Since 2013, rotational thromboelastometry has been available in our hospital to assess coagulopathy. The aim of the study was to retrospectively evaluate the effect of thromboelastometry testing in cardiac surgery patients. Altogether 177 patients from 2012 and 177 patients from 2014 were included. In 2014, the thromboelastometry testing was performed on 56 patients. The mean blood drainage volume decreased and the number of patients receiving platelets decreased between 2012 and 2014. In addition, the use of fresh frozen plasma units decreased, and the use of prothrombin complex concentrate increased in 2014. When studied separately, the patients with a thromboelastometry testing received platelets, fresh frozen plasma, fibrinogen and prothrombin complex concentrate more often, but smaller amounts of red blood cells. In conclusion, after implementing the thromboelastometry testing to the practice, the blood products were given more cautiously overall. The use of thromboelastometry testing was associated with increased possibility to receive coagulation product transfusions. However, it appears that thromboelastometry testing was mostly used to assist in management of major bleeding.

Objective Cytokine release syndrome is suggested to be the most important mechanism triggering acute respiratory distress syndrome and end organ damage in COVID-19. The severity of disease may be measured by different biomarkers. Methods We studied markers of inflammation and coagulation as recorded in 29 patients on admission to the hospital in order to identify markers of severe COVID-19 and need of ICU. Results Patients who were eventually admitted to ICU displayed significantly higher serum levels of interleukin-6 (IL-6), C-reactive protein (CRP), and procalcitonin. No statistical differences were found between the groups in median levels of lymphocytes, D-dimer or ferritin. Conclusions IL-6 and CRP were the strongest predictors of severity in hospitalized patients with COVID-19.

Introduction Dexmedetomidine has been suggested to be a promising sedative for patients with Covid‐19 infection (CV19). However, use of dexmedetomidine is limited by its heart rate (HR) and arterial blood pressure lowering effects. Moreover, CV19 is associated with cardiac manifestations including bradyarrythmias. The hemodynamic effects of dexmedetomidine have not been previously studied in CV19 patients. We evaluated the effects of dexmedetomidine on hemodynamic and respiratory parameters of CV19 patients. Methods In this single center study, all CV19 patients receiving dexmedetomidine for sedation during a one year period were included. Our primary outcomes included changes in HR, mean arterial pressure (MAP), respiratory rate (RR), partial oxygen pressure of arterial blood/fraction of inspired oxygen‐ratio (PF‐ratio), and Richmond Agitation and Sedation Score (RASS) during dexmedetomidine administration. Background: In the early phase of the coronavirus disease-19 (Covid-19) pandemic, Southwest Finland remained relatively spared. By the 3rd of May 2020, a total of 28 patients have been admitted to the Turku University Hospital. In this paper, we explore baseline characteristics in order to identify risk for severe Covid-19 disease and critical care admission. Methods For this retrospective cohort study, data were derived from hospital records. Basic descriptive statistics were used to characterise patients, including medians, percentiles and frequencies. Differences were tested with Mann Whitney U-test and Pearson's chi-square test. Results Pre-existent obstructive sleep apnoea (OSA) was present in 29% of patients admitted in the hospital for Covid-19, none of them having severe OSA. Overall, other findings on admission were comparable with those reported elsewhere. Organisms in the wild are likely to face multiple immune challenges as well as additional ecological stressors, yet their interactive effects on immune function are poorly understood. Insects are found to respond to cues of increased infection risk by enhancing their immune capacity. However, such adaptive plasticity in immune function may be limited by physiological and environmental constraints. Here, we investigated the effects of two environmental stressors - poor larval diet and an artificial parasite-like immune challenge at the pupal stage - on adult immune function, growth and development in the greater wax moth (Galleria mellonella). Males whose immune system was activated with an artificial parasite-like immune challenge had weaker immune response - measured as strength of encapsulation response - as adults compared to the control groups, but only when reared on high-nutrition larval diet. Immune activation did not negatively affect adult immune response in males reared on low-nutrition larval diet, indicating that poor larval diet improved the capacity of the insects to respond to repeated immune challenges. Low-nutrition larval diet also had a positive independent effect on immune capacity in females, yet it negatively affected development time and adult body mass in both sexes. As in the nature immune challenges are rarely isolated, and adverse nutritional environment may indicate an elevated risk of infection, resilience to repeated immune challenges as a response to poor nutritional conditions could provide a significant fitness advantage. This study highlights the importance of considering environmental context when investigating the effects of immune activation in insects.

We investigated the intra- and trans-generational effects of larval diet on immune function, body size and development time of the greater wax moth, Galleria mellonella (Lepidoptera: Pyralidae). We found that moths reared on a diet diluted with cellulose (a low-nutrition diet) were about one-third smaller, had about one-fifth longer development time and exhibited about 10% stronger encapsulation responses as compared with moths reared on the standard diet. The low-nutrition parental diet prolonged the development time of male offspring that were fed the low-nutrition diet by about 4% and the development time of female offspring that were fed the standard diet by about 1%. However, females that were fed the low-nutrition diet attained about 6% greater body mass when their parents were reared on the low-nutrition diet. Our results add to the growing number of studies demonstrating that the nutritional history of parents can affect the performance of their offspring.

The aim of this study was to assess the significance of the interleukin 6 gene polymorphism -174 in gastric cancer risk. The interleukin 6 -174 G/C (rs1800795) gene polymorphisms was analyzed in gastric cancer, peptic ulcer, and nonulcer dyspepsia patients and in healthy control subjects and the data were correlated with the histopathological features of the patients' biopsies. The interleukin 6 -174 GG and GC genotypes have been previously associated with high interleukin 6 serum levels. We discovered that the interleukin 6 -174 GG and GC genotypes are associated with an increased risk of the diffuse histologic subtype of gastric carcinomas (OR: 6.809, P = 0.034), but absent in the intestinal type carcinomas (OR: 1.109, P = 0.908). No significant associations with peptic ulcer, gastric atrophy, or intestinal metaplasia were seen. Our results demonstrate that the interleukin 6 -174 GG and GC genotypes increase the risk of the diffuse type gastric carcinoma, but not the intestinal type gastric carcinoma or its precursor conditions, including atrophy or intestinal metaplasia. Thus, interleukin 6 seems to be an important carcinogenetic factor in the diffuse type gastric adenocarcinoma and its carcinogenetic effect could be noninflammatory. © 2013 Wiley Periodicals, Inc.

The impact of nutritional deficiencies early in life in determining life-history variation in organisms is well recognized. The negative effects of inbreeding on fitness are also well known. Contrary to studies on vertebrates, studies on invertebrates are not consistent with the observation that inbreeding compromises resistance to parasites and pathogens. In this study, we investigated the effect of early nutrition on the magnitude of inbreeding depression in development time, adult body size and adult resistance to the bacterium Serratia marcescens in Drosophila melanogaster. We found that early nutritional environment had no effect on the magnitude of inbreeding depression in development time or adult body size but may have played a small role in adult resistance to the bacterial infection. Estimates of heritabilities for development time under the poor nutritional environment were larger than those measured under the standard nutritional conditions.

Susceptibility to pathogens and genetic variation in disease resistance is assumed to persist in nature because of the high costs of immunity. Within immunity there are different kinds of costs. Costs of immunological deployment, the costs of mounting an immune response, are measured as a change in fitness following immunological challenge. Maintenance costs of immunity are associated with investments of resources into the infrastructure of an immune system and keeping the system at a given level of readiness in the absence of infection. To demonstrate the costs of immunological maintenance in the absence of infection is considered more difficult. In the present study we examined the maintenance costs of the immune system in lines of Drosophila melanogaster that differed in their antibacterial innate immune response under starved and non-starved conditions. Immunodeficient mutant flies that have to invest less in the immunological maintenance were found to live longer under starvation than wild type flies, whereas the opposite was found when food was provided ad libitum. Our study provides evidence for the physiological cost of immunological maintenance and highlights the importance of environmental variation in the study of evolutionary trade-offs.

Salmonella typhimurium derivatives with either the original group B-type O antigens 1, 4, (5,) 12 or the transduced group D-type O antigens 1, 9, 12 have been previously shown to have different LD50 doses in intraperitoneal infection of the mouse, so that the 1, 4, (5,) 12 derivatives are more virulent than the 1, 9, 12 derivatives. The strains compared differed from each others in only a small part of their chromosome including the rfb cluster, which determines the O antigen specificity. We have now investigated a possible immunological basis of this difference by comparing the same strains as pathogens in thymectomized, X-irradiated mice. Although the LD50 doses were much reduced by the treatment, the difference between the 1, 4, (5,) 12 and the 1, 9, 12 strains was retained. No antibodies towards either type of bacteria were demonstrable in the uninfected mice.

Three-body processes go on in star clusters where binary stars meet single stars and frequently form temporary triple systems. The triples are typically unstable and break up into a new binary and a single star. Also a simple scattering of a single star from a binary may take place. Both processes can be handled by the statistical theories of three-body break-up and scattering. Here we apply the theory to binary stars, assuming that binaries have been involved in the three-body process. The distributions of binary periods, eccentricities and mass ratios are discussed from this point of view and compared with observational samples.

Peripheral concentrations of melatonin in the neonatal mammal are low and the diurnal rhythm is not apparent during the first days or weeks of life. Maternal milk provides a rhythmic although low source of melatonin to the newborn. Diurnal rhythm of serum melatonin was monitored in Friesian calves from birth to 4 weeks of age. Some of the calves under 1 week of age had no clear nocturnal elevation of serum concentrations of melatonin. Thereafter the nocturnal melatonin elevation was apparent in all calves and of the same amplitude as in adult cattle. The effect of oral intake of night-time milk or milk supplemented with low doses of synthetic melatonin was studied during day-time in a group of Ayrshire calves aged 1 to 5 weeks. Two and a half or 3 litres of night-time milk with a mean melatonin concentration of 40 pg/ml, providing 01 to 012 μg melatonin, did not cause any change in the day-time serum concentrations of melatonin, neither did addition of melatonin in doses up to 10 μg per animal. Only the highest dose of melatonin 200 μg in 2 l of milk (01 μg/ml) caused an elevation of day-time serum concentrations of melatonin. These studies show that the pineal gland of a very youg calf is functional and that the concentration of melatonin in blood does not reflect the concentration in milk at the time of feeding. The presence of melatonin in milk suggests, however, its potential significance for the neonate.

Interleukin-1 receptor antagonist genotype 2/2 is associated with a prolonged and enhanced inflammatory response. It is suspected of being a risk factor for atrophic gastritis and gastric cancer and for some autoimmune diseases. No specific genetic risk factors for oesophagitis have been identified so far and there are no reports of IL-1 polymorphism in relation to oesophageal disease.We studied the IL-1RN, IL-1beta-511 and IL-1beta + 3953 polymorphisms in an unselected series of 142 adult patients scheduled for gastrointestinal endoscopy because of dyspepsia. The control group consisted of university staff and students (n = 179). Helicobacter pylori status was determined by antibody testing and bacterial detection.Endoscopic oesophagitis was noted in 40 patients. The IL-1RN 2/2 genotype was significantly more prevalent in the patients with H. pylori-negative oesophagitis than in the control subjects (27% versus 9%; OR 3.574, CI 1.23-10.35, P = 0.034) or in the dyspeptic patients (27% versus 7%; OR 5.089. CI 1.51-17.11, P = 0.009). IL-1beta-511 T/T genotype tended to be more frequent in the H. pylori-negative patients with oesophagitis than in the control subjects (P = 0.071). The strongest association was between the simultaneous carriage of genotypes IL-1RN 2/2 and IL-1beta -511 T/T and H. pylori-negative oesophagitis. where the combined genotype was more prevalent than in the control subjects (23% versus 6%; OR 4.492, CI 1.40-14.46, P = 0.012) or the dyspeptic patients without oesophagitis (23% versus 3%: OR 9.706. CI 2.12-44.42, P = 0.003).The results indicate that the IL-1RN 2/2 genotype and the carriage of combined genotypes IL-1RN 2/2 + IL-1beta-511 T/T are associated with H. pylori-negative oesophagitis. This is the first report on the association between IL-1 gene polymorphism and oesophagitis.

In addition to nutritional conditions experienced by individuals themselves, those experienced by their parents can affect their immune function. Here, we studied the intra- and trans-generational effects of larval diet on susceptibility to an entomopathogenic fungus, Beauveria bassiana, in the greater wax moth, Galleria mellonella. In the first part of the study, a split-brood design was used to compare the susceptibility of full sibs raised either on low- or on high-nutrition larval diet. In the second part of the study, a similar experimental design was employed to investigate the effects of maternal and paternal diet as well as their interaction on offspring's susceptibility. In the first part of the study, we found that individuals fed with high-nutrition diet had higher mortality from infection than individuals fed with low-nutrition diet. However, diet did not affect post-infection survival time. Conversely, in the second part of the study, maternal diet was found to have no significant effect on final mortality rate of offspring, but it affected survival time: larvae with high-nutrition maternal diet survived fewer days after infection than larvae with low-nutrition maternal diet. Paternal diet had no significant effect on offspring's susceptibility to the fungus, indicating that paternal effects are not as important as maternal effects in influencing immune function in this species. Our findings provide further indication that maternal nutrition affects immune function in insects, and suggest that the direct effects of nutrition on immunity may be different, yet parallel, to those caused by parental nutrition.

It is possible that supermassive black holes are ejected from centers of galaxies at high speeds. If the ejection happens in a nearby galaxy, then some black holes will travel towards us and may appear quasar-like with a blueshifted spectrum due to the Doppler effect. But quasar spectra are as a rule interpreted as having been redshifted even when there is an equally good or better case for a blueshifted spectrum. Here we study the quasars which are apparently associated with galaxies. We look for alternative identification for the spectral lines reported in the quasar spectra and offer blueshift values whenever it is consistent with the available information. We present a list of five candidates predicting the lines for future observation to confirm blueshifts. We also present a composite IR spectra which can be adjoined to existing composite spectra of lines in the optical and the UV region. We suggest that many of these objects are much less massive satellite bodies which have been ejected from nearby galaxies during binary black hole mergers.

This triennial report (1996-1999) reviews the subject from a somewhat personal angle, mostly related to binary star formation and young binary star populations – a subject whose time had come in the early 1990s and is now in full swing.Many astronomers have searched for binary systems among main-sequence stars, and two large-scale surveys published in 1991 and 1992 have already become classics. Well before they became famous for finding extrasolar planets (see below), observing teams led by Michel Mayor (Geneva Observatory) and Geoffrey Marcy (San Francisco State Univ., now Univ. of Calif, at Berkeley) spent many years searching for low-mass stellar companions of nearby stars. The late Antoine Duquennoy and Mayor surveyed all solar-type dwarfs (spectral types F7 through G9) within 20 pc of the Sun, while Debra Fischer and Marcy studied stars with somewhat lower mass (M dwarfs) slightly nearer to the Sun.

We have obtained new optical spectra of the radio galaxy 3C 129 and the giant galaxy close to it. From these spectra we deduce a relative radial velocity of 710 km s−1 between the two galaxies. Using the orbit calculations of Byrd & Valtonen (1978) and the new observations we obtain a new value, 3.3 × 1014M⊙, for the mass of the system.

In gyrate atrophy of the choroid and retina with hyperornithinaemia (GA), a genetically determined deficiency of ornithine delta-aminotransferase activity leads to high ornithine concentrations in body fluids. GA is characterized by centripetally progressing retinal and choroidal destruction and selective atrophy with tubular aggregates in type II skeletal muscle fibres. These findings have been suggested to be mediated by hyperornithinaemia-induced deficiency of high-energy creatine phosphate. As abnormal brain magnetic resonance images and electroencephalograms are found in another disorder of creatine metabolism, guanidinoacetate methyltransferase deficiency, we investigated the central nervous system involvement in GA, which seems to be associated with a milder degree of phosphocreatine deficiency. We compared 23 untreated GA patients with age-matched healthy controls, and with 9 patients who had received creatine or creatine precursor supplementation daily for several years. The mean age of the patients (32 +/- 18 years) was similar to that of the controls (36 +/- 22 years). The MRI or EEG findings of the patients on creatine supplementation did not differ from those of the untreated group. Brain MRI revealed degenerative lesions in the white matter in 50% of the GA patients, and 70% of the patients had premature atrophic changes, with a striking increase in the number of Virchow's spaces. Of the patients whose EEG was recorded, 58% had abnormal slow background activity, focal lesions or high-amplitude beta rhythm (50 microV). The EEG findings were not associated with the MRI changes or with the age or the sex of the patients. Early degenerative and atrophic brain changes and abnormal EEG are thus features of GA, in addition to the well-characterized eye and muscle manifestations.

Because high ornithine concentrations inhibit creatine biosynthesis, the ensuing deficiency of high-energy creatine phosphate may mediate the pathogenesis. Materials and methods Relative concentrations of inorganic phosphate (Pi), creatine phosphate (PCr) and ATP in resting calf muscle were recorded in 23 GA patients and 33 control subjects using 31P-magnetic resonance spectroscopy (MRS). Eight patients with autosomal recessive retinitis pigmentosa with matched control subjects constituted an additional reference group. Results The PCr/Pi and PCr/ATP ratios (means ± SD) were lower for the GA patients than for healthy control subjects [4.66 ± 0.37 vs. 9.75 ± 2.17 (P

Over 13600 continuum observations of extragalactic sources are presented. These observations of 157 sources at 22, 37 and 87 GHz more than doubles the millimeter observations of these sources. The data are between 1990.5 and 1995.5, and combined with our earlier published data they form a 15 year database.

We investigated whether the melatonin levels in bovine milk exhibit a similar daily rhythm as serum levels. In 4 Ayrshire cows at the beginning of the lactation period in May the nocturnal rise in milk melatonin was moderate (from 7 +/- 2 pg/ml at noon to 15 +/- 1 pg/ml at night; mean +/- SEM) and did not correlate well with the melatonin level in serum (from 7 +/- 2 pg/ml to 27 +/- 7 pg/ml, respectively). On the other hand, 6 cows in a later phase of lactation, studied in February, showed a clear long-lasting nocturnal melatonin increase both in serum (from 9 +/- 1 pg/ml at noon to 26 +/- 3 pg/ml at night) and in milk (from 12 +/- 5 pg/ml to 26 +/- 7 pg/ml, respectively). Melatonin kinetics during lactation was studied in more detail in 4 Ayrshire cows and 4 dairy goats by giving an intravenous bolus injection of melatonin. A 3-compartment model with melatonin elimination from the central compartment was used to describe the data. The values (mean +/- SD) for the cows and the goats were: elimination half-life 27 +/- 4 min and 27 +/- 1 min, mean residence time 24 +/- 4 min and 18 +/- 4 min, steady state distribution volume 1.0 +/- 0.3 l/kg and 0.6 +/- 0.1 1/kg (p < 0.05), and plasma clearance 0.044 +/- 0.004 l/kg/min and 0.035 +/- 0.011 l/kg/min, respectively. Following injection, the melatonin concentration in milk increased rapidly and exceeded the corresponding serum value 15-30 min later, remaining thereafter above the serum level. Our results suggest that milk melatonin levels reflect blood concentrations of melatonin with a short delay.

In order to analyse possible triggering or contributing infections and HLA-B27 frequency in patients with acute febrile joint syndrome fulfilling the proposed criteria of adult Still's disease (AOSD), we studied prospectively the serological findings of 25 patients. They were aged 15–62 years and diagnosed between 1978–1992, We then compared results with a control group consisting of 119 healthy persons. Positive viral or bacterial serology was found in 12 patients (48%) in the AOSD group compared with 13 cases (11%) in the control group (p< 0.001). Fourfold or higher viral antibody rise was found in two patients and bacterial antibody rise in three patients. High stable viral antibody titre was observed in one patient and high stable bacterial antibody titre in six patients. HLA-B27 was not overrepresented in the study group (12%) compared with a healthy Finnish population (14%). We conclude that many different bacterial and viral infections may trigger or contribute to AOSD.

In order for the male courtship traits to honestly signal quality they need to be condition-dependent. Moreover, if these traits capture genetic variation in condition they should resemble life-history traits in being subject to strong directional selection and, consequently, suffer strong inbreeding depression. In this study we investigated the effect of high inbreeding on male attractiveness by assessing mating success, mating speed and copulation duration of inbred, outbred and crossbred (constructed by crossing separate, randomly chosen inbred lines) males of Drosophila melanogaster. When set to compete against a standardized competitor and compared to the success rate of the crossbred lines, inbreeding significantly reduced male mating success. Under competition, outbred males initiated copulation significantly sooner than crossbred and inbred males. Under non-competitive conditions, no effect of inbreeding was found on either mating speed or copulation duration. Both mating success and mating speed showed much higher inbreeding depression than male size.

Nine anesthetized and mechanically ventilated test pigs were intracavally infused with a 10% allogeneic bone marrows suspension at a dose of 100 mg/kg over 5 min; 9 control pigs received saline. The cardiac index decreased slightly in the fat embolism animals. Left ventricular stroke work remained unchanged in both groups, whereas right ventricular stroke work rose in the fat embolism animals. Mean arterial pressure remained stationary in the test group but rose in the controls. Pulmonary capillary wedge pressure and central venous pressure rose slightly in the test group. After infusion of the bone marrow suspension, mean pulmonary arterial pressure, pulmonary vascular resistance, pulmonary shunt and alveoloarterial oxygen tension difference increased immediately and remained elevated throughout the study period. Simultaneously, arterial and mixed venous oxygen tension and hemoglobin oxygen saturation decreased. Arterial carbon dioxide tension and hematocrit remained stationary. The fat embolism animals developed gradual fever. In conclusion, increased mean pulmonary arterial pressure, pulmonary vascular resistance, pulmonary shunt and alveoloarterial oxygen tension difference proved to be useful early indicators of experimental fat embolism.

Quantitive collection of night-time urine every 2nd week was used to elucidate the temporal relationship between changes in melatonin production, weight gain, and priming of the winter pelage under natural light conditions and under a long photoperiod (18L: 6D) imposed at different times in four groups of female mink during autumn. In mink maintained outdoors under the natural photoperiod (group 1), melatonin excretion was 1·80 (s.d. 0·80) ng per night (no. = 3) at the beginning of September, thereafter it began to decrease slowly until November when it was only 0·62 (s.d. 0·32) ng per night. These mink had a normal autumn moult during the first half of October and the winter pelage was mature at the end of November. When mink maintained outdoors were transferred to 18L: 6D on 7 September (group 2), excretion of melatonin decreased promptly, autumn moult was disrupted and winter fur priming delayed. Long photoperiod after mid October (group 3), did not interfere with pelage maturation despite the decrease in melatonin production. Mink implanted with melatonin on 14 July (group 4) showed very high melatonin excretion in early September. In these animals the winter pelage growth was unaffected by the long-day conditions; the pelage matured in mid October. Body weight increased in mink of all groups during autumn. This increase levelled off or a weight loss (group 2) was seen along with the final hair maturation. Activation of hair follicles occurred during a 4 to 6 week period. According to these results, melatonin is the photoperiodic signal to autumnal weight increase and autumn moult but seems not to be necessary for later pelage growth and maturation.

Gyrate atrophy of the choroid and retina with hyperornithinaemia (GA), an autosomal recessive disease, affects skeletal muscle in addition to the eye. Muscle biopsy samples show prominent type 2 muscle fibre atrophy. Atrophic fibres also contain accumulations of tubular aggregates in electron microscopy. To evaluate skeletal muscle involvement in detail, the thigh muscles of 7 patients with GA were examined using semi-open conchotome muscle biopsies, computed tomography (CT) and ultralow-field magnetic resonance imaging (MRI) at 0.02 T. In MRI, the T1 and T2 proton relaxation times were measured. Type 2 muscle fibre atrophy was found in all biopsy samples (100%) and tubular aggregates in 6 of the 7 samples studied (87%). The CT density of the muscle tissue was increased in the only child of the study, decreased in 3 elderly subjects, and normal in the remaining 3 patients. Mean T1 relaxation times of the patients were decreased (135 +/- 5 ms) as compared to those of 6 healthy controls (157 +/- 12 ms) (p = 0.002). The T2 relaxation time was slightly increased (40 +/- 3 ms) as compared to the controls (35 +/- 7 ms; not significant, p = 0.3). Our findings suggest that the thigh muscles of the patients with GA universally show changes in CT and MRI studies, but relaxation time measurements gave little additional information on the muscle metabolism.

Fifty-one patients with musculoskeletal infection were imaged by repeated MR imaging at ultra low-field and low-field strength. Soft-tissue infection, osteomyelitis, septic arthritis, and spondylitis were studied. The MR finding was scored according to the signal intensity (SI) on T2-weighted images (T2WI), and correlated with the values of serum C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and peripheral white blood cell (WBC) count. There was a positive correlation between the MR score and both CRP and ESR, but no correlation between MR score and WBC. The MR score between the follow-up studies decreased significantly in accordance with clinical reconstitution. The MR finding according to the SI on T2WIs corresponded better to disease activity than did the CRP or ESR.

The effects of antegrade and of combined antegrade and retrograde cardioplegia were compared in 101 patients undergoing elective coronary artery surgery. The patients were randomly allocated to two groups: antegrade cardioplegia was administered in 53 patients and combined cardioplegia in 43 patients. The patients of the two groups were similar in age, sex and left ventricular ejection fraction. Aortic clamping time and the number of coronary bypasses were equal in the groups. The ventricular septal temperature was measured continuously during cardioplegia administration, after each distal anastomosis accomplished, and continuously after aortic declamping. Serum CK-MB activities were serially measured for up to 3 days postoperatively. Electrocardiograms (ECG) were taken preoperatively, as well as on the first, second and eighth postoperative days. The left ventricular function was evaluated with a volume load test preoperatively and on the first postoperative morning. The two groups were similar with respect to myocardial cooling, response to volume loading, the number of patients with perioperative myocardial infarctions, cardiac arrhythmias or atrioventricular conduction blocks and clinical outcome. However, the CK-MB activities were lower in the antegrade group suggesting better myocardial protection in an unselected group of patients undergoing coronary artery bypass grafting.

We propose a kinematic approach to searching for the stars that could be formed with the Sun in a common "parent" open cluster. The approach consists in preselecting suitable candidates by the closeness of their space velocities to the solar velocity and analyzing the parameters of their encounters with the solar orbit in the past in a time interval comparable to the lifetime of stars. We consider stars from the Hipparcos catalog with available radial velocities. The Galactic orbits of stars have been constructed in the Allen--Santillan potential by taking into account the perturbations from the spiral density wave. We show that two stars, HIP 87382 and HIP 47399, are of considerable interest in our problem. Their orbits oscillate near the solar orbit with an amplitude of about 250 pc; there are short-term close encounters to distances, 17 pages, 9 figures

Diurnal variation in blood melatonin levels and renal melatonin excretion was monitored in five ewes by blood sampling and quantitative urine collection at 2-h intervals. A typical secretory pattern of melatonin was seen both in blood and urine levels and in the renal excretion of melatonin. Serum melatonin levels increased from daytime values of approximately 200 pmol/l to a mean of 800 pmol/l during darkness. Urine flow rate and urine osmolality did not show any clear diurnal rhythm. To examine whether urine flow rate affects renal melatonin excretion at night, urine was collected in three consecutive 30-min fractions, and blood was sampled in the middle of each urine collection period when the sheep were in normal water balance or after hydration. Hydration increased urine flow rate over sixfold and decreased urine osmolality well below plasma osmolality. Glomerular filtration rate, measured as creatinine clearance, did not change. Serum melatonin concentrations did not differ between hydrated and non-hydrated sheep. However, urinary melatonin excretion was 1·1 ± 0·3 (s.e.m.) pmol/min at midnight in normal water balance, and significantly higher (2·6 ± 0·4 pmol/min) in the hydrated state. In this study, the validity of urinary melatonin determinations as an indicator of pineal function was confirmed in normal water balance. In addition, our results suggest that a high tubular fluid load during diuresis increases urinary melatonin excretion because of decreased tubular reabsorption. This means that urine flow rate should be taken into consideration in studies where urinary melatonin levels are used as an index of production of the indole by the pineal gland. Because of their decreased overall fitness and genetic variability inbred individuals are expected to show reduced survival and lifespan under most environmental conditions as compared with outbred individuals. Whereas evidence for the deleterious effects of inbreeding on lifespan has been previously provided, only a few studies have investigated effects of inbreeding on survival under starved conditions. In the present study we compared the abilities of inbred and outbred adult Drosophila melanogaster to survive under starved and fed conditions. We found that inbreeding reduced lifespan but had no effect on starvation resistance. The results indicate highly trait specific consequences of inbreeding. Possible mechanisms behind the observed results are discussed.

The impact of nutritional deficiencies early in life in determining life-history variation in organisms is well recognized. The negative effects of inbreeding on fitness are also well known. Contrary to studies on vertebrates, studies on invertebrates are not consistent with the observation that inbreeding compromises resistance to parasites and pathogens. In this study, we investigated the effect of early nutrition on the magnitude of inbreeding depression in development time, adult body size and adult resistance to the bacterium Serratia marcescens in Drosophila melanogaster. We found that early nutritional environment had no effect on the magnitude of inbreeding depression in development time or adult body size but may have played a small role in adult resistance to the bacterial infection. Estimates of heritabilities for development time under the poor nutritional environment were larger than those measured under the standard nutritional conditions.

To test two hypotheses of psychiatric comorbidity in bipolar disorder (BD): (i) comorbid disorders are independent of BD course, or (ii) comorbid disorders associate with mood.In the Jorvi Bipolar Study (JoBS), 191 secondary-care outpatients and inpatients with DSM-IV bipolar I disorder (BD-I) or bipolar II disorder (BD-II) were evaluated with the Structured Clinical Interview for DSM-IV Disorders, with psychotic screen, plus symptom scales, at intake and at 6 and 18 months. Three evaluations of comorbidity were available for 144 subjects (65 BD-I, 79 BD-II; 76.6% of 188 living patients). Structural equation modeling (SEM) was used to examine correlations between mood symptoms and comorbidity. A latent change model (LCM) was used to examine intraindividual changes across time in depressive and anxiety symptoms. Current mood was modeled in terms of current illness phase, Beck Depression Inventory (BDI), Young Mania Rating Scale, and Hamilton Depression Rating Scale; comorbidity in terms of categorical DSM-IV anxiety disorder diagnosis, Beck Anxiety Inventory (BAI) score, and DSM-IV-based scales of substance use and eating disorders.In the SEM, depression and anxiety exhibited strong cross-sectional and autoregressive correlation; high levels of depression were associated with high concurrent anxiety, both persisting over time. Substance use disorders covaried with manic symptoms (r = 0.16-0.20, p0.05), and eating disorders with depressive symptoms (r = 0.15-0.32, p0.05). In the LCM, longitudinal intraindividual improvements in BDI were associated with similar BAI improvement (r = 0.42, p0.001).Depression and anxiety covary strongly cross-sectionally and longitudinally in BD. Substance use disorders are moderately associated with manic symptoms, and eating disorders with depressive mood.

SUMMARYEcological immunology is based upon the notion that activation and use of the immune system is costly and should thus be traded off against other energy-demanding aspects of life history. Most of the studies on insects that have examined the possibility that mating results in trade-offs with immunity have shown that mating has immunosuppressive effects. The connection between mating and immunity has traditionally been investigated using indirect measures of immunity. However, studies that have assessed the effects of mating on the resistance against real pathogens have had conflicting results. A previous study on Tenebrio molitor showed that copulation decreases phenoloxidase activity in the haemolymph, and concluded that copulation corrupts immunity in this species. In the present study we tested whether mating also affects the ability of T. molitor to resist the entomopathogenic fungus, Beauveria bassiana. Interestingly, we found that mating enhanced resistance against the fungal infection and that the effect was stronger on males than females. Furthermore, we found that male beetles were overall more susceptible to the fungal infection than were females, indicating an immunological sex difference in immunity. Our study highlights the importance of the use of real pathogens and parasites in immuno-ecological studies.

A patient who had had been treated by surgery for spinal echinococcosis in a Mediterranean country emigrated to northern Europe. After surgery, the echinococcosis disseminated and he developed chronic lytic and sinus-draining hydatid disease of the left hip and neuromuscular weakness of the left lower extremity. Seventeen years after the spinal surgery, he was referred to our hip service for a possible total hip replacement (THR) after receiving adequate chemotherapy against echinococcosis for nearly 3 years. Because of the poor results reported by others, we decided against THR; two of the four previous patients described in the current literature who had THR died due to complications.

Central haemodynamics, blood gases, visceral and peripheral tissue perfusion and oxygenation were studied in six patients undergoing coronary artery bypass grafting (CABG) - including one patient with aortic valve reconstruction and another with CABG and aortic valve reconstruction. The patients were operated upon under moderate haemodilution and systemic hypothermia. Visceral perfusion was indirectly assessed by determining the gastric intramucosal pH (pHi). Peripheral tissue perfusion was assessed by continuous recording of subcutaneous tissue PO 2 (PscO2), laser-Doppler skin red cell flux (RCF), transcutaneous PO2 (PtcO2) and fingertip temperature (Tft) in the upper extremity. During cardiopulmonary bypass (CPB) at the deepest hypothermia, pHi, PaO2 and PtcO2 reached maximum values simultaneously with the lowest calculated oxygen utilization: pHi reached its minimum at the end of the operation. During CPB, PscO2, RCF and Tft decreased markedly, rose during rewarming and fell again at the end of surgery. These data suggest that the patients' visceral perfusion is well maintained during CPB, and the patients develop hypoperfusion and hypoxia of peripheral tissues at the same time. After closing the wounds, pHi, PtcO2 index (=PtcO 2/PaO2) and other peripheral tissue perfusion parameters gained the lowest values indicating impending residual hypothermia and tissue hypoperfusion after rewarming.

The influence of the air-gap length on the performance of a 45 kW / 6000 min-1 axial-flux aluminium-cage solid-rotor-core induction motor is studied. The length of the air-gap has a significant influence on the characteristics of an electric machine. In the machines that are designed for speeds above 3000 min-1 (which can be reached with the 50 Hz network), the air-gap length has to be increased considerably from the value obtained for a standard electric motor. Especially, if the motor is equipped with a solid rotor core, the air-gap length has to be designed with special care. It is shown that the efficiency of the studied motor is highly dependent on the rotor eddy current losses. The determination of the losses is done by using numerical 2D FEM calculations and measurements for the prototype machine. In this paper, it is shown that by changing the air-gap length the loss minimum can be found between the rising stator copper losses and the diminishing rotor harmonic eddy current losses.

The historical optical light curve of OJ287 shows repeated outbursts at 11.65 yr intervals. Making use of computer modeling, we interpret this as resulting from a 9 yr orbital period in a binary system. Variability time scales are used to construct a mass model of the binary system, with derived masses of 5.109 M⊙ and 2·107 M⊙, We discuss how binary systems such as in OJ287 may result from mergers of galaxies. We infer that the system must contain also a third supermassive black hole.

The influence of the number of rotor slots on the performance - electromagnetic torque production, torque ripple, rotor losses and power factor - of an axial-flux (AF) aluminium-cage solid-rotor core induction motor is studied. The number of stator slots in the study was 36 and the number of the rotor slots was varied between 24 and 32. The width and depth of the rotor slots were kept at initial values. The maximum torque at a certain slip value was achieved at the number of 25 rotor slots. The rotor losses as a function of the number of rotor slots seem to decrease almost linearly when the number of the rotor slots was varied between 24 and 31.

The influence of the switching frequency of an inverter on the performance of rotor losses of an axial-flux (AF) solid-rotor core induction motor is studied. Inverter supply is known to create additional power losses in an induction machine over those occurring with grid supply. Inverter output creates an amount of losses especially in solid-rotor motors. Power loss measurements at no-load conditions are considered with the switching frequency from 1 kHz up to 12 kHz. Different rotor losses due to the switching frequency by pulse width modulation (PWM) inverter are considered by two dimensional (2D) finite element (FE) calculations. Measured stator current waveforms were used as inputs in the 2D finite element analysis (FEA) where the rotor losses were calculated. It is seen that the additional losses are about inversely proportional to the switching frequency between 1 - 6 kHz and after that constant. Both measurements and 2D FEM analysis support the observation.

Toll-like receptor 4 is a part of the innate immune system and recognizes Helicobacter pylori lipopolysaccharide. The goal of this study was to analyze the role of Toll-like receptor 4 polymorphisms +896 (rs4986790) and +1196 (rs4986791) in the pathogenesis of Helicobacter pylori related gastroduodenal diseases in relation to gastric secretion and inflammation. Toll-like receptor 4 polymorphisms, serum gastrin-17 and pepsinogen I and II concentrations were determined, and gastroscopies with histopathological analyses were performed to 216 dyspeptic patients. As genotype controls, 179 controls and 61 gastric cancer patients were studied. In our study, the Toll-like receptor 4 +896 and +1196 polymorphisms were in total linkage disequilibrium. The homozygous wild types displayed higher gastrin-17 serum concentrations than the mutants (p = 0.001) and this effect was independent of Helicobacter pylori. The homozygous wild types also displayed an increased risk for peptic ulcers (OR: 4.390). Toll-like receptor 4 genotypes did not show any association with Helicobacter pylori positivity or the features of gastric inflammation. Toll-like receptor 4 expression was seen in gastrin and somatostatin expressing cells of antral mucosa by immunohistochemistry. Our results suggest a role for Toll-like receptor 4 in gastric acid regulation and that the Toll-like receptor 4 +896 and +1196 wild type homozygozity increases peptic ulcer risk via gastrin secretion.
