1. Routine use of automated FiO2 control in Poland: prospective registry and survey

Abstract: ObjectiveThe performance of automated control of inspired oxygen (A-FiO2) has been confirmed in dozens of studies but reports of routine use are limited. Broadly adopted in Poland, our aim is to share that experience.MethodsWe used a prospectively planned observational study of the performance, general use patterns, unit practices, and problems with A-FiO2, based on a web registry of case reports, complemented by surveys of subjective impressions.ResultsIn 2019, a total of 92 A-FiO2 systems were in routine use in 38 centers. Of the 38 centers, 20 had agreed in 2013 to participate in the project. In these centers, A-FiO2 was applied in infants of all weights, but some centers restricted its use to weaning from oxygen and unstable infants. A cohort had reported their experience with each use (5/20 centers, 593 cases). A quarter of those infants were managed with a lower target range and three-quarters with alarms looser than European guidelines for manual SpO2 control. The perceived primary advantages of A-FiO2 were as follows: keeping the readings in the target range, reducing exposure to SpO2 extremes, reducing risk from nurse distraction, reducing workload, and reducing alarm fatigue. Practices did evolve with experience, including implementing changes in the alarm strategy, indications for use, and target range. The potential for over-reliance on automation was cited as a risk. There were a few reports of limited effectiveness (moderate 12/593 and poor 2/593).ConclusionsAutomated oxygen control is broadly perceived by users as an improvement in controlling SpO2 with infrequent problems.

2. Study of theoretical luminosity precision for electron colliders at higher energies

Abstract: Abstract We present an estimation of the theoretical precision of low angle Bhabha scattering at the proposed future ILC collider at 500 GeV. The analysis is an extension of the previous analysis done for the FCCee collider at $$\sqrt{s}=M_Z$$ s = M Z . As the state-of-the-art and the reference point we use the BHLUMI Monte Carlo event generator. Based on the current precision status of BHLUMI for LEP analysis, we estimate how various error components evolve from the LEP to ILC setups. The conclusion of our work is that for the ILC the precision of the current version of BHLUMI 4.04 deteriorates to 0.5%, by more than an order of magnitude w.r.t. the present precision for LEP. With the expected future improvements, the precision of BHLUMI can change to 0.016%, nearly as good as for the FCCee at the $$M_Z$$ M Z setup (0.01%). Based on the developed methodology we present also results for ILC $$_{1000}$$ 1000 , FCCee $$_{350}$$ 350 and CLIC $$_{3000}$$ 3000 setups.

3. QED exponentiation for quasi-stable charged particles: the $$e^-e^+\rightarrow W^-W^+$$ e-e+→W-W+ process

Abstract: Abstract All real and virtual infrared singularities in the standard analysis of the perturbative Quantum Electrodynamics (like that of Yennie–Frautschi–Suura) are associated with photon emissions from the external legs in the scattering process. External particles are stable, with the zero decay width. Such singularities are well understood at any perturbative order and are resummed. The case of production and decay of the semi-stable neutral particles, like the Z-boson or the $$\tau $$ τ -lepton, with the narrow decay width, $$\Gamma /M \ll 1$$ Γ/M≪1 , is also well understood at any perturbative order and soft-photon resummation can be done. For an absent or loose upper cut-off on the total photon energy $$\omega $$ ω , production and decay processes of the semi-stable (neutral) particles decouple approximately and can be considered quasi-independently. In particular, the soft-photon resummation can be done separately for the production and the decay, treating a semi-stable (neutral) particle as stable. QED interference contributions between the production and decay stages are suppressed by the $$\Gamma /M$$ Γ/M factor. If experimental precision $$\omega $$ ω is comparable with or better than $$\Gamma /M$$ Γ/M , these interferences have to be included. In the case of $$\omega \ll \Gamma $$ ω≪Γ decoupling of production and decay does not work any more and the role of semi-stable particles is reduced to the same role as that of other internal off-shell particles. So far, consistent treatment of the soft photon resummation for semi-stable charged particles like the $$W^\pm $$ W± bosons is not available in the literature, and the aim of this work is to present a solution to this problem. Generally, this should be feasible because the underlying physics is the same as in the case of neutral semi-stable resonances—in the limit of $$\Gamma /M \ll 1$$ Γ/M≪1 the production and decay processes for charged particles also necessarily decouple due to long lifetime of intermediate particles. Technical problems to be solved in this work are related to the fact that semi-stable charged particle are able to emit photons. Practical importance of the presented technique to the $$e^+e^-\rightarrow W^+W^-$$ e+e-→W+W- process at the Future electron–positron Circular Collider (FCC-ee) is underlined.

4. QED challenges at FCC-ee precision measurements

Abstract: Abstract The expected experimental precision of the rates and asymmetries in the Future Circular Collider with electron–positron beams (FCC-ee) in the center of the mass energy range 88–365 GeV considered for construction in CERN, will be better by a factor 5–200. This will be thanks to the very high luminosity, a factor up to $$10^5$$ 105 higher than in the past LEP experiments. Consequently, it poses the extraordinary challenge of improving the precision of the Standard Model predictions by a comparable factor. In particular the perturbative calculations of the trivial QED effects, which have to be removed from the experimental data, are considered to be a major challenge for almost all quantities to be measured at FCC-ee. The task of this paper is to summarize the “state of the art” in this class of the calculations left over from the LEP era and to examine what is to be done to match the precision of the FCC-ee experiments – what kind of technical advancements are necessary. The above analysis will be done for most important observables of the FCC-ee, like the total cross sections near Z and WW threshold, charge asymmetries, the invisible width of Z boson, the spin asymmetry from $$\tau $$ τ lepton decay and the luminosity measurement.

Abstract: Abstract We review the physics opportunities of the Future Circular Collider, covering its e+e-, pp, ep and heavy ion programmes. We describe the measurement capabilities of each FCC component, addressing the study of electroweak, Higgs and strong interactions, the top quark and flavour, as well as phenomena beyond the Standard Model. We highlight the synergy and complementarity of the different colliders, which will contribute to a uniquely coherent and ambitious research programme, providing an unmatchable combination of precision and sensitivity to new physics.

6. The path to 0.01% theoretical luminosity precision for the FCC-ee

Abstract: The current status of the theoretical precision for the Bhabha luminometry is critically reviewed and pathways are outlined to the requirement targeted by the FCC-ee precision studies. Various components of the pertinent error budget are discussed in detail – starting from the context of the LEP experiments, through their current updates, up to prospects of their improvements for the sake of the FCC-ee. It is argued that, with an appropriate upgrade of the Monte Carlo event generator BHLUMI and/or other similar MC programs calculating QED effects in the low angle Bhabha process, the total theoretical error of 0.01% for the FCC-ee luminometry can be reached. A new study of the Z and s-channel γ exchanges within the angular range of the FCC-ee luminometer using the BHWIDE Monte Carlo was instrumental in obtaining the above result. Possible ways of BHLUMI upgrade are also discussed.

8. Study on the DLOFC accident of the GEMINI+ conceptual design of HTGR reactor with MELCOR and SPECTRA

Abstract: The work presented in this paper was performed within the Euratom Horizon 2020 GEMINI Plus project. Behavior of the HTGR reactor under severe accident conditions was investigated and the maximum fuel temperature was observed. Due to application of the TRISO particles and SiC layers in the fuel element, no damage of the fuel is expected up to 1600°C. Under the cooperation in the project between Nuclear Research Group (NRG) and National Centre for Nuclear Research (NCBJ) a code-to-code calculations were carried out between the SPECTRA and MELCOR codes. SPECTRA code, developed by the NRG is a thermal hydraulic analysis code and MELCOR 2.1.6342 used by NCBJ developed by SANDIA National Laboratory is fast running severe accident code. Both codes have already HTGR specific models build in. The following accident was analyzed and will be presented: Depressurized Loss of Forced Circulation (DLOFC) with 65 mm break at the top of reactor vessel. The scenario was calculated applying following sets of assumptions: best estimate and conservative. Plant behavior was analyzed including primary and secondary side of the reactor. As the results of applying conservative assumptions, it was found that fuel temperature excides the acceptable limit of 1620°C. Therefore, changes in the core design were proposed by project participants. Analyses of the new core showed acceptable temperatures. In the paper the results of code-to-code comparison are presented. Both codes have shown a good agreement of presented following characteristics on maximum fuel temperature, relative power and Reactor Cavity Cooling System power, primary pressure and break flow.

Abstract: Our concept of nucleic acid biology has advanced dramatically over the past two decades, with a growing appreciation that cell-free DNA (cfDNA) fragments are present in all body fluids including plasma. In no other field has plasma DNA been as rapidly translated into clinical practice as in noninvasive prenatal testing (NIPT) for fetal chromosome abnormalities. NIPT is a screening test that requires confirmation with diagnostic testing, but other applications of cfDNA provide diagnostic information and do not require invasive testing. These applications are referred to as noninvasive prenatal diagnosis (NIPD) and include determination of fetal sex, blood group and some single gene disorders. As technology advances, noninvasive tests based on cell-free nucleic acids will continue to expand. This review will outline the technical and clinical aspects of NIPT and NIPD relevant to the daily practice of maternity carers.

Zuritathe FCC Collaboration, In response to the 2013 Update of the European Strategy for Particle Physics (EPPSU), the Future Circular Collider (FCC) study was launched as a world-wide international collaboration hosted by CERN. The FCC study covered an energy-frontier hadron collider (FCC-hh), a highest-luminosity high-energy lepton collider (FCC-ee), the corresponding 100km tunnel infrastructure, as well as the physics opportunities of these two colliders, and a high-energy LHC, based on FCC-hh technology. This document constitutes the fourth volume of the FCC Conceptual Design Report, devoted to the High-Energy Large Hadron Collider HE-LHC. It summarizes the HE-LHC physics discovery opportunities, presents the HE-LHC accelerator design, performance reach, and operation plan, discusses the underlying technologies, the civil engineering and technical infrastructure, and also sketches a possible implementation. Combining ingredients from the Large Hadron Collider (LHC), the high-luminosity LHC upgrade and adding novel technologies and approaches, the HE-LHC design aims at a hadron collider with about twice the centre-of-mass collision energy that the LHC can reach. Its performance aims at exploring physics beyond the Standard Model, signi cantly extending the LHC's direct and indirect sensitivity to new physics and discoveries.

Abstract: Infections in pregnancy represent a challenging and often underappreciated area of concern for many specialists and general practitioners and can cause serious sequelae. Antenatal status should be highlighted on pathology request forms, as this serves to alert the laboratory of the need to store serum for an extended period. Prior antenatal specimens can be forwarded to other laboratories to enable testing in parallel with the more recent sample. Women with a confirmed, potentially vertically transmissible infection should be referred to a specialist with expertise in the management of perinatal infections. Cytomegalovirus infection is the most common congenital infection. Women who care for young children are at greater risk of exposure to the virus. Preventive steps including hand hygiene and avoiding contact with children's urine, mucous and saliva are recommended for all pregnant women. The incidence of parvovirus B19 infection in pregnancy is unknown. This infection is highly contagious and may result in fetal loss; particularly in the first half of pregnancy, pregnant women should avoid contact with adults or children who may have an infection.

Abstract: All real and virtual infrared singularities in the standard analysis of the perturbative Quantum Electrodynamics (like that of Yennie-Frautschi-Suura) are associated with photon emissions from the external legs in the scattering process. External particles are stable, with the zero decay width. Such singularities are well understood at any perturbative order and are resummed. The case of production and decay of the semi-stable {\em neutral} particles like $Z$ boson or $\tau$ lepton, with the narrow decay width, $\Gamma/M \ll 1$, is also well understood at any perturbative order and soft photon resummation can be done. For the absent or loose upper cut-off on the total photon energy $\omega$ the production and decay process of the semi-stable (neutral) particles decouples approximately and can be considered quasi-independently. In particular soft photon resummation can be done separately for the production and decay process treating semi-stable (neutral) particle as stable. So far, consistent treatment of the soft photon resummation for semi-stable {\em charged} particles like $W^\pm$ boson is not available in the literature and the aim of this work is to present a solution for this problem. Generally, it should be feasible because the underlying physics is the same as in the case of the neutral semi-stable resonances -- in the limit $\Gamma/M \ll 1$ production and decay processes for charged particles also necessarily decouple due to long lifetime of the particles. Abstract: The current status of the theoretical precision for the Bhabha luminometry is critically reviewed and pathways are outlined to the requirement targeted by the FCC-ee precision studies. Various components of the pertinent error budget are discussed in detail -- starting from the context of the LEP experiments, through their current updates, up to prospects of their improvements for the sake of the FCC-ee. It is argued that with an appropriate upgrade of the Monte Carlo event generator BHLUMI and/or other similar MC programs calculating QED effects in the low angle Bhabha process, the total theoretical error of $0.01\%$ for the FCC-ee luminometry can be reached. A new study of the $Z$ and $s$-channel $\gamma$ exchanges within the angular range of the FCC-ee luminometer using the BHWIDE Monte Carlo was instrumental in obtaining the above result. Abstract: Background The number of procedures involving dermal filler injection is still on the rise. Although their safety is improving, not all complications can be avoided. The late complications manifesting as nodules or granulomas pose a particular diagnostic and therapeutic challenge, due to the lack of uniform standards or guidelines. High-frequency, ultrasound imaging appears to be a useful method for distinguishing between granulomas and nodular dermal filler deposits. Aim The aim of the paper was to evaluate the utility of high frequency ultrasound imaging for distinguishing between foreign body granulomas and nodular dermal filler deposits. Material and methods Eleven females aged 21-66 years (mean age of 43.6 years old) who had soft tissue fillers injected were enrolled. All patients had a high-frequency ultrasound scan of the involved skin area performed. The shape, margins, area, location and echogenicity of the lesions were assessed. Additionally, the lesions were evaluated histologically and photographs were taken. Results The analysis indicated differences between the ultrasound image of granulomas and dermal filler deposits. Characteristic ultrasound features of granulomas include oval shape and blurred, irregular outer edges. Small hyperechoic areas were seen inside the granulomas. The deposits were anechogenic, with sharp, regular borders. Conclusions High-frequency ultrasound imaging enables distinguishing between granulomas and nodules-dermal filler deposits, which form after dermal filler injections.

15. Student mobility. Between theory and practice?

Abstract: Introduction: The Bologna Process has initiated the “culture of reforms” in higher education, conditioning the economic and social development of Europe. Erasmus Plus is an EU programme of education in 2014-2020. Despite the numerous benefits it offers to students, their mobility within the Erasmus Plus Programme proves to be low (not satisfactory). Purpose: Review the theoretical and practical aspects of student mobility, with particular emphasis on the Erasmus Plus Programme. Materials and methods: We searched for extant networks through peer-reviewed literature and the world-wide web. Additionally, we used a data analysis on the mobility of students of medicine and health sciences in 2005-2015 within the Erasmus Plus Programme, at selected university in Poland. Results: According to the educational standards in the European Union, students of medical faculties are obliged to complete 12 semesters of studies comprising 5,500 hours under teachers’ supervision. In Poland there are 5,700 hours – these additional hours account for mandatory physical education and foreign language classes which are not taught in other countries. Data from the last decade indicate that student mobility in Poland is an overestimated phenomenon as it concerns a narrow circle of students. Conclusions: A student who benefits from a scholarship under the Erasmus Plus Programme enhances their self-esteem and development opportunities. Participation in an international programme seems to bring benefits alone. In practice, however, there prove to be numerous barriers related to student mobility, such as financial problems, language barriers and issues related to the recognition (crediting) of a period of study completed at a partner university.

Abstract: We formulate a challenge of constructing constrained Markovian MC algorithm for the evolution equations for the initial state QCD radiation with tagging the type and x of the exiting parton. This seems to be within the reach of the presently available computer CPUs and the sophistication of the MC techniques. We describe briefly an example of an efficient solution of this kind (constrained Markovian) for the pure gluon-strahlung. We show that it agrees numerically very well (0.2%) with other MC (unconstrained Markovian) of our own and another non-MC program, QCDnum16 .

Abstract: The dynamical properties of the small fullerene cluster (C60)14, confined between two parallel graphite walls, have been studied using a molecular-dynamics (MD) simulation method. An interesting instance of behaviour of C60 molecules in this environment has been discovered. For a range of distances d between the graphite walls (2.2 nm < d < 2.7 nm) the fullerenes form two monolayers parallel to the graphite surfaces (no migration of molecules between layers). The monolayers slide one against the other and perform a torsional motion.

Despite the simplified model of the process, presented numerical results prove that the basic concept of the new methodology works correctly in the numerical environment of the Monte Carlo parton shower event generator. The presented method is an alternative to the well established methods, MC@NLO and POWHEG. Refinements of the new method with better computer CPU time efficiency are also discussed.

Abstract: Presently available perturbative QCD calculations combining hard process matrix element with the Parton Shower Monte Carlo programs feature hard process matrix element calculated often beyond the leading order (LO), that is including complete next-to-leading-order (NLO), or even next-to-next-to-leading-order (NNLO) corrections, while Parton Shower is only at the leading order (LO). We report here on a work in progress which demonstrate feasibility of constructing Parton Shower Monte Carlo (PSMC) featuring complete NLO corrections to QCD evolution with respect to the logarithm of the factorization scale. This effort presently covers non-singlet subset of Feynman diagrams contributing to the above QCD evolution. It should be stressed that our approach to the NLO QCD evolution is exclusive, that is giving insight into the fully unintegrated phase space. However, at the inclusive level our implementation agrees exactly with the standard inclusive picture of the NLO DGLAP evolution. Our new approach (after including NLO singlet diagrams) provides a complete method of combining the resummed and fixed order perturbative QCD calculations beyond the LO in a form suitable for the MC implementation, an alternative to the existing ones. Abstract: A fragment of DNA from a yeast Pachysolen tannophilus, bearing the ornithine carbamoyltransferase gene (OCTase, EC 2.1.3.3) has been cloned from a genomic library by functional complementation of the Escherichia coli OCT-negative mutant. The gene was located within the cloned segment of DNA and its coding sequence identified by DNA sequencing. This has indicated that P. tannophilus OCT gene encodes a 347 amino acid polypeptide, which shows 60% identity to the homologous Saccharomyces cerevisiae protein. The amino acid composition of its N-terminus indicates that this protein is translocated across the mitochondrial membrane. The gene can be expressed in E. coli as well as in S. cerevisiae. Comparison with other OCTases confirms a high degree of conservation among these proteins.

Abstract: We discuss theoretical predictions for W-pair production and decay at LEP2 and higher energies in a form suitable for comparison with raw data. We present a practical framework for calculating uncertainties of predictions given by the KORALW and grc4f Monte Carlo programs. As an example we use observables in the $s\bar s c\bar c$ decay channel: the total four-quark (four-jet) cross section and two-quark/jet invariant-mass distribution and cross section, in the case when the other two may escape detection. Effects of QED bremsstrahlung, effective couplings, running W and Z widths, Coulomb interaction and the complete tree level set of diagrams are discussed. We also revisit the question of technical precision of the new version 1.21 of the KORALW Monte Carlo code as well as of version 1.2(26) of the grc4f one. Finally we find predictions of the two programs to have an overall physical uncertainty of 2%. Abstract: We calculate the effect of pair production on the luminosity process of low-angle Bhabha scattering at the SLAC Linear Collider (SLC) and/or CERN ${e}^{+}{e}^{\ensuremath{-}}$ collider LEP. We work in the leading-logarithm approximation to orders ${\ensuremath{\alpha}}^{2}$ and ${\ensuremath{\alpha}}^{3}$ for the nonsinglet and the singlet contributions, respectively. In particular we calculate the size of the singlet contribution to be below 0.007% for the energy cuts ${x}_{c}g0.1$, as was estimated by Jadach and co-workers. In this way, we remove the contribution of such pairs from the theoretical uncertainty in the SLC/LEP luminosity calculation of Jadach and co-workers via the Monte Carlo Bhlumi 2.01 at the precision level of 0.01%. Results are illustrated for typical SLC/LEP acceptance cuts.

Abstract: An electrophoretic karyotype of Pachysolen tannophilus has been obtained using Pulsed Field Gel Electrophoresis. Seven chromosomal bands were separated with one of the bands migrating probably as a doublet. The sizes of the chromosomes were estimated to be between 1 and 3.1 megabase pairs. Eleven loci have been assigned to chromosomal bands, including four involved in the metabolism of D-xylose.

Abstract: An Aspergillus nidulans DNA fragment composed of two adjacent Sal I subfragments (1.8 and 0.85 kb) that carries an argB complementing the yeast arg3 mutation has been isolated from two different gene libraries. Hybridization results and immunological tests indicate that the cloned fragment contains the A. nidulans structural gene coding for ornithine carbamoyltransferase (OTCase). Using the cloned gene as a probe, the specific mRNA was identified. The level of this RNA observed in A. nidulans strains grown under various conditions correlated with the level of the OTCase activity, suggesting transcriptional control of OTCase synthesis. Expression of the cloned gene in Saccharomyces cerevisiae does not depend on its orientation in the vector. In Escherichia coli , the cloned gene does not function; however arg - transformants revert to phototrophy with high frequency possibly due to DNA rearrangements within the recombinant plasmid.

Abstract: Summary: The Aspergillus nidulans argB gene coding for ornithine carbamoyltransferase (OTCase) is not expressed in Escherichia coli. However, E. coli OTCase-deficient strains transformed with plasmids carrying the argB gene from A. nidulans reverted to prototrophy at a high frequency. In these derivatives the argB gene became functional due to DNA rearrangements upstream of the coding sequence. Two types of rearrangement were characterized. One was identified as an insertion of IS2. The second was a deletion that resulted in transcription of the argB gene from the TcR gene promoter and translation from a newly created ribosome-binding site formed at the junction between the A. nidulans and vector DNA sequences.

Abstract: Biomarkers are critical for heart failure (HF) management by facilitating risk stratification, therapeutic decision-making, and monitoring treatment response. This prospective, single-center study aimed to assess predictors of death during one-year follow-up in patients with end-stage HF, with particular emphasis on the soluble suppression of tumorigenicity 2/left ventricular mass index (sST2/LVMI) ratio, modified Model for End-stage Liver Disease (modMELD), and Model for End-stage Liver Disease excluding INR (MELD-XI). This study comprised 429 consecutive patients with end-stage HF hospitalized between 2018 and 2023. The median age was 56.0 (50.0-60.0) years; and 89.2% were male. During the follow-up, 134 (31.2%) patients died. The area under the receiver operating characteristics (ROC) curves showed good predictive powers of sST2/LVMI-MELDXI (AUC: 0.90 [CI: 0.87-0.93]; specificity 85% and sensitivity 80%) and sST2/LVMI-modMELD (AUC: 0.92 [95% CI: 0.90-0.95]; specificity 92%, sensitivity 81%) for assessment of one-year mortality. In conclusion: the sST2/LVMI-modMELD and sST2/LVMI-MELD-XI ratios are independently related to one-year mortality in the analyzed group of patients. The prognostic power of these new models is significantly better than their individual components. This single-center study comprised a relatively small group of patients, so the prognostic value of these new models cannot be generalized to the entire HF population. Considering the limitations of this analysis, further randomized trials with a large cohort are necessary to confirm the utility of the new prognostic models in HF patients.

Abstract: BACKGROUND Effective assessment and diagnosis using simple nutritional screening tools are crucial for identifying malnutrition in older adults. The aim of the study was to evaluate how effectively different anthropometric parameters, indices, and body composition metrics can assess nutrition-related risks, using the Geriatric Nutritional Risk Index (GNRI) in a cohort of 185 patients >60 years. MATERIAL AND METHODS This study included 185 patients over 60 years old. Anthropometric measurements, indices, and body composition were examined. Nutritional status based on GNRI was categorized as major risk (1.1%), moderate risk (9.7%), low risk (15.1%), and no risk (74.1%). RESULTS The strongest correlations with the GNRI were observed for body mass index (BMI) (ρ=0.8628) and body fat in kilograms (ρ=0.8269), P<0.001. A unit increase in BMI decreased the odds of being in the risk group by 52.1% (OR 0.479; 95% CI 0.377-0.609; P<0.001). ROC analysis showed BMI ≤25.0 had the highest predictive value (AUC 0.93, 95% CI 0.89-0.97) in assessing nutrition-related risk in the elderly. Body fat (AUC 0.89, 95% CI 0.85-0.94), abdominal volume index (AUC 0.86, 95% CI 0.80-0.91), hip circumference (AUC 0.85, 95% CI 0.79-0.91), and waist circumference (AUC 0.85, 95% CI 0.80-0.91) also demonstrated significant predictive power (P<0.001). CONCLUSIONS Our study underscores the importance of using BMI and other related anthropometric measures and indices as part of routine assessments to identify and manage nutrition-related risks among elderly individuals in hospitals, care facilities, and dietetic clinics, particularly in situations where standardized tools for assessing malnutrition are not available or are impossible to use.

Abstract: Peripheral nerve regeneration remains a major challenge in neuroscience, despite advancements in understanding its mechanisms. Current treatments, including nerve transplantation and drug therapies, face limitations such as invasiveness and incomplete recovery of nerve function. Physical therapies, like pulsed electromagnetic fields (PEMF) and low-intensity ultrasound (LIPUS), are gaining attention for their potential to enhance regeneration. This study analyzes the effects of PEMF and LIPUS on gene expression in human primary Schwann cells, which are crucial for nerve myelination and repair. Key genes involved in neurotrophin signaling (NGF, BDNF), inflammation (IL-1β, IL-6, IL-10, TNF-α, TGF-β), and regeneration (CRYAB, CSPG, Ki67) were assessed. The results of this study reveal that combined PEMF and LIPUS therapies promote Schwann cell proliferation, reduce inflammation, and improve the regenerative environment, offering potential for optimizing these therapies for clinical use in regenerative medicine.

Abstract: Although the epidemiology and symptoms of major depressive disorder (MDD) have been well-documented, the etiology and pathophysiology of the disease have not yet been fully explained. Depression arises from intricate interplay among social, psychological, and biological factors. Recently, there has been growing focus on the involvement of miRNAs in depression, with suggestions that abnormal miRNA processing locally at the synapse contributes to MDD. Changes in miRNAs may result from altered expression and/or function of the miRNA biogenesis machinery at the synapse. The aim of our research was to assess the relationship between the occurrence of depression and single-nucleotide polymorphisms (SNP) in the following genes in the Polish population: DROSHA (rs6877842; rs10719) and XPO5 (rs11077). This study involved 200 individuals, including 100 with depressive disorders in the study group (SG) and 100 healthy people without MDD in the control group (CG). All participants were unrelated native Caucasian Poles from central Poland. Blood samples were collected to evaluate the single-nucleotide polymorphism of the genes. Findings indicated that within our patient cohort, the risk of depression is increased by polymorphic variants of the rs10719/ DROSHA and rs11077/ XPO5 genes and lowered by rs6877842/ DROSHA . Our study sheds light on the understanding of the genetic basis of depression, which can be used in the rapid diagnosis of this disease.

Abstract: Background: Neurofibromatosis type 1 (NF1) is an autosomal dominant genetic disorder, whose clinical picture is dominated by visible body changes as well as numerous somatic and behavioural abnormalities., Aim: The aim of the study was to explore the ways in which the individual experiences NF1 in everyday life, with particular emphasis on the impact of the altered body on family interactions, in addition to the personal and social identity of individuals with NF1., Methods: A qualitative study was performed using individual in-depth interviews with 93 individuals with NF1 (median age: 36.69; range: 18 to 64; 26% males)., Results: Body changes caused by NF1 determine the specificity of social interactions in the families of the sick. The strength and direction of the impact of body changes on social interactions depends on their type (visibility, invisibility), as well as the meanings given to them. The visibility of disease lesions triggers an attitude of excessive control and stigmatization in the family, especially on the part of the mothers of individuals with NF1, and prompts a tendency to define the individual through the prism of the disease and its bodily manifestations. In turn, the lack of visibility of disease symptoms gives rise to, especially on the part of the fathers of the sick, opposing attitudes of disease denial, normalization of its symptoms and a tendency to question the disease identity of individuals with NF1. The great intensity of interactional problems concerns especially those families in which NF1 was transmitted through inheritance, and family members blame each other for the disease. This leads to repression and denial of the disease, excluding it from the scope of issues discussed in the family, which is an attempt to avoid the attribution of blame for the disease., Conclusions: The body changes resulting from NF1 have social consequences that are of critical importance in the lives of the sick. The impact of NF1 on family interactions depends on the ways in which the disease is understood by the sick individual and his or her family members. 45. Fibrin clot permeability (Ks) in patients on left ventricular assist device.

Abstract: Patients on left ventricular assist devices (LVAD) are prone to excessive hemostasis disturbances due to permanent contact of artificial pump surfaces with blood components. We aimed to investigate if fibrin clot permeability is altered in patients on long-term continuous-flow LVAD therapy and if the clot permeability is associated with clinical characteristics and adverse events. We investigated 85 end-stage heart failure patients (90.6% men, age 48.6-63.8 years) scheduled for continuous flow long-term LVAD support according to current clinical indications. The patients were assessed periodically: prior to LVAD implantation (T1), 3-6 months (T2) after LVAD implantation, 6-12 months after (T3) and then every 6 months. We tested the first three blood samples (T1-T3) and the last available blood sample (T4), but no longer than 5 years after LVAD implantation. We assessed hemostasis parameters (Activated Partial Thromboplastin Time (APTT) Prothrombin Time, Activated Partial Thromboplastin Time, Fibrinogen, D-dimer, Antithrombin, Thrombin Time, Factor VIII, and von Willebrand Factor, aspirin-induced platelet inhibition, adenosine-diphosphate test) changes during the study period. Fibrin Clot Permeability was evaluated using a pressure system and Permeability Coefficient (Ks) was calculated. We observed a decrease in fibrin clot permeability (Ks) between T1, T2, T3 and T4 time periods; P < 0.01 for each comparison. Fibrin clot permeability was negatively correlated with fibrinogen concentration: r = - 0.51, P < 0.001, factor VIII activity r = - 0.42, P < 0.001. There was no association of Ks with age, Left Ventricular Ejection Fraction (LVEF) and medications P > 0.001, however cumulative measurements in patients on aspirin showed shortening of Ks in this group P = 0.0123. Major adverse cardiac and cerebrovascular events (MACCE) occurred in 36.5% patients, bleeding events in 25.9%, Net Adverse Clinical Events (NACE) in 62.4%; 31.7% patients died, and 17.6% underwent transplantation. The transplantation was considered as the endpoint. Discrepancies in Ks were observed between patients with MACCE, bleeding, and NACE, and patients without adverse events. Ks showed a constant trend towards normalization (P < 0.01) only in patients without adverse events. Patients with advanced heart failure have disturbed clot structure. A trend towards normalization of the Ks values is associated with fewer thromboembolic and bleeding complications in this group of patients., (© 2024. The Author(s).)

46. Lipoprotein(a) and its impact on cardiovascular disease - the Polish perspective: design and first results of the Zabrze-Lipoprotein(a) Registry.

Abstract: Introduction: Lipoprotein(a) [Lp(a)] is an independent risk factor for atherosclerotic cardiovascular disease (ASCVD). Increased Lp(a) concentration > 30 mg/dl (75 nmol/l) and especially >50 mg/dl (125 nmol/l) may cause faster atherosclerosis, being an important and underdiagnosed residual cardiovascular risk factor. Thus, there is a need to characterize further the clinical phenotypes in patients at risk for ASCVD with high Lp(a) levels now and during follow-up, while also looking for the possible impact of geographical differences., Material and Methods: The Zabrze Lipoprotein(a) Registry (Zabrze-Lip(a)R) was founded on the basis of data from 2,001 consecutive patients with very high cardiovascular risk treated in a tertiary hospital. The registry patients will be followed for at least 5 years with the possibility of extending this period as an open label study. All-cause and cause-specific mortality, hospitalizations, and cardiovascular events, such as myocardial infarction (MI) and stroke, will be assessed., Results: The mean age of patients was 66.4 years (females 37.1%). The median Lp(a) concentration in the entire population was 6.6 mg/dl (16.5 nmol/l) (mean: 14.3 ±19.4 mg/dl). 540 (27%) patients had elevated Lp(a) levels above 30 mg/dl (75 nmol/l); they were significantly older (68.8 vs. 66.3 years; p = 0.04), had significantly lower hemoglobin and hematocrit, and higher platelet count and levels of NT-proBNP and C-reactive protein. The prevalence of elevated Lp(a) > 30 mg/dl (75 nmol/l) concentrations was very high in patients with a chronic coronary syndrome (CCS) (52.2% (282/540) vs. 41.5% (607/1461); p < 0.001), in patients undergoing PCI during hospitalization (23.9 vs. 19%; p = 0.01), and in patients with previous MI (20.6% vs. 14.9%; p = 0.0022). In the multivariable analysis, the independent predictors of elevated Lp(a) > 30 mg/dl (75 nmol/l) were only lower Hb values (OR = 0.925; 95% CI: 0.874-0.978; p = 0.006) and higher platelet count (1.002; 95%CI: 1.000-1.003; p < 0.02)., Conclusions: In Poland, the largest representative of Central and Eastern European countries, 27% of patients at very high cardiovascular risk with established ASCVD experience additional risk related to an elevated Lp(a) level, with every second patient having CCS. Interestingly, only two factors were significantly related to elevated Lp(a) levels: lower Hb values and higher platelet count. 47. Hypertension-Mediated Organ Damage in Relation to Severity of Chronic Low Back Pain in Hypertensive Patients.

Abstract: Introduction: Chronic pain triggers a stress response, which results in increased blood pressure (BP). We investigated whether chronic low back pain (cLBP) in hypertensive patients is associated with an increased risk of hypertension-related organ damage., Methods: We studied 85 consecutive hypertensive patients with a median age of 62 years (55-67), who suffered from cLBP, the severity of which was evaluated according to the Oswestry Disability Index (ODI). Patients underwent transthoracic echocardiography, arterial ultrasonography and vascular tonometry. We assessed carotid artery atherosclerotic plaques, along with carotid-femoral pulse wave velocity (cf-PWV) and left ventricular mass index (LVMI)., Results: An equal to or higher than median (16 points) ODI score in 48 subjects (56.5%) was associated with the presence of carotid artery plaques ( p = 0.014). In multivariate analysis, after adjusting for covariates, the presence of carotid artery plaques remained independently associated with an ODI score equal to or higher than the median (OR, 3.71; 95% CI, 1.04-13.25; p = 0.044). None of the other analyzed parameters of hypertension-related organ damage demonstrated a significant relationship with the ODI score., Conclusions: We observed that more severe cLBP is associated with a higher prevalence of carotid artery atherosclerotic plaques among hypertensive patients.

48. The Barcelona Bio-Heart Failure risk calculator may predict 1-year mortality in patients with advanced heart failure.

Abstract: Introduction: Accurate risk assessment in patients with heart failure (HF) is crucial. Developing new models that combine biochemical and clinical variables with novel biomarkers is the best approach to improving the management and prognostic evaluation in this population., Objectives: We aimed to assess and compare the predictive utility of a new prognostic scale, the Barcelona Bio‑Heart Failure (BCN Bio‑HF) risk calculator, as well as traditional risk scores, the Heart Failure Survival Score (HFSS) and the Seattle Heart Failure Model (SHFM), in patients with end‑stage HF. We also searched for other risk factors associated with worse prognosis in the analyzed population., Patients and Methods: This was a prospective analysis of 279 patients with end‑stage HF listed for heart transplant between 2018 and 2021. The BCN Bio‑HF, HFSS, and SHFM scores were calculated in all patients, and the accuracy of these 3 models for predicting 1‑year mortality was assessed using receiver operating characteristic (ROC) analysis., Results: Median (interquartile range) age of the patients was 56 (50-60) years, and 87.1% of the study population were men. During 1‑year follow‑up, a total of 95 patients (34.1%) died. The areas under the ROC curves for predicting 1‑year mortality were 0.95 (95% CI, 0.92-0.97) for BCN Bio‑HF, 0.81 (95% CI, 0.76-0.86) for HFSS, and 0.7 (95% CI, 0.63-0.76) for SHFM. We found that the BCN Bio‑HF (hazard ratio [HR], 1.015; 95% CI, 1.012-1.019; P <0.001) and HFSS scores (HR, 2.801; 95% CI, 1.848-4.237; P <0.001), along with the circulating bilirubin concentration (HR, 1.015; 95% CI, 1.002-1.028; P = 0.02), were associated with 1‑year mortality in the analyzed population., Conclusions: The BCN Bio‑HF risk score had significantly better prognostic performance than HFSS or SHFM. Lower BCN and HFSS scores and a higher bilirubin concentration were independently associated with a higher risk of 1‑year death in patients with end‑stage HF.

49. Ceruloplasmin and Lipofuscin Serum Concentrations Are Associated with Presence of Hypertrophic Cardiomyopathy.

Abstract: Oxidative stress reflects an imbalance between the systemic manifestation of reactive oxygen species and cells' ability to neutralize them by antioxidant systems. The role of oxidative stress in hypertrophic cardiomyopathy (HCM) is not fully understood. The aim of the study was to examine selected parameters of oxidative stress in patients with HCM compared to the control group. We enrolled 85 consecutive HCM patients and 97 controls without HCM. The groups were matched for sex, the body mass index, and age. Oxidative stress markers included superoxide dismutase (SOD), ceruloplasmin (CER), and lipofuscin (LPS). The median age of the HCM patients was 53 (40-63) years, and 41.2% of them were male. HCM patients, compared to the control ones, had significantly increased levels of CER and LPS. The areas under the receiver operating characteristics curves (AUC) indicated a good discriminatory power of CER (AUC 0.924, sensitivity 84%, and specificity 88%), an acceptable discriminatory power of LPS (AUC 0.740, sensitivity 66%, and specificity 72%), and poor discriminatory power of SOD (AUC 0.556, sensitivity 34%, and specificity 94%) for HCM detection. CER with good predictive strength, as well as LPS with acceptable predictive power, allows for HCM detection. The utility of SOD for HCM detection is limited.

50. Mortality of patients presented with acute ST-segment elevation myocardial infarction according to the status of standard modifiable cardiovascular risk factors.

Abstract: Background: Standard modifiable cardiovascular risk factors (SMuRFs) remain well-established elements of assessing cardiovascular risk scores. However, there is growing evidence that patients presented without known SMuRFs at admission demonstrate worse post-myocardial outcomes. The aim of the study was to assess the influence of the SMuRF status on short- and long-term mortality rates in patients with first-time ST-segment elevation myocardial infarction (STEMI)., Methods: This observational, cross-sectional study covered 182,726 patients admitted between 2003-2020 to the CathLabs, according to data from the Polish Registry of Acute Coronary Syndrome. Both baseline characteristics and mortality (in-hospital, 30-day, and 12-month) were examined and stratified by SMuRF status. The predictors of mortality were assessed at selected time points by multivariable analysis., Results: The majority of STEMI patients had at least one SMuRF (88.7%), however, mortality rates of SMuRF-less individuals were greater at selected time points of the follow-up (p < 0.001), and persisted at a higher level during each year of the follow-up period compared to the SMuRF group and general population. Furthermore, the SMuRFs status constituted an independent predictor of mortality at the 30-day (OR: 1.345; 95% CI: 1.142-1.585, p < 0.001) and 12-month (OR: 1.174; 95% CI: 1.054-1.308, p < 0.001) follow-ups., Conclusions: SMuRF-less individuals presented with STEMI are at an increased risk of all-cause mortality compared to those with at least one SMuRF.
