first evaluate Levovist (Schering, Berlin, Germany), an echo-contrast agent, during power Doppler sonography (PDS) in patients with synovitis using asymptomatic joints as controls. Then we evaluated the accuracy of this technique against contrast-enhanced MRI. Forty patients (19 men and 21 women; mean age 40 years) were enrolled on the basis of clinical signs, laboratory tests, and radiographic findings positive for articular inflammatory disease. They were examined with conventional ultrasonography (US) and PDS techniques before and after intravenous contrast medium injection. Fourteen patients then underwent MRI with and without contrast medium 8-14 days after PDS studies. Three expert readers independently evaluated each examination. After contrast medium, synovium in inflammatory arthritis enhanced on PDS compared with normal joints in the same patient. Power Doppler sonography after contrast medium and MRI were concordant in all cases. Power Doppler sonography with contrast medium showed a qualitative increase in signal from synovial vessels, the first sign of synovial changes in inflammatory diseases. [ABSTRACT FROM AUTHOR]

Abstract

Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is characterised by the accumulation of pyrophosphate dihydrate crystals in articular and periarticular tissues and it can be classified as sporadic, hereditary or secondary. The diagnosis frequently rests on radiographic findings. Computed tomography scanning can detect well mineralised deposits in joints and also ultrasound may be useful in detecting CPPD crystal deposits. About MRI recent studies have demonstrated the utility of high field in depiction of CPPD crystal deposits. The aim of this review is to focus on the clinical-classificative and radiological aspects of CPPD, particularly the contribution of the different imaging techniques.

Abstract

To verify the hypothesis that a drug that causes vasodilation can contribute to improvement in MRA spatial resolution. To test this hypothesis, a comparison was made between the images obtained using the high resolution time-of-flight (TOF HR 3D) technique and the TOF 3D MTC TONE technique.From November 1993 to December 1994, we studied 40 patients, of which 20 patients (16 males and 4 females; average age of 10 years; range from 3 to 20 years) we examined after they had inhaled Isoflurane (experimental group), and 20 patients (16 males and 4 females; average age 9.7 years; range from 3 to 12 years) were examined with standard MRA (control group).The vasodilator in both HR MRA and MTC TONE MRA permits a better spatial visualization with respect to the clinical routine MRA. On the other hand, it is true that MTC TONE gives better visualization of the small vessels.In our experience, this preliminary study indicates that with respect to routine MR, the spatial resolution is notably increased when Isoflurane is used. The signal-to-noise ratio is increased but, moreover, the ability to visualize small vessels is increased.The preliminary results obtained in this study indicate that a pharmacological drugs is capable of increasing the vascular detail of MRA images of the intracranial vessels and that continued research this direction is called for.

Abstract

We optimized the technique of contrast-enhanced Magnetic Resonance Angiography (MRA) with a .5 T superconductive magnet.Forty patients with normal blood pressure and heart rate gave their informed consent to MRA studies with contrast agent administration. The carotid arteries were studied in 10 patients, the pulmonary arteries in 10, the thoracic aorta in 10 and the abdominal aorta and renal arteries in 10. All the examinations were performed with a .5 T superconductive magnet (Philips T5) acquiring 3D T1-weighted GE sequences with contrast agent administration. The parameters were: TR/TE/FA 13 ms/4 ms/60 degrees; 256 x 256 MA; 2 mm slice thickness; 1 NEX. The contrast agent was administered with an automatic injector (.2 mmol/kg at 1.5 flowrate) after a bolus test to evaluate circulation time. The images were studied by a radiologist rating artery visualization as "good" or "poor" on a multiple choice card. The signal-to-noise ratio was evaluated using regions of interest positioned on the examined vessels (signal) and muscles (noise).The mean start delay was 12.2 s for the abdominal aorta, 10.3 s and 8.7 s for the thoracic aorta and pulmonary arteries, and 10.3 for the carotid arteries. Fifty-eight of 70 vessels were well visualized and 12 were poorly visualized. The signal-to-noise ratio exceeded 1 in all districts.Our experience shows that enhanced MRA provides diagnostic images of body arteries even at .5 T.
