In den letzten Jahrzehnten hat sich die implantatbasierte Sofortrekonstruktion bei der prophylaktischen Mastektomie durch gesteigerte onkologische Sicherheit und die Entwicklung neuer Techniken und Materialien deutlich verbessert. Die subkutane prophylaktische Mastektomie hat zum Ziel, das komplette Brustdrusengewebe so sicher, aber auch fur den verbleibenden Haut-Fett-Mantel so schonend wie moglich zu entfernen. Die implantatbasierte Sofortrekonstruktion mit subpektoraler Implantatlage kann mit synthetischen Netzen (SN) oder biologischen, azellularen Matrices (ADM) durchgefuhrt werden. Es erfolgen die Erlauterung der praktischen Umsetzung der prophylaktischen, subkutanen, implantatbasierten Mastektomie mit subpektoraler Sofortrekonstruktion und Einordnung in den aktuellen Datenkontext. Die prophylaktische, subkutane, implantatbasierte Mastektomie mit subpektoraler Sofortrekonstruktion ist eine etablierte Operation, die bei korrekter onkochirurgischer Technik sicher und komplikationsarm durchgefuhrt werden kann. Das Ziel ist die muskulare Abdeckung des kranialen Implantatdrittels fur ein physiologisches Shaping im Bereich des oberen Implantatpols. Um den Muskel verlasslich zu positionieren und das Implantat zu fixieren, werden Interponate zwischen kaudalem Muskelrand und Inframammarfalte verwendet. Diese Interponate konnen SN oder ADM sein. Beide Materialien haben eine ahnliche Komplikationsrate, ein umfassender, prospektiver, multizentrischer Head-to-Head-Vergleich fehlt. Weitere Studien unter Schirmherrschaft der Arbeitsgemeinschaft fur asthetische, plastische und rekonstruktive Operationsverfahren in der Gynakologie (AWOgyn) laufen in Deutschland. Die prophylaktische, subkutane, implantatbasierte Mastektomie mit subpektoraler Sofortrekonstruktion und Unterstutzung durch SN oder ADM ist eine Standardoperation, die jedoch durch die zunehmende epipektorale implantatbasierte Sofortrekonstruktion Konkurrenz bekommen hat.

Up to 40% of patients with metastatic human epidermal growth factor receptor 2 (HER2)-positive breast cancer develop brain metastases (BMs). Understanding of clinical features of these patients with HER2-positive breast cancer and BMs is vital.A total of 2948 patients from the Brain Metastases in Breast Cancer registry were available for this analysis, of whom 1311 had primary tumors with the HER2-positive subtype.Patients with HER2-positive breast cancer and BMs were-when compared with HER2-negative patients-slightly younger at the time of breast cancer and BM diagnosis, had a higher pathologic complete response rate after neoadjuvant chemotherapy and a higher tumor grade. Furthermore, extracranial metastases at the time of BM diagnosis were less common in HER2-positive patients, when compared with HER2-negative patients. HER2-positive patients had more often BMs in the posterior fossa, but less commonly leptomeningeal metastases. The median overall survival (OS) in all HER2-positive patients was 13.2 months (95% confidence interval 11.4-14.4). The following factors were associated with shorter OS (multivariate analysis): older age at BM diagnosis [≥60 versus60 years: hazard ratio (HR) 1.63, P0.001], lower Eastern Cooperative Oncology Group status (2-4 versus 0-1: HR 1.59, P0.001), higher number of BMs (2-3 versus 1: HR 1.30, P = 0.082; ≥4 versus 1: HR 1.51, P = 0.004; global P = 0.015), BMs in the fossa anterior (HR 1.71, P0.001), leptomeningeal metastases (HR 1.63, P = 0.012), symptomatic BMs at diagnosis (HR 1.35, P = 0.033) and extracranial metastases at diagnosis of BMs (HR 1.43, P = 0.020). The application of targeted therapy after the BM diagnosis (HR 0.62, P0.001) was associated with longer OS. HER2-positive/hormone receptor-positive patients showed longer OS than HER2-positive/hormone receptor-negative patients (median 14.3 versus 10.9 months; HR 0.86, P = 0.03), but no differences in progression-free survival were seen between both groups.We identified factors associated with the prognosis of HER2-positive patients with BMs. Further research is needed to understand the factors determining the longer survival of HER2-positive/hormone receptor-positive patients.

Zusammenfassung Hintergrund Therapeuten und Behörden betrachten eine pathologische Komplettremission (pCR) als eigenständigen und relevanten Endpunkt zur Beurteilung des klinischen Nutzens einer neoadjuvanten Therapie beim frühen Mammakarzinom. Die vorliegende Studie sollte untersuchen, welche Behandlungsziele einer neoadjuvanten Therapie Patientinnen selbst als relevant erachten. Material und Methodik Mithilfe der Analytical-Hierarchy-Process-(AHP-)Methode wurden Patientenpräferenzen zu Behandlungszielen einer neoadjuvanten Therapie quantitativ gewichtet. Alle Probandinnen hatten eine neoadjuvante Therapie in Form einer Chemotherapie und bei HER2-Positivität einer zielgerichteten Antikörpertherapie gegen HER2 aufgrund der Primärdiagnose eines Mammakarzinoms 12 – 36 Monaten vor der Befragung erhalten. Die Kriterien des Hierarchiemodells wurden in einer vorangehenden qualitativen Befragung identifiziert. Die Patientinneninterviews wurden von 4 erfahrenen Interviewerinnen durchgeführt. Ergebnisse An der quantitativen Befragung nahmen 41 Patientinnen teil, davon 15 (36,6 %) mit HER2-positiver Erkrankung. Das Erreichen einer pCR stellte für die Patientinnen das wichtigste Therapieziel vor krankheitsfreiem Überleben, Gesamtüberleben und der Möglichkeit einer brusterhaltenden Operation dar. Der Vermeidung von Nebenwirkungen wurde die geringste Bedeutung zugemessen. Beim Vergleich der Nebenwirkungen hatte für die Patientinnen Fatigue den höchsten Stellenwert vor Übelkeit und Haarausfall. Fazit Das Erreichen einer pathologischen Komplettremission stellt für Patientinnen ein eigenständiges relevantes und vorrangiges Ziel einer neoadjuvanten Therapie dar.

Purpose: The WSG-PRIMe Study prospectively evaluated the impact of the 70-gene signature MammaPrint® (MP) and the 80-gene molecular subtyping assay BluePrint® on clinical therapy decisions in luminal early breast cancer. Methods: 452 hormone receptor (HR)-positive and HER2-negative patients were recruited (N0, N1). Physicians provided initial therapy recommendations based on clinicopathological factors. After prospective risk classification by MammaPrint/BluePrint was revealed, post-test treatment recommendations and actual treatment were recorded. Decisional Conflict and anxiety were measured by questionnaires. Results: Post-test switch (in chemotherapy (CT) recommendation) occurred in 29.1% of cases. Overall, physician adherence to MP risk assessment was 92.3% for low-risk and 94.3% for high-risk MP scores. Adherence was remarkably high in “discordant” groups: 74.7% of physicians initially recommending CT switched to CT omission following low-risk MP scores; conversely, 88.9% of physicians initially recommending CT omission switched to CT recommendations following high-risk MP scores. Most patients (99.2%) recommended to forgo CT post-test and 21.3% of patients with post-test CT recommendations did not undergo CT; among MP low-risk patients with pre-test and post-test CT recommendations, 40% did not actually undergo CT. Luminal subtype assessment by BluePrint was discordant with IHC assessment in 34% of patients. Patients’ State Anxiety scores improved significantly overall, particularly in MP low-risk patients. Trait Anxiety scores increased slightly in MP high risk and decreased slightly in MP low-risk patients. Conclusions: MammaPrint and BluePrint test results strongly impacted physicians’ therapy decisions in luminal EBC with up to three involved lymph nodes. The high adherence to genetically determined risk assessment represents a key prerequisite for achieving a personalized cost-effective approach to disease management of early breast cancer.

Reliable estimation from successful polymer pilots to multiple expansion scenarios (full-field and analogs) are of fundamental importance for booking polymer flooding reserves. Here we propose a methodology of four methods (geometrical WOR vs. Np, volumetric, ratio 1P waterflooding to P1 polymer flooding and numerical simulation) to estimate 1P, 2P, and 3P using multiple sources of information. Our results developed criteria to jointly define 1P, 2P and 3P reserves from a successful pilot implemented in Cuenca Golfo San Jorge. These four methods approach makes it easy to cross-check the key parameters that describe the efficiency of polymer flooding, i.e., displacement efficiency, volumetric efficiency. Therefore, they verified the consistency of the results. In addition, the results provide an estimation of the variability that allowed us to add additional criteria to determine 1P, 2P and 3P reserves. Our studies reveal new aspects of practical usage of the WOR vs Np approach for polymer flooding which need to be taken into consideration for booking reserves. The geometric WOR vs Np method is scale invariant, namely it can be applied with consistency across multiple group of well through the reservoir. We validated this approach with the results of the pilot and testing it with multiple scenarios generated by the simulator. The results obtained in terms of recovery factors throughout different layers and zones in the reservoir agreed well with the upscaled recovery factor obtained in multiple corefloods

Summary After the successful pilot (14%OOIP incremental oil, Juri et al. 2017 and utility factor of 2 kg polymer/bbl), a series of multiple simulations cases indicated an optimal extension of 3-year cycle factory-mode development. The initial cycle affects 80 polymer injectors distributed in multiple injection satellites across three multilayer reservoirs. After the 3-year cycle, we rotate the polymer skids to other satellites. The satellites emanate from a peripheral aqueduct that encircles the reservoirs. Each satellite has 8 to 10 injectors (each well injects 100 m3/day in an average net thickness of 18 m). The total number of injectors is 59 in the Grimbeek-2 block, 40 in Grimbeek-North block and 20 injectors in the Grimbeek-North-2 block. Injection in the remaining well will start in the second 3-year cycle. Here we report the use of reservoir simulation to design the entire architecture of the development which includes both the optimum injection period and the number of satellites under simultaneous injection. The strategy is based on the plug-in concept in which we minimise the footprint and we maximise the use of current surface facilties connecting the polymer skids to the waterflooding satellite. We tracked the oil that is swept by the injectors in each satellite. The simulation methodology extracted the incremental oil of each satellite because of polymer injection. We found that between 2.5 to 3 years polymer injection cycle and eight simultaneous polymer injection skids minimise the utility factor (kg of polymer injected per bbl of incremental oil above waterflooding baseline). After the 3-year cycle, the eight polymer injection skids rotate from the initial eight satellites to eight new locations, and water injection follows on the initial satellites. This strategy minimises CAPEX, OPEX and the risk of polymer production compared to the scenario of injecting in all wells in the same manner as waterflooding was implemented.

In den vergangenen Jahrzehnten hat sich die Technik der rekonstruktiven implantatbasierten Brustchirurgie durch die onkologischen Innovationen der haut- (SSM) und nippelsparenden Mastektomie (NSM) in Kombination mit neuen Materialien wie synthetischen Netzen (SN) und azellularen dermalen Matrizes (ADM) stetig weiterentwickelt. Ziel ist die sofortige komplette Abdeckung und Stabilisierung der gesamten Implantatloge, kranial durch den mobilisierten M. pectoralis major, kaudal durch das am Muskel fixierte Netz oder ADM mit gleichzeitiger Stabilisierung des Implantats. So kann reproduzierbar und zuverlassig eine deutlich bessere Asthetik erreicht werden als mit Standardtechniken ohne Netz oder ADM. Daruber hinaus konnen andere Muskelpraparationen des M. serratus anterior oder sogar des M. latissimus dorsi zur Implantatabdeckung vermieden werden. Die Operationstechniken der Netz- oder ADM-unterstutzten Implantatrekonstruktion sind anspruchsvoll, mit einer merklichen Lernkurve verbunden und nicht standardisiert. Es gibt immer mehr unterschiedliche Medizinprodukte an synthetischen Netzen und biologischen Matrizes, deren klinische Einfuhrung oft ohne nennenswerte vorausgegangene klinische Studien erfolgt. Das Implantat- und Netzregister der Arbeitsgemeinschaft fur asthetische, plastische und rekonstruktive Operationsverfahren in der Gynakologie (AWOgyn), in dem inzwischen uber 110 Brustoperateure ihre Falle dokumentieren, ermoglicht eine kontinuierliche Einordnung der verwendeten Materialien. Der differenzierte Einsatz der bereits in der klinischen Routine verwendeten oder auch neuer Netz- und ADM-Materialien ist ein Feld der aktuellen fachlichen Diskussion, des Erfahrungsaustausches und klinischer Forschung.
