The aim of the work was to evaluate the clinical and pathogenetic links of osteoarthritis (OA) with the qualitative composition of the drinking water in zones of patients’ residence. Material and Methods. The observation included 133 patients with primary OA aged 58.90 ± 0.83 years. In 33 urban and rural regions we have assessed levels of hardness and salinity of drinking water, concentration of chlorides, sulfates in it, phosphates and nitrates, content of toxic trace elements (Ba, Li, Ni, Pb). Results. Poor quality of drinking water in the regions where people live lead to the development of OA at a younger age, which subsequently determines the cumulative severity of articular syndrome, the nature of bone-destructive changes in the joints (osteoporosis, bone erosion, cartilage flaps), the formation of symptomatic reactive synovitis (chloride and nitrates concentration), the rate of disease progression, the emergence of intraarticular calcification deposits, osteophytosis and epiphyseal osteoporosis (degree of water salinity and hardness). The severity of OA and the rate of progression of bone-destructive changes in the joints depend on the level of trace elements, which determine the prevalence of joint syndrome, the development of Heberden’s and Bouchard’s nodes, spondylopathy, degree of reactive synovitis, osteoporosis, subchondral sclerosis, ligamentosis, intraarticular cartilage flaps and Hoffa’s bodies, and high content of Pb is a risk factor for unfavorable course of the disease. Conclusions. There were identified certain clinical and pathogenetic correlation of OA and the qualitative composition of drinking water in the regions where people live., Целью работы стала оценка клинико-патогенетических связей остеоартроза (ОА) с качественным составом питьевой воды в зонах проживания больных. Материал и методы. Под наблюдением находились 133 больных первичным ОA в возрасте 58,90 ± 0,83 года. Оценивали в 33 городских и сельских регионах уровни жесткости и минерализации питьевой воды, концентрации в ней хлоридов, сульфатов, фосфатов и нитратов, содержание токсичных микроэлементов (Ba, Li, Ni, Pb). Результаты. Низкое качество питьевой воды в регионах проживания людей способствует развитию ОА в более молодом возрасте, что в последующем определяет интегральную тяжесть артикулярного синдрома, характер костно-деструктивных изменений в суставах (развитие остеопороза, остеоузур, хондромных тел), формирование манифестного реактивного синовита (концентрации хлоридов и нитратов), темпы прогрессирования заболевания, появление внутрисуставных отложений кальцинатов, остеофитоза и эпифизарного остеопороза (степень минерализации и жесткости воды). Тяжесть ОА и темпы прогрессирования костно-деструктивных изменений со стороны суставов зависят от уровня микроэлементов, которые определяют распространенность суставного синдрома, развитие узелков Гебердена и/или Бушара, спондилопатии, степень реактивного синовита, остеопороза, субхондрального склероза, лигаментоза, интраартикулярных хондромных тел и тел Гоффа, а высокое содержание Pb является фактором риска неблагоприятного течения заболевания. Выводы: выявлены определенные клинико-патогенетические связи ОА с качественным составом питьевой воды в регионах проживания больных., Метою роботи стала оцінка клініко-патогенетичних зв’язків остеоартрозу (ОА) з якісним складом питної води в зонах проживання хворих. Матеріал і методи. Під спостереженням перебували 133 хворі на первинний ОA віком 58,90 ± 0,83 року. Оцінювали в 33 міських і сільських регіонах рівні жорсткості й мінералізації питної води, концентрації в ній хлоридів, сульфатів, фосфатів і нітратів, вміст токсичних мікроелементів (Ba, Li, Ni, Pb). Результати. Низька якість питної води в регіонах проживання людей сприяє розвитку ОА в молодшому віці, що в подальшому визначає інтегральну тяжкість артикулярного синдрому, характер кістково-деструктивних змін у суглобах (розвиток остеопорозу, остеоузур, хондромних тіл), формування маніфестного реактивного синовіту (концентрації хлоридів і нітратів), темпи прогресування захворювання, появу внутрішньосуглобових відкладень кальцинатів, остеофітозу й епіфізарного остеопорозу (ступінь мінералізації та жорсткості води). Тяжкість ОА і темпи прогресування кістково-деструктивних змін з боку суглобів залежать від рівня мікроелементів, які визначають поширеність суглобового синдрому, розвиток вузликів Гебердена та/або Бушара, спондилопатії, ступінь реактивного синовіту, остеопорозу, субхондрального склерозу, лігаментозу, інтраартикулярних хондромних тіл та тіл Гоффа, а високий вміст Pb є чинником ризику несприятливого перебігу захворювання. Висновки: виявлені певні клініко-патогенетичні зв’язки ОА з якісним складом питної води в регіонах проживання хворих.

Background. The urgency of the problem is of particular importance due to the increased incidence of lung cancer (LC) metastasis into the skeleton, which significantly worsens the survival rate of patients. This so-called “osteoarticular form” of LC is observed in 30–40 % cases of the disease. Purine bases belong to the biomarkers of bone metastases in LC, but their diagnostic and prognostic role has not been studied. The purpose was to determine the state of purine metabolism in the “osteoarticular form” of LC with the assessment of the clinical and pathogenetic significance of the identified changes. Materials and methods. The study enrolled 123 patients (all men) aged from 24 to 79 years. The ratio of the peripheral and central forms of cancer was 1 : 4, small-cell to non-small-cell histological variant — 1 : 4, and IB, IIA, IIB, IIIA, IIIB and IV stages of the disease — 1 : 2 : 4 : 6 : 5 : 6. The number of patients with metastases to the lymph nodes and distant organs was 89 and 62 %, respectively. The blood serum levels of products of purine metabolism (uric acid, oxypurinol, adenine, guanine, xanthine, hypoxanthine, xanthine oxidase, xanthine deaminase, adenosine deaminase, and 5'-nucleotidase activities) were studied. Results. Purine dysmetabolism is observed in 98 % of the patients with LC, which manifested by increased concentrations of uric acid and oxypurinol in the blood, xanthine oxidase and adenosine deaminase activities. The parameters of purine metabolism in LC reflect the severity of the disease, associated with the localization and morphological form of the tumor process, invasion in the breast, trachea, and esophagus, the presence of nerve root syndrome and obstructive atelectasis. Thirty-two per cent of the patients with LC were found to have the “osteoarticular form” of LC accompanying by an increase in the indices of uricemia, oxypurinemia, and adeninemia, involved in the pathogenetic constructs of the tumor process. The level of adenine (6-aminopurine) relating to the LC metastases in the skeleton has a predictive negative value. The survival of patients with “osteoarticular form” of LC is worse than in other patients.

Background. Low bone mass (LBM) in the form of osteopenia (Op) and osteoporosis (OP) is one of the most pres­sing public health problems. Рostmenopausal period in women is considered the main risk factor for the development of the pathological process. Objective of the research was to assess the clinical and pathogenetic features of LBM in women with menopause. Materials and methods. 261 non-menstruating women with an average age of 48 years were observed, physiological menopause occurred in 91 % of cases, and pathological in the rest. Exclusion criteria were women with inflammatory rheumatic diseases of the joints, oncological pathology, diseases of the blood system and thyroid gland, who received glucocorticoid hormones and anticonvulsants. In 133 (51 %) women, LBM (main group) was established in the ratio Op and ОР 4 : 1; diagnosis being made by the methods of conventional radiography and densitometry. Among the markers of bone metabolism, the following were used: blood levels of parathyrin (parathyroid hormone), calcitonin, osteocalcin, osteopontin, alkaline phosphatase, and osteo-associated chemical elements. During the examination, biochemical, enzyme immunoassay, atomic emission and atomic absorption methods were used. Results. The age of women correlates with the parameters of bone mineral density, and the LBM in all ca­ses is associated with high blood levels of osteocalcin, in 1/2 of them — osteopontin, depends on the presence of comorbid primary arterial hypertension, type 2 diabetes mellitus, and leukocytoclastic vasculitis. OP formation significantly differs from Op in higher blood levels of parathyrin (by 47 %), alkaline phosphatase (by 19 %), phosphorus (by 9 %) and lead (by 27 %), but lower calcium values (by 4 %), moreover, the acti­vity of alkaline phosphatase has predictive negative significance in relation to the severity of the LBM. Calcitonin, magnesium, strontium and zinc also participate in the pathogenetic constructions of the latter. Conclusions. LBM develops in every second postmenopau­sal woman, and the further development of pathogenetic therapy should be aimed at correcting the concentration of osteocalcin and osteopontin in the body, and inhibition of alkaline phosphatase activity may be a criterion for the effectiveness of therapeutic measures., Актуальность. Сниженная костная масса (СКМ) в виде остеопении (Оп) и остеопороза (ОП) является одной из наиболее актуальных проблем здравоохранения. Постменопаузальный период у женщин считается основным фактором риска развития патологического процесса. Цель исследования: оценить клинико-патогенетические особенности СКМ у женщин в постменопаузальном периоде. Материалы и методы. Под наблюдением находилась 261 неменструирующая женщина, средний возраст 48 лет, у которых в 91 % случаев имела место физиологическая менопауза, а у остальных — патологическая. Критериями исключения были женщины с воспалительными ревматическими болезнями суставов, онкологической патологией, болезнями системы крови и щитовидной железы, получавшие глюкокортикоидные гормоны и антиконвульсанты. У 133 (51 %) женщин установлена СКМ (основная группа) в соотношении Оп и ОП 4 : 1, для диагностики которой применяли методы рентгеновской абсорбциометрии и ультразвуковой денситометрии. Среди маркеров костного метаболизма изучали уровни в крови паратирина (паратгормона), кальцитонина, остеокальцина, остеопонтина, щелочной фосфатазы и остеоассоциированных химических элементов. При обследовании использовали биохимический, иммуноферментный, атомно-эмиссионный и атомно-абсорбционный методы. Результаты. Возраст женщин коррелирует с параметрами минеральной плотности костей, а развитие СКМ во всех случаях связано с высокими показателями в крови остеокальцина, у 1/2 из них — остеопонтина, зависит от наличия коморбидных артериальной первичной гипертензии, сахарного диабета типа 2 и лейкоцитокластического васкулита. От Оп формирование ОП достоверно отличается более высокими уровнями в крови паратирина (на 47 %), щелочной фосфатазы (на 19 %), фосфора (на 9 %) и свинца (на 27 %), но меньшими значениями кальция (на 4 %), причем активность щелочной фосфатазы имеет прогнознегативную значимость в отношении выраженности СКМ. В патогенезе последнего участвуют также кальцитонин, магний, стронций и цинк. Выводы. СКМ развивается у каждой второй женщины в постменопаузальном периоде, а дальнейшая разработка патогенетической терапии должна быть направлена на коррекцию концентраций в организме остеокальцина и остеопонтина, причем критерием эффективности лечебных мероприятий может быть угнетение активности щелочной фосфатазы., Актуальність. Постменопаузальний період у жінок вважається основним фактором ризику розвитку патологічного процесу. Мета дослідження: оцінити клініко-патогенетичні особливості ЗКМ у менопаузальних жінок. Матеріали та методи. Під наглядом перебувала 261 неменструююча жінка, середній вік 48 років, у яких в 91 % випадків мала місце фізіологічна менопауза, а у решти — патологічна. Критеріями виключення були жінки із запальними ревматичними хворобами суглобів, онкологічною патологією, захворюваннями системи крові та щитоподібної залози, які одержували глюкокортикоїдні гормони й антиконвульсанти. У 133 (51 %) жінок встановлено ЗКМ (основна група) у співвідношенні Оп і ОП 4 : 1, для діагностики якої застосовували методи рентгенівської абсорбціометрії та ультразвукової денситометрії. Серед маркерів кісткового метаболізму вивчали рівні в крові паратирину (паратгормону), кальцитоніну, остеокальцину, остеопонтину, лужної фосфатази й остеоасоційованих хімічних елементів. Вік жінок корелює з параметрами мінеральної щільності кісток, а розвиток ЗКМ у всіх випадках пов’язаний з високими показниками в крові остеокальцину, у 1/2 з них — остеопонтину, залежить від наявності коморбідних артеріальної первинної гіпертензії, цукрового діабету типу 2 і лейкоцитокластичного васкуліту. Від Оп формування ОП вірогідно відрізняється більш високими рівнями в крові паратирину (на 47 %), лужної фосфатази (на 19 %), фосфору (на 9 %) і свинцю (на 27 %), але меншими значеннями кальцію (на 4 %), причому активність лужної фосфатази має прогнознегативну значущість щодо вираженості ЗКМ. У патогенезі останнього беруть участь також кальцитонін, магній, стронцій та цинк. Висновки. ЗКМ розвивається у кожної другої жінки в постменопаузальному періоді, а подальша розробка патогенетичної терапії повинна бути спрямована на корекцію концентрацій в організмі остео­кальцину й остеопонтину, причому критерієм ефективності лікувальних заходів може бути пригнічення активності лужної фосфатази.

Background. Gastric cancer (GC) is considered an urgent problem of oncology, due to its high incidence and morta­lity, diagnosis at a later stage of the disease progression, anatomical and topographical features of the organ affected by the tumor and the low effectiveness of the existing methods of treatment. The role of purine dysmetabolism in carcinogenesis remains insufficiently studied, and its clinical significance requires clarification. An increased level of uric acid in the blood is a risk factor for the deve­lopment and further severe course of gastric cancer. Purine bases are considered to belong to the tumor biomarkers; however, their diagnostic and prognostic significance in GC remains uninvestigated. The purpose was to assess the state of purine metabolism in patients with gastric cancer, to establish the clinical and pathogenetic significance of the identified changes. Materials and me­thods. A total of 88 patients with cancer (all men) aged from 29 to 79 years (mean age 60 years) were analyzed. Localization of the tumor process occurred in the stomach pylorus in 76 % cases, in the body — in 16 %, in the antrum — in 3 %, and in 2 % of all cases examined the entire stomach was affected. Adenocarcinoma was diagnosed in 72 % of cases, undifferentiated adenogenic stomach cancer — in 18 %, squamous cancer — in 8 %, colloid GC — in 2 %. The ratio of IA, IB, IIA, IIB, IIIA, IIIB and IV stages of the GC was 1 : 3 : 9 : 12 : 6 : 5 : 6. The tumor spread to the spleen was found in 15 % of cases, the pancreas — in 7 %, the liver — 6 %, the esophagus — 3 %, the diaphragm — 2 %, the gallbladder — 1 %. Lymph node metastases were found in 60 % of the GC observations, distant metastases — in 14 %. The blood serum levels of products of purine metabolism — uric acid, hydroxypurinol, adenine, guanine, xanthine, hypoxanthine, xanthine oxidase, xanthine deaminase, adenosine deaminase, and 5'-nucleotidase activities were stu­died. Results. The changes in purine metabolism were found in all patients with gastric cancer, including compared with the control group of healthy people, the level of uric acid was 2.0 times higher, oxypurinol 4.5 times higher, adenine — by 16 %, xanthine oxidase activity by 2.1 times, xanthine deaminase — by 33 % and adenosine deaminase — by 7.1 times, the elevated rates of which were found in 99, 84, 39, 90, 53 and 100 % of cases, respectively, depending on the severity of the tumor process, total lesion of the stomach, spread of the tumor to the liver, pancreas, and esophagus, the presence of metastases in para-aortic lymph nodes, liver, lung, bones and brain. The altered purine metabolism worsens three-year survival of patients against the background of surgical, medical, and radiation treatment. Conclusion. Changes in purine metabolism are involved in the pathogenetic constructions of the GC, reflect the peculiarities of the course of the tumor process; and the parameters of uric acid and adenine have prognostic significance.

The aim and the objectives: to investigate the frequency and the character of renal lesion in systemic vasculitis, as well as aspects of the pathogenesis of nephropathy in this category of patients. Material and methods: investigate 525 patients, including 193 - with vasculitis Shenlayna-Henoch, 116- with microscopic polyangiitis, 93 - with cryoglobulinemic vasculitis, 39 - with polyarteritis nodosa, 29 - with aortoarteritis Takayasu, 28 - withWegener’s granulomatosis and 27 - with eosinophilic vasculitis Churg-Strauss. Results: the renal lesion is observed in 52-68% cases of vasculitis depends on the age and the sex of the patients, the duration of the disease, the degree of activity of the pathological process, alteration in the heart and lungs, integrated severity extrarenal signs of the disease, at that, the rate ofprogression of nephropathy associated with the character of the disorders of the immune system, the rheological properties of blood and endothelial vessels’function. Conclusions: kidney disease is one of the leading signs of systemic vasculitis and observed in most patients, it is determine the course of the disease.

Aim. To evaluate clinical-pathogenetic relationship between systemic vasculitis associated with anti-neutrophil cytoplasmic antibodies and vascular endothelial dysfunction. Materials and methods . In order to clarify relationships between clinical course of systemic vasculitis with the state of the endothelial vascular function the indicators of endothelial function of vessels in blood (the levels of endothelin-1, thromboxane A2, homocysteine, prostacyclins, nitrites, cyclic guanosine monophosphate) were studied with biochemical and immunoenzyme methods in 129 patients (76 - with microscopic polyangiitis, 26 – with Wegener's granulomatous polyangiitis and 27 - with eosinophilic polyangiitis Churg-Strauss). Results. The imbalance of levels of vasoconstrictors and vasodilators was determined. It was manifested by increased concentrations of endothelin-1 and nitrites in the background of the the amount of prostacyclin and homocysteine reduce. Conclusions . The abnormality of endothelial dysfunction of vessels in systemic vasculitis were involved in the vasculitis pathogenesis.
