Chirurgie und Anasthesie waren schon immer eng miteinander verbunden. Durch die Entwicklung immer komplexer werdender Therapien und der zur Verfugung stehenden technischen Moglichkeiten uberschneiden sich die beiden Facher in vielen Bereichen. Dieser Artikel beschaftigt sich mit der Frage, inwieweit die Anasthesie als Konkurrent oder als Partner in der Zusammenarbeit mit den verschiedenen operativen Fachrichtungen gesehen werden kann. Es hat sich in mehreren Studien eindeutig belegen lassen, dass das Outcome chirurgischer Patienten durch Interaktion und Kommunikation mit der Anasthesie verbessert werden kann und dass die Bildung multidisziplinarer Teams auf der Intensivstation der einzige Weg sein wird, der dem Wohl unserer Patienten dient.

Die Sepsis ist eine haufige Akuterkrankung mit einer nach wie vor hohen Letalitat. Durch fruhe Beteiligung des Gehirnes im Sinne eines sepsisassoziierten Delirs kommt dem Neurologen in der Fruhdiagnostik dieser interdisziplinaren Erkrankung eine besondere Bedeutung zu. Im folgenden Artikel sollen die wichtigsten diagnostischen Aspekte eines sepsisassoziierten Delirs dargestellt werden.

Regardless of improved diagnostics and therapeutic options, complicated intraabdominal infections remain a challenge in medicine. The prognosis of secondary peritonitis depends upon diagnosis and therapy in time. Diagnostic gold-standard is the computed tomography enhanced by contrast medium. Antimicrobial and supportive therapy is initiated before surgery and should be continued throughout and after the operation. Surgery aims on clearing the focus with a one-step-procedure in order to reduce perioperative morbidity and mortality.

Critically ill patients after extended surgical procedures are at high risk for postoperative infections. The overall incidence of sepsis increased constantly over the last decade, whereas sepsis-related mortality decreased, due to new intensive care options. After extended intra-abdominal surgery the abdomen is the predominant focus of sepsis, followed by respiratory tract infections. Unspecific clinical signs lead to the diagnosis of postoperative sepsis. Early focus identification by clinical, laboratory and radiologic examination is of major importance for a promising therapy, followed by immediately initiated procedures for source control and an adequate anti-infective drug therapy. For nosocomial sepsis, the use of antimicrobial regimens with extended spectra and potent activity against both Gram-negative and Gram-positive bacterial pathogens are warranted. Fungal infections play a great role for immunocompromized patients, e.g. after solid organ transplantation or under chronic corticoid therapy. An increasing incidence of multi-drug resistance in bacterial and fungal isolates has been observed in the last years. This is becoming a growing problem also for critically ill patients after abdominal surgery. Standardized treatment protocols including supportive and adjunctive therapy and the use of modern anti-infective agents may lead to a decrease in postoperative mortality due to sepsis.

Performance test results and operating characteristics of the Wedding & Associates' (W&A) PM10 Beta Gauge Automated Particle Sampler (Sampler), a U.S. EPA Designated Equivalent Method (Designation No. EQPM-0391-081), are reported. The custom, computer-controlled Sampler utilizes BETASOFT™, a multitasking operating system running the high level language application program. The Sampler employs the Wedding PM10 Inlet (Inlet) and Critical Flow Device (CFD). Analysis, calculations and mechanical control are achieved by use of analog sensing and digital logic. The development and testing program included wind tunnel and field tests to verify experimentally its conformity with theoretical calculations of the relationship of collected mass to 3- particle penetration.
